Seeing this on screen a simple procedure that almost everyone in Hollywood has undergone, yet is still discussed and rarely acknowledged is groundbreaking in itself, but the film goes further: In a later scene, Romy's teenage daughter (Esther McGregor) actually laughs about her plastic surgery. "Why would you do that to yourself?" she whines, pouting theatrically in imitation of her mother. "You look like a dead fish." Romy doesn't react, but we know it bothers her we see her examining her own reflection, quietly worried, moments later.

In a film rife with shocking, transgressive sex scenes, this is still the bravest and most revealing thing Kidman does. Traditionally, Hollywood stars have faced expectations to fight against visibly aging while avoiding the kinds of invasive treatments that might make them look “unnatural” and Babygirl confronts that hypocrisy head-on. Likewise, in The Substance, while Elisabeth’s efforts to keep her skin wrinkle-free aren’t explicitly communicated to the audience,
Moore’s unlined, carefully contoured face and glossy lips make it clear that her character has done everything in her power to look as youthful as possible for as long as possible, only to be let down.

In Marielle Heller's Nightbitch, which premiered earlier this month at the Toronto International Film Festival, Amy Adams's unnamed protagonist takes a radically different, though no less traumatic, approach to aging. Driven mad by her new responsibilities as a stay-at-home mom to the point where she eventually finds herself, er, turning into a dog she's seen, like Moore's Elisabeth and Kidman's Romy, referencing her own image in the bathroom mirror in an early scene. She feels broken, her hair is disheveled, wrinkles appear, and long hair grows in surprising places. "We're where we're at," she sighs.
