BOARD CHAIR AND CEO Dear Friends of HCS, A knock on the door from an HCS staff member offers the smile of hope, relief from worry, and the promise of quality care. That is one powerful knock on the door! You, as a vital member of our team, made those powerful knocks heard across our region this year.

Because of your generosity, Home Healthcare, Hospice, and Community Services provided an expanded level of caring, respectful service to our community.

In the following pages, you will see examples of new programs and improvements in operations that elevated our performance from the inside out.

Here are a few highlights: • Refreshed our Strategic Plan in January with a full board and senior leader retreat, deepening our commitment to serve as the regional leader for home health, hospice, and community services.

• Hospice Program expansion to care for people in every facility in the Monadnock region and service growth to 75% more families in 2023. Includes bedside musician program team expansion and local massage therapy program bringing massage opportunities to hospice families as well as patients.

• Launched the Hospice Enrichment Advancement Legacy (HEAL) program, designed to nurture the unique needs of a staff that continually encounters grief, stress, and trauma; launched new yoga classes, massages, paint nights, and group conversations.

• Converted all clinical programs and billing to a new electronic medical record —MatrixCare — creating streamlined processes and efficiencies across programs.

• Strengthened our commitment to diversity, equity, and inclusion (DEI) in collaboration with Franklin Pierce University’s chief diversity officer to examine DEI from the top down.

• Expanded our signature Butterfly Release event to three days and brought the event to multiple facilities where residents could not travel.

• Expanded our popular foot clinic and hired an additional clinic Registered Nurse.

• Added a Senior Pass program for the City Express to encourage independence through mobility.

• Provided support and resources to a record number of kinship caregivers through the Healthy Starts Kinship Navigator program. Thirty-nine caregivers are receiving support in navigating the world of parenting once again and keeping these children in a loving home.

COMMUNITY CONNECTIONS HCS swung open its doors to community collaborations this year throughout our service area. Collaboration and supporting our community are a tradition at HCS and this year these efforts were expanded and strengthened.

Local organizations are our strongest connections. HCS is a partner with the Monadnock Alliance for Families strengthening home visiting resources to at risk families and with the Monadnock Region Coordinating Council for Community Transportation shepherding resources to provide mobility and access for all ages. We regularly collaborate with Cheshire Medical Center to bring educational programs to the community and with the Mobile Food Pantry project at the Community Kitchen expanding access to fresh food and resources to rural communities.

HCS has reinvested in the local community by participating, collaborating and supporting events with the Greater Monadnock Collaborative Regional Chamber of Commerce, the Monadnock United Way campaign, and supporting the work of the Monadnock Diversity, Equity, Inclusion, and Belonging Coalition. The HCS Senior Dining program reopened weekly locations in Keene at the Senior Center and the Garden Café, and in Jaffrey. Once again, HCS is a Silver Sponsor for the Walk to End Alzheimer’s spotlighting this disease that is so prevalent in our community. HCS has also partnered with Rev. Jessica Johansson, End of Life Doula, to co-host quarterly Death Cafés in the HCS Keene location and is looking to expand to Peterborough.

Hospice at HCS has seen a significant increase in clients served and has met the need with tremendous success in partnership with many Assisted Living and Skilled Nursing Facilities in the region. Regular care coordination meetings with each facility allow our Hospice and Palliative Care certified staff to provide organized, individualized care for hospice patients and necessary education for facility staff on common end-of-life considerations. For appropriate situations, we are so happy to be able to offer massage and music which provides extraordinary comfort for our patients.

The HCS Butterfly Release expanded to three events this year, two at the Butterfly Garden in Keene and one with our event partner, the Cathedral of the Pines in Rindge. Each signature event featured live music from our Hospice Music Practitioners and a pop-up shop. Our Saturday Butterfly Release was geared toward families and those with children. We were joined by the Caterpillar Lab and Frisky Cow Gelato to make the day extra memorable.

To deepen our connections in the communities we serve, HCS will launch a Community Advisory Committee in FY24 to continue to strengthen and sustain our ties to our entire service area. The goal will be to engage partners with diverse experience to provide input on HCS projects, programs, and events and equip these partners to be advocates for HCS in their communities. OF CARE FULFILLED

At HCS we aspire to know our patients well, and to maintain warm connections to provide all possible support throughout the course of life. We feel especially privileged to have formed a strong connection with Wayne and Charlotte Crowell. Over the past several years, Wayne’s health management has included support from many of HCS’s comprehensive programs including the VNA’s dedicated RN, PT, and OT staff; use of the HCS Foot Clinic; and exploration of the Castle Center.

As Wayne’s situation and needs were changing, Palliative Support through the VNA was offered and accepted. A Palliative Nurse Practitioner provided consultative support, and a Palliative Social Worker guided the family through a series of Goals of Care and Advance Care Planning discussions. Wayne’s wife Charlotte recounted that those early discussions and opportunities to look ahead to changing needs and options allowed them to feel clearer and more comfortable about how Hospice at HCS could benefit them.

During a period this past summer at a skilled nursing facility, Wayne and Charlotte made the decision to move forward with Hospice care, and to move Wayne back home. In the days before he left the facility, Wayne and a roommate were treated to a private cello concert by a Hospice Music Practitioner.

With support from their daughters, Wayne and Charlotte found that for them, home is the best place for his Hospice support.

On the weekend before Wayne passed, his family lovingly surrounded him. And on the day of his passing, Charlotte and family were in turn surrounded by Hospice team members. For a man who served his country and gave so much to his community and his loved ones, HCS is profoundly honored to have served him in return.

HOME SUPPORT SERVICES Ann* has been a client of HCS Home Support Services since 2019 and has had Robin as a Home Support Provider since the first visit. Ann is “absolutely happy with everything” and “really, really loves Robin. She is just a morale booster.” Ann came to HCS through a Bureau of Elderly and Adult Services referral when she was living in a hotel room without transportation or much social support. When Ann moved into a lovely apartment, Robin started knocking on her door. Robin helps Ann with laundry, errands, and other household chores but most importantly to Ann, is “just there.”

When non-medical activities of daily living become too burdensome, Home Support Providers can assist older adults and individuals with disabilities in maintaining cleanliness in their homes and enhance their quality of Robin life. Each client has a personalized plan of care to address trouble areas so individuals can remain as independent as possible. Ann states that having Robin around makes her feel better and eases her mind when she gets scared or is down in the dumps. Ann maintains that interactions with HCS are always “a bright object of the day.” We are fortunate to have Ann as a client and look forward to supporting Ann for as long as she needs and accepts our support.

WELLNESS FOOT CLINIC EXPANSION The retirement of two local podiatrists sent hundreds of new clients to the HCS Foot Clinics.

“We went from getting a referral or two a week to four to five referrals a day,” says Laura Brow, Director of Clinical Operations. The program has grown 75% in the past year and shows no signs of slowing. HCS has met this need by hiring another dedicated Foot Clinic nurse and increasing the number of available appointments during the week.

The services provided at the HCS Foot Clinics are an integral part of healthcare for older adults and go far beyond clipping toenails and are eyes and ears into one of our community’s most vulnerable populations. Diagnoses like diabetes or circulatory problems that can cause painful conditions in the feet and greatly impact a person’s quality of life and ability to care for themselves.

Joe* walked in unsteadily for his regularly scheduled Foot Clinic appointment. The nurse noticed immediately that Joe’s pants were stretched tight over his legs and the skin underneath was swollen, red, and weeping through the fabric. After a quick assessment and with Joe’s permission, she

contacted our local Emergency Department and Joe’s daughter. Joe was diagnosed at the ED with cellulitis and was able to start lifesaving treatment immediately. Joe continues with his foot clinic appointments with a renewed sense of gratitude and of course “happy feet”.

Issues with feet can impact a person’s mobility and increase the risk of falls which can lead to further and more serious health complications. Regular foot care can improve a senior’s overall health and prevent complications to existing conditions. The HCS Foot Clinics are staffed by Registered Nurses who can identify issues early before they become more serious problems.

Fred* limped into the Foot Clinic Office looking disheveled and seeking help as he was unable to get his shoes on. After his appointment, Fred zealously followed all the nurse’s recommendations and by his second appointment, Fred was unrecognizable.

Bright eyed, groomed, and with a smile on his face, he proudly revealed how great he felt and how much more he has been able to do for himself each day now that his feet had been cared for.

TRANSPORTATION “This is the best service. Helps me so I don’t have to get family members from work.” Friendly Bus Rider, age 81 “Helps me to live well without my car for transport.” Friendly Bus Rider, age 79 “Wonderful service- I could not live comfortably in Keene without the buses.” Friendly Bus Rider, age 85 The Friendly Bus has been a fixture rolling through the streets of Keene from door to door for over 50 years. From the first Dodge van to the fully accessible Friendly Bus on the road today, the service has given mobility and independence to hundreds of older adults in the City of Keene and beyond.

The Friendly Bus is now just one of the transportation services operated by HCS. A fleet of vehicles provides access to medical care, shopping, and social services for people of all ages through the City Express public transportation, and Medical Express to the Veterans Administration facility in White River Junction, Vermont and to Dartmouth Hitchcock in Lebanon, NH. This year, additional funding for food equity provided weekly shopping trips in Keene, West Swanzey and Winchester. A Senior Pass program for rides on the City Express is being implemented, allowing older adults greater flexibility and choice for rides, and independence!

Food insecurity and the need for nourishment are just some of the reasons why Meals on Wheels is so important to older adults and individuals with disabilities in our area. Nourishment of the soul, with the daily “hello” and safety check by the delivery driver, is another key aspect of the program. Seventy percent of our Meals on Wheels participants live alone – for these people, the delivery driver is often the only person that breaks a day of loneliness and isolation.

Volunteers are the unsung heroes often delivering these meals, particularly in the very rural towns where HCS Meals on Wheels are delivered. There are sixty “good neighbors” on our volunteer roster, a very dedicated group who knocks on doors and delivers on good weather days and bad, always bringing sunshine along with the meals. As one volunteer said while encouraging others in her community to volunteer, “This is a great opportunity to get involved in a volunteer program that doesn’t require a big commitment of time but means so much to the community.”

The power of the knock on the door is tremendous. “[I] want to praise the volunteer who delivered my meal in the nine inches of snow in Jaffrey on that Friday. Didn’t expect a meal that day. There he was with my meal. Nice volunteer went the extra mile.” Jaffrey Meals on Wheels participant

EDUCATION SCHOLARSHIP AWARD

BARBARA R. DUCKETT

The Barbara R. Duckett Scholarship is celebrating ten years. This scholarship was created to honor Barbara on her retirement as CEO of HCS. She remains deeply committed to this community and to helping it continue to be able to meet its healthcare needs with a steady cadre of educated and trained professionals. The Scholarship is designed to assist those already in the healthcare workforce to advance and elevate their skills and competence and to attract new caregivers to the field. Scholarships are awarded annually to staff who are pursuing certification, licensure, or advanced degrees in support of their work at HCS. Barbara is personally aware of the significance of financial support in pursing educational goals. Upon graduating from high school, Barbara received multiple scholarships which allowed her to enter and complete her nursing education. Her understanding of the need to help and support others inspired the creation of the Scholarship Fund.

HONORING OUR OWN 2023 TRENDSETTER – VANESSA SHEEHAN Associate Director of Hospice, Vanessa Sheehan, was honored with a 2023 Trendsetter award. The Business Journal of Greater Keene, Brattleboro and Peterborough and Keene Young Professionals Network (KYPN) created this award in partnership with the Keene Sentinel to honor young professionals who are making a difference in the Monadnock region.

In line with her collaborative leadership style in caring for patients, Vanessa was nominated for this award by two colleagues. In her nomination, Supervisor for Palliative and Hospice Social Work and Support Programs Evelyn Erb noted, “Her integrity and authenticity shines, and her passion for Hospice and Palliative Care truly inspires. She is an ultimate trailblazer in every category, but most importantly she draws in and supports others to join her on the trail as her emphasis is always on ‘the team’.” Congratulations, Vanessa!

2023 OLDER ADULT VOLUNTEER AWARD – ALLEN MENDELSON Allen Mendelson recently won the 2023 Older Adult Volunteer Award presented by the NH State Commission on Aging. Allen is a long-time advocate for services for older adults in the Monadnock Region and served on the Board of Directors for HCS during a period of transition for the agency.

He has been involved in all aspects of HCS’s work, including relaunching our philanthropy efforts.

Allen has been responsible for planning and implementing educational programs on financial, funeral, and estate planning for the community and he was instrumental in reviving the Circle of Hope, a planned giving group for legacy gifts through estate planning. Allen has always led by giving his time, talent, and generosity to HCS and many other organizations in the community.

Allen is “more than you can imagine” and words don’t do justice to his immense contributions to HCS. Congratulations, Allen!

HEALTHY STARTS Resiliency is used to describe the work of the Healthy Starts at HCS program. More colloquially, Healthy Starts helps families prepare for and weather the inevitable storms that life brings.

Sometimes those storms are brief, just passing showers that temporarily interrupt our usual patterns, but we deploy our resources to get us back to where we were or help us adjust to the new growth the rain brought. Unfortunately, sometimes the storms make landfall in our lives as a hurricane, sweeping away our ability to buy groceries and diapers, repair the car, have a roof over our heads, or navigate a loved one’s mental health needs. We are then left wondering if we can find the resilience to hang on until the clouds break without the resources we need.

The Healthy Starts team of social workers, family support workers, and nurses build a storm shelter for each family that is unique to their opportunities and strengths. Last year, nearly 300 local children and their families received guidance, educational parenting opportunities, and community connections. Healthy Starts skillfully tethers the families to resources that ground them through the winds of unemployment, shades them from the housing deserts, and buoys them as the rising waters of grocery prices and utility bills threaten to break their family’s financial and emotional levies.

Healthy Starts offers opportunities to families so they may learn better how to find and foster their resiliency when they are caught in the storms, which increases the resiliency of the entirety of southwestern New Hampshire.

HEALTHY STARTS

The Dahl family, mom, dad, two “tweens” and a new baby due, were like many other families who find their way to the Healthy Starts Family Resource Center. The family became unhoused in this tight housing market, and with a child on the way, desperately needed help with their housing search.

Healthy Starts social workers and family support workers guide families through housing applications and accessing resources for food, medical and mental health care. They partner with families to learn about developmental milestones and fostering emotional awareness.

The Dahl family was featured in this Keene Sentinel Photo Essay by Hannah Schroeder in 2022 as the family began to work with the Healthy Starts team. Now, Mae, the happy, healthy, newest member of the Dahl family, is already crawling around their new apartment, getting into mischief, and bringing great joy to the family.

COMMITMENT AWARD Congratulations to Evelyn Erb, Supervisor of Palliative and Hospice Social Work and Support Programs, who was the 2022 winner of the Susan G. Ashworth Award for Commitment. The award is based upon written nominations that strongly reflect how the nominee meets four characteristics of Susan Ashworth’s 45 years of service: the person’s actions reflect a love of HCS as a service organization, a willingness to tackle difficult and complex work, forthright conversation, and equanimity in stressful situations.

Evelyn demonstrates total admiration for all the services HCS provides in her communications with patients, families, and staff. Unceasingly calm in stressful situations, Evelyn handles difficult conversations with a determined quietness.

She is genuine, approachable, and is instrumental in educating our patients, staff, and referral sources regarding HCS services. She was a leader in helping to institute the Hoffman Grant, which ensures all patients in our community have access to Palliative Social Work services, even if their insurance does not cover it.

Evelyn tackles challenging work head on, with emphasis on kindness and clarity. She never shies away from a difficult conversation with patients, families, or staff and takes a team approach to problem solving. Evelyn is an effective communicator, gentle in her approach but driven toward solutions. Families and staff trust her, and using her unique approach, she has been instrumental in increasing access to Palliative and Hospice services to many in need.

THE AYLENE WOZMAK

HOSPICE ENRICHMENT ADVANCEMENT LEGACY (HEAL) PROGRAM The Aylene Wozmak Hospice Enrichment Advancement Legacy (HEAL) program was created in 2022 to nurture and support organized programs for Hospice staff to enrich their continued work that focuses on the mind, body, and spirit. Being at the bedside with patients and families dealing with strong emotions and fears of dying is psychologically draining for even the most veteran of Hospice staff. It takes fortitude to continually listen compassionately and give of themselves to provide the best possible experience for the patient and their family.

The HEAL Program provides opportunities to restore, comfort, and support staff providing this essential care. In the first year, the HEAL Program has already provided massage, yoga, facials, and paint nights. There are plans for staff retreats, education in alternative therapies like Reiki, meditation, acupuncture, and ongoing facilitated support groups.
