LETTER FROM OUR

BOARD CHAIR AND CEO Dear Friends of HCS, As the global pandemic persisted, HCS innovated. Throughout the nationwide nursing shortage and chaos created by the pandemic, HCS listened to our community and recognized emerging needs.

We heard about the acute need for support for those who were committed to caring for loved ones with dementia 24/7 in their homes. In response, HCS created HCS Memory Care at Home. We heard that a record number of grandparents are caring for their children’s children and needed resources.

HCS responded with the Kinship Navigation program in Healthy Starts. We heard that Covid was creating extra challenges for seniors to receive appropriate nutrition. HCS responded by serving a much larger number of people Meals on Wheels and added a new volunteer meal delivery service in Richmond. We recognized from a recent Press Ganey employee survey that diversity was a priority among our staff. HCS responded by creating a Diversity, Equity, and Inclusion Advisory Committee to make this a priority.

Preparing this report gave us an opportunity to reflect on what we made possible in 2021 and to consider once again how special this agency is to those we serve and our employees. We are so grateful for all the opportunities that have come this past year and we look forward to hearing your feedback.

Our Annual Report highlights selected stories, metrics, and names that have been and continue to be impactful, but it cannot come close to telling you about everything we can do for you should you ever need us.

Perhaps your greatest contribution to HCS could be to “get the word out” after you have reviewed this report and pass it on to someone you think could benefit. Or perhaps you will consider joining our employee or volunteer work groups and find your own joy in serving here.

CARING & INNOVATION

DURING THE PANDEMIC KINSHIP NAVIGATOR: Healthy Starts received a referral about a situation where two elementary aged

children were struggling in school. Their grandmother had recently gained custody when their parents were incarcerated. The grandmother had no phone, no internet, no car, and remote learning was just beginning for the school system. Enter Rachel Kowalsky, Healthy Starts social worker and Brittany Daley, Healthy Starts Kinship specialist. They connected with the grandmother and were able to get her a telephone and internet in the home and connected with the school to obtain the devices that the children needed to learn from home.

With no car, grandmother was walking to the local convenience store and spending all of her food stamps on what food she could obtain there and carry home. The grandmother’s mobility issues made the trip to the convenience store expensive and physically taxing. Other HCS resources were engaged for a short-term fix – Meals on Wheels were delivered for the grandmother and the school provided lunches for the children.

Healthy Starts social workers collaborated with the Bureau of Elderly and Adult Services to obtain a new walker for this grandmother and found her a ride to the Walmart nearby once a week for groceries. The situation is now stable, and the children are back to school. The grandmother knows that Healthy Starts is only a phone call away if she needs support or resources.

HCS MEMORY CARE AT HOME: When the Castle Center closed due to the pandemic, the loss of

these support services was felt acutely by both caregivers and participants. HCS responded by launching an entirely new program – Memory Care at Home - to meet the needs of those with diagnosed dementia diseases and their caregivers. The program is already going strong. A nurse certified in dementia care works with caregivers to develop a plan of care that will be engaging, stimulating, and enjoyable for the person with dementia. LNAs provide personal care and meaningful activities to engage the person with dementia during a weekly three-hour block of time, while the caregiver takes a break to do errands, have lunch with friends, or just have some restful time. All staff in the Castle Center and Memory Care at Home have received special training in Dementia Care provided by the Home Care, Hospice and Palliative Care Alliance of New Hampshire.

This training was created by experts in dementia and behavioral health care in home-based settings. It includes webinars, supplemental education, quizzes, certificates and tracking sheets and is designed to comply with New Hampshire’s training requirements for NH RSA 151. A happy caregiver stated: “I am over the moon excited; in the past 14 months I have had to be here 24/7 caring for my husband. Now I have three hours to see friends, get a massage and do things for me!” Special thanks to 100 Women Who Care for providing start-up funding for this program!

CARING AND COMMUNITY CASTLE CENTER: At long last, the Castle Center re-opened in mid-August. Participants engage in daily exercises for their body and mind, help plan and make decorations for monthly events, take bus tours around Keene, and so much more with Kendra Pahl, our new Activities Coordinator. Individual and group activities provide meaning and value for each participant while dementia care trained nurses and LNAs provide personal care and monitor participant’s health and wellbeing. Vaccines are required for all participants, staff, and volunteers, and enhanced cleaning routines help keep participants and their families feeling safe. “Mom comes home tired and happy from her days at the Castle Center.

It gives me more peace of mind than I could have imagined.”

FOOT CLINICS: Mary* visits

the Peterborough Foot Clinic every eight weeks to receive specialized foot care to manage issues that are caused by her diabetes. Over 700 patients are regularly seen by the skilled Registered Nurses at the Keene and Peterborough clinics who provide care, teach about good foot health, and help patients avoid more serious issues. Mary recently donated to HCS saying, “I greatly appreciate the helpful and friendly service HCS provides. I hope my donation may help someone else afford this valuable service.”

NUTRITION: Just when MOW had record expansion due to Covid and vulnerable seniors, a new service

area was added. In Richmond, NH, a group of people saw that seniors were not getting the nutritional support they needed. They reached out to HCS and, working together, we were able to create a new volunteer delivery program that provides over 120 meals to seniors in Richmond every month.

HEALTHY STARTS: C,

young mother with MS, learned how to emotionally protect herself and her child with the support of the Healthy Starts program. “Healthy Starts helped her recognize her strengths, reminded her that she has value, and that she can break the cycle of abuse.” says Penny Vaine, the Healthy Starts Program Manager.

“C” says that Healthy Starts social workers were always encouraging her when she felt defeated. She now has the strength and emotional bandwidth to set clear boundaries to protect her child and herself from familial abuse.

TRANSPORTATION: Mark* rides the City Express to the

grocery store every week. HCS created a special shopping route so that Mark and his friends could get to Market Basket to participate in the seniors only shopping hour. He is so thankful for the HCS bus drivers that were willing to start their days early to keep him safe.

Shout out to our HCS drivers who never missed a beat and provided safety and kindness under extraordinary circumstances.

HOMEMAKING: Thelma* has a persistent hording disorder and began receiving Home Support services from HCS last fall.

A Home Support Provider, Jodene, worked closely with Thelma to help her throw away or donate many items and create safe access to many rooms in Thelma’s home. A year ago, Thelma could hardly open her front door. Now, Thelma now has room for her TV and stereo system in her living room and loves to dance! �5

CARING ONE BY ONE #1 HOME HEALTHCARE AGENCY 10 YEARS IN A ROW*:

Barbara Rockwell has always been someone who has given back to her community. In her rural town she has been the key person reinvigorating Old Home Day, starting up the local Friendly Meal dining for seniors program and getting a food pantry going. Sitting still is not her style.

But when she fell and broke her shoulder recently, she had to rely on others, especially the clinical staff at HCS for help. Her recovery has been complicated by other health conditions, but Susan Hayes RN has been there to troubleshoot, calling the doctor when necessary and heading off complications. With her arm strapped tightly to her body to immobilize it, Chris Persson and his years of occupational therapy experience helped her to find ways to safely move around her house, and even showed her new ways to shampoo her hair. “Everyone from HCS has been so good to me,” says Barbara. “They have explained what they are doing and why. I appreciate them so much. They have gone above and beyond.”

CARING ONE BY ONE PALLIATIVE CARE: For at least a decade, Frank* was served on and off by HCS Home Health services,

Meals on Wheels, and Homemaking, and he took great comfort in being in his home. Frank was in his 90s, in fragile health, and lived alone in terribly poor home conditions. Though he had no relatives nearby, Frank had incredible neighborhood support and care from several families and HCS services. An HCS social worker was instrumental in ensuring Frank achieved Long Term Medicaid so that he would have CFI Case Management and the ability to move to a facility if ever he chose to leave his home. His needs remained palliative, and he did not qualify for Hospice. He had the highest quality of compassionate and professional care from his devoted Home Health Nurse, who pulled in a Palliative Care Social Worker as well this past year. Frank was utterly clear his greatest wish was to stay in his home regardless of what others thought. HCS, his neighbors, and a CFI Case Manager together ensured that dream. He always emphasized: ‘I have everything I need’.

HOSPICE: Recently, a long-married couple were frightened of the

word “hospice” and the reality that their “forever” was coming to an end.

They knew they needed help but weren’t sure what to expect. They took the leap, and the wife was admitted to Hospice at HCS. After his wife passed, the husband wrote his deepest appreciation with a trembling hand to the amazing team who provided support and comfort to him and his wife in her final months of life. “I needed all of it and didn’t know what was available with hospice. The word scared me. I thought a Social Worker was for troubled families... I was mad at God, and rejected the chaplain, and then requested him. I called late at night panicking and was guided through my fears until I was exhausted and comforted at the same time.” HCS was able to surround this couple with support and is still supporting the husband with bereavement services.

WALK-IN WEDNESDAY: At a recent Walk-in Wednesday in

Charlestown, Susan Ashworth opened the door to find a delightful woman standing on the top step with her walker. She was a veteran and wanted to inquire about transportation to get to the VA in the winter when she could not drive. She also needed someone to do a bit of light housekeeping. Susan felt privileged to have a wonderful conversation with her about her time in the service and to help her connect with resources at HCS and beyond that might be helpful to her.

EDUCATION SCHOLARSHIP AWARD

BARBARA R. DUCKETT

The Barbara R. Duckett, RN, Scholarship Fund was created in November of 2013 to honor Barbara on her retirement as CEO of HCS. Barbara served HCS in the leadership position for thirteen years. She was and remains deeply committed to this community and to helping it continue to be able to meet its healthcare needs with a steady cadre of educated and trained professionals. The Scholarship is designed to assist those already in the healthcare workforce to advance and elevate their skills and competence and to attract new caregivers to the field.

Scholarships are awarded annually to staff who are pursuing certification, licensure or advanced degrees in any of the healthcare fields - nursing, physical therapy, occupational therapy, nursing assistants, or healthcare administration.

Barbara is personally aware of the significance of financial support in pursing educational goals. Upon graduating from high school, out of necessity, Barbara applied for and L to R, Barbara Duckett, Vanessa Sheehan, Katie Hart, Jaime Donovan received multiple scholarships which allowed her to enter and complete her nursing education. Her understanding of the need to help and support others inspired the creation of the Scholarship Fund.

On April 16, 2020, the Organization received a loan in the amount of $1,496,000 pursuant to the Paycheck Protection Program (PPP), a program implemented by the U.S. Small Business Administration (SBA) under the CARES Act and the PPPHCE Act. The principal amount of the PPP was subject to forgiveness, upon the Organization's request, to the extent that the proceeds were used to pay qualifying expenditures, including payroll costs, rent and utilities, incurred by the Organization during a specific covered period. The Organization followed the conditional contribution model to account for the PPP and recognized the full amount of the PPP as revenue after receiving notification of full forgiveness in June 2021.
