LETTER FROM OUR BOARD CHAIR AND CEO

Dear Friend of HCS,

Some days, in the midst of dealing with new government rules, advanced in-home therapies, escalating costs, and shifting patient demographics, it is easy to feel like we are running in place rather than making our way forward. There is ALWAYS so much MORE to be done! Writing this report provides a welcome opportunity to reflect upon the significant progress that the Board and staff of Home Healthcare, Hospice, and Community Services made in 2024.

HCS welcomed two new “tenant-partnerships” in 2024 that will pave the way for growth and collaboration. The Keene Senior Center relocated to 312 Marlboro Street, Keene (HCS Main Office) in January, taking advantage of increased space, convenient parking, and weekday Friendly Meals luncheons – all culminating in expanded membership for Keene Senior Center. This November, our HCS Peterborough location transitioned to the Monadnock Community Hospital campus, placing us right next to collaborative physician practices and nearer to patients about to be discharged from hospital to home.

Our Hospice and Palliative Care patient census doubled in size thanks to our reputation as the region’s same day Hospice response team. Additionally, our Home Health team of nurses, therapists, and aides achieved top rankings in New Hampshire and the nation for avoiding patient rehospitalization while under HCS care.

We are excited to announce that our Healthy Starts Family and Child support program and our VNA homecare program promoted three direct care staff to leadership roles; harnessing talent, organizational knowledge, and aspiration for professional challenge to activate new approaches to our work. We partnered with the Monadnock Mobile Integrated Health community paramedicine program to pilot prenatal and post-partum home visits for highrisk mothers and secured a grant to continue this service with additional RN staff.

After months of collaboration with our trusted regional insurance advisor, The Richards Group, we launched a new health insurance plan that offers multiple choices for our staff, allowing them to benefit financially through an Individual Coverage Health Reimbursement Arrangement (ICHRA)- an emerging trend in employee benefits sustainability.

Looking back for a moment, it appears we were not running in place after all. As we journey forward on the road together, we’re making progress with the support and encouragement of friends like you!

MEALS ON WHEELS MOMENTS OF CONNECTION

When Meals on Wheels driver Doug Pryor didn’t get a response at Jane’s* door, he was concerned. Jane always greeted him and was appreciative of the few minutes of conversation he brought along with the meal delivery. As a driver with Meals on Wheels for over many years in Hinsdale, he knew that participants relied on him not only for the nutritious meal, but also for a daily safety check. Doug used the ServTracker app on his phone to alert Gia Farina, the program manager for nutrition at HCS of his concerns. Every day, drivers report that 10 to 15 participants have not answered their doors. Gia calls emergency contacts and in some cases local police departments to do a “wellness check” to make sure that participants are safe. Gia notified Jane’s son of the driver’s concerns. He visited his mom that afternoon and found that she had fallen. Due to her multiple health conditions, she was hospitalized. John*, her son, called Gia to thank her for the alert. “Without the call from Meals on Wheels, my mom would have been on the floor for a long time since no one else usually visits her during the week. I thank Doug and Gia for their concern so that I could get mom the care she needed.”

Drivers report concerns not only when someone does not answer the door, but also when someone is not “themselves.” Recently William, who lives alone in Keene, said that he did not feel good, when Paul, his driver dropped off his lunch. At 84, this raised concerns for Paul, who reported this concern to Gia. William had listed all three of his children as contacts, so she left messages with all three. Later that day, she received a call back from his daughter that the whole family was relieved to know that Paul was checking on their Dad every day and were “very grateful for meals on wheels.”

“It is all part of my day,” says Gia. “I view the daily check as important as getting the meals out the door. Not only do the people feel better knowing that someone is checking on them, but I have many families let me know that they are relieved to know that this check on their family member is part of the program.”

THE ROAD AHEAD FOR TRANSPORTATION

People in our rural area with no transportation often face hurdles the height of Mt. Monadnock. Even those with limited access to a ride can find getting to essential medical appointments, groceries, and prescriptions insurmountable. The Transportation programs at HCS have been helping people of all ages in Keene scale these hurdles with the Friendly Bus, City Express and Medical Express. This year a Passenger Advisory Committee (PAC) was formed to hear directly from riders. The PAC’s mission is not only to give voice to riders, but also to engage them as advocates for improving public transportation.

After requests were made for stops at four new locations, including Cheshire Medical Center’s new Family Medicine facility and Monadnock Family Services, the PAC was called into action. The group critiqued proposed changes to the route and gave staff valuable insight into the rider experience. The changes to the route and schedule debuted on January 2, and even during the cold winter days ridership increased. Said PAC member and regular rider Sandra McDonald, “I have lived in many places throughout my life and always used public transportation. Since moving to Keene, I use both the City Express and Friendly Bus. The service here – the dispatchers and drivers and everyone involved – is second to none. I can’t say enough good things about public transportation and how important it is to a community.”

HEALTHY STARTS A ROADMAP TO THRIVING

Sherry* is a young mom of three who came to Healthy Starts when her second child was just under a year old. Sherry was living with her mother after leaving an abusive relationship and unfortunately, Sherry and her mother did not get along. Sherry needed help to get back on her feet, and as a first step her Healthy Starts worker connected her to supports for mental health at a local mental health agency. Next the Healthy Starts worker ensured she had access to all the benefits that would help Sherry care for her children and gain independence, such as WIC and Food Stamps.

Sherry was a resilient survivor of domestic violence and she continued to co parent for the betterment of her son. She had no job and she was trying to maintain abstinence from substances. The Healthy Starts worker partnered with Sherry to support her. Many times, this simply meant listening to her story and pointing her in a direction.

Soon after approaching Healthy Starts, Sherry was thrown by a new medical diagnosis of Multiple Sclerosis, but she did not let it stop her from continuing to parent her children the best she could. Over time, Sherry had found employment, was managing her MS, had an improved relationship with her mother, and both had moved from an apartment to a home together. Sherry no longer needed us, but always knew Healthy Starts would be there for her if she needed us in the future.

A few years later after her initial case closed, Sherry became pregnant again and ensured that the hospital referred her to Healthy Starts. A connection was rekindled with the same Healthy Starts worker, and it felt like services had never stopped. Except this time, Sherry’s toolbox was full of all she learned from the previous episode of treatment with Healthy Starts. Everything that the worker suggested was readily adopted by Sherry. She moved from the home she was in, to an apartment in Keene without her mother. She was continuing to hold down a job and managing both her sobriety and physical health. She also was managing the continued stress from co-parenting.

Sherri’s Healthy Starts worker moved from a gentle guide to an active supporter when they found out that the new baby’s kidneys had a complication. Sherry’s Healthy Starts worker was able to connect Sherry with the Healthy Starts Nurse to oversee the remainder of the pregnancy and monitor her baby’s kidneys. His delivery was not without drama, both from the adults in his life and his own medical issues; however baby Michael arrived in this world and Sherri was over the moon with joy. She continues to provide the best and most consistent care for him and his two older siblings. Sherry tries to give credit to Healthy Starts but she was the one who found the strength to provide a stable life for her children.

UNDER CONSTRUCTION: HCS’S COMMITMENT

TO QUALITY AND EFFICIENCY

At HCS, our mission to provide exceptional patient care is always evolving to meet the needs of our community. This year, we have undertaken several significant changes to improve our services and ensure that every patient receives the highest quality of care. Among these changes are internal process refinements and a renewed focus on quality of life, all aimed at enhancing the overall patient experience.

A HOLISTIC APPROACH TO QUALITY OF LIFE

At the core of our patient care philosophy is a focus on enhancing quality of life. We are currently addressing a crucial aspect of our service delivery—ensuring patients receive the most appropriate level of care for their needs. When patients receive the most appropriate level of care, quality of life improves for both the patient and caregivers. We begin with a comprehensive referral and initial assessment process to identify and address each patient’s unique needs. This stage involves conversations with referral sources to evaluate requirements such as pain management, wound care, caregiver support, and safety concerns. In cases where patients are identified as candidates for hospice care, we ensure that they receive our highest levels of support.

COLLABORATIVE CARE AND INFORMED DECISION-MAKING

Our process continues with a collaborative approach to decision-making. Complex referrals are discussed in our daily intake collaboration meeting. This meeting brings together VNA and hospice managers with intake staff to review referrals that may require special attention. By examining referrals with “red flags” for palliative or hospice care, we can more effectively align our services with the patient’s goals. After these meetings, a manager reaches out to the patient and their family to explain the various services and programs available. This dialogue helps us determine which services would be the best fit and provide the best quality of life. Depending on their needs, patients may receive comprehensive care from our team, including services from registered nurses, social workers, physical and occupational therapists, home health aides, or palliative care from our specialized nurse practitioner.

ENHANCING CARE THROUGH CONTINUOUS IMPROVEMENT

Our commitment to continuous improvement extends to our utilization of expert consultants who help us refine our processes and workflows. This ongoing collaboration ensures that our patient care strategies remain current and effective, reflecting the latest best practices in healthcare. In conclusion, the changes we have implemented this year at HCS underscore our unwavering dedication to providing exceptional care and enhancing patient quality of life. We are continuously striving to meet the evolving needs of our community with compassion and excellence and these changes ensure that people will have a smoother passage on their healthcare journey when supported by HCS.

Working at her computer in the HCS Peterborough office, Nanci was suddenly alerted to bellows of laughter. Walking out to the Foot Clinic waiting room, she found two Foot Clinic patients and the caregiver for a third joined in animated conversation. As she reflected on such a joyful exchange, it was clear how much the patients and caregivers at the Foot Clinic look forward to the social aspect of their much needed medical care.

Many patients have been seeing their Foot Clinic nurse for years and have shared the joys and challenges of their lives. It’s not unusual for a nurse to walk a patient back to the waiting room after an appointment with a patient exclaiming “I just feel so good,” and “Thank you for taking such good care of me,” or “I’ll keep you posted on what happens next time I see you.”

The holistic aspect of care at HCS shines bright. The attention of the wellness nurses goes far beyond the feet. Nurses may recommend that moles be examined immediately by their dermatologist; that a sudden increase in weight is indeed a reason to contact their PCP, and yes diet is important when managing diabetes. This additional support builds trust, a sense of security, and connections within our community.

As she returned to her work, Nanci felt a deep sense of gratitude, realizing that it’s these shared moments made a world of difference in the lives of everyone at the Foot Clinic.

CELEBRATING THE IMPACT OF HOME SUPPORT SERVICES

At HCS, we are reminded daily of the profound difference we make in the lives of our clients. In Winchester, we had the pleasure of supporting Francine,* a delightful client who often felt lonely as she frequently transitioned in and out of skilled rehabilitation facilities. Despite her circumstances, she always radiated kindness and appreciation for her Home Support Providers, Cynthia and Miguel, often stating, “I love them,” when reminded when they would be coming to her home to assist her.

The work is so much more than light housekeeping. Throughout Francine’s journey, she found solace in the companionship offered by her Home Support Providers. Their support brought her comfort during a difficult time, reminding her that she was not alone on this road. Although Francine has since transitioned to long-term care, we know that her time with Cynthia and Miguel made her days at home more hopeful and pleasant.

This story serves as a powerful reminder of why our work matters. We are committed to providing care for the whole person—ensuring that every client feels valued and connected. Companionship organically flourishes and we celebrate the relationships built through our service as we support people on their journeys.

BARBARA R. DUCKETT

EDUCATION SCHOLARSHIP AWARD

The Barbara R. Duckett Scholarship was created to honor Barbara on her retirement as CEO of HCS. She remains deeply committed to this community and to helping it continue to be able to meet its healthcare needs with a steady cadre of educated and trained professionals. The Scholarship is designed to assist those already in the healthcare workforce to advance and elevate their skills and competence and to attract new caregivers to the field. Scholarships are awarded annually to staff who are pursuing certification, licensure, or advanced degrees in support of their work at HCS.

Barbara is personally aware of the significance of financial support in pursing educational goals. Upon graduating from high school, Barbara received multiple scholarships which allowed her to enter and complete her nursing education. Her understanding of the need to help and support others inspired the creation of the Scholarship Fund. Over the past fourteen years this scholarship fund has helped over 30 individuals at HCS begin, advance, or enhance their careers in healthcare. This year we are awarding eight more scholarships!

SUSAN G. ASHWORTH COMMITMENT AWARD

2023 - Dan Mastronunzio – The 2023 Susan G. Ashworth Commitment Award was presented to social worker Dan Mastronunzio. The award is given annually to the staff member who exemplifies the four pillars of love for HCS as an organization; a willingness to tackle difficult and complex work; forthright in conversation, and fair and calm in stressful situations. Presenting the award was Susan Ashworth and Evelyn Erb, last year’s award recipient. Evelyn, who nominated Dan, wrote: “Dan never flinches when asked to see clients in the most adverse situations or challenging family dynamics. He is fearless to respond to human need and always sees the person while never rendering judgement. He is always in a positive spirit to listen first and to ask questions to ensure understanding. It is one thing to have a tremendous knowledge base of their field; it is wholly a different skill set to demonstrate total respect for human dignity and to honor the right to self-direction while giving sound advice. There is no agenda with this person. Other than to be of service.”

2024 - Tina White, is the recipient of the 2024 Susan G. Ashworth Commitment Award. Tina, PT and Rehab Team Leader, was surprised to receive this award during the Annual Employee Recognition last month. Laura Brow, Director of Operations, wrote Tina’s nomination and the four VNA managers joined in to cosign! Tina has consistently demonstrated an unwavering commitment to providing the highest standard of care at HCS. Beyond her direct patient care responsibilities, Tina plays a crucial role in the Quality department, assisting with the quality review of patient records. Her meticulous attention and commitment to excellence ensure that our records are accurate, comprehensive, and up-to-date. In her role, Tina consistently takes on the most challenging and complex cases with a positive attitude and a determination to succeed. She engages in open, transparent conversations with patients, caregivers, and colleagues, ensuring everyone is well informed and on the same page. This forthrightness fosters trust and confidence, creating a supportive and collaborative environment. In the face of stressful and high-pressure situations, Tina remains fair, composed, and level-headed. Her calm demeanor and ability to handle crises with grace have made her a reliable and trusted resource within our team. Whether managing a sudden change, in a patient’s condition or addressing complex administrative challenges, she approaches each situation with a balanced and thoughtful perspective. She always has a smile on her face and remains focused and approachable when engaging in any situation.
