A Paean To The Piñon

F

or me, and perhaps for you, one of the signature scents of the Christmas season is the resinous aroma of piñon smoke.

One whiff of that natural incense transports me magically to the plazas of Zuni or Jemez or San Ildefonso, watching a line of winter ceremonial dancers. Snowflakes drift down and piñon smoke from houses and kivas swirls through the crisp air. In homes surrounding the plaza, feast day tables are piled high with the traditional foods of the season: posole, tamales, oven bread and hearty stews. And always, there’s a bowl of roasted piñon nuts.

In a corner of the living room stands a Christmas tree, often a piñon cut in the nearby hills.

For far longer than such things as Christmas or Christianity have existed, these humble pines have been a vital source of food, fuel, and building material for the people who have occupied this corner of North America. Even before the holy trinity of corn, beans and squash came to the Southwest, there was piñon.

The proper botanical name for our familiar New Mexico piñon is Pinus edulis, which distinguishes it from three other species, also commonly called piñons (or pinyons, to use the anglicized spelling) that grow in the American Southwest and northern Mexico. Seldom exceeding 25 feet in height, they can usually be found growing alongside one or more of our five species of native junipers, with whom they share an ecological niche between high desert grassland and the ponderosa pine forests of higher elevations.

Edulis means “edible,” and that they certainly are. One pound of piñon nuts is said to pack about 3,000 calories in the form of healthy unsaturated fats and a full complement of essential protein-forming amino acids.

Ethnobotanists, who study such things, say that they likely were a mainstay in the diet of the earliest Paleo-Indian hunter-gatherers and all who came after. Ancestral Puebloans used piñon boughs for the framework of their pit houses and made glue from the pitch, which they used to attach feathers and points to arrow shafts. The pitch was also used medicinally and in the manufacture of paints to decorate pottery.

In the southwest corner of our state, the range of P. edulis overlaps with that of the closelyrelated Mexican piñon, or Mexican stone pine, P. cembroides. It may have been the latter species that was encountered by a little band of four castaways who trekked across northern Mexico in 1535.

These were the last survivors of the ill-fated Narváez expedition, shipwrecked on the Florida coast seven years earlier. One of the four, Alvar Núñez Cabeza de Vaca, later published an account of their odyssey in which he described the gnarled little pines and noted that their nuts were “better than those of Castile,” the earliest known reference to any of the piñons by Spanish chroniclers. Since the castaways had been subsisting on mesquite bean mush and prickly pear cactus for some weeks previously, those nuts must have been a welcome treat.

Another close cousin, the singleleaf piñon, P. monophylla, is one of the two State Trees of Nevada.

There, in the 19th century, hundreds of square miles of piñon/juniper woodland were clear-cut to make the charcoal that fueled the furnaces that smelted the ore from which the Silver State got its nickname. From the eastern foothills of the Sierra Nevada to Utah’s Wasatch Front, forests that had taken centuries to mature disappeared in a generation.

Here in New Mexico, with much less ore to smelt, our piñons were largely spared that fate, although many were felled for firewood to heat homes. The overgrazing of sheep in some districts led to soil erosion, the destruction of the piñon/juniper ecosystem, and the loss of many trees in those areas.

Today, climate change is taking its toll, as rising temperatures and drought stress make the trees more susceptible to insects and diseases.

Both in the wild and in cultivated landscapes, piñon mortality is increasing, which is all the more unfortunate because healthy forests can help to absorb the carbon dioxide emissions that fuel climate change.

Nevertheless, P. edulis is still widespread across New Mexico, and since 1949 has been our official State Tree. Could there have been any other choice?

leep is a conserved behavior across evolution and a behavior found in almost all known animal species. Studies have shown sleep is essential for optimal health and the lack of it or inefficient sleep is associated with

poor health outcomes. In today’s world many of us do not get enough sleep. Reasons for not getting enough sleep vary but may include stress from daily life, distraction from news and information 24/7 through our digital devices, or lack of sleep hygiene. A third of U.S. adults report they usually get less than the recommended amount of sleep.

How much sleep do I need and how does aging affect my sleep?

Healthy sleep includes the right amount, good sleep quality, appropriate timing and

regularity, and the absence of sleep disorders or disturbances. Good quality sleep refers to how well you sleep. It means falling asleep in 30 minutes or less, sleeping soundly through the night with no more than one awakening and being able to fall back to sleep within 20 minutes if you do wake up.

The amount of sleep needed changes with age. Experts recommend adults should sleep seven or more hours per night on a regular basis to promote optimal health.

Older adults do not sleep as well as younger adults. Studies have shown changes in sleep patterns in the fifth decade of life and older.

Older adults go to bed earlier and rise earlier, take a longer time to fall asleep, sleep for a shorter amount

of time, have more awakenings, have altered sleep stages, and have more awake time during the night.

To be more specific, the sleep stages where there is no rapid eye movement, also known as dreamless sleep, is affected the most as we age.

In the recent decades a lot of research has been done and continues to shed light on circadian rhythm, sleep, chronic diseases, aging, and how it relates to dementia. But the connections are still largely unknown. The question remains: Do older adults just need less sleep, or are they incapable of getting the amount of sleep they need?

Some common sleep disorders include insomnia, narcolepsy, restless leg syndrome and sleep apnea. If left untreated, these disorders can affect sleep quality and health.

Insomnia is trouble falling asleep and/or staying asleep. People with insomnia do not feel rested when they wake and feel tired during the day. They also have less energy and often worry about the lack of sleep because it is affecting their ability to function.

Narcolepsy is excessive daytime sleepiness combined with sudden muscle weakness. These are called “sleep attacks” because the person is awake one moment and falls asleep without warning and during situations when it would normally be unusual to fall asleep, such as walking, having a conversation, or worse, while driving.

Restless leg syndrome occurs when one has unpleasant sensation in the legs, which causes difficulty in falling asleep. Sensations have been described as itching, creeping, tension or crawling. Moving the legs brings relief. Having restless legs can cause insomnia, frequent awakenings at night and can lead to depression.

Sleep apnea is when one stops breathing for short periods of time while asleep. There are two types of apnea. Obstructive sleep apnea occurs when the soft tissues of the throat collapse and block the airway. Central sleep apnea happens when the brain does not send the appropriate signals to the muscles to make one breathe. Studies have shown that people with untreated sleep apnea are more likely to have heart disease and high blood pressure and an increased risk for diabetes and obesity.

What can I do to get better sleep?

A strategy to improve your quality of sleep is called “sleep hygiene.”

This means having a consistent sleep schedule, avoiding beverages with caffeine and alcohol late in the day, keeping your room dark and quiet, not forcing yourself to sleep, sleeping long enough to feel rested and then getting out of bed, exercising several times a week but not right before bed, avoiding looking at smart phones or digital

devices that give off light before going to bed, avoiding long naps in the late afternoon, and solving problems before you go to bed.

If necessary, work with a counselor or psychologist to deal with any problems that might be causing poor sleep. Finally, if you think you might have a sleep disorder, speak with your health care provider about it; studies can be done to evaluate and help develop a treatment plan.

Local nonprofit leaders report increased need for contributions, volunteers

ealth care experts have advised everyone, especially seniors, to stay home for the holidays due to Covid-19 risks associated with travel and intimate indoor gatherings. While avoiding beloved traditions around the tree, at the dinner table, and by the fireside next to family may feel like a hardship, their absence pales in comparison to the lack of paychecks, food, and basic financial stability facing thousands of New Mexicans due to the pandemic-induced economic crisis. ‘Tis the season to help those in need.

With the global generosity movement Giving Tuesday celebrated December 1, International Volunteer Day occurring December 5, and annual holiday charitable drives underway, local nonprofits hope the money and time people

aren’t spending away from home this month might be diverted to donations and volunteer hours spent locally.

Despite the pandemic, forecasts call for charitable giving to have increased in 2020, compared with 2019, which saw a jump in donations for the 39th time in 41 years, according to a November report

by WalletHub. But final tallies ride heavily on the generosity of people right now. Giving in December represents on average 22 percent of all annual online giving, according to Charity Navigator.

“Individuals typically want to donate a lot around the holidays, but we need donations to continue and not stop. To say it’s going to be needed during the holidays is a misstatement. It’s going to be harder on people – period,” said Frank R. Lopez, executive director of the New Mexico Association of Grantmakers (NMAG).

The good news is that local philanthropic leaders say donations are steady if not slightly higher so far this year, in line with national giving reports. Foundations have increased giving, with fewer restricted funds, enabling New Mexico nonprofits to keep people on payroll, pay bills and generally remain solvent so they can continue serving the community.

“For those organizations fortunate enough financially to have a stable income, the demand for their services is up,” Lopez said. “They’re not accumulating wealth.”

Larger organizations accustomed to padding coffers into the next year via large end-of-year fundraising events have had to cancel them due to Covid-19 risks; smaller

Organizations, too, have seen success through targeted campaigns.

For example, United Way of Central New Mexico raised $1.1 million in three months during a drive earlier in the year, and Sandia National Laboratories launched a relief effort with a goal of $25,000 for New Mexico pueblos and reservations.

“They ended up in less than 10 days raising $250,000. So, they blew their goal out of the water,” said Melissa Dracup, director of investor relations for United Way of Central New Mexico.

The not-so-good news is that fundraising success to date in no way means that nonprofits are sitting pretty heading into winter.

seeking assistance from the local nonprofit’s “drive-up, pick-up” food markets, where volunteers place bags of groceries in trunks to minimize physical contact and Covid-19 risk.

“The need is exploding,” Hidalgo said, estimating that each market, held a few times per month, averages 350-500 households. Plus, many of the drivers are requesting more groceries as “each car represents more people living in a household,” he said. A single market in August served 700 seniors and their family members.

If not for the near record fundraising of 2019, Hidalgo said, Silver Horizons wouldn’t be able to respond to the need. Between higher grocery costs and stalled fundraising, he said last year’s cushion isn’t going to last forever.

Further, as services get stretched, more personal protection equipment and cleaning supplies will be needed to ensure organizations can operate safely.

“Nonprofits don’t yet see another round of federal help (as of midNovember), and we can see in a few months they are going to be strapped again. We can’t rest on recent generosity (from foundations and individuals) and would like to see double down support,” said Larry Alei, director of the Center for Nonprofit Excellence, which merged

However, attracting volunteers has proven challenging as people isolate at home. Nonprofits, like local businesses, have had to become creative to ensure they can do their work safely. In addition to employing social distancing and requiring masks, for example, food banks are taking volunteers in “bubble groups,” isolating a person or family unit from other volunteers on site, Dracup said. The local Agora Crisis Center, a suicide prevention lifeline, is conducting virtual trainings for those interested in volunteering to answer its hotline.

Individuals can also help the community at a distance by adopting a family and mailing gifts on wish lists through HopeWorks or PB&J Family Services.

The Center for Nonprofit Excellence even provides a volunteer matching database on its website, CNPENM.org, where you can enter information about your skills and interests and get paired with nonprofits that fit your capabilities and Covid-19 comfort zone.

Changes To New Mexico’s Holiday Traditions

ew Mexico is rich in holiday traditions. In years past, we’ve celebrated in Southwest fashion with Las Posadas, Albuquerque’s Luminaria Tours, the Twinkle Light Parade, and New Mexico’s largest walkthrough light show, the River of Lights. Each year we kick off the holiday season with the annual lighting of the Christmas tree at Historic Old Town Plaza and various holiday strolls throughout the city.

But with the COVID-19 pandemic disrupting countless live events, there has been much speculation about how local celebrations will be pulled off, if at all, this year.

Some celebrations have been reimagined in hopes of keeping the holiday magic alive. These include live streamed and virtual celebrations that may be enjoyed from the comfort of your own home and drive-through formats from a distance in your car. Many events

around the state have been modified or were awaiting confirmation to occur as of deadline for this story.

Due to the State of New Mexico’s public health orders, the following events have been canceled: • Albuquerque BioPark’s River of Lights.

• 1/2 teaspoon peppermint extract • 2 eggs • 1 tablespoon all-purpose flour Brownie Batter: • 1 cup all-purpose flour • 1/2 cup cocoa powder • 1/2 teaspoon salt • 1 cup granulated sugar • 1/4 cup vegetable oil • 1/4 cup milk • 2 eggs • 1 teaspoon pure vanilla extract Topping: • 1/2 cup peppermint baking pieces

Preheat oven to 350 F. Coat 8-inch square baking pan with nonstick cooking spray; set aside.

To make cheesecake batter: Place cream cheese in bowl of stand mixer.

Beat on medium speed until smooth and creamy. Add sugar and peppermint extract; beat well. Add eggs, one at a time, beating well after each addition.

Add flour and beat mixture until blended.

To make brownie batter: In medium bowl, whisk flour, cocoa powder and salt until combined. In separate bowl, whisk sugar, oil, milk, eggs and vanilla until well combined. Add wet ingredients to dry ingredients and mix until blended.

Pour brownie batter evenly into prepared pan. Carefully pour cheesecake batter over top, spreading evenly. Carefully swirl layers together using knife tip. Bake 20 minutes.

Sprinkle top of brownies with peppermint baking pieces and bake 10 minutes until brownies are set. Cool brownies completely in pan on wire rack before cutting into 16 squares.

To make pudding: Cut bread into bite-sized cubes. If bread is soft and fresh, place cubes on half sheet pan and let stand at room temperature overnight to dry out.

Preheat oven to 375 F. Grease 13-by-9-by-2-inch baking dish with softened butter. Place bread cubes in prepared baking pan. Scatter cranberries over bread.

Beat eggs with sugar, milk, orange zest, orange juice, melted butter, vanilla and cinnamon until well blended. Pour evenly over bread and cranberries to coat. Sprinkle with coarse sugar, if desired. Bake 60-65 minutes, or until custard is set and edges of bread are browned.

To make orange custard sauce: In small saucepan, whisk egg yolks with sugar until lightened to pale yellow color, about 2 minutes. Stir half-andhalf into mixture until combined. Cook over low heat, stirring constantly, 15-20 minutes, or until sauce thickens enough to coat spoon. Stir orange zest into custard sauce.

Serve bread pudding with warm custard sauce over top.

about 3 minutes. Add molasses and beat until incorporated. Beat in eggs, one at a time, then beat in vanilla. Add flour mixture and mix on low speed until just combined.

Divide batter evenly among muffin cups, filling each about 3/4 full.

Bake 30 minutes, or until toothpick inserted in center cupcake comes out clean or with moist crumbs. Let cupcakes cool 10 minutes then transfer to wire rack to cool completely.

To make whipped vanilla buttercream: In large bowl of stand mixer, beat butter on mediumhigh speed until light and fluffy, about 3 minutes.

Add vanilla and, with mixer on low, slowly add in powdered sugar, cream and salt, scraping down sides of bowl as needed.

Once incorporated, whip frosting at least 3 minutes on medium-high or high speed. If frosting is too thick, gradually beat in additional cream. Pipe on top of cooled cupcakes and sprinkle with cinnamon.

Holidays Can Be Celebrated – The Doc With Precautions Dr. Gerard Muraida

h, the virus outside is frightful, and the fire is so delightful. My apologies to Sammy Cahn for altering the lyrics to his song Let it Snow. As the holidays approach and the number of novel coronavirus cases surge, many of us must decide whether to eschew traditional gatherings with ANSWER TO #5146

family and friends or risk spreading the virus.

Infectious disease specialist Anthony Fauci has warned us to limit celebrations to household members. We only have to look back to other holidays this pandemic year to see the danger in acting otherwise. After Memorial Day and Fourth of July, infection rates spiked nationwide.

Weddings, religious celebrations, and funerals have also been linked to Covid-19’s spread.

This doesn’t mean all holiday plans need to be cancelled.

However, celebrations should be altered so that safety takes priority.

We are all in this together, and if we want to see one another next holiday, this season must be celebrated differently.

Virtual gatherings using free online applications such as Zoom and household members-only events are considered safe. If you must gather in person with people beyond your household, limit the number to

that mandated by the State of New Mexico in the governor’s most recent public health order. (That number was five at deadline for this article.)

Gather outside, make sure everyone wears masks, and maintain a 6- to 10-foot distance between households.

Keep the time of the gathering short.

Discourage out-ofstate attendees, unless they have time to quarantine for 10 to 14 days before the gathering, as recommended by the Center for Disease Control.

In the absence of these precautions, an event that may start as an innocent family gathering can become a “super-spreader,” contributing to asymptomatic community spread. Studies show it is 10 times easier to transmit the virus inside as opposed to outdoors.

If it is too cold outside and the celebration must move indoors, open as many windows as possible, separate households at different tables, and ask people to bring their own food. Wash your own dishes to limit cross-contamination. Paper plates are safer to use than regular dishes. Wipe down common areas.

Don’t use serving utensils, and refrain from passing dishes. Avoid caroling or singing in person.

Be safe, be well, and may your days be merry and bright.

oday’s technology can be both life enhancing and life deteriorating.

Phones and computers offer the gift of connecting virtually online when gathering in person isn’t an option, and they allow access to critical information at the push of a button. But when technology becomes ubiquitous in all areas of life, potential adverse side effects may occur.

The radiation these gadgets produce may be dangerous - a good reason to consider a technology detox this holiday season.

Remember when everyone was worried about microwave frequencies being a health disrupter? Now we have dozens of devices that emit radiation in the home. Do you have a Bluetooth or Wi-Fi-capable television, a door camera, earbuds, watch, cell phone, computer, mouse, keyboard, speakers, wireless fan, crockpot, or refrigerator? There are dozens of devices that run off of Wi-Fi at home. Induction cooktops also generate

The Gift Of Presence radiation frequencies. The level of persistent and layered radiation exposure is far beyond what any other generation has experienced. It’s worth asking, “Are there potential dangers to this exposure?”

Both the U.S. Food and Drug Administration (FDA) and the Federal Communications Commission (FCC) have statements on their websites that attempt to dispel any concerns for radio frequency, or ‘RF,’ risk to health and safety. Additionally, the Environmental Protection Agency, the National Institute for Occupational Safety and Health, and the Occupational Safety and Health Administration have been active in the conversation about the potential health risks associated with RF exposure. None of these agencies present conclusive scientific evidence that indicate a health risk in association with RF exposure.

However, the FCC does recommend the following: • Reduce the amount of time spent using your wireless device.

• Use a speakerphone, earpiece or headset to reduce proximity to the head (and thus head exposure). While wired earpieces may conduct some energy to the head and wireless earpieces also emit a small amount of RF energy, both wired and wireless earpieces remove the greatest source of RF energy (the cell phone or handheld device) from proximity to the head and thus can greatly reduce

total exposure to the head.

• Increase the distance between wireless devices and your body.

• Consider texting rather than talking but don’t text while you are driving.

As 5G spreads to more of the country to increase internet speed, more towers and antennas will exist. Even if some of the experts aren’t ready to accept potential biological side effects from all this radiation, consider at a minimum the amount of heat our bodies absorb from proximity to RFs. The allowed RF frequency levels in the U.S. are relatively high compared to the standards set by other countries.

Radiation expert Martin Pall wrote in the Journal of Environmental Research, July 2018, that “Wi-Fi is a threat to human health,” that the effects are “cumulative,” and that there is enough evidence to question the existing science.

radiation, or ‘EMF.’ Technology as a whole has done a lot of good for the environment and the world. Daily, we are learning how to discern our best relationship with it. Every device for legal sale in the U.S. must pass FDA testing to be declared “safe” for human use.

However, the consistent use of many different devices simultaneously over time isn’t well studied, rendering it advisable to take reasonable precautions. Follow that list the FCC proposes. Distance yourself physically from devices whenever possible. Try not to carry your phone on your body. Keep your devices on a table away from you with your hands off of them between texts and typing. Around the house, turn off any electronic device you aren’t using. Detach from tech when you can, and discover the gift of your presence this holiday season!

T is a 61-year-old woman with a seven-year history of type II diabetes mellitus who had enrolled in the New Heart New You program for weight management and exercise training. Four-year previously, she had a heart attack and a coronary artery stent placed.

Subsequently, she remains free of further cardiac problems. She is obese with a BMI of 31, sits at work 7 hours and hates exercise. Her blood pressures have not been well controlled. She was being treated for high LDL cholesterol with a statin.

Her diabetes is well controlled. Her current medications are atorvastatin, lisinopril and metformin.

Diabetes carries an increased risk for heart disease of several types.

Diabetes causes a unique type of heart damage termed diabetic cardiomyopathy. . Cardiomyopathy means heart muscle disease. In diabetes, cardiomyopathy results from obstruction of the small blood vessels or arterials that supply oxygen to the heart muscle. The result is severe reduction in the hearts ability to provide adequate circulation leading to congestive heart failure and a risk for sudden cardiac death The magnitude of the problem of heart failure in diabetes has recently been recognized as an important issue that doctors need to address.

The cause of the heart failure either due to large artery disuse or small artery disease determines the diagnostic and therapeutic approaches rather the focus of pharmaceutical treatments.

Our focus has been preventing atherosclerosis in the major coronary arteries and controlling blood sugar.

preventing and treating heart failure.

Hypertension is a major cause of heart failure both in patients with and without diabetes. At least 40% of patients with diabetes have hypertension. The American diabetes Association recommends a systolic blood pressure of 125 or 130 mmHg. If you use an automated blood pressure device the number should be 120 mmHg as these devices tend to record 5 mm lower than the standard blood pressure measurement.

The choice of medication to treat hypertension is not as important as assuring that the hypertension is controlled. Two classes of drugs protect against kidney failure and the need for dialysis. Angiotensin inhibitors (angiotensin-converting enzyme [ACE] inhibitors and angiotensin receptor blockers [ARBs]) protect the kidneys and help avoid dialysis.

Controlling blood sugar with insulin and a variety of classes of drugs hat do that. Until the development of the new class of drugs, control of blood sugar did not

provide any cardiac protection. The development of heart failure is not protected by blood sugar control, medication. The new class of drugs called sodium-glucose cotransporter (SGLT2) inhibitors (dapagliflozin, empagliflozin, or canagliflozin) has changed all of that .The initial . clinical trials of these drugs intended to demonstrate their benefits on lowering blood sugar and reduce the eye and kidney complications of diabetes. The SGLT2 inhibitors provided an unexpected benefit for the heart. Scientific studies have shown SGLT2 inhibitors reduced the risk of developing heart failure and hospitalization for heart failure in patients with never had heart failure. The damage to the heart causing heart failure was reversed (a term called reverse remodeling).

Additionally, patients who have most severely damaged hearts also benefited from these agents.

diabetes. Ironically a drug originally developed to lower blood sugar protected the heart. Researchers have not established the precise mechanism why this class of drugs acts on the heart.

We reviewed JT’s medication and found it to be based on solid science following national guidelines. She is receiving a cholesterol-lowering drug or statin because patients with

diabetes should be assumed to have coronary artery disease (she already has it) and should have their cholesterol lowered to target.

The current target in our practice is LDL cholesterol lower than 50mg/ dl. At this level, studies have shown reversal of disease is possible.

She is taking lisinopril. The class of drugs called ACE inhibitors protect kidney function and can control her blood pressure.

JT should be utilized for heart

protection. The decision to institute an SGLT2 blocker is very reasonable. to attempt to prevent our failure. These agents have proven safe, help with weight loss and are well tolerated. They are expensive at nearly $600.00/month. They will be the standard of care for patients with diabetes and will have an important role in caring for patients with heart failure of a variety of causes.
