Rolling Out The Red Carpet Of Higher Learning For Albuquerque’s Senior Students

hile students across the state stuff their backpacks and sharpen their pencils, Albuquerque’s own halls of higher learning are rolling out the red carpet for some its more nontraditional students. Seniors can enjoy cool discounts, special programs and more to continue their life of learning.

Because of a New Mexico state statute, Central New Mexico Community College and the University of New Mexico offer students older than 65 classes for just $5 per credit hour, up to six credit hours per semester. With only a few restrictions, the bulk of the CNM and UNM course catalogs are open to seniors who want to expand their minds and meet new people. Oasis Albuquerque, part of a national lifelong learning organization, aims to be a hub of activities and information for seniors who want to learn more. Albuquerque is one of nine Oasis adult education centers nationwide. Among its offerings this fall, seniors can sign up to be a part of conversations about the Supreme Court and timely government issues, learn about African history and archaeology, or they can learn to dance like no one is watching. Each course is designed for personal enrichment and senior engagement, and classes cost members as little as $8. The University of New Mexico’s Continuing Education department also has built a program specifically for seniors that includes discounts on more than just class time. The Osher Lifelong Learning Institute is a membership program for people 50 and up who are committed to learning. Classes are held at the UNM Continuing Education building, and course topics include art and art history, economics and finance, history, science and technology, philosophy and religion, travel and culture, and more. Membership costs $20 per year, and additional benefits

include a free monthly lecture, free materials check-out from the library, theater discounts, and discounts at local restaurants and fitness centers.

Local senior and multigenerational centers are offering classes throughout the fall, including for exercise and education. Upcoming courses include gardening topics and fitness.

The Albuquerque Public Library system offers a host of classes to help adults improve their tech skills.

Courses are ongoing throughout the year, but each library also offers one-on-one support. Classes include social media, employment basics, typing and email, digital photography, and computer basics. Visit your local library to learn more or go to abqlibrary.org/adultlearning/classes.

Albuquerque Little Theater and New Mexico Art League are two great options to get engaged with the local art scene. ALT offers classes to help adults get into acting, while NMAL helps aspiring artists improve techniques and learn new skills. When Pianos Duel Everyone Wins

his year, with the help of Albuquerque’s own Uptown Funk Dueling Piano Bar, the attendees of the Harvest Wine Festival will be treated to live performances of some of the best piano players in the country while they duke it out, piano-topiano, key-to-key. Two Men Of Vision And The Parks They Created

A

nyone walking today in the shade of the majestic cottonwoods that line both banks of the Río Grande where

it flows through Albuquerque probably wouldn’t guess that much of the Bosque once served as a city dump.

“There are 10 trainloads of tin cans out there,” declared conservationist and civic booster Aldo Leopold in 1917, launching a campaign to clean up the Bosque and preserve it for future generations as a riverfront park.

Nevertheless, he noted, people went there to swim, to fish, and simply to connect with nature “in spite of the fact that the river has been made unattractive to the limit of human ingenuity.” He encouraged landowners in the area between Old Town and the Barelas Bridge to donate land for the proposed park. Some agreed, but others balked, and so the park did not materialize.

Perhaps it would have if Leopold had remained in Albuquerque to push it forward, but he made his home here for only five years, from 1915 to 1920. Then he was called away by his employer, the U.S. Forest Service, to duties elsewhere.

In 1933, he retired to his farm in Wisconsin, where he wrote the environmental classic, A Sand County Almanac, which includes reminiscences about his time in the Southwest. It was published posthumously in 1949 and is still in print.

At around the same time Leopold was lobbying for the preservation of the Bosque, another young man was beginning his rise to prominence in Albuquerque politics. His name was Clyde Tingley. A native of Ohio, Tingley brought with him a Midwesterner’s love of greenery when he and his wife, Carrie, settled here in 1911. He soon entered local politics and was elected Alderman in 1916. Then, when the city charter was revised to create the City Commission, he became

its chairman and ex-officio Mayor — a title he readily laid claim to.

He went on to serve two terms as New Mexico’s Governor (1935 to 1938), then returned to his seat on the City Commission, which he held until 1955.

When the Middle Río Grande Conservancy District was created in 1925 to oversee flood control and irrigation projects in a broad stretch of the river that included Albuquerque, Tingley flexed his political muscle to persuade the agency to set aside an area for recreation adjacent to what is now the Albuquerque Country Club.

Work on the project began in 1931.

By then, in addition to the mountains of tin cans, the Albuquerque Tribune reported that some 13,000 junked cars also lay rusting in the Bosque. That figure may be an exaggeration, but there was undoubtedly an awful lot of trash to be hauled away. Once that

was removed, an artificial lake was scooped out of the sand and river water diverted to fill it. Lifeguard towers, a bathhouse, a snack bar and other amenities were added, and Conservancy Beach, as it was then known, became the city’s favorite swimming hole. Later the name was changed to Ernie Pyle Beach, and later still to Tingley Beach.

Polio panics closed the lake to swimming in 1946 and ’47, and led to its permanent closure in 1952, although no cases of polio were ever linked to it. The bathhouse and other structures fell into disrepair and were subsequently torn down. After

Clyde Tingley’s beautification efforts did not end at Conservancy Beach. Throughout his long career he worked for the creation of other city parks, the largest of which was Terrace Park, which Tingley soon renamed Roosevelt Park, in recognition of the fact that President Franklin D. Roosevelt’s New Deal programs had provided most of the funding.

Located south of Coal Avenue between Spruce and Maple Streets, the site had been an arroyo cutting through the sandhills and, like the

decades of neglect, a $16.5 million renovation of Tingley Beach was completed in 2005. It has now been incorporated into the Albuquerque BioPark and is once again a popular spot for fishing, strolling and feeding the resident population of waterfowl.

Conservancy Beach was a start, but the rest of the Bosque remained a no-man’s land, crisscrossed by rutted roads and still used for illegal dumping, woodcutting and other destructive activities. It was far from the expansive riverside park that Leopold had envisioned. That did not become a reality until the establishment of Río Grande Valley State Park and the Río Grande Nature Center in 1983, 65 years

Bosque, had frequently been used as a dump. Skeptics scoffed that nothing but tumbleweeds would ever grow there, but Tingley pressed ahead in his characteristic get-itdone style, enlisting a labor force of some 200 workers idled by the Depression. Building stone salvaged from the recently-demolished county jail was reused to construct a retaining wall that still exists at the park’s southern end. It opened in 1934 and was instantly popular as a picnicking spot, as it still is.

How Medicare Covers Diabetes diabetes services and supplies along with some other tips that can help you save.

ear Savvy Senior, How well does Medicare cover diabetes?

I’m 66 years old and was recently told by my doctor that I have pre-diabetes. If it progresses to full-fledged diabetes what can I

expect from Medicare.

Recently Retired Dear Recently, Medicare actually provides a wide range of coverage to help beneficiaries who have diabetes, as well as those who are at risk of getting it – but they don’t cover everything.

Here’s a breakdown of what Medicare covers when it comes to

Screenings: If you have prediabetes or some other health conditions that put you at risk of getting diabetes – such as high blood pressure, high cholesterol and triglycerides, are overweight, or have a family history of diabetes – Medicare Part B (medical insurance) will pay 100 percent of the cost of up to two diabetes screenings every year.

Doctor’s services: If you’re a Medicare beneficiary, Medicare will pay 80 percent of the cost of all doctor’s office visits that are related to diabetes.

You are responsible for paying the remaining 20 percent after you’ve met this year’s $183 (for 2018) Part B deductible.

Prevention program: Just launched in April, the Medicare Diabetes Prevention Program provides lifestyle change programs offered by health professionals to help you prevent diabetes. This is available for free to all Part B beneficiaries who have pre-diabetes.

Self-management: If you have diabetes, Medicare covers 80 percent of the cost of selfmanagement training to teach you how to successfully manage your diabetes.

Supplies and medications: Medicare Part B covers 80 percent of the cost of glucose monitors, test strips (100 per month if you use insulin, or 33 per month if you don’t), lancets, external insulin pumps and insulin (if you use a pump), after you’ve met your deductible.

If, however, you inject insulin with a syringe, Medicare’s Part D prescription drug benefit will help pay your insulin costs and the supplies needed to inject it – if you have a plan. Part D plans also cover most other diabetic medications too. You’ll need to check your plan for coverage details.

Nutrition therapy: Medicare will pick up the entire tab for medical nutrition therapy, which teaches you how to adjust your diet so you can better manage your condition. You’ll need

a doctor’s referral to get this service.

Foot care: Since foot problems are common among diabetics, Medicare covers 80 percent of foot exams every six months for diabetics with diabetes-related nerve damage. They will also help pay for therapeutic shoes or inserts as long as your podiatrist prescribes them.

Eye exams: Because diabetes increases the risks of getting glaucoma and diabetic retinopathy, 80 percent of dilated medical eye exams are covered each year, but eye refractions for glasses are not.

Other Insurance If you have a Medicare supplemental (Medigap) policy, it may pay some of the costs that Medicare doesn’t cover. Call your plan’s benefits administrator for more information.

Or, if you’re in a Medicare Advantage plan (like an HMO or PPO), your plan must give you at least the same diabetes coverage as original Medicare does, but it may have different rules.

You’ll need to check your policy for details.

Financial Assistance If you’re income is low, and you can’t afford your Medicare out-of-pocket costs, you may be able to get help through Medicare Savings Programs. To find out if you qualify or to apply, contact your state Medicaid program.

ummer grilling season is slowly winding down, but there is still plenty of time to celebrate the sizzling hot month of August by firing up the grill to make delicious food created by three of my favorite chefs: Michael Symon, Carla Hall and Clinton Kelly. Their show, The Chew, was sadly recently canceled.

First on the menu is from The Chew’s seventh annual summertime episode, Symon’s jerk chicken skewers, which he made to pair with Kelly’s watermelon agua fresco. Top off your menu by whipping up Hall’s grilled banana boats.

Symon’s jerk chicken skewers look delicious, and I think Burqueños will enjoy them as they pack a nice punch.

Ingredients: • 4 scallions (root ends removed, roughly chopped) • 2 (or more to your desired level of heat) scotch bonnets or habanero peppers (seeds and ribs removed, roughly chopped) • 4 cloves garlic (peeled, roughly chopped) • 1 (2-inch) piece ginger (peeled, roughly chopped) • 1/4 cup fresh thyme leaves • 3/4 tablespoon cinnamon • 1 tablespoon allspice • 3 tablespoons light brown sugar

• 2 limes (zested, juiced) • 2 tablespoons apple cider vinegar • 1 tablespoon soy sauce • 3 pounds chicken thighs (boneless, skinless, cut into 1-inch pieces) • 6 (6- to 8-inch) wooden skewers soaked in water • Olive oil (to drizzle) • 3 limes (halved) • Kosher salt and freshly ground black pepper (to taste)

Directions: In a bowl of a food processor fitted with a blade attachment, add the scallions, scotch bonnet or habanero peppers, garlic cloves, ginger, thyme leaves, cinnamon, allspice, brown sugar, lime zest and juice, apple cider vinegar, soy sauce, and salt and pepper to taste. Puree mixture until smooth. Remove the mixture to a large zip-top bag along with the chicken, and allow to marinate for at least two hours or overnight.

Preheat grill or grill pan over medium-high heat. Remove chicken from marinade and thread chicken pieces halfway up the skewers, about four pieces per stick. Drizzle with olive oil and place on the grill. Repeat with the rest of the chicken. Cook until charred on all sides and chicken is cooked through or a meat thermometer reaches 165 degrees F, about four to five minutes per side. Remove to a large serving platter.

Drizzle the cut side of the lime halves with olive oil and place flesh down on the grill. Cook until charred, about two to three minutes. Add the limes to the serving platter and enjoy!

Tip: Store any leftover jerk marinade in an airtight container, and use on any chicken, pork or swordfish dish.

Sparkling watermelon agua fresco: • 1 (14- to 15-pound) seedless watermelon (peeled, cut into 2-inch cubes) • 1 cup silver tequila • 1/2 cup water • 7 limes (six sliced into rounds, one zested and juiced) • 1/4 cup Kosher salt • Prosecco (to top) Directions: 1. For the basil simple syrup: In a small saucepan over medium-high heat, add the water, sugar, and basil leaves, and bring to a boil. Cook, stirring occasionally until sugar has dissolved, about one to two minutes.

Strain through a fine mesh sieve, and discard basil. Add the simple syrup to a large pitcher and set aside.

2. For the sparkling agua fresco: In the carafe of a blender, add the watermelon, and puree in batches. Strain the pureed watermelon into the large pitcher as you go, and repeat until all of the watermelon has been strained into the pitcher. Add the tequila, water and lime rounds to the pitcher and stir to combine.

3. In a shallow bowl, add the kosher salt and lime zest, and stir to combine. Dip the rim of each coupe glass in the lime juice and then into lime-salt. Repeat with remaining glasses and fill with crushed ice. To serve, nearly fill each glass with watermelon mixture, top with Prosecco, and serve!

4. Tip: Substitute the watermelon with your favorite tropical fruit, like pineapple, for a tasty twist.

Ingredients: • 4 bananas • 1/2 cup mini chocolate chips • 1/2 cup mini marshmallows • 8 Milk Maid caramels (cut into smaller squares) • 1 orange (1-inch strips of zest) • Maraschino cherries (optional, to garnish) • Sprinkles (optional, to garnish) • Chopped toasted peanuts (optional, to garnish) • Vanilla ice cream (optional, to garnish) • Chocolate ice cream (optional, to garnish) • Strawberry ice cream (optional, to garnish) Directions: 1. Preheat a grill pan to medium heat.

2. Remove the banana from the peels. Slice the bananas down the middle lengthwise, being sure not to cut all the way through. Carefully open up the banana creating a space in the middle to fill.

3. Place each of the bananas on its own piece of aluminum foil. Divide the chocolate chips, marshmallows, caramel and orange zest evenly between the bananas, filling the space that was created in the middle.

4. Loosely fold up the sides of the foil packet toward the center, being sure to leave a little space between the banana and the seam at the top.

5. Place on the grill and cook for 10-12 minutes.

6. Remove from the grill and carefully open the packet. Serve in the foil and garnish with your favorite toppings.

Grandparents Preparing For A New School Year

hirley and Doug Mitchell have been raising their 13-year-old granddaughter, Haley, for the last three years.

She’s bright, kind, helpful and a straight shooter, much like Shirley.

And together, the family is gearing up for a major moment in the life of any child: eighth grade.

Experts who track such figures say that the Mitchells will spend between about $700 and $1,500 this year on school supplies, a number that has nearly doubled in the last decade. Huntington National Bank’s annual Backpack Index follows school supply trends, including the fact that rising spending figures regularly outpace inflation.

In New Mexico, according to Census figures, 26,241 grandparents are householders responsible for their grandchildren who live with them. About a third of that figure includes grandparents who are raising their grandchildren without their parents present at all. The Mitchells are in that latter group.

Shirley, now 57, and Doug, 71, had been married for four years when Haley was born, their only grandchild and the only girl in a family full of boys. Shirley and Haley were instantly bonded, and Haley spent most of her weekends with her grandparents. Her parents struggled with addiction issues that interfered with their ability to raise Haley, and the Mitchells quickly stepped in to be Haley’s full-time guardians.

They join a growing number of grandparents who are raising their grandchildren. The Mitchells are part of what the New Republic earlier this year dubbed “The Grandparenting Generation” and

what nonprofit group Generations United calls “grandfamilies.”

“I have been wanting to start a grandparents-as-parents support group or something,” said Shirley.

“I think it’s more common than people realize, but I think it creates a unique set of problems we have to work through.

Sometimes Doug and I feel like we don’t fit in anywhere; people our age don’t have little kids at home, and the people who do have kids Haley’s age aren’t our age.”

They are shopping for school supplies and new clothes for a 13-year-old girl for the first time in their lives. Haley will attend a new school this year, which means a new set of clothing guidelines, new expectations and rules, new supply requirements, and new grandparent/guardian-teenage girl negotiations. Shirley had three adult sons before marrying Doug, who had two adult sons of his own (continued on next page)

from a previous marriage. They say that in the time between their children going to middle school to Haley’s entrance into eighth grade, so much has changed.

“Things are more expensive now. I don’t remember needing to buy five folders in different colors in each class; they require so much more now,” Shirley says. “I don’t remember buying reams of paper. I remember buying Kleenex, but now they also ask for Clorox wipes. The (school supply) list is longer, and the requirements for what they need at home are different.

They need a computer to submit their papers now,]; they don’t just turn in a paper. We order her lunch electronically. We just have to make so many more decisions now.”

She is also giving back to her community in ways that she knows would have been helpful to her as a single mother. At work, she renovated a supply closet by adding shelves and hangers, where she personally invites fellow UNM employees to find outfits and accessories for their children and grandchildren. She also delivers bags of curated outfits to single moms at her church.

She does this, she said, because she knows that school shopping is overwhelming at every age, including for grandparents caring for a teenage girl.

Should I get tested for osteoporosis?

The National Osteoporosis Foundation has several recommendations for who should receive a

bone density test, a painless test that uses very low levels of radiation to measure the amount of minerals in your bones, usually in your hip and spine. In general, women and men ages 50 and older who have risk factors for osteoporosis should be screened. For additional information, check with your doctor or visit the National Osteoporosis Foundation website at https://www.nof.org.

Are women more likely than men to get osteoporosis?

While women have a greater risk of getting osteoporosis, the disease can affect both men and women. Older Asian and white women are at highest risk, but younger people can also get osteoporosis. Other risk factors include insufficient dietary calcium intake, eating disorders such as anorexia, and medications such as prednisone. People who have lived in northern areas of the country that do not get enough exposure to sunlight can also be at higher risk.

I’ve read that calcium can help prevent osteoporosis. How can I make sure I am getting enough calcium?

Calcium is good for bone health, and the best approach to make sure your body is getting enough of this important mineral is to maintain a nutritious diet that includes vegetables, fruits, whole grains, lean proteins, beans and dairy. Men and women between the ages of 18 and 50 require 1,000 milligrams of calcium every day. Women older than 50 and men over the age of 70 should have 1,200 milligrams of calcium daily. Vitamin D is also important for bone health. The best amount of vitamin D is not clear, but a good starting point is 600-800 international units (IU) a day.

What kinds of foods are high in calcium?

Dairy products – such as yogurt, milk and cheese – are rich in calcium. Ricotta cheese is especially high in calcium; according to an estimate from the National Osteoporosis Foundation, part-skim ricotta cheese contains more than 300 milligrams of calcium in a four-ounce serving.

Be sure to check food labels for nutritional information.

What if I don’t eat dairy?

Many non-dairy foods are high

in calcium. Sardines, edamame, tofu, broccoli rabe and white beans are good sources of calcium.

Certain leafy greens such as kale, collard greens and spinach are also calcium-rich.

What else should I be doing to take care of my bones?

Avoid smoking, limit alcohol intake and stay active. Exercise can help build strong bones and slow bone loss. Combine weight-bearing exercise with strength training. If you have access to a swimming pool, water exercising is a great way to work out. Peak bone mass is usually attained by your early- to mid-20s. Individuals will get the most benefit if they start exercising when they’re young and stay active throughout their lives. If you have adult children, encourage them to exercise regularly and get plenty of calcium and vitamin D. Are there medications for osteoporosis?

There are medications that can decrease the risk for fracture from osteoporosis. One class of medications is called bisphosphonates, which can be prescribed based on your risk factors. If you are concerned that you might have osteoporosis, talk to your doctor.

here are approximately 225 species of ants in New Mexico. That is more species of ants than live in all of Europe. While the vast majority never enter homes and you will never see them, there are a few

species that will occasionally.

Those need to be properly identified before a treatment is performed.

Proper Identification Critical For Solving Some Ant Problems Recently, I went to a house where a family had an infestation of ants. Two companies gave them prices on the phone without knowing what kind of ants the family had. Both suggested spraying and baits. When the family called me, I was unable to identify the ants even after visiting the home. I knew they were one of 33 species of big-headed ants (genus Pheidole), but I couldn’t determine which until I put them under a microscope.

In this case, the family had the species Pheidole desertorum, which has very large colonies and usually goes under large boulders when winter approaches. They are not common house pests, but in this case, they burrowed beneath the home. They needed water, so they came into the house via the expansion joints and headed for the sink. The family tried some store-bought baits to

control the population, but the ants ignored them.

Baits don’t work very well with these ants because they feed on seeds. They won’t take a bait unless it is in a seed-like form. The only bait I am aware of that may work with these ants is Niban Bait in fine grade, made from boric acid.

This bait is only available online.

I recommended the family control the ants by spraying along the baseboards with a cedar oil product.

There are several brands. One is Wondercide. This will only last a week or so, but it is absolutely safe for pets. I also suggested they fill their sinks with a few inches of water and a little soap. If the ants wanted water, they would go into the sink, which is what they did. They caught hundreds of ants in the sink this way. I also recommended that they put some diatomaceous earth behind or

under the baseboards, wherever it was possible, since it would not be practical or possible to seal all the expansion joints around the perimeter of the slab of the house.

Diatomaceous earth will also last a long time. Using these remedies, the family got the ants under control, almost eliminating them without using any pesticides.

There is more to controlling ants than identifying them as “sugar ants,” spraying the baseboards and putting out toxic bait stations.

Make sure any company you use to control ants is competent in myrmecology and can properly identify them. In most cases, you can control the ants yourself as long as you get them identified. Classifieds The Foster Grandparent Program (FGP) Advisory Council Needs members to join its efforts in promoting awareness and educating the community about FGP and senior issues. Council members advocate on behalf of FGP volunteers, evaluate the Program’s effectiveness, and assist in the recognition of Foster Grandparents by raising funds and in-kind resources. Foster Grandparent Program (FGP) is looking for volunteers age 55 and older who are interested in making a difference in a child’s life. Foster Grandparent volunteers must be willing to serve an average of 20 hours per week and must be able to pass a background check, physical, and tuberculosis test.

Foster Grandparent volunteers serve one-on-one with children, working with them to build their social and emotional development as well as help improve their literacy skills. Volunteers receive mileage reimbursement, supplemental liability insurance, meals while on duty, pre-service and monthly trainings, and recognition throughout the year.

How can today’s astrobiologists and interested citizens be “Leonardo-like” as we contemplate the question of whether there are other lifeforms in our own Solar System and beyond on the myriad of exoplanets around our neighboring stars?

Dr. Boston, Director of NASA’s Astrobiology Institute, is an internationally recognized scientist who also explores art along with the interdisciplinary aspects of astrobiology. She studies extremophiles (extreme microbial lifeforms) on Earth and the possibilities of similar life on other planets. Prior to her appointment with NASA she was founder and director of the Cave and Karst Studies Program, and faculty member at New Mexico Institute of Mining and Technology in Socorro. She has been featured in many science documentaries shown on PBS and National Geographic. She has a B.S. in microbiology, geology, and psychology, and a M.S. in microbiology and atmospheric chemistry. She earned her Ph.D. from University of Colorado Boulder.

Healthy Fascia Key To Pain-Free Movement Healthy, flexible fascia can positively support a moving physical body, and unhealthy, rigid fascia can lead to pain, injury and overall lowered body functioning.

The fascia is involved in how the body communicates its spatial relationships (proprioception) to the brain and has a significant role not only in maintaining static posture but also in dynamic movement and efficient energy transfer throughout the body.

Trauma affects the fascia where collagen fibers adhere together, causing fixtures that act like “snags” in body movement. These “snags” cause resistance within the interior fabric of the body.

A healthy physique with supple, flexible fascia allows for dynamic pain-free local movement. Fascial congestion, or “snags,” may result in painful movement that tugs and pulls upon separate areas of the body and tissues. Tight fascia can be painful, exhausting and overall bad news for optimal body functioning.

For healthy, functioning organs, agility and less pain, pay close attention to your fascia. The research on the role fascia plays in many illnesses is growing. A research paper published in April 2018 by the International Journal of Chronic Obstructive Pulmonary Disease (COPD) found multiple aspects of fascial involvement in COPD pain. Fascia is intricately innervated and envelopes organs, so when fascial tissue contracts, it does so to all that it holds, anywhere in the body.

Stretching, exercise and massage can help. When you stretch or manipulate your body, you exercise, nourish and unleash fascial layers from the inside out.

Purchase a foam roller (or begin with a tennis ball), and experiment with rolling your fascia out like you would pizza dough. You can use a foam roller head to toe, on the floor or while in bed. Just position it below the weight of your body and gently move back and forth comfortably. Some folks with plantar fasciitis like to lie in bed and roll out their calves. Those with back pain prefer to place the roller beneath the gluteus to roll out the muscles that become tight from too much sitting.

However, supplementing collagen is controversial since it breaks down in the digestive system into the same amino acids food sources provide. Also, bone and cartilage are concentrated parts of an animal, and unless you go with a high-quality source that tells you exactly where they source the animal parts and how they test their products, you don’t know what you’re getting.

For this reason, instead of supplements, I recommend hydrating soups that include bone, skin and cartilage parts (pastured chicken, grass-fed beef or sustainably raised fish). This will help increase dietary consumption of amino acids that may assist in providing the body with ingredients for collagen production. If you are compelled to supplement, look into hydrolyzed collagen with U.S. Pharmacopeial Convention (USP) or National Sanitation Foundation (NSF) approval.

fter a recent summer evening dining out, I found myself unpleasantly reliving the tastes of what had been a most exquisite meal. How does this gastric phenomenon occur, and why?

Called gastroesophageal reflux, commonly referred to as “GERD,” this is a condition in which food or liquid travels backward from the stomach into the esophagus (the tube from the mouth to the stomach). This reversal of stomach contents can irritate the esophagus, causing heartburn and other symptoms. The reflux may be related to a problem with the lower esophageal sphincter, an area of increased pressure in the lower esophagus that essentially closes off the esophagus from the stomach. If this sphincter doesn’t close properly,

Don’t Take Reflux Lying Down food and liquid can move backward into the esophagus and may cause the symptoms. Risk factors for reflux include hiatal hernia, obesity, scleroderma and pregnancy. Even people who don’t suffer from any of these maladies can be victimized.

For simplicity, I will limit this discussion to hiatal hernia and reflux. Although it can occur at any age, the disorder has the highest incidence in seniors. Hiatal hernias are also common in overweight people and in people who frequently sustain intense physical effort (weight lifting). A hiatal hernia is a common type of hernia that occurs when the wall of the stomach presses against the esophageal opening in the diaphragm. When the opening is weak, the stomach may be pushed upwards above the diaphragm causing the stomach and its contents to travel upward into the esophagus.

Seniors can occasionally be devoid of clear cut symptoms, but most commonly reflux causes heartburn, which usually intensifies after meals. It often gets worse when a person bends over or lies down. Reflux disease may present

as difficulty swallowing, belching, (acidic or sour-tasting) and a chronically sour or bitter taste in the mouth. Chest or upper abdominal pain, especially pain that disrupts sleep, hoarseness, especially in the morning, a sore throat, coughing, wheezing or a repeated need to clear the throat are symptoms often seen in the senior population.

How is a hiatal hernia treated?

Treatment options include both surgical and non- surgical approaches. Treatment of a large para-esophageal hernia causing symptoms requires surgery. During surgery, the stomach is pulled down into the abdomen, the esophageal hiatus is made smaller, and the esophagus is attached firmly to the diaphragm. This procedure restores the normal anatomy.

Non –surgical approaches to hiatal hernias mimic the approach to GERD. Some general recommendations include: • Weight reduction • Avoiding lying down after meals • Sleeping with the head of the bed elevated

• Taking medication with plenty of water • Avoiding dietary fat, chocolate, caffeine, peppermint (they may lower esophageal pressure) • Avoiding tomatoes and citrus fruit, which may precipitate symptoms • Avoiding alcohol and tobacco

Your health care provider may prescribe antacids after meals and at bedtime. Histamine (H2 receptor) blockers, promotility agents and proton pump inhibitors are other medications that may be prescribed.

Most individuals respond to the behavioral modification measures mentioned above. Avoidance of aggravating foods is key.

The consequences of ignoring reflux include inflammation of the esophagus, stricture (closure) or ulceration of the esophagus, hoarseness, bronchospasm and chronic lung disease. Barrett’s esophagus (a change in the lining of the esophagus) can also develop and can increase the risk of cancer.
