How much alcohol is too much?

Factors such as age, gender, beverage type and alcohol content are considered when determining how much alcohol is too much. In the United States, a standard drink is any drink that contains 14 grams of pure alcohol, which is about 0.6 fluid ounces or 1.2 tablespoons. Generally, this means one 12-ounce beer equals one standard drink, one bottle of wine equals five standard drinks, and a fifth of hard liquor equals 17 standard drinks. Mixed drinks can contain one or more standard drinks depending on the recipe.

For adults age 65 or older, moderate drinking — or relatively low risk for alcohol problems — is

no more than two drinks on any single day and no more than seven drinks per week. Anything above this is considered heavy drinking and an increased risk for alcohol problems.

For some, no level of alcohol intake can be regarded as safe.

Examples of these situations include having a personal or family history of alcoholism, being pregnant, or having liver disease.

What do studies show on alcohol and increased age?

As the body ages, the movement of drugs within the body changes. Older adults have slower rates of metabolism and lower body mass. This means alcohol levels can accumulate in the body, leading to intoxication with lower amounts of alcohol intake. Older adults are more vulnerable to the side effects of alcohol than younger adults.

Alcohol can affect overall health. Some studies show reduction in cardiovascular death with low to moderate alcohol intake. If you have a cardiovascular condition, be sure to discuss your alcohol intake with your health care provider. Problem drinking can decrease one’s quality of life, increase disability, and increase mortality risk. Alcohol use has been shown to increase risk for cancers in the digestive system, breast cancer in women, and liver cancer that results from alcoholic liver disease. People with gout are at increased risk for flares with too much alcohol intake.

Osteoporosis, which is thinning of the bones, can be related to heavy alcohol use and result in falls and broken hips. Accidents, violence, depression and suicide are also associated with alcohol use. Long-term alcohol abuse can result in heart, nerve and brain damage. Alcohol can lower the

This is the medical term for alcoholism or addiction. People who have alcoholism have two or more of the following problems: drink more or for a longer time than planned, wish they could cut down but can’t, spend a lot of time trying to get alcohol or being drunk, crave alcohol, can’t do the things they want to because of their alcohol use, keep drinking when things in life get worse, keep drinking even in dangerous situations or when they know it affects their mental or physical problems,

need to drink more to get the same effect, or have withdrawal symptoms when they stop drinking. Withdrawal symptoms include sweating, racing heart, trembling, insomnia, anxiety, seizures, nausea/vomiting, restlessness and generally not feeling well.

What can I do if I think I have a problem with alcohol?

Many older adults do not share information about drinking concerns because of embarrassment or shame. Many are also isolated from family and social networks, especially during the COVID-19 public health emergency; isolation can lead to increased mental stress and increased alcohol intake.

available. There are several treatment options ranging from medications, counseling and support groups. Admission into a recovery program is available for those with a more serious alcohol problem. The Little Dinosaurs Of Ghost Ranch

E

very U.S. state has official mascots of various kinds: a state bird, a state tree, a state flower, and so on. New Mexico has a slew of them, including even a state fossil, a dinosaur that lived near the end of the Triassic Period, more than 200 million years ago. The story of its discovery is a fascinating one involving a bit of serendipity, as well as solid scientific detective work.

The initial find was made by self-taught fossil hunter David Baldwin, working under contract

for Philadelphia paleontologist Edward Drinker Cope. In 1881, Baldwin unearthed a few scattered, fragile bones at two locations in southern Rio Arriba County and shipped them off to Cope. After studying them, Cope assigned them to a previouslyunknown genus of early dinosaurs that he named Coelophysis (pronounced SEE-lo-FY-sis), meaning “hollow form,” because the bones were hollow, like those of modern birds. But the find was considered of only minor importance, and for decades the bones lay mostly forgotten in the catacombs of the American Museum of Natural History in New York, which had acquired Cope’s collections.

and specialist in the vertebrate fauna of the Triassic, was headed west as the leader of an expedition sponsored by that same museum.

He was to conduct a dig at Petrified Forest National Park in northern Arizona. On the way, the group decided to stop off at Ghost Ranch in the Chama River Valley near Abiquiu and spend a few days prospecting for fossils there.

Ghost Ranch (so called because local legend claimed the area was haunted) is today a retreat and conference center operated by the Presbyterian Church. But in 1947, it was a private ranch owned

by Arthur and Phoebe Pack.

The Packs were supportive of scientific endeavors and welcomed the visitors.

There they made a startling discovery, described by Colbert in his delightful 1995 memoir, The Little Dinosaurs of Ghost Ranch, published by Columbia University Press. At the base of a cliff, in a layer of crumbly siltstone, an upper member of the Triassic Chinle Formation, some dinosaur bones were exposed.

A little digging revealed dozens more. Colbert, who was familiar with the Cope specimens, quickly identified them as belonging to Coelophysis.

Now they faced a dilemma.

The site looked promising, but until they dug deeper into the rock they couldn’t know for sure.

Obtaining the permission of the National Park Service for the proposed dig at Petrified Forest had entailed a lot of bureaucratic red tape, an effort that would be wasted if they abandoned that plan in favor of spending the summer excavating the new bone bed at Ghost Ranch. Colbert called a meeting of his group and together they decided that the Ghost Ranch site was too rare of a find to pass up. They voted to remain there.

The Packs were agreeable and even gave them the use of one of the ranch buildings to serve as their headquarters.

It was a fortuitous decision.

As the dig progressed, it became apparent that this was one of the largest concentrations of dinosaur fossils ever discovered. Hundreds of skeletons were revealed, many of them complete in every detail.

It appeared that the reptiles had all died at the same time, possibly in a flash flood, and that their bodies

had been swept away and buried in streambed sediment before scavengers could dismember them. Over tens of millions of years the silt that entombed them hardened into stone, which was then buried under hundreds more feet of sedimentary rock.

The sedimentary rock was then uplifted to become the colorful layer-cake cliffs that attract artists, as well as scientists, to the Ghost Ranch area today.

Ansel Adams and Eliot Porter photographed here. Georgia O’Keeffe lived here for decades and made several visits to the Coelophysis quarry during the time it was being excavated. She, too, had a fascination with old bones, as anyone who has seen her paintings can attest.

Work at the site continued through that summer and the one following. In all, 13 large blocks, weighing more than two tons each, were quarried out, jacketed in burlap and plaster to protect them, and trucked off to museums around the country. Skilled preparators using dental tools and tiny chisels then began the painstaking process of scraping away the rock matrix to expose the bones. Work on a single block can take years to complete, and some blocks removed by a later excavation at Ghost Ranch in the 1980s are still being processed.

The fruit of all this labor is that we now know much more about these ancestral dinosaurs than we do about some species that lived millions of years later. An adult Coelophysis measured about 10 feet from the snout to the tip of its long tail, but would have weighed less than 100 pounds.

This dinosaur was light and agile and ran on hind legs, probably at a top speed of about 25 mph. It was a meat eater and may at times have been cannibalistic. Its brain was larger, in proportion to body size, than that of most dinosaurs, so it was probably an alert and intelligent predator. Coelophysis may have been warm-blooded, like modern birds and mammals.

Altogether, an elegant beast to serve as our state fossil.

Practical Tips For Caregivers Facing A Pandemic

W

hether your role as a caregiver has you looking out for an elderly relative, children or both, chances are good that you’ve worried about how the COVID-19 pandemic may continue to affect your ability to provide necessary care.

Following the most up to date guidelines from the Centers for Disease Control and Prevention is the first step for caregivers who are looking after loved ones.

As caregivers continue to adjust during the pandemic, keep these considerations in mind: • Contact health care providers to obtain extra necessary medications and stock up on over-the-counter drugs.

• Monitor needed medical supplies related to a loved one’s condition or treatments (oxygen, incontinence, dialysis, wound care, etc.) and common supplies such as tissues and cough syrup.

• Those with a loved one in a long-term care facility should inquire about any positive COVID-19 cases in the facility,

testing procedures for the residents and staff members and be aware of the facility’s protocol if there is an outbreak or positive tests among residents and staff.

These additional tips can help caregivers reduce the pandemic’s impact: Make backup plans. Most caregivers have plans in place for temporary assistance when things go awry, but COVID-19 is putting many of those short-term solutions to the test. It’s a good time to pull in additional resources so you have extra help waiting if someone you’re counting on falls ill or can’t fill in as planned. A meal delivery service may be a good option if grocery shopping and meal preparation continue to be affected.

Reduce exposure. Those who take care of loved ones in their homes or are regular care providers to family members and friends have concerns about exposing this vulnerable group to the virus.

Many long-term care facilities have changed their visitation policies.

You might be able to visit a loved

one through a window, via a balcony or through video chat. It’s also important to minimize time spent out in the community where you could unknowingly contract the virus and pass it to a vulnerable loved one.

Shop smart. Because supermarkets and stores with goods identified as “essential” are still bustling with people, it’s important to minimize extra trips and wear a mask when in public. If possible, drop groceries and essentials at the door or arrange for delivery. In addition, some major pharmacies, where AARP members get special benefits on health, wellness and beauty purchases, have introduced special shopping hours for seniors and drive-thru shopping options to minimize person-to-person contact.

Reschedule wellness appointments. Not only are doctor’s offices short on resources, a waiting room can be filled with germs that may cause illness. Try to arrange for telephone or videobased appointments when possible

and cancel any appointments that aren’t urgently necessary.

Keep germs away. Thorough handwashing with soap and water is critical. In addition to washing hands after eating and using the restroom, anyone entering and leaving the house should wash his or her hands. Also wipe down high-touch surfaces like doorknobs, remotes and phone keypads.

Combat boredom. Despite the good intentions of staying away, social isolation can be a real concern for seniors. Practicing social distancing is important for their health, but you can help keep them engaged by increasing phone, video and online interaction, and encouraging family and friends to do the same. If your loved one doesn’t already have a cell phone, contract-free plans are available with free activation and special rates for senior users. Many longterm care facilities also offer social distancing activities for residents.

However, there is one seasonal event that New Mexicans look forward to that hasn’t ceased: the arrival of chile. It’s time for the rich smells from roasters to begin filling parking lots. While the chile industry has had to make some adjustments due to COVID-19, this year’s harvest is plentiful, vendors say.

“Agriculture in New Mexico is an essential business, so pretty much everything has continued as usual in the production and harvesting of our labor-intensive crops,” said Taylor Mitchell, vice

GH is a 64-year-old man who had been well except for a history of hypertension and hypercholesterolemia. Both were controlled with medication. One afternoon, he developed sudden weakness of his left arm and slurred speech. After 15 minutes, his symptoms resolved. He did not seek help until the symptoms recurred but this time after an hour, the symptoms persisted. Because of his concern about going to the hospital, he delayed two hours before finally calling 911 . In the emergency department, he was told he was having a stroke.

A stroke results from a sudden loss of blood supply to an area of the brain. The symptoms depend on the area of the brain affected.

GH had a warning called a TIA (transient ischemic attack) a week before his stroke. Often there is no warning. Thirty percent of strokes occur during sleep. The lack of blood supply to portions of the brain cause specific neurological deficit.

In GH’s case, the stroke involved the right side of his brain causing weakness and garbled speech.

Types of Benefits Depending on your income level and where you live, here are some benefits you may be eligible for: Food assistance: Programs like the Supplemental Nutrition Assistance Program (SNAP) can help pay for groceries. The average SNAP benefit for 60-and-older households is around $125 per month. Other programs that may be available include the Emergency Food Assistance Program, Commodity Supplemental Food Program, and the Senior Farmers Market Nutrition Program.

Healthcare: Medicaid and Medicare Savings Programs can help or completely pay for out-of-pocket health care costs. And, there are special Medicaid waiver programs that provide in-home care and assistance too.

Prescription drugs: There are hundreds of programs offered through pharmaceutical companies,

government agencies and charitable organizations that help lower or eliminate prescription drug costs, including the federal Low-Income Subsidy known as “Extra Help” that pays premiums, deductibles and prescription copayments for Medicare Part D beneficiaries.

Utility assistance: There’s the Low-Income Home Energy Assistance Program (LIHEAP), as well as local utility companies and charitable organizations that provide assistance in lowering home heating and cooling costs.

Supplemental Security Income (SSI): Administered by the Social Security Administration, SSI provides monthly payments to very low-income seniors, age 65 and older, as well as to those who are blind and disabled. In 2020, SSI pays up to $783 per month for a single person and up to $1,175 for couples.

In addition to these programs, there are numerous other benefits they can help you locate such as HUD housing, home weatherization assistance, tax relief, veteran’s benefits, senior transportation, respite care, free legal assistance, job training and employment and debt counseling.

esticides are weapons of mass destruction and can be hazardous during the pandemic. They can compromise your immune system and make you more susceptible to developing viruses, including coronavirus.

To avoid being exposed to pesticides and still implement insect control, you can kill any bug in your house with a mixture of 40 percent water, 40 percent alcohol and 20 percent dish soap.

I

n January 2019, I wrote an article about using a breathing device called a nebulizer to help with the common cold, allergies or chronic concerns due to lung-related illnesses. A nebulizer converts liquids (including medications) into a mist for inhalation into the lungs for absorption. The flu of 2020 and the novel coronavirus have led to an increased interest in nebulizers. This article highlights the relevance of nebulizer use in today’s climate of upper respiratory illnesses.

A virus can trigger existing lung-related illnesses or create a new one. Being proactive about treatment is essential. A virus enters the body through the mouth, nose or eye, triggering symptoms such as a cough, sneezing, or a stuffed

or runny nose (with or without phlegm), and head and body aches. Nebulizers may be useful in managing respiratory environments to prevent or isolate viral and bacterial infections. Nebulizer medication function knowledge helps to equip patients during medical consultations.

Bronchodilators are most commonly prescribed to individuals with asthma or chronic airway conditions to open the airway (Albuterol, Ipratropium, Xopenex, DuoNeb). Anti-inflammatory medications are often taken daily to help prevent asthma flare-ups (Budesonide, Pulmicort, Cromolyn).

A doctor chooses antibiotics when they identify an infection that can be treated best locally, inhaled directly into the lungs (Amikacin, Colistin, Gentamicin, Tobramycin, Vancomycin). Antibiotics aren’t used for viral toxins but rather to address bacterial infections and pneumonia. Sterile saline treatments open the airway and thin and loosen mucous, allowing easier expectoration. Dr. Michael Blaiss, executive medical director of The American College of Allergy, Asthma, and Immunology, says that nebulizer treatment for COVID-19 patients in hospitals has been halted. According to the research, aerosolized COVID-19 particles could be exhaled by infected patients while nebulizing. The potential dangers of infecting others created a shift to inhalers in hospitals. The big difference between a nebulizer and an inhaler is the dose received.

Typically, someone given an inhaler is advised to take two puffs during an asthma symptom flare. “A nebulizer treatment is equivalent to about 10 puffs of an inhaler,” Blaiss said.

It is essential to update our understanding of nebulizer use for the best benefit and to prevent potential harm. Only hygienic use of

sterile fluids intended for nebulizer equipment can be used. Keep your hands and working area clean, and most importantly, use a nebulizer in a room where others cannot be infected with a viral or bacterial toxin that you exhale. Mobilize and expectorate mucus with great care to protect against infecting others.

Watch the chamber for droplets to collect on the sides, then tap them to the base to ensure you have a full dose delivered.

Consider purchasing a pulse oximeter to measure your blood oxygen saturation. This device can help you monitor the effectiveness of your treatments between doctor visits. Normal sterile saline treatments (0.9% sodium chloride solution) are popular among those who desire to keep their lungs moist, expectorate phlegm, and reduce the need for a rescue inhaler.

Consult with your doctor about nebulizer medications, and do not self-prescribe solutions without understanding potential side effects.
