Asking These Questions Can Save a Life!

Thursday, 09 September 2021 19:42 Written by Deb Saine, MS, RPh, FASHP Administration of concentrated potassium chloride (KCl) by IV push is deadly — it will stop the heart. In June, ISMP reported the first KCl IV push accidental fatality in 14 years.

This event occurred during a code in the intensive care unit and involved a series of miscommunications and incorrect assumptions. The hospital stored the concentrated KCl vials only in the pharmacy department, consistent with The Joint Commission (TJC) standards since 2003.

Upon pharmacist request, a vial was delivered to the patient’s room, where it was subsequently administered IV push, resulting in the patient’s death.

Ask yourself - could this happen at your organization? If you stock vials of KCl or have access to it, the answer is YES, and it’s time to take action! Read the case available on ISMP’s website. Take time to discuss it with your staff and leadership, and plug the "holes" now to avoid a similar event.

ALL pharmacy staff should be aware of it, and signage should be posted near storage of all the "never drugs" and near your tube station. Quiz often.

Does everyone in your department feel comfortable asking questions about potential safety concerns? It might be time to take the pulse of your safety culture. Empower everyone - at all levels - to ask for clarification. This might include a pharmacy technician who retrieves the vial from the storage area, the person who is asked to deliver the vial to the patient care unit, the pharmacist who authorizes the vial to leave the pharmacy and the pharmacist who requested the vial based on the doctor’s incomplete request. Even one question might be enough to prevent tragedy.

Medications that should not be stored or dispensed outside the pharmacy department are listed below. (If your organization requires storage in a specific patient care area due to a unique situation, requirements for special handling, labeling, dispensing and accountability should be defined in policy and easily accessible to all users.)

- Concentrated electrolytes (e.g. KCl, NaCl 3% or greater) - Concentrated insulin U-500 - Concentrated oral opioid solutions - Sterile water in bags - Concentrated epinephrine multi-dose vials - Neuromuscular blocking agents

Raptou Today I’d like to share with you a product that was created expressly for my organization to solve a JCAHO requirement. Health Care Logistics® Item #MFG 39755 - PETG Sink Splash Guard allows medications to be prepared on a counter that is immediately adjacent to a sink.

My health system had dozens of medication rooms that needed some type of partition next to the sink. We were able to collaborate with HCL® to design a low-cost solution to our problem, which looks and performs great! We were able to dictate the dimensions and the need for it to be made of a single piece of plastic that was easy to clean and without areas where germs could easily hide.

In the creation of this splash guard, we were presented with different plastic material options that allowed us to pick the one best suited to the cleaning solutions we were actively using for our cleaning processes.

I find it so refreshing to not just have a company that will furnish needed products that we can rely on, but also to have a partner capable of innovating and designing a product from a concept.

Our pharmacy still gets requests for the source of our splash guards from clinical managers who think a splash guard makes good sense next to sinks that may not necessarily be in a medication room and are not be required to have one. During this last year, I even heard of instances where these were used as COVID partitions for personnel that had to communicate face-to-face with the public.

Disposable Bin Liners: Waste Less Time, Clean Fewer Messes Thursday, 15 October 2020 16:12 Written by Allison Lawhorn Save time cleaning up spilled messes, be it powder or sticky liquid, with these Disposable Bin Liners. If there’s a spill – no problem!

The mess is contained in the disposable liner. Simply toss and replace with a new one. The liners keep drawers clean and items easily accessible.

Rather than trying to fish out each item from a small compartment, simply lift the liner out and pour out the contents. It makes reaching to the very bottom or checking the date on products a breeze!
