Fall 2018 Dear Albuquerque Caregivers, Taking care of someone who needs you can be one of the most important roles you will ever play in your life. And while caregiving can be challenging, it also offers some of the most rewarding life experiences. I’m grateful to share the latest edition of the Family Caregivers Guide, which is full of valuable resources that I hope you will find helpful. Albuquerque’s family caregivers dedicate countless hours of care so their loved ones can continue to live independently. As the largest generation in our lifetime continues to age, the need for family care only increases. This adds to the physical, emotional, and financial stress that some caregivers experience, and that is why it’s on us to stand up as One Albuquerque to help ensure caregivers have access to the resources and assistance they need to care for their loved ones. One of the common threads that weaves its way through Albuquerque’s multicultural diversity is the way in which we value our seniors and look to them for wisdom as we work together to move our city forward. The generations that have come before us have worked hard to grow our communities and create a special place for future generations to live, work, and play. And because of this, it becomes our responsibility to care for Albuquerque’s seniors and help them with their needs so they can live independently and thrive in our community. Fall 2018 Dear Compassionate Caregivers, It is our privilege at the City of Albuquerque’s Department of Senior Affairs to work alongside you to care for individuals in our community. Since coming on board as Director of the Senior Affairs Department in March, I am impressed by the compassion and commitment of caregivers in our community and how fortunate we are, as a Department, to have various programs and services available to support you through your work. The Department of Senior Affairs is proud to be included in this latest edition of the Family Caregiver Resource Guide. We recognize that close to 80 percent of all long term care in the United States is provided free by family members from the National Alliance for Caregiving, which is also what we experience here in our beautiful state. As our population continues to grow, there is an increased need for caregivers in our community, and we are here to be a resource to you and connect you to senior services throughout the city, including home services like minor home repairs and home delivered meals for the frail, homebound older adults who cannot prepare meals for themselves. CHRONIC DISEASE SELF-MANAGEMENT EDUCATION PROGRAMS SUMMARY The Chronic Disease SelfManagement Education Programs (CDSMEP) include six (6) evidencebased self-management programs developed by Stanford University’s School of Medicine Patient Education Research Center. Each CDSMEP is designed to support individuals in developing skills and coping strategies to manage their symptoms related to chronic health conditions such as diabetes, arthritis, hypertension, lung disease, etc.

The Chronic Disease SelfManagement Program (CDSMP) is designed for adults with ANY chronic health condition and caregivers; the Diabetes SelfManagement Program (DSMP) is for people managing Type 2 diabetes and their families / caregivers; the Cancer: Thriving and Surviving (CTS) program is for cancer survivors and caregivers; the Chronic Pain Self-Management Program (CPSMP) is for anyone dealing with chronic pain; the Positive Self-Management Program (PSMP) is for people living with HIV; and the new Workplace Chronic Disease Self-Management Program (wCDSMP) is for those still working and also managing a chronic health condition.

All CDSMEP are guided workshops delivered two and one-half hours, once a week, for six weeks, in community settings such as senior centers, churches, and health care facilities, etc.
These workshops are licensed through Stanford / SelfManagement Resource Center and are facilitated by two certified Workshop Leaders, one or both of whom live with a chronic health condition. Subjects covered include: • Techniques for dealing with problems such as frustration, fatigue, pain, and isolation; • Appropriate physical activity for maintain and improving strength, flexibility, and endurance and preventing falls; • Making decisions • Appropriate use of medications; • Communicating effectively with family, friends, and health professionals; • Healthy eating; and • How to evaluate new treatments. CDSMEP workshops are highly participatory where approximately 10-15 participants focus on building skills to manage their conditions by sharing experiences and providing mutual support. These programs do not conflict with an individual’s existing health improvement program or treatment plan, but rather are designed to enhance regular treatment and disease-specific education and management. More than 38,000 individuals in New Mexico suffer from Alzheimer’s disease or other dementia. Statistics show that about 60% of these individuals will wander at some time or another. The Albuquerque Police Department and the Department of Senior Affairs are working together through the Silver Alert program to ensure missing persons with Alzheimer’s disease or other dementia are quickly returned to their family or caregiver. Silver Alert is a public notification system to broadcast information about missing persons who suffer from Alzheimer’s disease of other dementia. The system notifies the public of a missing senior using television, radio, electronic billboards, reverse 911 and traffic signs. The public is asked to call a phone number if they see the missing person. Individuals who are 60 or older and suffer from Alzheimer’s disease or other dementia are eligible to register for Silver Alert. The Department of Senior Affairs will place vital information and a photo of the individual on a USB drive and give the drive to the family member/caregiver who accompanied the person to the Silver Alert registration. When the family member/caregiver determines that their loved one is missing, they are asked to call 242-COPS. An officer from the Albuquerque Police Department will come to the residence, or the place from which the person is missing, and initiate an investigation. If the person is not found immediately, the Silver Alert will be issued, notifying all media sources of the importance of finding the individual as quickly as possible. The alert will contain a photograph of the person and state their name, description, age, relevant medical conditions, vehicle type if applicable and their last know whereabouts. If you had previously registered your loved one with Silver Alert, this important information will be on the USB drive in your possession that you already completed with the assistance of the Department of Senior Affairs. At that time, you will turn over the USB drive that contains this vital information to the Albuquerque Police Office working with you. Registration for Silver Alert is on-going. Those familiar with resources, programs or organizations use short cuts or acronyms to talk about resources. Here is a glossary for some of the terms.
AAA: Area Agency on Aging
(The Triple A) Six Area Agencies on Aging serve the State of New Mexico; the Albuquerque/Bernalillo County area has one.
ADLs: Activities of Daily Living.
Routine personal activities such as bathing, dressing, toileting, eating and walking (ambulation).
Advance Directives:
Legal forms such as a “Durable Power of Attorney for Health Care and Instructions for Health Care and End-of-Life Decisions”. Also referred to as a “living will.”
APS: Adult Protective Services
Assisted Living: Assisted
living is an intermediate level – between independent living and nursing homes – of long-term care appropriate for many seniors.
Benefits Counseling:
A program of the NM Aging and Long-Term Services Department which provides information that might reduce your Medicare payments or help you with the costs of food, utilities and medications.

physical, financial or emotional support for a relative or loved one whose daily activities are limited by diminished mental or physical function.
Centennial Care: is the
name of the New Mexico Medicaid program. Centennial Care is
provided by three managed care organizations (MCOs). These services include physical health, behavioral health, long-term care and community benefits. Most people who are currently enrolled in a New Mexico Medicaid program are eligible for Centennial Care. Eligibility for all Medicaid programs requires that individuals meet certain federal guidelines. These include citizenship, residency and income requirements. To apply for Centennial Care coverage, you can complete an application on-line beginning October 1, 2013 at www.yes. state.nm.us. You may also apply in person at your local ISD office, by fax, or by mail. To download an application, go to ww.hsd.state. nm.us/isd/apply.html. If you have application questions, you can call HSD’s new Medicaid Expansion Hotline toll-free at 1- 855-6376574 or for general questions, please call 1-888-997-2583.
Continuity Of Care:
Range of resources and services which help caregivers in their efforts to maintain the care recipient’s quality of life.
Cremation Authorization Form:
An advance directive which allows you to authorize your body be cremated and to designate how you want your ashes disposed of.
Discharge Planning:
Medicare defines discharge planning as: “A process used to decide what a patient needs for a smooth move from one level of care to another.”
DNR: Do Not Resuscitate
DSA: City of Albuquerque/
Dual Eligibility: a person who qualifies for both Medicare and Medicaid
Durable Medical Equipment (DME):
Items such as wheelchairs, canes, walkers, ramps, etc.
HIPAA: Health Insurance
Portability and Accountability Act Under this 1996 Federal law, health care providers cannot release your medical information without your express consent.
HUD: U.S. Department of
Housing and Urban Development.
IADL: Instrumental Activities of Daily Living.
Institutional Medicaid: A Medicaid program that pays for nursing home care for individuals who are in a qualifying medical institution for at least 30 days when nursing home care is determined to be medically necessary.

Level of Care (LOC):
LIHEAP: Low-Income Home Energy Assistance Program
Medicaid: is a state and
federal government health insurance program that is administered by the state where eligibility is determined by the states established criteria. It is for people with low income and limited assets. Medicaid covers medical and personal care options, along with benzodiazepines, behavioral health drugs, not covered by Medicare. With prior approval/ authorization, it may also cover eyeglasses, hearing aids and dental work. MEDBANK For information about prescription drug help for insured and non-insured contact 1-800-432-2080. VA benefits – Medications may be provided at low or no cost depending upon your VA status.
PACE: A Program of All-
inclusive Care for the Elderly (PACE) Generic term for day health centers that include a medical clinic, adult day care, social services, rehabilitation, transportation, in-home services and inpatient care (when needed). In Bernalillo County, this service is available through InnovAge. To qualify for PACE, the senior must be aged 55+ and live within the county or within 30 minutes travel from center. Free for recipients of both Medicare and Medicaid. Private pay also accepted.
POA: Power of Attorney – a
legal form designating someone else to act on your behalf if you become unable to chose medical care for yourself.
