Dear Albuquerque Caregivers, This year, more than ever, providing support for Albuquerque’s caregivers is a critical need. While caregiving is often a labor of love, it is also a very demanding role. Especially during the public health emergency, caregivers, like so many others, have been faced with unprecedented challenges. But you do have support—from many City departments, including Senior Affairs, to outside organizations—and this guide is meant to help you find the resources that suit your needs.

Since the first day the pandemic hit New Mexico, the City of Albuquerque has worked hard to ensure we are offering support to seniors, including standing up meal pickup sites and expanding home delivery, organizing outreach to nursing homes and home-bound seniors, dropping off care packages, and expanding rent assistance programs to keep people in their homes. Between mid-March and midSeptember 2020, we’ve served more than 350,000 meals to seniors.

But ensuring all residents have their needs met requires our community to continue to stand together as One Albuquerque. That’s why every year we also support the Caregiver’s Guide as a way to help connect caregivers and those they care for with resources that can help.

Whether you have been a caregiver for a long time or are just starting on that path, know that we are here to support you on your journey. CHRONIC DISEASE SELF-MANAGEMENT EDUCATION PROGRAMS SUMMARY The Chronic Disease SelfManagement Education Programs (CDSMEP) include six (6) evidencebased self-management programs developed by Stanford University’s School of Medicine Patient Education Research Center. Each CDSMEP is designed to support individuals in developing skills and coping strategies to manage their symptoms related to chronic health conditions such as diabetes, arthritis, hypertension, lung disease, etc.

The Chronic Disease SelfManagement Program (CDSMP) is designed for adults with ANY chronic health condition and caregivers; the Diabetes SelfManagement Program (DSMP) is for people managing Type 2 diabetes and their families / caregivers; the Cancer: Thriving and Surviving (CTS) program is for cancer survivors and caregivers; the Chronic Pain Self-Management Program (CPSMP) is for anyone dealing with chronic pain; the Positive Self-Management Program (PSMP) is for people living with HIV; and the new Workplace Chronic Disease SelfManagement Program (wCDSMP) is for those still working and also managing a chronic health condition.

All CDSMEP are guided workshops delivered two and one-half hours, once a week, for six weeks, in community settings such as senior centers, churches, and health care facilities, etc.

These workshops are licensed through Stanford / SelfManagement Resource Center and are facilitated by two certified Workshop Leaders, one or both of whom live with a chronic health condition.

Subjects covered include: • Techniques for dealing with problems such as frustration, fatigue, pain, and isolation; • Appropriate physical activity for maintain and improving strength, flexibility, and endurance and preventing falls; • Making decisions • Appropriate use of medications; • Communicating effectively with family, friends, and health professionals; • Healthy eating; and • How to evaluate new treatments.

CDSMEP workshops are highly participatory where approximately 10-15 participants focus on building skills to manage their conditions by sharing experiences and providing mutual support.

These programs do not conflict with an individual’s existing health improvement program or treatment plan, but rather are designed to enhance regular treatment and disease-specific education and management.

The CDSMEP are FREE and run throughout the year.

More than 38,000 individuals in New Mexico suffer from Alzheimer’s disease or other dementia. Statistics show that about 60% of these individuals will wander at some time or another. The Albuquerque Police Department and the Department of Senior Affairs are working together through the Silver Alert program to ensure missing persons with Alzheimer’s disease or other dementia are quickly returned to their family or caregiver.

Silver Alert is a public notification system to broadcast information about missing persons who suffer from Alzheimer’s disease of other dementia. The system notifies the public of a missing senior using television, radio, electronic billboards, reverse 911 and traffic signs. The public is asked to call a phone number if they see the missing person.

Any individual who suffers from Alzheimer’s disease or other dementia are eligible to register for Silver Alert. The Department of Senior Affairs will place vital information and a photo of the individual on a USB drive and give the drive to the family member/caregiver who accompanied the person to the Silver Alert registration.

When the family member/caregiver determines that their loved one is missing, they are asked to call 242-COPS. An officer from the Albuquerque Police Department will come to the residence, or the place from which the person is missing, and initiate an investigation.

If the person is not found immediately, the Silver Alert will be issued, notifying all media sources of the importance of finding the individual as quickly as possible. The alert will contain a photograph of the person and state their name, description, age, relevant medical conditions, vehicle type if applicable and their last know whereabouts.

If you had previously registered your loved one with Silver Alert, this important information will be on the USB drive in your possession that you already completed with the assistance of the Department of Senior Affairs. At that time, you will turn over the USB drive that contains this vital information to the Albuquerque Police Office working with you.

“The last time I had my hearing checked for hearing aids, the person in charge said I needed hearing aids without doing near the testing that Mr. Lam did. He was completely thorough and made me feel relaxed and feeling he did the testing to find out what my hearing loss was and what would be done to correct my situation.

Mr. Lam then showed me where my hearing loss was, acoustically, and let me decided which hearing aids would be best suited to me.

He never steered me toward to a particular hearing aid that was more expensive. He showed me several types best suited to my hearing loss and let me decide what hearing aid to use. I believe all the testing done, the aids needed were all in my favor.

In Bernalillo County, this service is available through InnovAge. To qualify for PACE, the senior must be aged 55+ and live within the county or within 30 minutes travel from center. Free for recipients of both Medicare and Medicaid.

POA:

Power of Attorney – a legal form designating someone else to act on your behalf if you become unable to chose medical care for yourself.

LEVEL OF CARE (LOC):

is a state and federal government health insurance program that is administered by the state where eligibility is determined by the states established criteria. It is for people with low income and limited assets. Medicaid covers medical and personal care options, along with benzodiazepines, behavioral health drugs, not covered by Medicare. With prior approval/ authorization, it may also cover eyeglasses, hearing aids and dental work.
