FAMILY MATTERS: BEING INVOLVED IS KEY TO LIVING A FULL LIFE

R

ecently I had the opportunity to meet the sister and brother-in-law of someone we have supported for many decades. They have been very involved in their sister’s life, but not very involved in the larger life of Hammer. They wanted to meet us, learn more about Hammer, and offered to get involved in a greater way. It was a delightful conversation filled with stories about their loved one’s life. We left with a commitment on both sides to deepen this relationship so that we can better serve their sister and others at Hammer Residences.

To me, this is an example of the best of Hammer at work. Our primary instinct is to build relationships. We have many programs and innovative service models. However, all of this is built upon the relationships we nurture with the people we support, their families, and friends. We strongly desire that families and friends be involved in the lives of their loved ones and with Hammer. We rejoice when people become advocates, volunteers, and donors.

The stories you will find in this issue of Discoveries are all about relationships.

You will meet Matt, who is supported by Hammer in our Tyler home. Matt loves music, the theater, and movies. You will also read about a married couple who co-manage our Lawndale home in Maple Grove. Together with their team of staff, they have supported six individuals for over fifteen years.

We are also sharing a story about our innovative tool called myEvolv, designed to manage the transition from pen and paper documentation to electronic health records. This will allow access to all the records in our homes from our office and will speed up the required documentation process for our staff.

This will allow staff to spend more time with the individuals they support and reduce our administrative costs.

Even if you are just getting to know Hammer, we are grateful for your presence with us. While we may frequently talk about our services, there is nothing that makes us happier than seeing relationships and the Hammer family grow and thrive, because family does matter. In the end, being involved makes the lives of people living with disabilities even better.

A TRUE PARTNERSHIP

I

t’s hard to imagine what it’s like to know that your thirteen-year-old child needs to leave your home to live in a group home. But that’s what we needed to do for Aaron. He was a big kid, and with his autism and outbursts, my wife, Julie, and I couldn’t safely care for him in our home.

It was an agonizing decision, but one that needed to be made for the entire family.

Before Aaron was born, I had worked in social services for organizations directly working with people who had developmental disabilities, so I had a pretty good understanding of how the “business” worked. Because of my work and knowledge of the industry here in the Twin Cities, when Aaron’s case manager asked us if we had any company preferences, I immediately said Hammer. Much to our delight, and relatively soon after receiving Aaron’s waiver, Hammer had an opening in their children’s program. When we arrived at Hammer’s Arrowwood home for the tour we were nervous, concerned, and to be honest, our hearts were breaking.

However, after meeting with Tom Gillespie and Ken Harper at Hammer, we just knew it was the place for Aaron.

We asked Tom and Ken every question we could think of, and they were not fazed a bit. They understood what this meant to us, and they answered all of our questions honestly and compassionately.

We knew this was the partnership we were looking for and Hammer was the place that would help Aaron reach his full potential.

Hammer is truly our partner. Together we are advocates for Aaron. If there’s something Aaron needs, we need, or the house needs, we talk openly with Ken and or staff about what’s going on. Together we will talk with Aaron to formulate a plan to meet his needs and goals. If my wife Julie and I find a community program we think will benefit Aaron, Ken and the staff do everything they can to make it happen. In turn, if the team

needs help driving to an event, we will take Aaron ourselves when they are stretched. When working through a problem Aaron is having, they know we are there to support them as well. It is truly a collaborative environment. Everyone is working toward the same goals for Aaron: His maturation, growth, and happiness.

Julie and I also feel we have a responsibility, as part of the Hammer family, to support the organization.

Aaron is now an adult and will probably always need the assistance of others.

I cannot express the peace of mind it gives us knowing Hammer will be there for Aaron — even after we are gone.

It’s priceless and one of the reasons we support Hammer financially. We want to help make sure Hammer stays strong for many years to come!

Serving on the board and the finance committee is one of the ways I personally give back. Julie has taken part in parent panels and supported Hammer's educational programs. Our daughter, Hilary, works for Hammer as a DSP while getting her master’s degree in education.

We also feel it is important that, as we can, we make monetary gifts to support Hammer through initiatives such as Reach for Ralph and other appeals.

Some gifts benefit Aaron’s house directly and some support the greater Hammer community. We have also gotten friends and family involved by inviting them to join us at Reach for Ralph and in making Hammer the designee for memorials. In this way, we hope friends and family can see and learn about the organization that means so much to our family.

It has been almost ten years since Aaron moved into a Hammer home, and he is thriving! Tom is now Hammer’s Chief Program Officer, and Ken continues to manage Aaron’s home. The staff

continues to play an import role in his life. While no one can ever take care of your child the way you would, the Hammer family comes very close, and for that, the Kirsch family will be forever grateful and supportive of Hammer.

Love of Music Brings Joy

M

att was just a regular kid growing up, until his tragic accident at age fifteen. Matt was hit by a car while riding his bike and suffered a severe head injury.

He was in a deep coma and underwent several emergency surgeries due to a skull fracture and resulting infections.

Matt was treated at Mercy Hospital before being transferred to Cambridge State Hospital for long-term care. At the time, there were no group home openings for him.

After spending years at Cambridge and months on wait lists as his family searched for a suitable placement, Matt came to Hammer in 1986 at the age of twenty-one. Hammer was opening a new group home, one specifically for those with traumatic brain injury (TBI). Dianne Berg, the current program manager at Hammer’s Lakeside home, and former CEO, Tim Nelson, helped open the Tyler home.

The original Tyler home, where Matt first moved in, was located in St. Louis Park. Years later, the home moved to its current location in Plymouth. The Tyler home has been remodeled to accommodate the high-level of care Matt and his housemates require. There is a ceiling track lift system that runs throughout the house and connects each of the rooms. All the doorways have been widened to accommodate wheelchairs and the bathroom is equipped with a roll-in shower and handicapped-accessible tub. Staff use Tyler's lift van to take individuals out to various activities or to doctors appointments.

One of the greatest joys in Matt’s life is music. He was a talented musician as a child and played both the violin and the piano. In fact, Matt was accepted into Julliard — prior to his accident. Today, music continues to bring Matt a great deal of joy. On any given day there is always good music coming from his stereo. When his favorite artist or genre is played, it can instantly brighten his mood and bring a smile to his face.

A music therapist has been coming to the house for many years. Each week Matt and his housemates get together for an hour of music in the living room. Matt also loves the orchestra, music in the park during the summer months, plays, musicals, and catching a movie at the theater.

Matt has lived at Hammer for most of his adult life. It has become his home and staff are like family. A few staff at Tyler have been working here for over fifteen years. It is important to Matt to have staff he knows and trusts work with him, because building relationships with the people we support is what Hammer is all about.

This past spring, Matt was hospitalized with a severe case of pneumonia. He spent ten days in the hospital, some of which was in the ICU. I spent days with him at the hospital, and his mom called almost daily to speak with the doctors and nurses.

Members of the Tyler staffing team (past and present) were there for him when he needed them most. It was a scary time, but Matt has made a full recovery and is living a full life thanks to his staff at Hammer, and also from the support of donors like you who give to Hammer.

IT’S MORE THAN JUST A PAYCHECK — WORKING AT HAMMER GIVES PURPOSE

H

ow would you answer this question: Other than your paycheck, what kind of work environment or company benefits are most important for your job satisfaction?

When you conduct interviews every day as I do, you expect to hear about insurance, work-life balance, collaboration, and respect. If you are interviewing at Hammer, you can expect to hear that the applicant’s satisfaction requires an opportunity to give back and make a difference. Applicants often say this is their calling, and they believe in the Golden Rule: to treat others in the same way they would like to be treated.

Everyone brings their values and experiences with them. For those who are returning to the job market since they immigrated or retired, this can be a place where they contribute to our mission and continue to serve others.

The best part of my job is when I match a person to a job and then hear about how they are contributing. Hammer offers great benefits, on-going training, and a warm and friendly culture. But for real satisfaction, we make sure the job and culture are a good fit so employees have a positive experience and daily opportunities to make a difference.

MY JOURNEY TO HAMMER My connection to Tyler goes back to when I was in high school. My mom was the nurse for Tyler when it first opened in St. Louis Park. She would bring us along to picnics or just to hang out every now and again. Fast forward to when I graduated from St. John’s University. I wasn’t sure what I wanted to do, so my mom encouraged me to apply at Hammer and work there until I figured it out. I was hired to work at the Tyler home.

I remember those first couple months were really fun. The environment at Tyler was so different than any other place I had worked. We had three to four staff working at a time so it was pretty much one-to-one staffing with the people living at Tyler. The house was full of so much personality, whether it was the staff or the people living there. This is where I learned the basics of cooking – what to do and more often than not what not to do when cooking (my first try at mashed potatoes was a disaster).

All four of the individuals living there were nonverbal, but the staff were able to help interpret their needs and wants so effectively — it made it fun. It didn’t matter what you tried, if it helped with communication you were encouraged. It was fun to go to work — lots of laughing too . . . and it was also OK to make mistakes.

Through it all I learned a whole lot. It was a great experience, because the people around me made it fun to come to work everyday.

THE COMMON THREAD THAT WEAVES US TOGETHER

B

ecoming part of the Hammer family was always going to be part of my path, partly because fate brought me to the same resort in Punta Cana as Joe Koltes (Joe serves on the Hammer Board of Directors).

We just happened to meet and find our Hammer connection. The thread that links us is because of my family history. I am Ralph Rosenvold’s great, great niece.

Ralph Rosenvold, was one of the first individuals supported by Hammer.

When I switched majors in college to focus on Special Education, I felt like it was my calling. Now I know for certain, it was literally in my blood all along. Along with Alvina Hammer, my relatives, Judy and Rueben Rosenvold, helped to start and build Hammer with Ralph as one of the first residents.

In June, I began my journey with Hammer working as a Direct Support Professional at Lake Place apartments and as the Marketing and Communications intern in the central office. I tell anyone who will listen how much I love my job and all the amazing things Hammer does on a daily basis and the passion everyone in the organization has to help individuals live the best life possible.

There are thirteen individuals who live at Lake Place apartments and are supported by Hammer. My first couple days working there were slightly overwhelming. I met a lot of people and learned the routines they have developed independently and with staff.

I was taken by surprise at how quickly I was able to bond and develop personal relationships with each of them. Before long, walking in the door at Lake Place felt like walking through the door at home. They had become my Hammer family. Each day was filled with a different prank, car karaoke, or living room dance-off that reinforces that I am exactly where I am supposed to be.

In July, I was able to attend Reach for Ralph, Hammer’s fundraising event, with my grandma (Ralph’s niece), my dad, my uncle (Ralph’s great nephews), and a few other relatives. It was a very moving experience for many reasons.

The biggest revelation was experiencing the pride my grandma had in seeing Ralph’s legacy living and blooming still today. I am lucky enough to get to be a part of that. After the event ended we reminisced with each other about the evening and were hit with one very profound thought.

Out of everyone in my entire family, the one person who has left the greatest legacy is a wonderful man, Ralph, who happened to have a disability.

QUANTIFYING HAMMER’S MISSION

T

here are many things to love about working at Hammer, but my favorite “fringe benefit” is the stories you accumulate over time. Anyone who has spent time with the men and women we support is bound to have a go-to story of laughter or triumph and maybe a few tear jerkers too.

I experienced this first-hand when I worked at our Knollway home as a Direct Support Professional (DSP).

provided by DSPs over the past year and see the frequency of anxiety documentation increase around the end of the month.

This could start a conversation with the person supported about the end of the month and if there is something we can do to support them during that time, like therapy or a visit to the doctor for an evaluation.

As an organization, we need these stories for our advocacy work, to build up people with disabilities in society, and to develop relationships in the community to keep doing the incredible work we do. I believe we do the best we possibly can for the people we support as represented by the wonderful stories we all have. But how do we prove it?

Over the past two years, I have had the pleasure of overseeing our transition from pen-and-paper documentation to an electronic health record. This record, called myEvolv, houses nearly all our information and documentation from our Direct Support Professionals (DSPs) and customized support services. Because of the time and effort put in by everyone working at Hammer, we now have an incredibly robust database of information we can use to show how our services impact the men and women we support.

As a quick example, one of our DSPs may notice an increase in a supported person’s levels of anxiety and log documentation of these events in the Daily Service Record in myEvolv and alert the manager of the home to it. The manager, using reports generated from myEvolv and other analytics software, can dial in to the documentation 8 | Hammer Discoveries

We could have figured all of this out prior to myEvolv as well, but consider the process to create a graph, similar to the one below. You would need to collect the pen-and-paper documentation for the past year, tally the daily total of the anxiety level, enter the information into a spreadsheet, and then create the chart.

For some of our computer savvy managers it might take a day, but for those who have not yet plunged into the wonderful world of spreadsheets, this might seem like an insurmountable task. With these new tools, this information can be accessed with a few clicks of a mouse button.

This means all of our valuable information is available in the most accessible format for our program supervisors to help the people we support live life to its fullest.

VOLUNTEER

MAKE A FRIEND, BE A FRIEND: KINDRED SPIRITS AND LIFELONG FRIENDS

T

here are many paths and reasons that bring volunteers to Hammer. There are some who stay for a short while, others who stay for many years, and yet others who need to leave and then return.

Bob Ziegeweid starting volunteering with his wife, Judith, in 2005. As members of Holy Name of Jesus Church, they became church drivers for those we serve at Hammer. Bob and Judith continued in this role until 2012, when Judith’s diagnosis of ALS had progressed to the point where they could no longer continue volunteering.

Bob recently reached out to Hammer, offering to volunteer again. Judith had passed in 2014 and Bob had just retired. He asked us, “What is your greatest volunteer need?” The answer was simple: We need one-on-one volunteers; people who would reach out to an individual we support, who had mutual interests with that individual, and spend time with them. This could be as simple as going for a cup of coffee, seeing a movie, or playing cards.

Bob decided he would like to become a one-on-one volunteer. After careful consideration, Bob was matched with John T. at Zealand, who was featured in our Make a Friend, Be a Friend video campaign.

Bob and John were first introduced to each other at Zealand in April 2017.

They have been meeting once a week ever since. They spend their time together miniature golfing, going to the movies, working out at the YMCA, and participating in John’s favorite sport, bowling.

We asked John to tell us about his volunteer. He said, “I like Bob. We hang out together, at home, and in the community. Bob is funny, and nice, he

Bob and John love to go grocery shopping together and then head over to Bob’s house and make dinner. After dinner they watch a movie or just hang out and talk.

talks to me. He helps me cook, we watch TV and movies. My favorite activity is going to Perkins to have pancakes, sausage, and eggs.”

Another favorite activity is cooking dinner together. After grocery shopping, they go to Bob’s house and make a meal consisting of hamburgers, baked beans, and sponge cake topped with fresh strawberries and whipped cream.

Bob says he doesn’t feel like a volunteer.

He is John’s friend. He looks forward to seeing John each week and spending time together. They have a lot of fun.

Bob wanted other potential volunteers to know that one-on-one volunteering will be the most rewarding time you will spend all week. All you have to do is show up and everything else will take care of itself.

THE STRUGGLE TO SAVE MEDICAID: ALL HANDS ON DECK

T

he summer of 2017 is a time advocates in Health and Human Services will not soon forget. It was the summer our skills were put to the test by “it.”

You may have heard the media frenzy refer to “it “by different names such as Medicaid, Medicare, Affordable Care Act, Trumpcare, Obamacare, GrahamCassidy or various other names. Regardless of how it was referred to, we all knew the fierce battle being fought was about health care for the elderly, people with disabilities, and those with pre-existing conditions just to name a few.

At Hammer, every individual we serve relies on Medicaid. Whether they live with a disability or a pre-existing condition, their potential loss of funding if Medicaid had been cut would have been devastating. The losses to Hammer as an agency would have been staggering. Within the next three years, the losses to the state of Minnesota could have been billions of dollars.* We would like to think the safety zone we are in now exists because of a few brave senators who voted no, like Senator McCain… but in reality, it is because of local advocates like the staff, families, citizen advocates, and volunteers of Hammer who participated in grassroots efforts like This is Medicaid.

This is Medicaid* is a Hammer supported coalition of 158 non-partisan organizations in Minnesota that are collaborating and partnering to protect Medicaid from harmful changes and funding cuts. The nonprofit organizations advocate for and directly serve people who access health care and support through Medicaid.

It was amazing to see how each organization gave whatever staff or resources they could to help. We stepped up in true Hammer fashion. We can be proud to say we were part of this historic save(s) to our services…. at least for the summer of 2017.

MEET THE TEAM

MAKING A HOME FOR A LIFETIME

hen you walk up to Hammer’s Lawndale home, the first thing you notice is the giant willow tree in the front yard. It is a large, beautiful tree, and one cannot help but be taken by its sheer immensity.

The trunk stands tall and sturdy with branches crawling and weaving their way skyward. The leaves and smaller branches cascade down and sway gently in the breeze.

When asked about how they manage to work together and live together and still keep a happy marriage, they laugh.

“We delegate tasks at work,” Phelomina responds. “I handle administrative tasks like the budget, because I know Michael does not like to do those things. And he handles things like medical appointments, payroll, and scheduling.”

It is because of leaders like Phelomina and Michael and their team that Hammer continues to be a top provider for people with disabilities in the Twin Cities. "Mom and dad taught us to give back to the community with their actions. When I was very young, I remember them volunteering for church projects. Dad was involved in a re-entry program for inmates and they helped out at Sheltering Arms, one of Minnesota’s first programs for my sister, Liz. Mom and dad became involved at Hammer when Liz became part of the Hammer family. Even when there was no extra money, Dad would always give what they could financially to the church and to Hammer. They felt it was their responsibility to give what they could afford no matter their situation, in order to help others.

MY ROAD TO COMMUNITY LIFE

T

he Community Life department was developed from Hammer Healthy Living Initiatives with the support of grant funds awarded from the Medica Foundation in January 2014. Today, the Community Life department functions as a great resource for both the individuals supported by Hammer and the staffing teams that serve throughout our residential settings and in our offices. Since its inception, the core values of the Community Life department remain steadfast with a strong mission focused on encouraging and supporting healthy, informed, and well-balanced choices.

My Hammer story began several years ago when I came to work part time at Hammer as a Direct Support Professional (DSP) in the Individualized Community Services (ICS) department. That position soon led to serving as the Program Manager at the Rockford group home in Plymouth, a position which helped to further my realization that Hammer was the place for me. I went back to support the recreational activities of ICS and then moved to ICS Program Manager. Currently, I am in my current role as the Community Life Coordinator.

I am not a Minnesota native, but I found my home in Minnesota surrounded by the loving support of many amazing friends. Being an active member of my community is important to me, and I enjoy volunteering with a variety of community organizations. In my free time, I enjoy spending time with my dog, Clarence, travel, fitness, outdoor adventure, theater, baking, and reading.

We are all faced with the task of planning the activities, pursing the interests, and making the social connections that help us enjoy life. Sometimes this task of planning life can be overwhelming, confusing, and even frustrating, but that is where the work of the Community Life department provides a helping hand.

I bring with me knowledge not only of the programs and services Hammer offers, but also insight into the needs of the individuals supported by Hammer and those of my Hammer co-workers. The Community Life department is here to help with resources, activities, and encouragement, so letâ&#x20AC;&#x2122;s enjoy this wonderful adventure together!

INNOVATION: SMALL CHANGES HELP STAFF SUCCEED!

B

efore I started my role as Nursing department coordinator, I was the program manager of our Welcome Place and Nelson Place homes. One of my favorite things about that position was having the freedom and ability to use my strengths and talents to lead and empower my staff and to create a fun and nurturing home for our individuals. As a former teacher, my houses were filled with fun decorations, art projects, and staff meetings full of silly games and activities. Now, would my style work at every Hammer house? Perhaps not. But, that is one of the cool things about Hammer. No two houses or apartments are the same. And while they all share the same mission and values of Hammer as a whole, each manager and staff team are free to bring their own style and vision to their site.

When I started this new role, I was assigned the task of revamping the medication cabinets to look the same in all our homes. It seemed the size, location, and organization of the cabinets were different at each house I visited. The main goal of this project was to make things easier and more consistent for staff, especially those working at multiple sites.

The med cabinet makeovers took quite a bit of time and resources. New bins, labels, and pictures were added and all houses now share a common cabinet with the same layout. While these changes can take some getting used to, in the end, they will help staff members in the homes to minimize medication errors.

Another adjustment in this area was the way in which we track medication errors. This system was revamped and now ties medication errors to a points system. If staff members receive a certain number of points, they must retake the meds training class.

With the addition of both of these systems, we are confident we will reduce medication errors across the Hammer organization. As I said earlier, change is hard, but staff are seeing this change is in place to help them succeed!

DANCING, DINING, AND MAKING NEW FRIENDS: HAMMER TRAVEL MAKES IT ALL HAPPEN

V

acations mean many different things to many different people.

To some it means adventure, relaxation, education, or experiencing something new, to others it means an escape from reality.

A Hammer Travel trip leader wrote, “Cathy was pretty reserved for the first couple days, but the last couple days of the trip, she really opened up and had a great time. She became friends with one of the other gals on the trip, and had the whole room laughing and having a good time. It was really cool for me to see someone achieve so much — from just convincing herself to go on the trip to opening up with people and having a great time.”

Cathy says, “I love to travel because it means an escape from my reality. I enjoy the different surroundings and meeting new people. The trip and the daily itinerary keeps me moving. I also like to be waited on, which doesn’t happen often. And the dancing…I love the dancing. Even though I get depressed again, every time I come home, the trips give me a break from my reality.”

Hammer Travel helps all individuals, who need support, have a positive travel experience. We go the extra mile to accommodate our travelers’ needs, so they can have a great time!

Meet Cathy — a very interesting woman with many stories. Cathy makes friends easily and enjoys meeting new people. She loves to walk and take in the sights of nature, eat at Burger King (her favorite), and color mandalas to relax. Cathy is also a mother to two adult sons and has been supported by Hammer for four years.

Cathy is living with bi-polar disorder and struggles with depression. She works hard to stay in control of her body and emotions, but sometimes it can get the best of her. There are days when it is just too hard to even get out of bed. These days are the toughest.

Cathy has traveled with Hammer Travel six times. Her trip destinations have included: an Alaskan cruise, Hollywood Movie Madness, a dude ranch, the Orlando Dolphin Encounter, the Grand Canyon, and a Disney cruise. When asked which trip was her favorite, the response was swift, the cruises!

Cathy had signed up for more, but at times her depression got the better of her. It takes Cathy extra strength and courage to convince herself to travel. However, once she is on a trip, Cathy becomes a whole new person.

Inspired by our event theme, Caring for the Heart of Hammer, over 450 of you Hammer enthusiasts gathered to hear about the special bond between our incredible caregivers and the people we serve, together. Michael joined us on stage with his caregiver, Mike, and shared their story. “Without my care giver Mike, I don’t know how I would get to my dialysis appointment every week. Mike is always on time. He is really helpful.

He also plays a good game of cribbage. Before Hammer, I did not have many friends, mostly just my mom and dad. Now, I have a lot of friends and I am really active thanks to my care giver Mike and Hammer.”

The room was alive with conversation, and the aroma of homemade Italian spaghetti filled the air, as we enjoyed a meal together to celebrate raising $89,595.00 for Give to the Max Day 2017!

Thanks to the 159 generous Give to the Max donors, individuals supported by Hammer will be able to experience 24 hour a day, seven day a week care, as well as the opportunity to participate in their communities.

"To find words to express how wonderful the Hammer organization is, it's impossible. Every time I turn around there is something else that just raises my admiration. As large as Hammer is, everyone works together like a loving family.

My family and I are grateful for Hammer and it is a dream come true for our family. I lost both by sister Beth who lived at Hammer this summer, and also my brother, Al Tennessen, who served on Hammerâ&#x20AC;&#x2122;s Board of Directors. Although Al was deeply involved with many nonprofits throughout his life, he was adamant that all of his memorials be directed to Hammer. So far, more than 125 gifts of all sizes have been received in memory of both Al and Beth. They would be so proud!"
