New Mexico PBS / KNME-TV Joins Community Partners In Promoting National Estate Planning Awareness Week, October 15 - 21, 2018

ew Mexico PBS / KNME-TV announced that the station is joining local community organizations in a public awareness campaign to promote National Estate Planning Awareness Week (NEPAC), October 15 – 21, 2018.

Estate planning is one of the most overlooked areas of personal financial management. It’s been estimated that more than 120 million Americans do not have proper estate plans to protect themselves or their families in the event of sickness, accidents, or untimely death. This circumstance can cause many families unnecessary expense and financial hardship.

Proper planning can help avoid these unintended consequences.

In 2008, Congress passed a resolution proclaiming the third week in October as National Estate Planning Awareness Week. “Public television has long served as an advocate for financial literacy and estate planning is a critical component of the financial health of most adults,” said Theresa Spencer, Development Director, New Mexico PBS / KNME-TV. “Essentially, you are making sure that your personal wishes

are clearly defined, communicated and understood. Putting those financial desires in place, also provides peace of mind – both for you and your family.”

New Mexico PBS / KNME-TV is recognizing National Estate Planning Week by providing New Mexico residents with a FREE estate planning booklet. This booklet will allow the user to start organizing their financial assets and thinking through their own personal goals. place in mixing bowl. Mix shallot, 3/4 of sweet onion, two stems chopped parsley, softened butter and chicken stock into cornbread mixture. Add salt and pepper to taste.

all is here and October is a good month to celebrate hearty foods. Sometimes you just have a hankerin’ for thick, juicy pork chops. My pork chops fall short of delicious, are often times overcooked and, well, rubbery. That’s why I was thrilled when my beautiful niece and goddaughter, Marisa Tran, a teacher from Rialto, Calif., made her delicious, rich and tender stuffed pork chops on her recent trip to Albuquerque.

Directions: Preheat oven to 350 degrees. Slice pork chops vertically, creating a small pocket. Drizzle olive oil generously over pork chops. Sprinkle salt and pepper over pork chops.

Heat skillet with olive oil. Coarsely chop remaining 1/4 sweet onion, place in skillet and sauté. Grill pork chops until lightly browned on each side.

Coarsely chop garlic cloves, rosemary and remaining parsley.

Sprinkle herbs over all sides of the meat.

Press brown sugar into the meat, ensuring that it is evenly coated on all sides.

Stuff the pork chops with the stuffing. Place meat in roasting pan. Cover with foil and bake about 25-30 minutes, until the juices run clear. Enjoy!

October is also Halloween month. While searching for fun Halloween drinks, I came across these spiced apple cider pomegranate Moscow mules created by Ree Drummond. The “Pioneer Woman” serves these fun cocktails to her guests on Halloween.

These hauntingly good spiced apple cider pomegranate Moscow mules will be perfect to serve at Halloween. Not only are these boo-zy drinks pretty, but they are also perfectly festive for your ghostly soirée. I’d omit the alcohol and substitute with ginger ale for a kid-friendly drink.

Ree Drummond’s Spiced Apple Cider Pomegranate Moscow Mules Makes two drinks Ingredients: • Pomegranate seeds and fresh apple slices, for garnish • 4 ounces, fluid vodka, chilled • 2 pinches cinnamon • 1/4 cup apple cider, chilled • 1/2 cup sparkling pomegranate juice, chilled • 12 ounces fluid ginger beer, or as needed to top off glasses, chilled

Marisa’s Stuffed Pork Chops Ingredients: • 4 thick cut, bone in pork chops • 3 stems fresh parsley (flat) • 1 large shallot • 1 medium-large sweet yellow onion • Pre-made cornbread (store bought) • 4 cloves garlic • 2 sprigs fresh rosemary • 1 box chicken stock • 2 Tablespoons softened butter • 2-3 (or more as needed) Tablespoons olive oil • 2 Tablespoons brown sugar • 2 teaspoons salt • 1 teaspoon pepper

Instructions: Fill large glasses with ice cubes. Add pomegranate seeds and apple slices. Pour two ounces of vodka in each glass and add a pinch of cinnamon. Add apple cider and sparkling pomegranate juice to each glass. Fill each glass the rest of the way with cold ginger beer. Stir gently to combine and serve immediately. Cheers!

Ask A Health Care Professional Eugene Sun, M.D., Vice President and Chief Medical Officer, Blue Cross and Blue Shield of New Mexico

T

his month’s column focuses on accessing health care in the community. To answer my questions, I have enlisted the help of Lance Wilson, M.D., chief medical

officer of DaVita Medical Group, a multi-specialty physician group in the Albuquerque metro area.

Dr. Sun: It can be difficult to understand what type of health care is available in the community and how to access care when needed. What thoughts or guidance can you share on this topic?

Dr. Wilson: First, and most importantly, I think everyone needs to establish a relationship with a primary care provider (PCP). The relationship between the PCP and the patient allows for conversations about what type of health care is available and how best to access it. I would encourage everyone to get the hours of operation, phone numbers and addresses of the health care resources that they might need, even before they actually need such care. That way, if something comes up, it’s easy to know what to do.

Important resources include your primary care office, specialty care offices, care management team, urgent care centers and hospitals.

Dr. Sun: What can you tell our readers about how to understand whether their health concern should be cared for by their PCP or by an urgent care center or emergency room?

Dr. Wilson: I practiced emergency medicine in this city full time for about 14 years. It’s hard to answer that question without a lot more information. Each health care concern needs to be treated as an individual event, and the level of care needed will not always be the same. I would encourage patients to ask their PCP this question to better understand the capabilities of their primary care office and urgent care centers. Some urgent care centers, including the DaVita Medical Group Urgent Care at Journal Center, can treat a wide variety of concerns that many patients think need to be cared for in the hospital or emergency room. Again, I encourage patients to use their best judgment and to get information on what is available in the community before they actually need to access that care.

Dr. Sun: What services and specialties does DaVita Medical Group offer for seniors?

Dr. Wilson: DaVita Medical Group offers adult primary care and specialty care in over 30 medical and surgical specialties. We also have a care management department that helps patients navigate the complicated health care system and coordinate their care with clinical teams. We are the only medical group in the state of New Mexico that is offering a truly comprehensive annual visit for seniors; this includes a no-charge visit with a medical assistant for 30 to 45 minutes and then a follow-up appointment with a clinician for an additional 30 minutes. We call these appointments Comprehensive Wellness Visits, and they allow us to spend more time with our patients to ensure that all of their health care needs are being met.

Dr. Sun: Blue Cross and Blue Shield of New Mexico encourages our members to seek and obtain timely and appropriate preventive care services. What are some of the preventive care services your group provides? Are these services easy to access?

Dr. Wilson: We do offer a variety of preventive care services, such as breast cancer screening with 3-D mammography; colon cancer screening in our endoscopy center; resources for patients with diabetes, including diabetes education; and an ophthalmology department that has expertise in preventing and treating the potentially devastating complications of diabetes on eyesight.

All of these services are easy to access – most offer same-day or same-week appointments.

Dr. Sun: What closing thoughts do you have for our readers with regard to their health and how to access care in our community?

Dr. Wilson: My take-home message would be to make sure that you are educated about your existing medical issues, your medications, and when and where you should get care if things aren’t going according to plan. I would also personally encourage everyone to explore their mental, emotional, and spiritual health practices, as these are incredibly important in creating the best physical health possible.

Bewitching New Mexico

A

s Halloween approaches, the familiar images of ghosts, witches, bats and big hairy spiders appear everywhere. Nowadays it’s all in fun. But in colonial New Mexico an accusation of witchcraft or sorcery was taken very seriously and could result in a sentence of death.

Throughout human history and in all parts of the world, the belief has existed that some individuals possess supernatural powers that they employ to cause harm to others. Any unexplained calamity—severe weather, a crop failure, a disease epidemic, the death of people or livestock—might be attributed to witchcraft, and any person whose behavior deviated from socially accepted norms might become a suspect.

When Spain began to colonize the New World in the 16th century, Europe was in the throes of a witch panic that had been going on for decades and that had resulted in the torture and execution of tens of thousands of suspected witches. The Medieval world order was breaking down as the new ideas of the Renaissance and the Protestant Reformation spread across the continent. Anthropologists say that it is at such times of social upheaval that outbreaks of witch hysteria are likely to occur.

While most of the witch burnings occurred in northern Europe, Spain also had its share. So when the first Spanish colonists arrived in New Mexico, their cultural baggage included a body of ancient folklore concerning witchcraft or brujería.

The Pueblo religion they found here, with its pantheon of strange deities, underground kivas, and masked ceremonial dances, which sometimes involved live snakes, seemed to them diabolical.

As the Franciscan missionaries set about converting the Indians to Christianity, some friction was inevitable. Friars who had experience ministering to native populations elsewhere were inclined to be tolerant of native beliefs and to allow traditional practices to coexist with Christian worship. Others were more zealous, sometimes ordering the burning of ceremonial objects and the whipping of Pueblo religious leaders.

Persecution became especially virulent in the 1670s, when several medicine men were hanged for sorcery.

Historians generally agree that this was a leading cause of the 1680 Pueblo Revolt. After the revolt, Spanish magistrates were more reluctant to convict accused witches or to inflict harsh punishments when they did.

The pueblos and other Southwestern tribes had their own traditions of

witchery that predated Spanish contact, and some of these were remarkably similar to imported European beliefs.

In both cultures it was thought that witches can assume the form of an animal (usually an owl or a coyote) or even become invisible, that they can fly through the air as a fireball, or place a hex on someone merely by gazing at them—the so-called mal ojo, or evil eye. The pueblos were as diligent about rooting out witches as were the Spanish, if not more so, and the penalties could be severe. There are reliable accounts of alleged witches being put to death at some pueblos as late as the 1880s.

Belief in witchcraft lives on in the modern era, and not only in the pueblos or in rural Hispano villages.

In the spring of 1990 in the city of Clovis on New Mexico’s eastern plains, a 6-year-old boy named Matthew Roberts mysteriously went missing. He had been playing in the front yard of his home on a sunny afternoon, and then suddenly he was gone. The entire town mobilized in the search for Matthew, but no trace of the boy could be found. The case attracted national media attention.

I was working as a reporter at The Albuquerque Tribune at the time and was sent to Clovis to cover the story.

cult, to be offered as a human sacrifice.

I was assured by several sober citizens that such a cult existed, led by a local woman who was a known witch.

Naturally, I sought out the “witch” for an interview.

She turned out to be a friendly 62-year-old grandmother, something of a free thinker, who dabbled in New Age spirituality and hosted a weekly get-together of like-minded individuals in her home on Sunday mornings.

That, apparently, was enough to make her a “witch” and a “cult leader” in the eyes of some of her neighbors.

But the question remained: What had happened to poor Matthew?

Two weeks after his disappearance, his body was discovered inside a storage compartment in the rear of a family station wagon that had been parked at the side of the house, undriven during that time. It appeared that he had crawled into it while playing an impromptu game of hideand-seek and had suffocated in the cramped compartment. There had been no abduction, no satanic ritual, no witches, no evidence of foul play— only a tragic accident.

Fortunately, apart from a few crank phone calls, no serious harm was done to any alleged “Satanists” before the truth came out. In another time and place, the story might have ended differently.

Medicare Open Enrollment

edicare open enrollment begins this month, and health insurance officials are excited about a slate of new options for members. The Centers for Medicare and Medicaid Services has issued a short list of changes and possible additions for 2019, including removing the caps on therapies, and expanding telehealth programs and lifestyle benefits such as wheelchair ramps.

Every year, the Medicare enrollment period is Oct. 15-Dec. 7 and comes two weeks after Medicare distributes its Annual Notification of Change, which details any shifts in the program from year to year.

One such change is that CMS is giving Medicare recipients the opportunity to switch plans one time during the first 90 days of the year.

This means that if a member chooses one plan during open enrollment but discovers that this plan doesn’t meet her needs for prescription drugs or providers, the person can

jump to another plan before March 31. Janice Torrez, vice president of external affairs and chief of staff for Blue Cross and Blue Shield of New Mexico, said that she is glad New Mexicans will have the opportunity to make choices, and she encourages members to spend time evaluating every option available before signing up.

“I would really like to encourage all seniors to take time to look at the different plans, research plans that are available to them,” Torrez said.

“Medicare.gov lets them compare plans, and most Medicare Advantage plan providers will host different educational events throughout the enrollment period.”

Blue Cross and Blue Shield of New Mexico and Presbyterian Health Plan both plan to host a slew of seminars that allow members and prospective members alike to ask detailed questions and learn general information about Medicare. Presbyterian Health Plan and BCBS say that members this year can take advantage of zero-dollar premiums on some plans, as well as new benefits this year, including for hearing aids and transportation.

CMS’ new rules for 2019 and looking toward 2020 allow for Medicare Advantage plans to offer coverage for lifestyle comforts, such as handrails. Medicare is also closing the dreaded “donut hole” coverage gap under Part D for brand-name prescription drug coverage in 2019; the gap for generic drugs closes in 2020.

Presbyterian’s Lauri J. Michael, director of brand management strategy and customer engagement, said the organization has worked hard this year to beef up ease of access for its Medicare Advantage members.

“One of the things we’re most excited about from a benefit perspective is the video visit benefit at zero co-pay,” Michael said.

“It’s a nice, easy, convenient way for members to access licensed providers 24/7 at no cost. They simply log into their MyPres account (online) and request to start a visit.”

Additionally, that same MyPres account can allow members to have online visits via email, where providers can assess simple ailments without requiring the member to head into an office. Presbyterian has also expanded its urgent care services to include scheduling, limiting the long wait times that often come with such visits. It isn’t a requirement, but it may help make visiting an urgent care provider easier, Michael said.

“These are all fun, innovative ways that we are attempting to ease folks into accessing care more easily,” Michael said. “These are evolved services, improved for 2019.”

Torrez said that Blue Cross and Blue Shield of New Mexico is focused on helping members access preventive care. To that end, Torrez said, the insurer is offering zerodollar copays for primary care.

Additionally, members can earn rewards for healthy behavior, such as bone density tests, mammograms and vaccines. Each of BCBS’ plans includes a Silver Sneakers benefit, a no-cost fitness program that also supports prevention and healthy lifestyles.

“When they get a flu shot or get a physical,” Torrez said, “they let us know and we provide them with member rewards up to $100 a year.”

United Healthcare’s Lisa Contreras, regional communications director, said that UHC is working hard to prepare Medicare beneficiaries with information and resources to make the best decision for them. Its website, MedicareMadeClear.com, is packed with tools to help members during open enrollment.

“We suggest people ask themselves a few key questions,” Contreras said. Among them: “Is your current coverage still meeting your personal health needs, and will your benefits change next year? Is your coverage a good fit for your budget? Are you comfortable with the doctors and hospitals that are available with the coverage you’re considering? Are you medications on the approved drug list?”

“If you’re enrolled in a Medicare Advantage plan, you should have received a document called the Annual Notice of Changes.

Make sure you take a look at that document so you’re not caught off guard by any changes to your plan,” Contreras said. “It’s important to pay attention to more than just the monthly premium. You should also understand the other out-of-pocket costs, including the deductible, copays and coinsurance.”

Like Torrez, Contreras advises Medicare recipients to ask questions, diving deep into what each plan offers. Help is available at the plans’ websites and at educational events, where people can find out if a doctor they love is in-network, or if a specific medication is covered.

Comparison shopping will take some time, but it is worth it for seniors to be comfortable with their coverage.

Foster Grandparent Program (FGP) is looking for volunteers age 55 and older who are interested in making a difference in a child’s life. Foster Grandparent volunteers must be willing to serve an average of 20 hours per week and must be able to pass a background check, physical, and tuberculosis test. Foster Grandparent volunteers serve one-on-one with children, working with them to build their social and emotional development as well as help improve their literacy skills. Ronald McDonald Family Rooms at Presbyterian and UNM Hospitals The Ronald McDonald Family Rooms at Presbyterian and UNM hospitals offer a place for parents to rest and regroup right at the hospital, just moments away from their sick child. The Family Rooms are open seven days a week and are staffed by hospital volunteers. The Family room volunteers greet families, maintain visitor data, stock food, drinks, and help families with their needs.

Hospice serves just more than 1 million patients per year. Almost 98 percent of these patients receive their hospice care at home, where half of hospice deaths occur.

The hospice benefit was designed for individuals whose terminal illness renders a life expectancy of six months or less. In 2016, the average length of stay in hospice care from admission until death was 17 days. One third of patients placed on hospice die within 22 days. The reason for this may be that in our country hospice is too often thought of very late in the course of an illness.

When Kubler-Ross testified at the first national hearings, she said, “We live in a very particular deathdenying society. We isolate both the dying and the old, and it serves a purpose. They are reminders of our own mortality. We should not institutionalize people. We can give families more help with home care and visiting nurses, giving the families and the patients the spiritual, emotional, and financial help in order to facilitate the final care at home.”

Helping families cope with the dying process through compassionate care is a team process. The hospice team accomplishes this with a registered nurse case manager, a chaplain, a social worker, a certified nursing assistant and a physician assigned to each patient.

Hospice and palliative care is now a fully recognized medical subspecialty. Physicians can become board certified and obtain fellowship training after residency.

Nurses and social workers can also receive specialized hospice training and certification.

The modern hospice movement arose in the United Kingdom in 1963, when Dame Cicely Saunders established St. Christopher’s Hospice in a London suburb. Saunders’ creed was “to cure sometimes, but to comfort always.” Saunders created a movement of caring for the total patient - from emotional, physical and spiritual approaches. She was trained as a social worker, a nurse and a physician. Dame Saunders chose to spend her last days in the very hospice unit she founded.

No one wants to admit that death is approaching. Our culture is not one of giving up. A series of studies in 2005-2006 showed that 22 percent of all Medicare cancer patients started a new chemotherapy drug in the last month of life, and 34 percent of cancer patients received chemo treatment in the last two weeks of life. This highlights the need for further patient enlightenment regarding options for care.

In a landmark study of more than 9,000 hospitalized patients published in the Journal of the American Medical Association, the support authors showed indirect evidence of the increased need for end-oflife care expertise. One half of the conscious patients complained they had inadequate pain control, with severe pain present 30 percent of the time. Almost half of these patients had a “do not resuscitate” order written within the last two days of life. Greater than one third of the patients in the study that died spent at least 10 days in an intensive care unit.

In 1969, Dr. Elisabeth KublerRoss published a book based on more than 500 interviews with dying patients. In On Death and Dying, she details the five stages through which many terminally ill patients progress.

hy do I focus on the hazards of pesticides? There are several reasons.

Pesticides are hazardous to people when they are misused and even in cases where they are properly used. The most ANSWER TO #5122 O

common pesticide group are called synthetic pyrethroids.

Studies have found that they can cause children to develop leukemia and lymphoma and that they can cause pregnant women to have autistic children. These pesticides are frequently sprayed along the baseboards in homes and businesses.

Pesticides when generically used outside will kill a lot of bugs, but that also impacts animals that eat those insects. Lizards, some mammals and many songbirds get sick as a result. In fact, songbirds that eat insects are protected in New Mexico, and generically spraying pesticides can cause legal problems.

Pesticides are often used carelessly, and they are very poorly regulated in many areas, including in New Mexico. There is no reason to use toxic pesticides to control most pests as there are plenty of non-toxic methods available. Why use pesticides to control roaches when you can use beer and duct tape. Why use pesticides to control ants as you can use various baits? You can even use non-toxic products to control termites. XT2000 Orange Oil is very effective when used against dry wood termites, and Altriset is a very good product to use for subterranean termites.

Herbicides, particularly those containing glyphosate, can be dangerous to human health when generically used. Examples are Roundup and RangerPro. A person who used Roundup developed cancer and sued its manufacturer,

Many herbicide applications kill beneficial plants, as well as so-called weeds. These beneficial weeds include plants that are good for bees, butterflies and other insects, and even some that are edible by humans.

Toxic pesticides are nothing more than weapons of mass destruction and should never be used in schools, in medical facilities or in any public places.

ometimes the best thing to take for one’s health is a walk.

New Mexico fall temperatures are perfect for aerobic activity, but what about those of you who just don’t get inspired enough to actually hit the pavement? Perhaps you haven’t found a consistent source of motivation. A fitness tracker could help.

When I bought one it helped me center my mindset on performance and goal-setting. It promotes my awareness of daily movement – the number of steps I take during the day - and as a result, I have become more motivated to reach my goals.

My fitness device has both a clock and a calendar, and it ends my daily count of steps at 11:59 p.m. each day. This often motivates me to keep track of how much I walk – or don’t - throughout the day, so I don’t feel

I’ve had several devices to track daily movement over the past decade.

I started with a simple but accurate pedometer that calculated my steps.

I noticed right away that I was motivated to increase my number each day. I also was forced to take responsibility for inactivity. Soon, I migrated to a device I could plug into my phone (now they connect wirelessly). This device had a mutual accountability setting where I could share my effort with friends and see their steps each day and week.

Outsmarting these devices sometimes becomes a humorously paradoxical side effect. But who really loses when we trick our fitness trackers? A friend told me once that she put her tracker on the ceiling fan! As I began to consider how that might work for me, I forced myself to find other options.

I needed a fitness tracker that measured not just steps but also heart rate. Measuring heart rate can be a better way to determine aerobic activity. While I had been getting my steps in, it turned out I wasn’t

getting much cardiovascular activity.

A heart rate monitor allows me to test the impact of body motions and pace on my heart while walking. If I want to get more aerobic activity, I can swing my arms or gently twist my torso left and right to watch my rate increase.

Seth Martin, a cardiologist at Johns Hopkins Medicine agrees that fitness trackers benefit heart health: “Tracking likely helps a lot of people when combined with a clear goal to shoot for.”

We enjoy rewards. If you haven’t invested in a fitness tracker, I strongly encourage it. It doesn’t matter if you have a difficult time getting around, or if you’re a swimmer - there are waterproof versions too we all need a way to monitor our daily effort.

Wouldn’t it be great to offer them a fitness tracker for the holidays and get the whole family competing for most daily steps taken? I bet many of you move more than your children and grandchildren do.

Research a device that meets your needs. Prices can range from $5$500. My insurance company offered to reduce my monthly premium this year if I wore an approved fitness tracker and met goals. Find out if your plan might provide a similar incentive. Perhaps the device will pay for itself over time. Set some goals and get tracking!
