igarette smoking is the leading preventable cause of death. Quitting smoking is associated with many health benefits. Earlier this year the United States Preventive Services Task Force (USPSTF) updated the recommendations on screening, counseling and treatment. The USPSTF recommends that clinicians ask all adults about tobacco use, advise them to stop using tobacco, and provide behavioral interventions and U.S. Food and Drug Administration (FDA)-approved pharmacotherapy for cessation to nonpregnant adults who use tobacco. A similar recommendation was issued for all pregnant persons.

Why should I quit smoking?

A very important reason to quit is that quitting smoking can give you and those around you — such as your family, friends and pets — a better quality of life.

The problems smoking can cause include cardiovascular disease; many different types of cancers; lung disease, such as emphysema; bone loss leading to osteoporosis; and increased risk for broken bones, stomach ulcers, eye diseases — such as cataracts and vision loss — teeth and gum disease, and complications after surgery. Smoking is also associated with an increased risk in developing type 2 diabetes mellitus.

The benefits of quitting are many.

When you stop smoking, you’ll be able to breathe better, your taste and smell should improve, you’ll sleep better, you’ll have more energy and you’ll increase your lifespan. You will be amazed at how quickly your body begins to heal itself — the blood pressure will lower, blood flow will improve, and your chances of a heart attack will decrease.

What is the smoke doing to my body?

Smoking cigarettes exposes a person to more than 6,000 chemicals, many of which are toxic. The levels of toxins can vary depending on several factors, which include but are not limited to the amount of smoke inhaled, the type of cigarette smoked and how the cigarette is smoked. When

a cigarette is lit, the chemicals are released and deposited in the mouth, throat, lungs and bloodstream. Some of these toxins are called nicotine, aldehydes, amines, hydrocarbons and heavy metals. These toxins have been shown to cause damage to human genetic material, cause inflammation, and destroy cells in the body, resulting in disease and cancer.

Secondhand smoke is the smoke a non-smoker breathes that is generated by a burning cigarette or exhaled from a smoker. There are just as many toxins in secondhand smoke, which can put a non-smoker at risk for disease and cancer.

Imagine how damaging this can be for an infant or child who is exposed to secondhand smoke. Infants exposed to secondhand smoke have a higher risk of sudden infant death syndrome (also known as SIDS) and more ear and lung infections.

Why is it so hard to quit smoking?

It isn’t easy to quit. For most people it takes at least seven quit attempts to be successful. Nicotine addiction is the main reason why it is so hard to quit. The effect of nicotine is very quick, within 10 seconds of inhaling. When nicotine enters the body it causes a surge of endorphins (chemicals produced by the body to relieve stress and pain).

This causes euphoria or a “high” that can increase the reward centers of the brain. This cycle reinforces the behavior to keep smoking.

When the body becomes used to nicotine it becomes dependent, and a smoker will need to keep smoking to maintain pleasure and prevent (continued on Page 4)

withdrawal symptoms. Withdrawal symptoms include irritability, anxiety, restlessness, difficulty concentrating, depressed mood, insomnia and possibly increased appetite. The symptoms usually decrease after three to four weeks, but the cravings for a cigarette can last a lot longer.

How do I quit smoking?

Ask yourself: What would you look forward to if you quit? This is important to understand and can help you focus on quitting.

A great online resource that explains common thoughts and concerns about quitting is the Emmi program Thinking About Quitting Smoking: https:// www.my-emmi.com/SelfReg/ YHSMOKE. Ready to quit? That’s great news! Studies have shown that behavioral counseling is effective. Counseling can be in the form of advice, counseling sessions with a specialist or with a group, participation in a smoking cessation program, or even by telephone. Studies support the use of medical treatment with nicotine replacement therapy — such as nicotine patch, gum, inhaler or tablets — as effective with helping people quit smoking for 6 months or more. Medications such as antidepressants (varenicline or buproprion) are also effective.

In fact, combining behavioral counseling with medical treatment could be even more effective. Be sure to talk to your health care provider and share the great news

with your family, friends and co-workers — you will find lots of support!

What if I switch to e-cigarettes?

Less toxins have been found in e-cigarettes. However, there is no long-term evidence to support that smoking e-cigarettes is healthier or less dangerous. There is some new knowledge suggesting that using both cigarettes and e-cigarettes might increase the level of toxins in the body. The use of e-cigarettes cannot be recommended.

What Is The Difference Between Medicare and Medicaid?

edicare and Medicaid are two different government programs for healthcare. It is important to understand the difference between them. Here, we will discuss how the program benefits differ and how eligibility for each program is established.

Medicare vs. Medicaid – What are the program benefits?

Medicare is a program administered by the federal government to provide healthcare to certain populations.

Original Medicare is divided into Parts A and B. Medicare Part A covers hospital care and a limited period of nursing home care, home health services, and hospice care. Medicare Part A will only cover nursing home care if – 1. There was first a qualifying hospital stay of 3 days of inpatient care; and 2. Nursing home care was needed relating to the hospital stay; and 3. The patient entered the nursing home within a short time of the hospital stay (usually within 30 days).

Thereafter, only the first 20 days of nursing home care are paid for by Medicare Part A. Days 21 through 100 of care require a partial payment by the patient. Any care after 100 days is not paid at all by Medicare Part A. Medicare Part B covers traditional healthcare expenses, such as visits to a doctor, blood tests, and X-rays. In most cases, a referral is not needed to see a specialist. Original Medicare does not cover prescription drug coverage; however, you can enroll in Medicare Part D through a private insurance company with paid premiums.

Medicaid is also a program intended to provide medical benefits to certain populations. It is a joint federal-state

program. While states receive federal funding and must follow specific federal rules, each state administers its own Medicaid program. Medicaid covers all types of medical care, including long-term care, such as a nursing home. However, eligibility criteria are more stringent when trying to qualify for long-term care.

Medicare vs. Medicaid– How is eligibility established?

Eligibility for Medicare is simple – if you are over age 65 and have paid Medicare tax through your employment for at least ten years, you qualify. You can get Medicare Part A at age 65 without paying any premiums if: • You receive Railroad Retirement Board benefits; or • You are eligible to receive Railroad Retirement Board benefits or Social Security benefits but have not yet filed for them; or • You or your spouse had Medicarecovered government employment.

If you or your spouse don’t qualify for Medicare Part A because neither of you paid Medicare tax through your employment, you may still be able to obtain Medicare Part A via paid premiums. Eligibility for Medicare Part B is the same as for Part A but requires a paid premium. Some folks qualify for Medicare benefits even though they are under age 65, including younger people with disabilities and those with End-Stage Renal Disease.

Eligibility for Medicaid is needsbased. Meaning, income restrictions for programs cover pregnant women, children, the disabled, and the elderly.

If your income is under the amount specified for your state, then you likely qualify if you are in one of those groups.

If long-term care is needed, however, there are also asset restrictions. An applicant cannot have over a certain

amount of assets and still qualify for nursing home care benefits. However, applicants can retain an elder law attorney to do legal planning to protect assets while still getting qualified for benefits. This way, money and property are preserved for their family and won’t have to be spent on care.

In addition to income and asset rules regarding nursing home Medicaid benefits eligibility, there is a look-back period. Suppose you had transferred assets during a specific time period before the Medicaid application was submitted.

In that case, you will likely receive a penalty where you are not eligible for benefits for a period of time. Again, an experienced elder law attorney can best help you navigate the application process to best manage any prior transfers for your benefit.

Medicare and Medicaid are two very different programs; each provides certain benefits has certain criteria for enrollment. Between the two, however, only Medicaid will cover long-term care expenses for

more than 100 days. Getting longterm care Medicaid can be a tedious process, and legal strategies can be employed that will help you protect assets while getting needed care. Yes, We Had One Too

I

n recent weeks Canada has been rocked by news of the discovery of unmarked mass graves at several First Nations boarding school sites across that country. Similar gravesites have also been uncovered at Bureau of Indian Affairs boarding school locations in the U.S., prompting interior secretary Deb Haaland to call for an investigation into possible abuses at the schools, which operated from the 1870s into the 1980s. And yes, we had one right here in the Duke City, and it did have its own small cemetery.

The Albuquerque Indian School, or AIS, opened in 1881. Originally operated by the Presbyterian Church, the school was transferred to the BIA around 1884. The campus, at 12th Street and Menaul in the North Valley, eventually grew to 46 acres with the addition of land donated by neighboring property owners. The school had its own vegetable gardens,

orchards, and dairy cows, and produced much of its own food.

The majority of the students came from the Navajo, Jicarilla, and Mescalero reservations, and from the pueblos.

For the first few decades of its existence discipline at AIS was strict. Students wore military-style uniforms and marched to class in formation. Boys were given short haircuts. Traditional cultural practices were suppressed and the speaking of native languages was forbidden.

By the 1930s however, this Spartan regimen had softened considerably, partly in response to pressure from parents. Campus life soon began to resemble that of any other American high school.

Extracurricular activities included the publication of a yearbook, team sports, and a first-rate marching band. Native arts and crafts were taught and encouraged. But the curriculum still emphasized vocational training for boys and

home economics for girls, rather than academic subjects. And speaking Navajo or Tewa could still result in being assigned extra chores.

Not every student thrived in this environment, and both AIS and its counterpart in Santa Fe were plagued by high dropout rates throughout their existence.

But many students did thrive.

Some went on to college and earned advanced degrees, then returned home to become tribal leaders, educators, nurses, and entrepreneurs.

Agnes Dill of Isleta Pueblo entered AIS as a fourth grader and stayed for nine years, receiving her high school diploma in 1932. She went on to earn a BA in education, taught school, and later became a community leader and an advocate for women’s rights. The late historian Joe Sando (a member of Jemez Pueblo and a graduate of the Santa Fe Indian School) interviewed her for his 1998 book,

Pueblo Profiles. She had this to say about her education: “The BIA schools have been criticized for many reasons. But I . . . say that if it weren’t for those schools I might not be what I am today, as well as many other Indians throughout the country. It’s usually the young activists who damn Indian schools and government programs.

They don’t understand how much those programs have helped Indians.

Most of the militants are urban Indians. They haven’t lived the hardships of reservation life.”

Clearly, not everyone who attended these schools saw them as the Dickensian hell-holes they have been portrayed as. And Dill was not alone. In 1947, when budget cuts threatened to close AIS and the Santa Fe Indian School, the All Pueblo Council sent a delegation to Washington, DC, to lobby to save the schools. They remained open for another 30 years. (continued on next page)

AIS eventually did close in 1982 and the site was turned over to the pueblo council. After decades of neglect, it is now being developed as a commercial hub.

The school’s small cemetery was located a few hundred yards northwest of the main complex of buildings. An estimated 25 to 30 individuals, primarily students, were interred there between 1882 and 1933. It was not unusual in that era for institutions like boarding schools, military posts and tuberculosis sanatoriums to maintain their own burial plots.

Transporting bodies home for burial was impractical, if not impossible, so people were buried where they died. This was not a “mass grave” as it has sometimes been called. These were individual burials that took place over many years. The cemetery was fenced and maintained, and had a caretaker until 1964. Its existence was never a secret.

The land was acquired by the City of Albuquerque in the early 1970s and today the graves lie under the sod in a pleasant, treeshaded area at the east end of 4-H Park, away from play areas and picnic tables. Until a couple of years ago a simple bronze plaque marked the location. It disappeared in 2019. The theft didn’t receive much media attention at the time, but the recent discovery of gravesites elsewhere has thrust the AIS burial plot and its missing marker back into the news. The Albuquerque Parks and Recreation Department has issued a statement assuring the public that the plaque was not removed by the City. Who did take it, and why, remains a mystery. In its place, a makeshift memorial or Descanso has appeared under a nearby tree.

The Health Benefits Of Owning A Dog

ogs love unconditionally, they fill our hearts with joy, and often are considered

to be cherished family members as much as any human. However, dogs also provide health and wellness benefits that may go unrecognized by those lucky enough to be around a dog on a regular basis. This is especially true for seniors who have a canine companion.

Albuquerque resident Darla Carl, 79, shares her home with her beloved 9-year-old male Shih Tzu, Yogi, who is a centerpiece of her life. “I never had a dog; I’ve always had cats,” Carl said.

“After getting Yogi, I was more content and less lonely.

Having someone else made my house much more alive, and that was very comforting to me.”

Dogs facilitate physical exercise and play, help people cope with crises, and decrease symptoms of depression and feelings of loneliness. They also help to lower stress, which is a significant cause of cardiovascular problems. The American Heart Association says, “Dog owners are 31% less likely to die from a heart attack or stroke than non-dog owners; people with prior heart events who had a dog living at home had a 65% reduced risk of death; dog owners who walk their dogs face one-third the risk of diabetes of those who don’t own a dog.”

One of the most challenging aspects of aging is isolation and loss of social interaction. Schmidt said having a pet tackles loneliness and introduces more structure to seniors’ lives, especially when they retire.

Ellen Schmidt, senior director of Adoptions, Outreach & The Center at the nonprofit Animal Humane New Mexico in Albuquerque, said dogs are excellent companions for seniors and that a lot of seniors decided to bring a pet into their lives during the isolating months of the pandemic.

“When a senior adopts a dog, there are increased opportunities for socialization because they go out more,” Schmidt said. “They can also meet other people who are just as just crazy about their dogs. Talking about our pets is a generational topic, and it gets us out of the house.”

An active dog, for example, can encourage a senior to get more exercise, whether just by walking more, going to a park, or by getting involved in training activities, like

In cases where a senior can’t have a dog live with them, there are others ways to safely be around dogs and gain some of the same benefits. Schmidt suggests seniors consider volunteering at a local animal shelter.

“The beauty of volunteering is flexibility, structure, and purpose,” without the commitment, Schmidt said.

For seniors considering getting a dog, check local shelters first. They have a revolving population that

needs your help.

At deadline for this story, Animal Humane New Mexico had 95 cats and kittens and 70 dogs and puppies at its shelter alone, Schmidt said.

It’s also prudent to spend a bit of time with any dog before taking it home. Some shelters, including Animal Humane New Mexico, have a program that allows people to conduct “meet and greets” to help determine if a dog will be a good fit.

Yogi was the right dog for Carl from Day 1. “He has always been such a good dog,” she said. “He has never been hard work or difficult in any way. He is such a joy in my life.

I don’t know what I’d do without him. He’s such a comfort to me.”

Ken: It was all really confusing.

I remember wondering if I should practice yoga, be acupunctured, get Rolfed, or attempt a primal scream.

For the record, I tried them all!

When I moved to Berkeley from Big Sur, a diverse and high-minded group of doctors, psychologists, tai chi teachers, acupuncturists, psychics, biofeedback scientists, bodyworkers, and nutritionists got together and formed a study group to try to imagine a new model of medicine in which the mind and body weren’t separated and all therapeutic possibilities could be examined.

Similarly, the relatively straightforward world of yoga practice has morphed into hot yoga, vipassana yoga, hip-hop yoga, yin yoga, yang yoga, kundalini yoga, earth and sea yoga, indigo yoga, et cetera. Now in this far more entrepreneurial, capitalistic era, we’re being sold skin preparations, hormone therapies, vitamins, prepared meals, pillows, mantras, brain wave supplements, self-help apps, and exercise technology along with books and seminars.

Casey: Are you saying that these people are frauds? Are you a skeptic regarding the very holistic health movement you helped create?

Ken: Some are frauds and some aren’t—Over the past fifty years, I have heard and seen a thousand different “experts” make the case that they’ve got the secret sauce or treatment that can cure almost everything. Usually they make their case by first proclaiming, “You’ve got some things wrong with you that your Western trained doctor doesn’t properly understand.”

Then they usually layer in how “Big Pharma doesn’t want you to get better because they make more money when you’re sick.” I’ve heard that from nutritionists, supplement sellers, energy medicine practitioners, anti-aging doctors, psychologists, chiropractors, life coaches, and biohackers.

Casey: Don’t you believe in people taking responsibility for their own health?

Ken: Yes. I’m a big believer in self-empowerment. But because there’s so much information out there and so many choices, it can be utterly confusing and overwhelming.

Sometimes it’s even just plain wrong, since much of what’s now posted on the Internet is not vetted for truthfulness.

Casey: Okay, Dad. So you’ve been there, done that for a lot of what I’m now just learning about. What’s in your crystal ball now? What do you see as the future of medicine?

Ken: A big part of the challenge is that while there are many similarities among us humans, there are lots of differences, too. And since now there might be literally thousands of things to measure and thousands of ways to improve one’s health, how can any one person know for sure which approach or which combination of approaches—with which practitioners—will get the best results? I can’t imagine that any one practitioner can mastermind or curate the right approach to a more holistic medicine for each and every individual.

However, I think that within a few years, due to advances in artificial intelligence and machine learning, (continued on next page)

we’re going to transition relatively swiftly from this somewhat chaotic period of health diagnosis and treatment to an era of “precision medicine.” We’re soon going to see scientific breakthroughs that will allow us to arrive at a far deeper and richer understanding of the interactions between our genes, nutrients, molecular activity, and brain-body interactions so that we’ll be able to “holistically” track thousands of biomarkers.

And if the AI is informed by a wide range of potential solution paths—allopathic, naturopathic, homeopathic, Ayurvedic, and others (always evolving based on outcomes research)—it could be far more effective at precisely diagnosing what’s not working right and then proposing the ideal constellation of solutions for each individual.

This will create an entirely new science and practice of medicine, geared to optimizing physical and mental health, enhancing well-being and happiness, and maybe even forestalling aging.

Casey: Will this new health platform be available to everyone?

Ken: Eventually, but I worry that probably not at first. A few years ago I was giving a keynote address for the esteemed Business Council, which is composed of the chairs of the Fortune 100. I was feeling a bit playful and asked the group, “Imagine that you’re a bit older than you are now and you’re beginning to get worried about physical and mental aging.

If I had a pill or a therapy of some sort that could cause you to look and feel like you were forty again, what percentage of your total net worth would you give me for that?”

Assuming that the average net worth in the room was around $100 million, I was curious about their reactions. I asked, “How many of you would give me five percent of your total net worth for that?”

Every hand in the room went up.

I pressed further: “Ten percent?

Fifteen percent? Twenty percent?

Fifty percent?” Every hand in the room remained up until I eventually hit 90 percent. The room grew silent

as everyone realized that something highly revealing and provocative had just surfaced. That evening at dinner, this theme became the topic of conversation. These powerful folks said that it wasn’t so much about wanting to be a young person again.

They liked who there were. Rather, it was their growing awareness that if their bodies were beginning to fall apart, they would wish they could have youthful vitality and health once more.

And here we’ll have an ethical issue. If optimal health is available to everyone, that would be grand, but if it’s only available to corporate billionaires, that doesn’t seem fair. If you think that income inequality is a big deal, how do you think folks will feel about longevity inequality, which already exists but could multiply?

How To Replace Important Documents That Are Lost Or Missing

D

ear Savvy Senior, Can you tell me what I need to do to replace a variety of important documents?

Our house burned down a few months ago, and we lost everything including our home property deed, car titles, old tax returns, Social Security, Medicare and Covid vaccine cards, birth certificates, marriage license and passports.

Stressed Seniors Dear Stressed, I’m very sorry for your loss, but you’ll be relieved to know that replacing important documents that are destroyed, lost or stolen is pretty easy once you know where to turn. Here are the replacement resources for each document you mentioned.

Birth certificates: If you were born in the United States, contact the vital records office in the state where you were born (see CDC. gov/nchs/w2w/index.htm for contact information). This office will give you specific instructions on what you need to do to order a certified copy and what it will cost you – usually between $10 and $30.

Car titles: Most states offer replacements through a local department of motor vehicles office. You’ll need to complete a replacement title application form and pay the application fee, which varies by state. You’ll also need to show ID and proof that you own the car, such as your vehicle registration or your licenseplate number and VIN (vehicle identification number). To get an application, go to DMV.org, pick your state, and print it or fill it out on the site.

Property deed: To access your house deed, contact your county clerk’s office, where deeds are usually recorded – you may be charged a small fee to get a copy.

Marriage certificate: Contact the vital records office of the state you were married in to order a copy (see CDC.gov/ nchs/w2w/index.htm). You’ll need to provide full names for you and your spouse, the date of your wedding, and the city or town where the wedding was performed.

Fees range from $10 to $30.

Social Security cards: In most states (except in Alabama, Minnesota, Nevada, New Hampshire, Oklahoma and West Virginia), you can request a replacement Social Security card online for free at SSA.gov/ myaccount.

If you live in a state that the online service is not available, you’ll need to fill out form SS-5 (see SSA.gov/forms/ss-5.pdf to print a copy) and take it in or mail it to your nearby Social Security office along with a number of evidence documents that are listed on this form. For more information or to locate the Social Security office that serves your area, call 800-7721213 or see SSA.gov/locator.

Medicare cards: If you are enrolled in original Medicare, you can replace a lost or damaged Medicare card by calling Medicare at 800-633-4227, or by logging into your MyMedicare.gov account.

If, however, you get Medicare health or drug benefits from a Medicare Advantage Plan, such as an HMO, PPO, or PDP, you’ll need to call your plan to get your card replaced.

COVID-19 vaccination card: Your first step is to go back to your vaccination site and see if they’ll give you a replacement.

Bring an ID and try to recall the date you were vaccinated. If that’s not feasible, contact your state health department immunization information system (see CDC.gov/ vaccines/programs/iis/contactslocate-records.html) where you should be able to print out a replacement sheet.

Tax returns: To get copies of old tax returns start with your tax preparer, who usually keeps copies of your returns on file. You can also get copies of federal returns directly from the Internal Revenue Service. You’ll need to fill out and mail in IRS form 4506. To download this form IRS.gov/pub/ irs-pdf/f4506.pdf or call 800-8293676 and ask them to mail you a copy. The cost is $43 for each return requested.

Passports: You can apply for a replacement passport at a Passport Application Acceptance Facility.

Many post offices, public libraries and local government offices serve as such facilities. You can search for the nearest authorized facility at iafdb.travel.state.gov. The fee is $145.

he most common fly is the house fly. House flies have a gray thorax (where head is connected and wings are attached) with four dark stripes and a mottled abdomen (posterior portion).

These flies are considered “filth flies” and will feed on excrement, garbage, carcasses and even human secretions from wounds and mucous membranes. If you accidentally eat the larvae in contaminated food, they can survive in your intestine. They can harbor over 100 pathogenic organisms and are capable of transmitting more than 65 diseases and bacteria that can cause duodenal and stomach ulcers.

House flies are the most common fly in the world.

When you swat a fly, remember that is has an unblurred range of vision of only about 1½ feet. You should aim your flyswatter about 1½ inches behind the fly because when houseflies take off from a horizontal surface, they jump upward and backward.

Set out a saucer filled with bubble soap to attract and kill flies. Adult flies eat only sugar, so use some light Karo Syrup, honey or sugar water with 5 percent boric acid or borax baits.

You can also make a good fly trap with some plastic water bottles, apple cider vinegar and some sugar.

Cut the tops off the bottles and invert the top into the lower section forming a funnel and tape them together. Put about two inches of apple cider vinegar in the bottle with a quarter teaspoon of sugar.

This will attract and catch many kinds of flies, no matter their normal food preference. If you have a large building or yard, you can use gallon size milk jugs cut the same way, with about three times the vinegar and sugar. A good commercial fly trap is Rescue Fly Traps, available in stores.

Another common fly is the little house fly. Little house flies are dull gray with yellow on the upper abdomen and three dark longitudinal stripes on the top of the thorax. These flies resemble house flies, but they fly in circles in the middle of a room or on a porch and don’t appear to land. They can lay their eggs in any organic material, including compost piles, pet feces, dead leaves, etc. They have been known to enter the urinary tract of naked sleeping persons and causing urinary myiasis. To prevent these flies from appearing, empty and clean all food handling equipment, dishes and garbage containers, and daily remove and/or bury all animal droppings, fruit and organic debris inside and/or outside. They do like beer, so you can put two packets of aspartame in two inches of beer in an open container to act as a bait for these flies. You can also use a fly swatter with a sticky side to swat them when they are circling.

Also very common are blow flies.

Blow flies are larger than house flies and are normally shiny green, blue, bronze or black. Blow flies feed on decaying animal matter. If you have them in your house, it is an indication of a dead animal in the wall or ceiling. Occasionally, the only sign of these flies in an early infestation is when the larvae fall from the ceiling void onto the floor. If you can find and remove the carcass of the dead animal they are feeding on, it will speed up their leaving. If you can’t, there isn’t much you can do except be patient and wait for the dead animal to dry up. They can also lay their eggs in dog feces or in any animal matter with a high protein content, including dry cat food.

Blow flies are also used by forensic entomologists to establish the time of death in human fatalities.

Remember, pesticides are never necessary for fly control or for almost any other household pest.

As we age those reflexes that have kept us out of trouble on the highway slow down. Not only does our reflex time increase but our vision becomes less sharp Let’s examine our eyesight first.
