Don’t neglect Varicose veins can cause aching pain and discomfort. Sometimes the condition leads to more serious problems. Varicose veins may also signal a higher risk of other disorders of the circulatory system.

Varicose veins are gnarled, enlarged veins. The word “varicose” comes from the Latin root “varix,” which means “twisted.” Any vein may become varicose, but the veins most commonly affected are those in your legs and feet. That’s because standing and walking upright increases the pressure in the veins in your lower body. For many people, varicose veins and spider veins — a common, mild and medically insignificant variation of varicose veins — are simply a cosmetic concern. For other people, varicose veins can cause aching pain and discomfort. Sometimes the condition leads to more serious problems. Varicose veins may also signal a higher risk of other disorders of the circulatory system. Varicose veins are a common condition in the India, affecting up to 15 percent of men and up to 25 percent of women. Treatment may involve self-help measures or procedures by your doctor to close or remove veins. ‹‹ Signs and symptoms Some people with varicose veins don’t experience any discomfort from the condition. When painful signs and symptoms do occur, they may include:

An achy or heavy feeling in your legs, and burning, throbbing, muscle cramping and swelling in your lower legs. Prolonged sitting or standing tends to make your legs feel worse. ªª Itching around one or more of your veins. ªª Skin ulcers near your ankle, which represent a severe form of vascular disease and require immediate attention. Varicose veins are dark purple or blue in color and may appear twisted and bulging — like cords. They commonly appear on the backs of the calves or on the inside of the leg. However, they can form anywhere on your legs, from your groin to your ankle. Spider veins are similar to varicose veins, but they’re smaller. Spider veins are found closer to the skin’s surface and are often red or blue. They occur on the legs, but can also be found on the face. Spider veins vary in size and often look like a spider’s web or a tree branch. ¾¾ Causes: Arteries carry blood from your heart to the rest of your tissues. Veins return blood from the rest of your body to your heart, so the blood can be recirculated. To return blood to your heart, the veins in your legs must work against gravity. Muscle contractions in your lower legs act as pumps, while toned, elastic vein walls help blood return to your heart. Tiny one-way valves in your veins open as blood flows toward your heart then close to stop blood from flowing backward. As you get older your veins can lose elasticity, causing them to stretch. The valves in your veins may become weak, allowing blood that should be moving toward your heart to flow backward. Blood pools in your veins, and your veins enlarge and become

ªª Obesity. Being overweight puts added pressure on

your veins.

ªª Standing for long periods of time. Your blood doesn’t flow as well if you’re in the same position for long periods. ‹‹ Treatment Fortunately, treatment usually doesn’t mean a hospital stay or a long, uncomfortable recovery. Selfhelp measures — such as exercising, losing weight, not wearing tight clothes, elevating your legs and avoiding long periods of standing or sitting — can ease pain and prevent varicose veins from getting worse. Varicose veins that develop during pregnancy generally improve without medical treatment within three months after delivery. If you don’t respond to self-help or if your condition is more severe, your doctor may advise one of these varicose vein treatments: Sclerotherapy, Laser surgeries, Catheter-assisted procedures, Vein stripping, Ambulatory phlebectomy and Endoscopic vein surgery ªª Prevention ªª There’s no way to completely prevent varicose

Women are more likely than men are to develop the condition. Hormonal changes during pregnancy, premenstruation or menopause may be a factor. Female hormones tend to relax vein walls. Taking hormone replacement therapy or birth control pills may increase your risk of varicose veins. ªª Genetics. If other family members had varicose veins, there’s a greater chance you will too.

veins. But improving your circulation and muscle tone can reduce your risk of developing varicose veins or getting additional ones. ªª Exercise. Get your legs moving. Walking is a great way to encourage blood circulation in your legs. ªª Watch your weight, and your diet. Shedding excess pounds takes unnecessary pressure off your veins. What you eat makes a difference, too. Follow a low-salt, high-fiber diet to prevent the swelling that may result from water retention and constipation. ªª Watch what you wear. Avoid high heels. Lowheeled shoes work calf muscles more, which is better for your veins. Don’t wear tight clothes around your waist, legs or groin. Tight panty-leg girdles, for instance, can restrict circulation. ªª Elevate your legs. To improve venous circulation, take several short breaks daily to elevate your legs above the level of your heart. For example, lie down with your legs resting on three or four pillows. ªª Avoid long periods of sitting or standing. Make a point of changing your position frequently to encourage blood flow. Try to move around at least every 30 minutes. ªª Don’t sit with your legs crossed. This position can aggravate circulation problems. Prominent varicose veins on the back of  the right leg caused by a leaking valve at the juction between superficial and deep veins behind the knee (sapheno-popliteal junction). Varicose veins on the inner aspect of the right thigh and calf caused by a leaking valve in the groin (sapheno femoral incompetence). If required, punishment could include things like helping with small chores at home, denying sweets (not food), denying television time etc. 5. Parents may also take help of psychological counselors in the school in dealing with such behaviour. ‹‹ Management of behaviour disorders: ªª Doctor and behavioral psychologist should be consulted. Medicines are effective in managing the behaviour disorders resulting from past misconducts, injury (physical and mental) and environmental reasons. ªª It is important to keep the child away from the illfitting environment before starting the treatment. ªª A combined effort of parents, Doctor and behavioral psychologist can bring about the desired change in the abnormal behaviour of the child. ªª Behaviour psychologist does individual psychotherapy of the child and also gives training to the parents in child management skills. ‹‹ Common habit disorders ¾¾ Thumb sucking ªª Thumb sucking is considered normal behaviour in young children but is often maladaptive in older children and adolescents. ªª Children suck their thumbs or fingers as it makes them feel secure and has soothing effect on them. ªª In some babies thumb sucking can signal hunger,

fatigue, sleep, teething or shyness. Excessive thumb sucking can harm alignment and positioning of teeth. It also affects jaw bone as it is soft and easy to bend in babies. Nail biting Nail-biting is the most common of the typical “nervous habits” which include nose-picking, hair-twisting or pulling, tooth-grinding and picking at skin. It affects as many as one half of all school age children, Nail biting is considered to be a disorder when the habit interferes with his/her daily functioning or with his/her social relationships. Nail-biting can cause your fingertips to be red and sore and sides of nails to bleed. Nail-biting also increases risk for infections around nail beds and in mouth. Cause of habit disorders: Exact cause of these habits is not known. Genetic predisposition and many psycho-social factors have a role in causing habit disorders. A child who constantly bites his nails may be suffering from severe anxiety or a poor selfimage. Do’s and don’ts in habit disorders: Keep child’s hands occupied; try substituting another activity, such as drawing or writing or squeezing a stress ball, when you find your child biting his nails. Keep a close record of the times when child bites his nails or does thumb sucking in order to support and encourage the child to deal with the situation and be able to stop the habit. It is always good to educate the child regarding the harmful consequences of nail biting and thumb sucking such as distorted teeth alignment, damage to fingers, repeated throat and nail bed infections etc. Making children wear gloves or adhesive bandages is generally advised but is not very effective in treating habit disorders.

Marathi Karivana Indian pennywort is a perennial prostrate medicinal herb flourishing on water and damp areas of tropical regions. It is native to India, China, Indonesia, Madagascar and Africa. It is generally applied in the cure of leprosy, skin diseases, diarrhoea and is known for its antitumour and antiulcer activity. The fresh herb has an aromatic odour when crushed and has a nauseous bitter taste. Whole plant of Mandukaparni is popular as a kitchen herb in certain parts of India finding a place in chutneys, pickles, refreshing drinks etc. It is also reported to possess insecticidal properties. It improves immunity in children below one year when they are given this juice with honey. Insomnia patients can take 2-4 spoons of the juice with honey twice daily and the oil prepared is to be applied to the scalp. The whole plant is crushed, boiled with milk called “Ksheera Paka”. This improves memory power. If this is given in the form of food the memory power can be improved in mentally retarded children. Decoction prepared with the plant and Tulasi (Holy Basil), black pepper cures fever and cold, RECIPE Curry: Prepared by cooking the Pennywort leaves along with dhal, grams or any other vegetables. Thambuli: Roast pennywort leaves, cumin seeds and pepper with a little ghee. Grind along with grated coconut, Add salt to taste. Mix the paste with curds or butter milk. Salad: Pennywort leaves, Cucumber, Carrots, Cut all the vegetables, mix and add salt to taste. Cool drink: (A soothing drink for summer) Make the juice of pennywort leaves. Add sugar or jaggery. Warm drink: During rainy season, prepare a decoction of pennywort leaves. Drink by adding jaggery. Chutney: Cut the leaves, Add grated coconut, 1 or 2 green chillies, Salt and tamarind, if required little jaggery can be added. Grind all the ingredients. While preparing Idli, dosa or roti the pennywort leaves can be added to the dough. Apart from being healthy, it adds to the taste.

What is Dry Eye? Tears keep the surface of the eyes wet, clean and protect your eyes from infections. Tears also provide constant moisture and lubrication to maintain the vision and comfort to the eyes. It your eyes do not produce enough tears or if the tears dry up too quickly, the condition is termed as dry eye, ‹‹ What causes Dry Eye? Dry eye may be a temporary or a chronic condition. It measures moisture content in the eye and determines the severity of the problem ªª A dye, such as fluorescein or rose Bengal is placed on the eye to stain the surface.This can measure the surface of the eye is affected by the dryness or not ‹‹ What is the treatment for dry eye? Numerous treatment strategies exist to battle the signs and symptoms associated with dry eye. Fibroids: What You Should Know You’ve found out that you have fibroids (myomas). Fibroids are very common and treatable. They are round muscle growths in your uterine wall. ¾¾Pelvic Exam

During a pelvic exam, your doctor examines your reproductive organs. The tenderness, texture, and overall size of your uterus are checked. If you have abnormal bleeding, your doctor will also check your vagina and cervix for signs of infection or small breaks in the tissue. Samples of cervical cells may be taken (a Pap test) to check for abnormal cell growth. Since fibroids can grow on the back wall of the uterus, your doctor may also do a rectal exam. A Pelvic exam allows your doctor to feel fibroids in or around your uterus. ¾¾ Diagnostic Tests Three common tests can provide close-up views of the inside or outside of your uterus and confirm the size and general location of fibroids. Ultrasound is quick and painless. Hysteroscopy and laparoscopy are more involved procedures. Ultrasound is often used to check fibroid growth. A device placed on your lower abdomen or inside your vagina bounces sound waves off your tissues, creating a video picture. Other imaging tests may also be done to check fibroid growth and location. Hysteroscopy evaluates fibroids that extend into the uterine cavity. Your doctor looks directly at the inside of your uterus through a hysteroscope, a thin “telescope” with alight attached. This device is passed through your vagina and cervix. Laparoscopy is used to look at the surface of your uterus. This procedure uses tiny incisions and special instruments to check for fibroids. ¾¾ Your Treatment Options How your fibroids are treated depends on the number, size, and location of the fibroids, how fast they’re growing, and your symptoms. You and your doctor may decide to monitor them, remove them, or remove your uterus. But if your uterus is removed, you’ll no longer be able to have children. Talk with your doctor about your plans to have a family, as well as the risks and benefits of your treatment options. ¾¾ Keeping Your Symptoms in Check Fibroids may change size or shape. If they shrink, symptoms may lessen or even go away. Also, fibroids often won’t affect a woman’s ability to have a healthy pregnancy. If your doctor thinks that your fibroids are likely to shrink, he or she may suggest simply monitoring them instead of removing them. To monitor your fibroids, your doctor may schedule you for regular ultrasound tests or pelvic exams. Monitoring your fibroids may be a good option if they’re small or if you’re nearing menopause (the end of menstrual cycles). At menopause, fibroids often shrink naturally, due to decreasing levels of the hormone estrogen. Other things can affect the size, shape, number, and location of fibroids: ªª Pregnancy. Fibroids may grow rapidly during pregnancy. But they may not cause serious problems. If you’re thinking of getting pregnant, discuss this with your doctor. ªª Birth control pills. Using birth control pills may change the size of your fibroids. Your doctor may

change your birth control prescription to better control your fibroids. ªª Hormone replacement therapy. If you are receiving this therapy, it may cause your fibroids to grow. If so, your doctor may suggest that you try a lower dose. ¾¾ Removing the Fibroids Surgery to remove a fibroid is called myomectomy. This surgery can help preserve your uterus and your ability to have children. But new fibroids may grow back later. Choosing the best type of surgery depends on the location, size, and number of fibroids.  Some fibroids located inside the uterus may be removed through the vagina using hysteroscopic myomectomy.  An abdominal myomectomy (laparotomy) removes fibroids through an abdominal incision.  Small fibroids on the outside of the uterus may be removed through small incisions using laparoscopic myomectomy. Before surgery, your doctor may prescribe medications to shrink the fibroids. ¾¾ Removing Your Uterus After a hysterectomy (removal of the uterus), fibroids won’t return. But you won’t be able to have children. Depending on the fibroids and your symptoms, this surgery may be a good solution. The uterus is removed through an abdominal incision, through the vagina, or through a combination of both (LAVH). The ovaries are often retained to allow continued hormone .production. Also, the cervix ,may he retained. Before having a hysterectomy, talk with your doctor about the option of keeping your ovaries and cervix. ‹‹ Other Options to Treat Heavy Bleeding or Shrink fibroids:  Endometrial ablation: destroys the uterine lining with electric or laser energy to reduce heavy bleeding.  Uterine artery embolization shrinks fibroids by cutting off their blood supply. ¾¾ Your Future Checkups Treating your fibroids is likely to relieve your symptoms. But your doctor will want to schedule regular checkups to monitor your progress. If you have surgery, ask your doctor about any additional follow-up visits you might need.

You need to be extra careful about cleansing your skin without stripping it of its natural moisture. After half an hour take out the leaves from ball , grind them to a paste and bind it on affected area. ¾¾ Rheumatoid Or Usually Known As Inflammation Of Lining Of Joints Or Internal Organs In Rheumatoid Arthritis the lining of joints or lining of internal organs are inflamed. This disease may be chronic (lasts for a long time) or may flare up intermittently. The inflammation of lining of joints causes swelling, pain, redness, stiffness and warmth in the affected region. The cells of inflamed lining release enzymes which may harm the related bone and cartilage. This process may cause deformity of the joint pain and may restrict joint movements (stiffness). In ayurveda there is very detailed description about rheumatoid arthritis or “Amavata”. become inflamed and painful. The pain in affected joints resembles the pain of a scorpion’s sting.” ¾¾ Causes The exact cause of Rheumatoid arthritis is not known. But immunologists have discovered that Rheumatoid Arthritis is an auto immune disease. Some may have a tendency to inherit this disease. The cause of rheumatoid arthritis is very well explained in ayurveda. The rheumatoid arthritis is caused by formation of “Ama” (a toxin that is produced by imbalanced body fire). The toxin Ama is carried by imbalanced Vata (one of the three energetic forces) and reaches the Kapha (one of the three energetic forces) dominated places like joints, stomach, chest, brain etc. This toxin becomes sticky due to imbalanced doshas and blocks the vital channels which nourish the body. The sticky toxin ama not only affects the joints but also spreads all over the body. The channels (channels like arteries, intercellular spaces, pores in cells etc are called srotas) which allow nutrients to flow through and nourish body tissues, cells get partially blocked. This leads to a decreased supply of nutrients to body tissues and cells. Thus the immunity of the body gets lowered, the appetite reduces, the body becomes weak due to insufficient nourishment of body tissues. The Ama which gets harbored in joints acts like a foreign substance and triggers the immune system. They help in letting in and letting out the blood, opening up and closing in, in a wink. ¾¾ Facts about heart ªª It weighs approximately 250 to 300 grams (9 to 11 oz) in females and 300 to 350 grams (11 to 12 oz) in males. ªª In the rest condition it beats between 60 to 80 times in a minute and when the body is working, stressed or excited the beat rate may increase 200 times a minute. ªª It pumps between 70 and 100ml of blood with each beat, and in one day heart circulates up to 10,000 litres of blood ªª The inner layer of the heart (endocardium) is in contact with the blood that the heart pumps. ªª The blood is pumped into the lungs through Pulmonary Artery ªª The heart gets the supply of blood needed for its muscles from the two small arteries called coronary arteries. When the heart fails to function properly defects develop in the circulation of blood, then different parts of the body fails to get proper supply of blood and organs may get damaged. Let some one else prepare food. Cooking odor may provoke nausea. Food that is served at room temperature or cold gives less odor than hot food. Have food with others in pleasant social atmosphere. Ask family members to bring home-cooked food to hospital after checking by dietician. To over come nausea, try chewing on ice chips or sucking on a ginger candy or lemon before eating food. Rest in sitting or upright position for half an hour after having food. Lying flat may trigger reflux, nausea and vomiting. Plan Your meal for the time of the day when you are least likely to experience vomiting and nausea i.e. early morning. Otherwise eat small frequent meals through out day. If mouth sores are the problem, eat bland food for example, rice and custards puddings made with milk and blended soup avoid salty, spicy or acidic foods. If a dry mouth makes swallowing difficult, liquefy foods in blender and moisten them with low-fat milk and gravies. If Diarrhea is a problem (while chemotherapy) avoid fatty food, raw fruits whole grain product and other foods that can make it worse. Instead eat bland, binding food such as rice, banana. Cooked apples. Say “no” to all these: ÂÂ ALCOHOL: Alcohol consumption hinders the body’s ability to use B-Carotene, which appears to protect against cancers. So, heavy drinkers have associate risk of cancer of month, larynx and liver. Alcohol reduces the reservation of folate, thiamine, vitamin-B and selenium. ÂÂ SMOKINAG: Top most risk factor. Fat, especially from animal sources are discouraged because they are believed to support the tumor growth. In contrast, fruits and vegetables contains natural plant chemicals that are thought to retard the growth of tumor. Protein is essential as it helps to repair the body. Daily 2 to 3 servings of cereals and sprouts can be used. ¾¾ Nature (body) is wise: Growing numbers of cancer specialists discourage urging some cancer patients to eat when they don’t feel like. In past, forced feeding in the form of enriched dietary supplements, intravenous nutrition, or gastric tube was recommended to maintain nutrition, but these all approaches usually did not result in weight gain. Instead, many who were forced fed actually died sooner; experts now believe this may be because the feeding actually spurs tumor growth. Many medical scientists believe that the anorexia and cachexia (severe form of malnutrition and body wasting) that occurs in advanced stage of tumor may be an example of “Wisdom of body” as it attempts to starve the tumor. In some conditions, cachectic patients are allowed to do starving while doctors undertake aggressive therapy to destroy tumor. ¾¾ Fake marketings: Millions of Indians take vitamin and mineral supplements, often in high dose, sometimes without expert opinion. There is increased intake of B-carotene and vitamins A, C and E in form of medicines. Unfortunately this dose not seem protect body from cancer. When these all vitamins consumed in form of fruits and vegetables, these do have an antioxidant effect, but when taken in form of supplements or medicine, these substances may have an adverse effect, which increase the growth of tumor.

What is cancer? Cancer is defined as the uncontrolled growth of any particular cells of a tissue due to inherent change in the nature of their DNA code due to different causes like viruses, radiation or other carcinogens. Cancer tends to invade neighboring structures or distant organs by spreading through blood or lymph vessels. ¾¾ How is cancer diagnosed? Patients with cancer present to physicians with signs and symptoms which are characteristics of that cancer. Then doctor orders special tests to confirm and

These tests are powerful tool for both workup and follow-up of cancer Other tests include genetic studies to identify the mutations in the DNA which are becoming useful in making decisions for personalized treatment in each patient. These tests include immunohistochemistry, FISH, RT-PCR, cytogenetic and flowcytometry ¾¾ How is cancer treated? Cancer treatment has evolved greatly in the recent days from crude to a sophisticated treatment modality which has helped patient to become disease free or increase the survival while also reducing the side effects of treatment. The treatment has been advancing to such degree that in coming days cancer will be like Diabetes or Hypertension where a patient can just can take a pill every day and carry on with his daily life. ¾¾ Major treatment modalities are: ¾¾ 1. There are many good reasons to lose weight. Improving your sex life might be one of them.

to be false. One problem with obese people is that they lack stamina, therefore being unable to sustain high energy activities for long periods of time. The research revealed that the average amount of calories burned during a session of sex was less than 50. This means that it cannot be considered an effective weight loss tool by itself. It can, however, still be a powerful motivator to keep going when combined with a healthy diet and a workout regimen. Obesity does not, by itself, lead to a bad sex life. However, there are physical conditions that go handin-hand with obesity that can cause such problems. One study revealed that as much as a third of all obese people have issues with their sex drive, performance, desire or a combination of the three. One problem that can cause this is high cholesterol. Most people that are overweight have elevated cholesterol levels as an unhealthy diet is oftentimes a main cause of obesity. Such a problem can also have an impact on sexual performance and desire. This issue is much more common in men than in women. Furthermore, the same symptoms can be caused by insulin resistance which is, again, something related to a poor diet. This can also be an early indicator of type II diabetes. These two medical issues can cause men to experience sexual problems because they can clog up the tiny arteries found in the penis with deposits of fat. As you would expect, this can lead to erectile dysfunction or even impotence. Men are not the only ones that can have sexual problems related to improper blood flow. This can also happen in women. They experience a diminished sex drive because the blood vessels that lead to the vagina can also get clogged, thus making their bodies less responsive to sexual stimuli. In certain cases of morbid obesity, men can suffer from a medical problem known as buried penis syndrome. As its name suggests, this problem involves extra layers of fatty tissue above the pubic area overlapping and hiding the penis. This can present physical difficulties for the man, making it harder for him to achieve penetration. However, from a medical standpoint, this condition can also be a direct cause for erectile dysfunction. Surgery is available as a treatment, but it is never the recommended option and, in some cases, it is not even possible. Weight loss through diet and exercise is always the best solution to treat this problem. There is another medical issue that further complicates matters and has a negative impact on the sex life of people struggling with weight problems. Excessive amounts of body fat will increase the levels of SHBG (sex hormone binding globulin) in the system. This is a natural chemical that binds to sex hormone testosterone, which is also used to provide a healthy sexual libido and increase sex drive. Higher amounts of SHBG means that there is less testosterone left to handle the duties associated with a normal sex life and therefore there is a decrease in desire. Obesity also interferes with a person’s sex life in other ways. One study found that it is much harder for obese women to find a sexual partner than it is for women of normal weight. They were 30% less likely to have had a sexual partner within the last year. The differences between obese men and regular men were

not significant. Not only that, but the same study revealed that obese women were also more likely to engage in risky sexual behavior. One common issue was the lack of contraceptive measures as overweight women were not as likely to anticipate a sexual encounter and plan ahead. Therefore unplanned pregnancies can also be an issue associated with obesity in women. žž Dealing with Obesity for a Better Sex Life The good news is that most of the issues in your sex life that arise as a result of obesity can be dealt with a little effort. The best way to combat this problem is by losing weight through diet and exercise. You do not even have to wait that long for results. Even losing as little as 10 pounds can start to stimulate sex hormone production, thus resulting in a healthier sexual appetite. This means that your sex drive should improve gradually throughout the weight loss process and you can see tangible results from the beginning. This can act as a powerful motivator to keep going until you reach your weight loss goal. Your love life can improve from this regardless of whether your problems were of a physical or mental nature. Some of the benefits of weight loss that pertain to this situation are increased stamina, bigger supplies of energy, better mobility, increased selfesteem and even help deal with depression. Syphilis is a sexually transmitted disease (STD) & is a bacterial infection usually transmitted by sexual contact. Syphilis is found worldwide, and is the third most common STD. The disease affects genitals, skin and mucous membranes, but it may also involve many other parts of the body, including brain and heart.

Causes: The bacterial organism that causes syphilis is Treponema pallidum. This organism is extremely sensitive to light, air and changes in temperature. It can live only within the human body. It enters body through minor cuts or abrasions in skin or mucous membranes. The most common route of transmission is through contact with an infected person’s sore during sexual activity. Other routes are through transfusion of infected blood, through direct unprotected close contact with an active lesion (such as during kissing), and through an infected mother to her unborn child during pregnancy. ¾¾ Bacteria causing Syphilis Treponema pallidium. Symptoms: Syphilis, typically first appears as a sore on the area where there has been sexual contact with the infected person. If the infection goes untreated, it can progress to affect the entire body. Syphilis typically has three stages with varying symptoms - primary, secondary and tertiary. Primary: These signs may occur from 10 days to three months after exposure: ÂÂ A small, painless sore on the part

of the body where the infection was transmitted, usually genitals, rectum, tongue or lips. A single sore is typical, but there may be multiple sores(As seen in above pictures).  Enlarged lymph nodes in the groin. ¾¾ A sore over the upper lip of a person  a. Sore over vagina.  b. Sore over Penis. ¾¾ Primary Syphilis seen over sex organs. Secondary: The symptoms of secondary syphilis may begin three to six weeks after the sore appears, and may include: ªª Rash marked by red or reddish-brown, small coin sized sores over any area of the body, including palms and soles ªª Fever ªª Fatigue and a vague feeling of discomfort ªª Pain in bones or joints. ¾¾ Rashes over the hand of a person – Secondary syphilis. Tertiary: If the Syphilis is not treated in the first two stages, it will lead to tertiary stage which is a serious stage involving spread and damage of internal organs which may lead to death. Neurologic problems. These may include stroke, infection of the brain and spinal cord (meningitis), poor muscle coordination, numbness, paralysis, deafness or visual problems, personality changes, and loss of memory (dementia). Cardiovascular problems. These may include bulging and infection of large blood vessels. These signs and symptoms may disappear and reappear for up to two years. Congenital: This type of Syphilis passed from mother (who is suffering from Syphilis) to child during fetal development. The bacteria will pass through the placenta to the fetus. This type of disease is a serious condition and can cause abortion. Even if the child is born it will have lot of abnormalities and defects. Some of the commonest problems seen in the infant are- nasal discharge, nasal defect, bone diseases, defect in the teeth, defect in the brain, defect in the eyes and defect in the ears. ¾¾ Congenital syphilis- Child with defective teeth

Left untreated, the disease can lead to serious complications or death. Some of the medicines in use are penicillin, , doxycycline, amoxycillin combined with probenocid, ceftriaxone & azithromycin. Prevention: To reduce the risk of syphilis and other sexually transmitted diseases, one has to adhere to practice of safe sex and follow ABC guideline, that isªª Abstinence - Not having sex before marriage or practicing it with more than one sexual partner. ªª Being faithful towards your spouse. ªª Condoms should be used for any type of sex

Diagnosis: If any painless sores in the genital area and enlarged lymph nodes in the groin area is noticed then immediately it should be reported to doctor. Treatment in the early stages of syphilis can prevent serious, long-term illness and spread of the disease. To diagnosis syphilis, following tests may be done Tests: Some of the tests used in diagnosis are, ÂÂ Microscopic examination of the lesion looking for Treponema palladium bacteria. ÂÂ Serological Tests: - These tests detects antibody for T. palladium bacteria. E.g.: Fluorescent Treponemal Antibody absorption test (FTA-abs). ÂÂ VDRL Test: Venereal Disease Research Laboratory test. This is a non specific test for Syphilis. This is a routinely conducted test. Complications: Untreated syphilis can lead to destruction of soft tissue and bone, heart failure, blindness and a variety of other conditions. A female with untreated syphilis may transmit the disease to her unborn child, which may result in death or deformity of the child. Doctors are required to test pregnant females for syphilis at pregnancy. Tests of newborns of the infected women are required at the time of delivery. People with syphilis also have an increased risk of contracting HIV-AIDS. A syphilis sore can provide an easy way for HIV to enter the bloodstream during sexual intercourse. Treatment: Early diagnosis and treatment with penicillin can kill the organism that causes syphilis and stop the progression of the disease.

Although fortunately there is a cure for syphilis, the ultimate goal is to prevent and control the spread of this infectious disease. Sunita Pant Bansal “My child refuses to eat fruits or vegetables. How can I get her (or him) to have a balanced diet?” Most of the pre-school teachers get to hear these complaints from the parents of their students. Nothing seems to get a parent more upset than a picky or a fussy eater. They feel that their child isn’t getting proper nutrition. Most children aged two to five have an uncomfortably short list of foods that they like to eat, and ‘food fights’ between parents and children are a common scene in many households. But then look at the fact that there is a positive side to your child’s eating habits. This is probably one of the first areas where your child is demonstrating his/her independence and personal taste! Over the ages parents have tried everything from “You’ll sit there until you finish every bite on your plate” to “What do you want for dinner, aloo-parantha or burger?” The answer lies somewhere in between these two extremes. There seems to be a power struggle when you are saying, “Do it because I’m the parent” and that rationale doesn’t work for long. But if your child understands the why behind the rules, those values can lay the groundwork for a lifetime of sound food choices, whether you are there to enforce them or not. You should discuss with your child. Tell your child what is good for him or her, and if they say that don’t like to eat certain things, give them options to choose from. Reason with your children; do not try to order them. My own experience as a parent of two girls has made me reach a conclusion that we must ask our children to lend a hand in the kitchen with easy tasks. If they participate in helping to make the meal, they are more likely to want to try it. It works with my 8-year-old nephew too! Just say pleasantly, “Oh well, maybe you’ll like the next meal better.” ªª Be a good role model for your child. Eat a variety of foods and show your appreciation for them. Children often get curious about food when it’s giving pleasure to others. ªª Do not be too strict about table manners with small children. Keep mealtimes enjoyable. For small children, part of the fun of eating is feeling the texture of the food in their hands. ªª Limit the fluid intake of your child to two to three cups of milk a day, and a glass of juice. Children very often fill up their stomachs with these or soft drinks and then have no room for other regular foods. ªª Avoid giving your child bribes or rewards for eating certain foods. These forms of enticement might work in the short term, but they don’t help your child develop a lifetime pattern of healthy eating. ªª Don’t make your child’s eating habits part of the mealtime discussion, otherwise the meal will become a stressful event. Talks about the importance of good eating should be reserved for bedtime or story time. One should always try to sleep in a relaxed and comfortable posture, rather than cramping oneself. Sleep is such a process where all the parts of the body, namely, the brain, nerves, organs, glands, muscles etc., are relaxed and function slowly after long hours of work. While sleeping, keep your body loose and relaxed so that you wake up fresh in the morning. Special care should be taken about the position of your hand. Hands should not be taken under me head or above the head while sleeping, as it disturbs the cervical bones and creates anginal pain, spondylilis etc. Unmarried girls should not sleep with one leg crossed over the other. While discharge and irregular periods can he a result of these postures. One should sleep in a room which has proper ventilation, so that carbon dioxide exhaled should go out. Otherwise, the head lets heavy and remains heavy for the whole day and there is a feeling of tiredness. While sleeping, air ventilation should not be stopped. Even if the room has one window it is sufficient as the hot air which is breathed out is light and will go out on its own and the cool fresh air comes in. One should not hold urine and stools tor long time. Periodical emptying of bowels and bladder will ensure good health. water. One should have a minimum of 8 to 10 glasses of water daily. No artificial drink or aerated water can replace plain water. After eating water - melon or any fruit do not drink water for atleast 10 to 15 minutes, as this may lead to fever. One should not drink water immediately alter tea or milk, as this can lead to loose motions. Atleast give a gap of 10 to 15 minutes. In NT it is stressed that everybody must drink at least 8 to 10 glasses of plain water every day. Water can reach the cells easily by osmotic principle only if nothing is added to

it. At the same lime, if digestion is to be done properly, water should not be taken along with meals, as it dilutes digestive juices. Water is to be drunk only after half an hour after meals. After walking in the midday sun, do not drink cold water directly from the fridge. Natural cold water may be taken. Always sit and drink water. There will be pain in calf muscles, if you drink water in a standing posture. Also, very sweet drinks draw water into the intestinal tract (at the expense of muscles) and may result in cramps during exercise. When one notices bubbles rising in stored water, it indicates that the water is polluted. During loose motions one should not drink plain water but should add a little sait and sugar to the water. Plain water will lead to more loose motions. U only salt is put m water, that also will lead to loose motions. One should have fruits and vegetables according to season. It is a natural phenomenon that fruits like mangoes are available only in hot season. Preserved fruits and vegetables are harmful to the health. vegetables and fruits disturb the chemical balance of the body and cause disease. A person who eats food quickly without chewing properly will tend to put on weight. The food that is well chewed is not only easy to digest but also helps to control obesity; as proper chewing leads to early satisfaction and so one tends to eat less amount of food. Also, food should be eaten in small bites, to facilitate digestion. Fruits which can be eaten by patients undergoing treatment for pain are papaya, custard apple, guava, banana, chickoo, figs, sugarcane etc. Sugarcane juice without lemon, coconut water and watermelon also may be taken. One should not eat any thing which contains white flour (‘Maida’). Since maida is bad for health, one should avoid food items such as bread, biscuits, noodles, samosa etc. Liquors, cigarettes and tobacco also should be avoided. Those patients who get fits should not eat cucumber, pumpkin, musk melon, water melon and gram flour. Diabetics should not eat rice, potato, sugar or sweet fruits. If you are having cold, do not blow your nose very hard. It can damage the ear through the eustachian tube. Nasal drops to relieve blocked nose should not be used, as their effect is temporary. NT treatment gives lasting effect. When giving bath to small children, first put a little water on the head avoiding the ears, then on the shoulders; thereafter put more water over head and ears; otherwise the head will suddenly become cool and the gasping child may inhale water along with air. echnically, the term ‘spondylosis’ refers to any ailment of the spine. ‘Spondylitis’ in its strictest sense, indicates inflammation involving the spine. This inflammation could be non-infective or ‘infective. The terms ‘spondylosis’ and ‘spondylitis1 are often used loosely for all kinds of pain involving the spine, more commonly the neck region. ¾¾ What are the symptoms? This pain is generally noted while moving one’s neck while coughing or sneezing. Similar to Sciatica which occurs in the lower hack, when the pain radiates into the upper limbs, it is referred to as Radiculopathy. Another complaint of cervical spondylosis is unsteadiness while walking. ¾¾ What are the causes? This kind of pain is commonly caused by a slipped disk or overgrown bone within the spinal canal. However, more often than not, lifestyle factors such as posture and even anxiety and tension tend to aggravate a seemingly innocuous slipped disk problem. ¾¾ Who is affected? It is seen more commonly in older people. Twowheeler riders may suffer from the problem. It also has a familial tendency. ‹‹ Types of spondylitis ªª Non-infective spondylitis often manifests as early morning stiffness. It is aggravated in cold weather. ªª Infective spondylitis manifests as pain in the neck or back, which is present even when the body is at rest. It is typically more pronounced at night. It may be accompanied by a history of fever. Other features include loss of weight and appetite. ‹‹ What is the treatment? ªª In the absence of any obvious feature suggesting significant pressure on the nerves, simple steps such as lifestyle factor modification, appropriate attention to posture and measures to alleviate stress can help. ªª Judicious use of medicine such as painkillers and muscle relaxants along with physiotherapy modalities may be warranted in some cases. In extreme situations with severe nerve compression, surgery may be the only option. DENGUE FEVER D

engue fever is an acute febrile disease, found in the tropics, and caused by four types (serotypes) of virus of the genus Flavivirus. It is also known as “breakbone fever” or “Dandy Fever”, slaves in the West Indies who contracted dengue were said to have the posture and gait of a dandy. Infection by these viruses may be asymptomatic or may lead to – ªª Classical Dengue fever ªª Dengue hemorrhagic fever(DHF) ªª Dengue shock syndrome (DSS). It is a mosquito-borne (arthropod-borne) disease, transmitted by Aedes mosquitoes, particularly A. aegypti and A. albopictus. Dengue may also be transmitted via infected blood products (blood transfusions, plasma, and platelets). Dengue fever is usually a self-limiting disease and DHF/DSS outbreaks are mainly seen in areas of high prevalence of Aedes mosquitoes. Dengue fever is generally a tropical disease and is seen in tropical and sub-tropical climates, mainly affecting urban and periurban areas (due to high population density). The WHO estimates that nearly 2.5-3 billion people live in areas where the virus can be transmitted, with around 50 million cases worldwide each year. Dengue fever id prevalent in India, with all 4 serotypes found. The origins of the word dengue are not clear, but one theory is that it is derived from the Swahili phrase which describes the disease as being caused by an evil spirit. Or, it may possibly have its origin in the Spanish word meaning fastidious or careful, which would describe the gait of a person suffering the bone pain of dengue fever. ¾¾ TRANSMISSION The Aedes mosquito becomes infective by

feeding on a patient (from the day before onset of fever to the 5th day of illness).The virus then takes 8-10 days to develop inside the mosquito, after which, the mosquito can infect a healthy individual. Furthermore, once the mosquito becomes infective, it remains so for life. This is referred to as “ManMosquito-Man” transmission. The mosquitoes commonly bite during dusk and dawn, and are more abundant during the rainy season, when they breed. ‹‹ SIGNS AND SYMPTOMS ¾¾ Dengue fever: ªª It may affect people of any age or sex, with children often having milder symptoms. ªª Onset is sudden, after an incubation of 3-10 days. ªª Patient may have high fever, rash, chills, intense headache, muscle and joint pains. ªª Patient may also be weak, lose his/her appetite, and have altered taste sensation. ªª Other symptoms include constipation, abdominal tenderness, pain behind the eyes with painful eye movements, groin pain, and sore throat, diarrhea, vomiting. ªª Fever has typical periods of remission and is often accompanied by a rash with flushing and pin-point eruptions on the face. The rash may be present all over the body. ªª Fever usually subsides after 5-7 days, and case fatality is very low. ¾¾ Dengue Hemorrhagic fever: ªª Severe form of dengue fever ªª Thought to be due to a double infection by dengue viruses. ªª Has the same features as dengue fever, but with greater severity, accompanied by hemorrhagic manifestations like – hemorrhagic spots on

the skin (ecchymosis, petichae, and purpura), bleeding from the nose (epistaxis) and gums, blood in vomitus and stools. ªª Infants may have febrile convulsions. ªª The liver may be enlarged ªª Blood tests may reveal low platelets in the blood. ªª DHF in cases of liver cirrhosis has been linked with the development of Liver cancers. ¾¾ Dengue Shock Syndrome: ªª Occurs when patients with DHF lose enough blood to go into shock. ªª It represents an emergency condition where patients have a fast pulse, low blood volume and blood pressure, cold skin and are restless. ¾¾ DIAGNOSIS The diagnosis of dengue is usually made clinically from the classic picture of a high fever with a (petechial) rash with low platelet and white blood cell count. It is also important to diagnose signs of DHF and signs of shock in DSS. Care has to be taken as diagnosis of DHF can mask end stage liver disease and vice versa. Dependable immediate information of the Dengue diagnostics in the rural areas can be performed by the introduction of Rapid Diagnostic Test kits which also differentiates between primary and secondary dengue infections. Serology and polymerase chain reaction (PCR) studies are available to confirm the diagnosis of dengue if clinically indicated. If a patient has persistent fever for more that 2 days and/or constant headaches then they should go for a complete blood check. If the platelet count and WBC count are below than their usual range then a Dengue Antigen test is done. To go for Dengue Test is decided depending on the result of CBC Counts. ¾¾ TREATMENT ªª The mainstay of treatment is timely supportive therapy. ªª The patient is advised bed rest and is given antipyretics (for fever) and oral fluids and electrolytes (especially in cases of diarrhea). Intravenous fluids are advised for patients with vomiting. ªª Aspirin and non-steroidal anti-inflammatory drugs are avoided as they aggravate bleeding tendencies. ªª The mainstay of management of DSS is to tackle

shock due to hemoconcentration and bleeding. Close monitoring of vital signs is critical. Oral or Intravenous fluids are given to prevent dehydration. A blood transfusion or just platelet transfusion is indicated in rare cases if the platelet level drops significantly (below 20,000) or if there are significant bleeding. The presence of melena(Blood in the stools) may indicate internal gastrointestinal bleeding, requiring platelet and/ or red blood cell transfusion. ‹‹ CONTROL MEASURES ¾¾ Mosquito Control There are two primary methods: larval control and adult mosquito control. In urban areas, Aedes mosquitoes breed on stagnant water collections in artificial containers such as plastic cups, used tires, broken bottles, flower pots, etc. Periodic draining or removal of artificial containers is the most effective way of reducing the breeding grounds for mosquitoes. Larvicide treatment is another effective way to control the vector larvae there are some very effective insect growth regulators (IGRs) available which are both safe and long-lasting (e.g. pyriproxyfen). For reducing the adult mosquito load, fogging with insecticide is somewhat effective. Prevention of mosquito bites is another way of preventing disease. This can be achieved by using insect repellent, mosquito traps or mosquito nets. In 1998, scientists from the Queensland Institute of Medical Research (QIMR) in Australia and Vietnam’s Ministry of Health introduced a scheme that encouraged children to place a water bug, the crustacean Mesocyclops, in water tanks and discarded containers where the Aedes aegypti mosquito was known to thrive. This method is viewed as being more cost-effective and more environmentally friendly than pesticides, though not as effective, and requires the continuing participation of the community. ¾¾Vaccines There is no commercially available vaccine for the dengue flavivirus. However, a number of vaccine candidates are entering phase I or II testing. Aromatherapy is a treatment which cures or heals both mind and body, through the correct use of fragrances. These fragrances or aromas are obtained from essential oils which are present in the different parts of the plants- fruits, leaves, flowers, bark, roots etc. These essential oils are antiseptic, anti inflammatory, antibacterial, antitoxic etc. These days, parlors, massage centers, spas, all advertise about aromatherapy. With a little effort, you can turn your daily activities, even bathing, into an aromatherapy treatment. List of the different ways which you can use the essential oils at home. ªª Take 20ml of any vegetable oil, preferably cold pressed, add 7-8 drops of essential oil and

Rose is good for relaxing, Lavender is relaxing and refreshing, and Jasmine and Sandalwood act as an aphrodisiac. ªª Add 6 drops of essential oil in your bath water and feel good throughout the day. ªª Use the essential oils as room fresheners. Just add a few drops in a bowl of hot water or smear a few drops on the tube light or any source of heat. The fragrance will spread in the room and create a cheerful atmosphere. Lavender, Geranium, Pine etc can be used. ªª For colds and blocked noses, you can add 6-8 drops of Eucalyptus or pine in hot water and inhale. ªª Essential oils can be used as compresses also, both hot and cold. For bruises, use Lavender in cold water, headaches can be relieved by using Peppermint or Lavender in cold water. Sprains also benefit with the use of peppermint in cold water. For boils, sandalwood in hot water is effective. Rose, Lavender in hot water can be used as a hot compress to get relief from cramps and painful menstruation. ªª Use Rose oil to make rose water. Buy 1ltr bottle of mineral water, add 8-10 drops of Rose essential oil and l/2tsp of glycerin. Shake it every time you want to use it. ªª Apply Lavender oil directly on insect bites or burns. ªª Cedar wood oil can keep moths and silverfish away from your woolens and books. ªª Mix any mineral oil with Geranium oil and apply on the exposed parts of the body to keep away the mosquitoes. ªª Tea tree oil is a very powerful disinfectant. Just 1 drop in al bucket of water is enough. Aromatherapy is also effective in taking care of your skin and hair. More of that next time. Till then take care- the aromatherapy way.
