LETTER FROM OUR BOARD CHAIR AND CEO Dear Friends of HCS, This annual report briefly summarizes HCS’ experience of 2020. We hope that you will come to know HCS better through these pages and before you ever need our services. We trust that as you catch a glimpse of our organization’s caring through adversity, you will realize HCS is more than you can imagine.

The year “Twenty-Twenty” began with promise in its very name. Clear vision and balance. The HCS board and senior leaders had just crafted a fresh Strategic Plan and vowed that it would not sit neatly on a shelf but would be a work plan for the next two years. Little did we realize how useful that plan would be in dealing with the COVID 19 Pandemic.

How could we protect our patients and staff from the approaching virus? Our strategic plan prioritized workforce investment. Focusing on that aim, HCS acquired full personal protective equipment for every clinician even if visiting in the absence of symptoms; equipped all office staff for remote work; converted to telehealth home visits for families caring for children and families attending to loved ones at end of life; and allocated federal relief funds to supplement the wages of all staff during the pandemic peak.

As of this report, no HCS employee has tested positive nor become ill with the COVID 19 virus despite serving more than 600 patients and families in their southern New Hampshire homes each day.

Our Strategic Plan called for growth. How were we to grow when our referring health facilities were in “lockdown” and when patients actually telephoned us to cancel visits due to fear of viral contagion?

Our Strategic Plan pointed to investing in infrastructure. Focusing on that aim, we sought grant dollars to increase telemonitoring; converted congregate dining to an expanded 500 delivered meals per day; deep cleaned busses and provided PPE to our drivers to add early morning grocery runs for vulnerable elders; and expanded nursing services to include drive-through consultations.

These are a few examples of how our Strategic Plan has charted our course and avoided distraction from our mission to serve. We have all been tested by the events of 2020 and as we look back, we do so with gratitude for our board, our leaders, our amazing staff, and you, our committed supporters.

BARBARA R. DUCKETT EDUCATION SCHOLARSHIP AWARD The Barbara R. Duckett, RN, Scholarship Fund was created in November of 2013 to honor Barbara on her retirement as CEO of HCS. Barbara served HCS in the leadership position for thirteen years. She was and remains deeply committed to this community and to helping it continue to be able to meet its healthcare needs with a steady cadre of educated and trained professionals. The Scholarship is designed to assist those already in the healthcare workforce to advance and elevate their skills and competence and to attract new caregivers to the field. Scholarships are awarded annually to staff who are pursuing certification, licensure or advanced degrees in any of the healthcare fields - nursing, physical therapy, occupational therapy, nursing assistants, or healthcare administration.

Barbara is personally aware of the significance of financial support in pursing educational goals. Upon graduating from high school, out of necessity, Barbara applied for and received multiple scholarships which allowed her to enter and complete her nursing education. Her understanding of the need to help and support others inspired the creation of the Scholarship Fund.

THOSE WHO NEEDED US IN 2020 Healthy Starts Melissa was a heavy heroin user and five months pregnant with her fourth child when she met HCS Social Worker and Family Resource Specialist, Amy Delisle, in the Mothers in Recovery Program. Melissa had temporarily lost custody of her older three children due to her ongoing drug abuse.

Mothers in Recovery is a program wherein HCS works collaboratively with the Cheshire Medical Center to help pregnant and parenting women receive the medical intervention they need to overcome the physical effects of drug abuse and to provide ongoing social and emotional support to sustain recovery.

Melissa

While Melissa’s medical provider adjusted her dosage of Suboxone, a medication used to treat opiate addition, Amy and Melissa met weekly at a group gathering of Mothers in Recovery and Amy made additional home visits to personally support Melissa. Amy became someone who Melissa turned to for a pep talk and guidance when facing criticism from doubtful onlookers who publicly questioned Melissa’s chances of overcoming addiction. Melissa missed her children and needed a map to reach her goal of family togetherness.

Amy skillfully outlined the territory.

Melissa’s baby was born healthy. To start anew, Melissa took parenting classes, learning to encourage her children in their schoolwork. She was able to rediscover her passion as a fan of race car driving and is enjoying her own work as an auto mechanic. Melissa learned to be a strong advocate for her family and has secured reliable daycare – a necessity now that she has worked at a steady job for a year.

Finally, having demonstrated her commitment to the ongoing labor of recovery, Melissa attained full custody of her three older children. She permitted use of her real name for this story in hopes of encouraging others struggling with addiction to seek help and to be more than they can imagine.

Palliative Care

Throughout the decades of their life together, Ramona and Dick acted as two halves of a whole. Their partnership grew ever deeper as they shared the caregiving role for each other through a range of health challenges. Whether Dick was caring for Mona, or she was supporting him, they were in turn supported by HCS visiting nurse services at different times. When Dick became a Hospice patient at the beginning of this year, Ramona was there for him in her role of devoted caregiver. Losing her beloved spouse at the dawning of a global pandemic made for unimaginable worries and stress, and Mona’s health suffered.

HCS walked with Mona through the Hospice caregiving process and remained closely tied through bereavement support from the Hospice team in the wake of Dick’s death. Mona’s devoted Meals on Wheels drivers provided sustenance and daily check-ins. As her health challenges grew more complex, Mona became eligible for Palliative Social Work support through HCS’s “Hoffman Fund”. This grant funding, provided by the New Hampshire Charitable Foundation’s Kathy Hoffman Follo Memorial Leukemia Fund, allows HCS Palliative Social Worker, Evelyn Erb, to assist patients at difficult crossroads when their insurance does not otherwise cover skilled Social Work interventions.

During a time of extreme social isolation from March to August 2020, Mona and Evelyn worked to identify Mona’s needs and seek solutions. Often this work occurred over the phone to ensure safe social distancing. Palliative Social Work made the critical difference to provide emotional support, recruit community volunteers, apply for long term care benefits, obtain Homemaking services - also through HCS - and ensure that Mona was matched with a Case Manager for the State of New Hampshire’s Choices for Independence Program. Continuity of caregiving from wellknown HCS professionals created a partnering relationship with Mona. Our staff feel privileged to work alongside her as she navigates new situations, fortified with more resources than she would have imagined.

THOSE WHO NEEDED US IN 2020 Hospice Care When Lee’s husband, Bruce, a former Marine who built their home in Fitzwilliam became short of breath and weaker following a hospitalization, she knew as a long time nurse, that she was going to need more help in the home as well as help in paying for additional medical supplies if she was to continue to work full time and care for twin great grandsons.

Bruce experienced two hospitalizations for pneumonia in short succession and had taken to staying in bed with worsening COPD. At the suggestion of hospital case managers, Lee called Hospice at HCS and soon had a hospital bed and oxygen delivered. She scheduled Licensed Home Health Aide assistance from HCS to bathe Bruce and provide lunch Monday through Friday. Hospice also arranged for a volunteer visitor to keep company and conversation with Bruce weekly and the two men enjoyed exchanging their stories for months.

These events occurred more than two years ago. Hospice is still appropriate for Bruce in his frail state and Bruce pushes on, determined to “make it to 90” years old. More time than Lee could have imagined.

Lee describes Bruce as a solitary man who has always been comfortable living a quiet, private life; but a highlight of Bruce’s care came one day in a special “We Honor Veterans” celebration in his home. HCS Hospice team members arrived during a planned visit to recognize Bruce’s Korean War military service with music, a pinning and plaque that commemorated Bruce’s Marine Corps service, then topped the occasion off by serving cake to Bruce, Lee, and their twin great grandsons who still remember “the party.”

Today, Bruce’s Hospice nurse visits twice weekly to partner in his ongoing hospice care with Lee. COVID 19 brought an end to the weekly volunteer visits but Lee, who is newly retired, is able to spend quality time with Bruce thanks to the physical, emotional, and financial support of HCS hospice.

STATE OF NEW HAMPSHIRE LNA OF THE YEAR The Home Care, Hospice and Palliative Care Alliance of New Hampshire has awarded Ruth Orkins, Licensed Nursing Assistant at HCS, the prestigious honor of LNA of the Year. The statewide award recognizes an LNA who offers excellence in patient care and is a superb communicator with patients, families, and other staff. Ruth has been committed to a career as an LNA with HCS for 29 years and is a role model for fellow professional LNAs. LNA supervisor Mary Lucas, RN, nominated Ruth for the award writing, “Ruth is an incredible woman. She has dedicated her life to the service of many with courage, strength, and wisdom. Clients and staff alike are always treated with dignity and respect at every visit. Ruth’s constant calm demeanor and great rapport with her clients leaves families comfortable knowing that they can trust her with their loved ones.” As an LNA, Ruth provides personal care to assist patients to recover and remain safely at home. She is part of the Hospice at HCS care team supporting patients and their families at the end of life. Her clients are always treated with dignity and respect at every visit. Ruth’s constant calm demeanor and great rapport with her clients leaves families comfortable knowing that they can trust her with their loved ones.

The Home Care, Hospice and Palliative Alliance of New Hampshire awards the LNA of the Year annually from nominations from throughout the state. The organization promotes high quality services for individuals at home with the goal of helping people maintain or restore their health and well-being. There are more than 40 member agencies.
