WELCOME I am delighted to welcome you to the second edition of the Aging Life Care™ Association’s (ALCA) Mid-Atlantic Resource Directory! We’re incredibly grateful to Steve Gurney and the Retirement Living Sourcebook for their support in producing this guide, as well as promoting the sponsorship of our upcoming Chapter Conference on October 22 – 23.

We are proud to be the 4th largest of the 9 regional chapters of ALCA and our membership has grown about 20% over the last 5 years. Our members are social workers, nurses, therapists, gerontologists and certified care managers and 80% are certified at the two highest levels of membership: Advanced Professional and Professional. We also enjoy very collegial relationships with one another and provide referrals to care managers locally, regionally and across the country to help guide families and responsible parties (such as court appointed guardians) and provide support to our clients who are experiencing challenges with aging or disability.

We also continue to value our partnership with members of the National Association of Elder Law Attorneys (NAELA), and they are an integral part of this guide. Members of NAELA were among the first professionals to encourage social workers and nurses to assist with their clients’ care needs. They provided support during the early days of our association and we continue to work closely with them today.

Our valued ALCA corporate partner members provide ongoing support for the work of our chapter, and we also welcome the additional support of our community partners across the region including home care companies, assisted living communities, skilled nursing facilities, hospitals and hospices.

Understand Aging Life Care™ Aging Life Care / geriatric care management is a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges.

What is an Aging Life Care Professional?

An Aging Life Care Professional™, also known as a geriatric care manager, is a health and human services specialist who acts as a guide and advocate for families who are caring for older relatives or disabled adults. The Aging Life Care Professional is educated and experienced in any of several fields related to aging life care / care management, including, but not limited to gerontology, nursing, social work, psychology or occupational therapy.

The Aging Life Care Professional assists clients in attaining their maximum functional potential. The individual’s independence is encouraged, while safety and security concerns are also addressed. Aging Life Care Professionals have extensive knowledge about the costs, quality and availability of resources in their communities.

Aging Life Care Professionals are members of the Aging Life Care Association™ (ALCA) and differ from Patient Advocates, Senior Advisors, Senior Navigators and Elder Advocates. ALCA members must meet the stringent education, experience and certification requirements of the organization, and all members are required to adhere to a strict code of ethics and standards of practice.

What Services do Aging Life Care Professionals Provide?

Aging Life Care Professionals provide guidance to help families ensure quality care and an optimal life for those they love through:

• Assessment and monitoring • Planning and problem-solving • Education and advocacy • Family caregiver coaching • Long-distance caregiving Aging Life Care Professionals are engaged to assist in a variety of areas, such as: • Housing – helping families evaluate and select the appropriate level of housing or residential options.

• Home care services – determining the types of services that are right for a client and assisting the family to engage and monitor those services.

• Medical management – attending doctor appointments, facilitating communication between doctor, client and family, and if appropriate, monitoring client’s adherence to medical orders and instructions.

• Communication – keeping family members and professionals informed as to the well-being and changing needs of the client.

• Social activities – providing opportunities for the client to engage in social, recreational or cultural activities that enrich quality of life.

• Legal – referring to or consulting with an elder law attorney; providing expert opinions for courts in determining level of care.

• Financial – may include reviewing or overseeing bill paying or consulting with accountant or client’s Power of Attorney.

• Entitlements – providing information on Federal and state entitlements; connecting families to local programs.

• Safety and security – monitoring the client at home; recommending technologies to add to security or safety; observing changes and potential risks of exploitation or abuse.

• Long-distance care – coordinating the care of a loved one for families that live at a distance, including crisis management. Local, cost-effective resources are identified and engaged as needed.

A care plan tailored for each individual’s circumstances is prepared after a comprehensive assessment.

How do you know that you need an Aging Life Care Professional?

You may need an Aging Life Care Professional if: • The person you are caring for has limited or no family support.

• Your family has just become involved with helping the individual and needs direction about available services.

• The person you are caring for has multiple medical or psychological issues.

• The person you are caring for is unable to live safely in his/her current environment.

• Your family is either “burned out” or confused about care solutions.

• Your family has limited time and/ or expertise in dealing with your loved ones’ chronic care needs.

• Your family is at odds regarding care decisions.

• The person you are caring for is not pleased with current care providers and requires advocacy.

• The person you are caring for is confused about his/her own financial and/or legal situation.

• Your family needs education and/or direction in dealing with behaviors associated with dementia.

• You live at a distance from your loved one.

Mid-Atlantic Chapter The Aging Life Care Association (ALCA) developed the standards of practice and code of ethics to guide Aging Life Care Professionals in their daily professional and business practices. Additional information, as well as the full code, is available on ALCA’s website: aginglifecare.org.

THE CODE OF ETHICS PROVIDES: Accountability to our Clients ALCA members recognize diversity in our society and embrace a multi-cultural approach to support the worth, dignity, potential and uniqueness of each client. The Code of Ethics acknowledges the vulnerable population we serve and makes explicit the highest standards of practice.

Accountability to the Public The Code of Ethics sets a national standard for the professional practice of Aging Life Care™. It defines for the public the ethical responsibilities expected of ALCA’s members and the organization’s role in maintaining the highest standards of practice and promotion of ethical behavior.

Education of Aging Life Care Professionals ALCA recognizes the diversity of the experience and education of its members and the needs of members for guidance in both their professional and business roles, and thus the Code of Ethics was developed to guide members in each of these roles. It states the core values and principles to current and future members, to the public, and to allied professionals. All members of ALCA are expected to understand and behave in a manner that is consistent with the provisions of the Code of Ethics.

A Framework for Analyzing & Resolving Ethical Dilemmas The Code of Ethics offers a framework for ethical decisionmaking when conflicts arise in either the practice or the business of Aging Life Care. It assists Aging Life Care Professionals in examining the ethical issues present in all aspects of their work by identifying what principles need to be considered and how to prioritize them when it is necessary to make a choice. It asks Aging Life Care Professionals to be aware of their own biases as they seek to resolve ethical dilemmas.

Assistance in Reviewing Complaints In ALCA’s Peer Review Process the Code of Ethics and Standards of Practice serve as the basis for assessing and resolving ethical or business practice complaints against members in their practice of Aging Life Care.

THE CODE: ETHICAL PRINCIPLES AND STANDARDS OF PRACTICE SUPPORTING THESE PRINCIPLES Integrity An Aging Life Care Professional is honest, diligent, and accountable in the provision of service. An Aging Life Care Professional always acts in a manner that is consistent with the professional values stated in this Code.

Loyalty and Responsibility An Aging Life Care Professional is trustworthy and dependable in all aspects of both professional and business relationships. An Aging Life Care Professional maintains confidentiality, avoids conflicts of interest, and always pursues the best interest of clients.

Promoting Benefit and Avoiding Harm An Aging Life Care Professional promotes clients’ interests, values, and welfare in order to maximize benefits and avoid harm. An Aging Life Care Professional is aware of potential conflicts that may arise when balancing the benefits and risks of interventions being considered. An Aging Life Care Professional strives to assure that vulnerable clients’ individual choices are maximized to the greatest extent possible.

Respect for Clients’ Rights and Dignity An Aging Life Care Professional treats clients with respect, as complete individuals with their own history, narrative, and unique cultural identity. An Aging Life Care Professional respects the rights of each client, including the right to privacy, and, for the vulnerable client, strives to balance client autonomy with the need for protection and safety.

Justice An Aging Life Care Professional behaves in a just and fair way in all professional and business relationships. An Aging Life Care Professional does not promote or sanction any form of discrimination such as discrimination based on race, ethnicity, gender, religion, sexual orientation, national origin, disability, or socioeconomic status.

How to Find an Aging Life Care Manager™ Near You All Aging Life Care Managers™ have similar skill sets, but the care managers best equipped to help you are the ones in your area. Aging Life Care Managers are experts on the resources available in their various geographic regions.

We have done our best to group the Mid-Atlantic Aging Life Care Association (ALCA) members into geographic regions based on the states served (Pennsylvania, Delaware, Maryland, Virginia, West Virginia and DC), but many of our members serve large areas that include several jurisdictions. For instance, some care managers in Pennsylvania also serve border areas over the state line in Delaware, and vice versa.

Why Choose an Aging Life Care Manager?

AALCA promotes the highest standards of practice. Membership in ALCA is open only to qualified individuals with specialized degrees and experience in human services, including social work, psychology, gerontology or nursing.

Those who are at the Advanced Professional level of membership hold one of four ALCA-approved certifications.

Only members of ALCA can call themselves Aging Life Care Managers.

Aging Life Care Managers have diverse experiences, credentials and backgrounds. Many are licensed in their state in specific fields (such as nursing or 4

social work).

Aging Life Care Managers may call themselves professionals, consultants, advisors or by other titles. All care managers do not specialize in all areas of aging life care. When looking for a care manager, find out his/her areas of expertise. You will want to hire someone who regularly handles clients with needs similar to yours.

Aging Life Care Managers who primarily work with older adults bring more to their practice than an expertise in geriatrics. They bring a deep knowledge of aging issues that allows them

and their staff to overcome the myths related to aging and focus on the problems at hand. At the same time, they have tremendous experience working with resources in your community. They are fully aware of real life problems, health and otherwise, that emerge as individuals age and the tools that are available to address those issues. They are also connected with a community of social workers, nurses, psychologists, elder law attorneys, advocates, and other elder care professionals who may be of assistance to you.

QUESTIONS TO ASK WHEN LOOKING FOR AN AGING LIFE CARE MANAGER It is important for consumers to ask questions. Some of these include: • What are the primary services provided by your agency/business?

• How many Aging Life Care Managers are in your agency/ business?

• Is there a fee for the initial consultation and, if so, how much?

• What are your professional credentials?

• Are you licensed in your profession?

• How long have you been providing aging life care or care management services?

• Are you available for emergencies?

• Does your company also provide home care services?

• How do you communicate information?

• What are your fees? (These should be provided to the consumer/ responsible party in writing prior to services starting.)

• Can you provide me with references?

The answers to your questions will assist you in determining whether a particular Aging Life Care Manager or agency/business has the qualifications important to you for a successful relationship.

If you have a specific issue that requires immediate attention, be sure to inform the Aging Life Care Manager of this during the initial conversation.

Aging Life Care Association™ (ALCA) Membership Levels The Aging Life Care Association™ (ALCA) offers several levels of individual membership and benefits that respond to the needs of Aging Life Care Professionals™ at every step of their careers.

The profession of Aging Life Care™ / geriatric care management is growing and different professionals within the field have different levels of experience, education and certification that support their practice. ALCA is proactively meeting the needs of the profession by creating membership categories that recognize high certification standards and professionalism, while also being inclusive to allow all practitioners of Aging Life Care/care management a home within ALCA. The end goal of ALCA and all its members is improved service and quality of life for older adults and their families.

Advanced Professional Member Advanced Professional Members must: • Hold a Bachelor’s, Master’s or Ph.D. with at least one degree in a field related to Aging Life Care/ care management (i.e. counseling, gerontology, mental health, nursing, occupational therapy, physical therapy, psychology or social work); • Be primarily engaged in a paid position in the direct practice, administration or supervision of client-centered services to the elderly and their families; • Have two years of post-degree supervised experience in a paid position in the field of Aging Life Care/care management within the last ten years; and • Hold a current ALCA-approved certification.

Professionals can also qualify for the Advanced Membership level if they hold a Bachelor’s, Master’s or Ph.D. 6

MEMBERSHIP LEVELS AT A GLANCE

• Advanced Professional - AVP • Professional - PRO • Associate - ASC • Student - STU • Emeritus - EMR degree in a field unrelated to Aging Life Care/care management. However, these members must meet all of the other criteria outlined above, and they must have three years of post-degree supervised experience in the field of Aging Life Care/care management within the last ten years.

The Advanced Professional level also includes members designated as Fellows of the Leadership Academy.

Professional Member Professional Members must: • Hold an Associate’s, Bachelor’s, Master’s or Ph.D. degree with at least one degree held in a field related to Aging Life Care/care management (i.e. counseling, gerontology, mental health, nursing, occupational therapy, physical therapy, psychology or social work); • Be primarily engaged in a paid position in the direct practice, administration or supervision of client-centered services to the elderly and their families; and • Have two years of post-degree supervised experience in a paid position in the field of Aging Life Care/care management within the last ten years.

Members may also qualify if they hold an Associate’s Nursing degree or Diploma in Nursing and meet all of the other criteria outlined above.

Practitioners can also qualify for the Professional Membership level if they hold a Bachelor’s, Master’s or Ph.D. degree in a field unrelated to Aging Life Care/care management. However, these members must meet all of the other criteria outlined above, and they must have three years of post-degree supervised experience in the field of Aging Life Care/care management within the last ten years.

Associate Associate Members must: • Hold an Associate’s, Bachelor’s, Master’s or Ph.D. degree; • Be primarily engaged in a paid position in the direct practice, administration or supervision of client-centered services to the elderly and their families.

Members may also qualify if they hold an Associate’s Nursing degree or Diploma in Nursing and meet all of the other criteria outlined above.

Practitioners can also qualify if they hold an LPN, PTA or OTA license and meet all of the other criteria outlined above.

Student Member A non-voting member who is presently enrolled in an undergraduate, graduate or certificate program from an accredited University or College with an interest in Aging Life Care/care management. Members must show proof of enrollment with each renewal.

Emeritus Member A non-voting member who is retired from the active practice of Aging Life Care/care management and has been an ALCA member in good standing at the Advanced level (formerly Certified Geriatric Care Manager) for a minimum of five (5) years.

Study Shows Value of Aging Life Care Managers to Caregivers n

A recent study of family caregivers and others who authorize and pay for care management services reveals that these “responsible parties” find great value in hiring an Aging Life Care Professional. Until now, researchers had paid scant attention to the perceived worth of care management services to caregivers, focusing instead on clients’ perceptions.

Almost all of the caregivers and other responsible parties surveyed in the study, 97 percent, described Aging Life Care Management as having a positive effect on the individuals in their care. And even more of those surveyed, 99 percent, reported that use of Aging Life Care Man-

agers had improved their own lives.

“This type of service seems to greatly benefit the family caregivers – a group that is statistically dwindling in size, struggling with increasing responsibilities, and looking for help,” say the study authors.

The findings, presented by researchers from Webster University in Orlando, Florida, come from a 2015 survey of 177 responsible parties who used the services of members of the Florida Chapter of the Aging Life Care Association(ALCA).

Family members caring for an older adult ALCA client comprised 82 percent of the survey sample. Trust officers, attorneys, professional guardians, friends,

and neighbors made up the remainder of the responsible parties surveyed.

Participants in the survey answered 16 questions focused on three themes. The nearly unanimous praise for Aging Life Care Management, cited above, arose from a theme on the perceived effects of care management. Questions in this theme addressed effects both on caregivers’ lives and on the lives of those in their care. These questions revealed that 94 percent of respondents felt peace of mind as a result of having an Aging Life Care Manager on the job. of which they had been unaware.

Another theme focused on how and why responsible parties chose to use an Aging Life Care Professional. The most common reason, chosen by 73 percent of respondents, was the need for a local person to coordinate care when the family did not live nearby. Many respondents, 69 percent, cited the need for help in dealing with a change in the client’s health status or for aid during a crisis.

And 45 percent of participants reported a desire for assistance in exploring options for the client’s care.

These results “show how professionals can best assist informal caregivers who may have limited ability to be directly involved,” the authors note. “These informal caregivers appear to highly value having a ‘professional to call on at all times,’ and feel that the older adult clients value this arrangement as well.”

When asked what factors influenced their choice of a specific Aging Life Care Professional, 69 percent of respondents identified the professional’s background and credentials as key. Many respondents, 63 percent, also preferred care managers referred by a professional source.

Questions in the third theme asked participants to rank the importance of various care manager functions. Assessing, monitoring, and updating responsible parties on client needs received a top ranking by 89 percent of study participants. Close behind, at 80 percent, was the professional’s role in managing crises, safety concerns, and conflicts in the client’s life. Many respondents, 75 percent, also appreciated professional help in coordinating and advocating for the client’s medical concerns. When asked which functions of Aging Life Care Professionals were most important to them personally, the top answer, given by

90 percent of responsible parties, was to provide assistance during emergencies.

Surprisingly, this part of the survey also found that many respondents, 56 percent, relied on Aging Life Care Professionals to coordinate or monitor care at residential facilities. “Aging Life Care services have often been associated with the home health care industry and with helping older clients in a home setting,” the study authors point out. “Our study indicated that while Aging Life Care Professionals serve some clients in their homes, many others work with clients in assisted living or nursing home facilities.

This finding points to a new area of discussion for families and other caregivers who interact with older adults no longer living in their own homes.”

The survey of perceptions of family caregivers and other responsible parties was funded by the Florida Chapter of the ALCA. It is published in the March 2017 issue of the Journal of Aging Life Care.

The expertise of Aging Life Care Managers can be summarized into 8 knowledge areas. They include: Health and Disability - Aging Life Care Managers attend doctor appointments and facilitate communication. These professionals help determine types of services – including home health and hospice – that are right for a client and assist in engaging and monitoring those services.

Financial - Services may include reviewing or overseeing bill paying or consulting with a client’s accountant or Power of Attorney.

Housing - Aging Life Care Managers help families and clients evaluate and select the appropriate level of housing or residential options.

Families - Aging Life Care Managers help families adjust, cope and problem-solve issues surrounding long-distance and in-home caregiving.

Local Resources - Aging Life Care Managers know the local resources in their communities like the back of their hands and know how services are accessed.

Advocacy - Aging Life Care Managers are strong and effective advocates for clients and their families, promoting the client’s wishes with health care and other providers, ensuring that client’s needs are being adequately addressed.

Legal - Aging Life Care Managers refer to legal experts, like elder law attorneys, estate planners, and Powers of Attorney.

Crisis Intervention - Aging Life Care Managers offer crisis intervention when it is needed. For families that live at a distance, this can be a muchneeded 24/7 emergency contact.

How Care Managers and Elder Law Attorneys Work Together Partnering our way through happily ever afters and the occasional horror story

Elder Law attorneys focus much of their time on drafting documents to ensure for quality of life through Advance Directives. They also make sure the financial picture is handled according to the client’s needs. From revocable or irrevocable trusts to Power of Attorney documents (POAs) to naming joint or successor agents, Elder Law attorneys can handle all the legal needs of seniors and their families.

Some Elder Law attorneys also act as a variety of agent-types, just like family members of clients. They can serve as Guardians Conservators, POAs, and Trustees (hereafter simply known as “agents”).

Sometimes the client’s family members are available to serve as agents, and do a great job. There are plenty of happily ever afters out there.

Other times, though, there are no suitable family members available, either because there actually are none, or because they aren’t “suitable.”

“Un-suitable” can result in a wide variety of scenarios, some of which are sadly comparable to either dark comedies or horror stories.

Enter the Aging Life Care Manager also known as Care Managers or Geriatric Care Managers (GCMs). Aging Life Care Managers are professional guides and advocates for families who are caring for older relatives or disabled adults.

On the national level, these professionals are represented by the Aging Life Care Association.

Elder Law Attorneys Hiring Care Managers One common joint effort between Care Managers and Elder Law attorneys

occurs when the Elder Law attorney is the agent for an individual client. The attorney agent hires and directs the Care Manager to assess the client and come up with a care plan.

Once the attorney approves the care plan, he or she hands the Care Manager the power of implementation. The Care Manager sets off on their gallant horse to ensure the best possible care for the client over sometimes quite treacherous terrain.

Care Managers are often called upon to mediate family feuds, locate the best placement or secure home care for a client. They also make sure that care plans continue to be the best fit over time, as well as change course when needed.

They make and attend appointments and procedures. Finally, they are the advocates in all sorts of circumstances: medical, entitlement benefits, social and emotional wellbeing and more.

Planning Ahead for Care Management Needs A growing trend is for people who are having Estate Planning and Advance Directives drafted by an attorney to include language in anticipation of needing a Care Manager in the event of incapacity. For instance, a Care Manager may be needed if there is an illness or disability that leaves an individual unable to manage his or her own care. Many people realize that their “agent-family-memberin-charge:” a) likely has a job and a family of their own, and b) doesn’t happen to be an expert on elder care.

“direct” the agent to hire a Care Manager (meaning the agent pretty much has to hire one), and some documents state the agent “may” hire one. This distinction can have to do, for example, with whether the document is a POA, with “may/ suggest” type language, or a Trust, with “may, direct, or shall type language.”

The estate planning client who may be interested in including Care Management planning should discuss reasons for differences in wording with their attorney. Scenarios where such a discussion may be necessary could include: the Client’s level of confidence in the agent (will they really do what I want them to?), or the client’s confidence in the future care budget (will my funds support making my agent hire a Care Manager, or should I just suggest it)?

Selecting a Senior Housing Option Retirement Communities Offer Something for Everyone Offering everything from lowmaintenance homes to luxury apartments, retirement communities in the Mid-Atlantic Region are as distinctive as their residents. Local retirees have a seemingly endless array of options to choose from when selecting their new home. However, before they can select the appropriate community, retirees and their loved ones should be sure to fully understand all the options.

Learning the basic terminology used in the retirement living industry will help consumers begin to consider and compare options. The chart after this article features a list of communities that offer a variety of retirement living options.

Continuing Care Retirement Communities The wide range of retirement living options starts with Continuing Care Retirement Communities (CCRCs), which generally feature care options ranging from independent living to long-term nursing care on one campus. Residents can transfer to higher levels of care as needed and enjoy a wide array of amenities and activities.

CCRCs offer long-term contracts that guarantee lifelong shelter and access to specified health care services. Most CCRCs establish requirements for incoming residents based on age, financial assets, income level and physical health.

Residents are typically expected to move in while they are still independent to get to know the community. In return for guaranteed lifelong shelter, amenities and health care, residents usually pay a lumpsum entrance fee and regular monthly payments.

Independent Living Communities For seniors who want to leave the burdens of home maintenance behind, Independent Living Communities may be the ideal option. In addition to property

upkeep, these communities also generally offer 24-hour security and activities to keep residents engaged.

Independent Living Communities are similar to any apartment, condominium or single-family development, except that they provide special services including security and activities. These communities typically offer a full range of activities such as shopping trips, outings to cultural events and organized gatherings that promote socialization. Many communities also feature tennis courts, swimming pools, activity rooms and other amenities to keep residents active and engaged.

Assisted Living Communities Individuals who need assistance with the activities of daily living—including eating, dressing, walking, transferring and toileting—may want to consider an Assisted Living Community. In addition to daily assistance, these communities also offer activity programs to fit the interests of almost any resident.

As opposed to Independent Living Communities, health care services are available at Assisted Living Communities.

Residents of assisted living generally need some assistance with at least one of the activities of daily living. They may also need transportation assistance and help with housekeeping and laundry, all of which are typical offerings. Additional amenities can include private units, state-of-the-art facilities, beautifully decorated common areas, barber shops and beauty parlors, pharmacies, physical therapy services, recreation rooms, libraries, gardening areas, fitness centers, and many others.

Nursing and Rehabilitation Centers Finally, whether for a short-term rehabilitation or a long-term medical stay, Nursing and Rehabilitation Centers are a vital senior living option. These centers are equipped to help residents heal and/ or maintain their best quality of life and

often feature a full schedule of activities.

At a Nursing and Rehabilitation Center, care is administered by professionals under the direction of a physician. Many facilities also offer sub-acute, respite, rehabilitation and other short-term care.

Rehabilitation services can be especially helpful for individuals who are recovering from surgery or an illness.

Choosing a Community After selecting the type of community that best fits their wants, needs and budget, retirees should start calling individual communities to request additional information. Communities will also likely have detailed websites, as well as staff members who will be happy to answer questions about the property.

When researching potential communities, retirees should be sure to ask about admission and discharge criteria. Prospective residents should fully understand the entrance and monthly fees, as well as any additional fees that may be incurred.

Finally, potential residents should ask about amenities, activities and resident involvement.

Once they have narrowed their search down to a few communities, potential residents should plan on-site visits with friends or relatives. Only by visiting a community in person can individuals get a true feel for the lifestyle residents enjoy.

Focusing on Individuals Understand person-centered living The non-profit national advocacy organization previously known as the Consumer Consortium on Assisted Living (CCAL) is now known as CCAL-Advancing Person-Centered Living. They helped coin the term “person-centered living” that is now commonly heard in the assisted living and personal care field. While the term may be familiar to professionals, it may still be confusing to consumers who are just beginning to investigate assisted living/ personal care options. So what is personcentered living?

CCAL coined the term person-centered living (PCL) as a reminder that as people grow older or have disabilities they should not experience a loss of humanity or choice regardless of where they live. PCL means living as one chooses to.

PCL is based on humanistic beliefs that every individual has a unique background, human experience, and the right to determine how to live his/her own life. PCL is also relationship-based and focused on supporting the individual’s emotional, social, physical and spiritual well being (e.g., belonging, experiencing a continuation of self-worth, purpose, meaning, enjoyment, comfort and opportunities for growth).

CCAL upholds the PCL principle, “Nothing About Me Without Me.” This simple phrase means that every elder and individual with a disability is provided choice, privacy, dignity, respect, independence and autonomy.

Elders and individuals with disabilities have the right to determine their needs; decide how best to have those needs met; and

give feedback about the quality and nature of the care, services and supports they receive. If support is needed, supports are centered on their personal preferences and values that honor their selfhood.

A person-centered culture is nurturing, empowering and respectful, and it optimizes the well being of not only the elder and individual with a disability but also the family and larger caregiving and/or services and support network.

Ultimately, person-centered living means being kind, respectful and sensitive to those being served and honoring their right to make their own choices, regardless of the setting.

New Medicare Cards Offer Greater Protection n

Good news from The Centers for Medicare & Medicaid Services (CMS) for the more than 57.7 million Americans on Medicare. To combat fraud and illegal use, the new Medicare cards will no longer contain Social Security numbers.

How will Medicare Cards change?

The new cards will use a unique, randomly-assigned number called a Medicare Beneficiary Identifier (MBI).

This will replace the Social Securitybased Health Insurance Claim Number

(HICN) currently used on the Medicare card. CMS will begin mailing new cards in April 2018. Furthermore, this will meet the congressional deadline for replacing all Medicare cards by April 2019.

“We’re taking this step to protect our seniors from fraudulent use of Social Security numbers which can lead to identity theft and illegal use of Medicare benefits,” said CMS Administrator Seema Verma. “We want to be sure that Medicare beneficiaries and healthcare providers know about these changes well in advance and have the information

Providers and beneficiaries will both be able to use secure look up tools. These Tools will support quick access to MBIs when they need them. .

Planning for the Care You Need at Home n

If you took a poll of older adults and asked each person if they wanted to remain in their own home until the end of their life, most people would say, “of course.” That is a wonderful goal, which often takes a lot of sound planning to be successful. Anything can get in the way—accidents, illness, lack of energy, declining physical abilities—of achieving your desired independence. It is hard to give up even a little autonomy, but asking for assistance can help keep you where you want to be—in your own home.

What if You Become Homebound?

If you are recovering from an acute illness, you may be eligible for home health services that are paid for by the Medicare program. An Aging Life Care Manager™ can help you access licensed home health services by facilitating a conversation with your physician about your need for skilled nursing services, physical therapy, occupational therapy, and/or speech therapy in your home.

This is an important conversation to have with your doctor and discharge planner if you are in the hospital or a

nursing facility and need to decide on and plan for services in your home once you are discharged.

However, Medicare will only pay for these services if you are “homebound.”

Homebound is defined as only leaving your home for “medical treatment or short, infrequent absences for nonmedical reasons, like attending religious services.”

You may also receive help from a home health aide under the supervision of a professional nurse or therapist. A physical or occupational therapist can

Accessing services These services must be obtained from a state licensed home health agency certified to provide Medicare-reimbursed services. They may be freestanding agencies, or they may be part of a large, hospital-based organization. Licensed home health services are usually timelimited, ending when you have met the goals of treatment and the need for the skilled care has ended.

If you have enrolled in a Medicare Advantage program, you may also have access to additional services such as home visits after a hospitalization.

Other Home Care Services Many other care services are pro-

vided in the home but are not licensed or paid for by Medicare. An Aging Life Care Manager can help you understand what they can offer, access reputable services, and monitor them on an on-going basis.

Non-Medical Aides, Companions and Homemakers There are many types of paid care giving services available—often called non-medical aides, companions and homemakers—who many people hire privately. If you do, you or your family should check at least two work-related references, develop a specific set of tasks and expectations for the caregiver, arrange to pay all appropriate taxes, and have the required insurance coverage.

Caregiver services can also be arranged through an agency. While this may cost more per hour than hiring an

aide yourself, hiring an agency assures you of coverage when the aide is sick, on vacation or quits. The agency also carries liability insurance and is responsible for all employment taxes and employee bonding. An Aging Life Care Manager can help you find a reputable agency and get important questions answered, such as: do they check police and driver license records; do they provide supervision; how do they handle cancellations; and how long have they been in business?

Your family can come together and construct a plan for supporting you in your wishes to remain in your home. If it becomes clear that you all need help in doing that, hiring an Aging Life Care Manager is a wise investment. Understanding Costs Can Help With Decision Making As the decision to remain at home is becoming an increasingly popular option, it is important to understand the costs of care and your payment options while researching different home healthcare companies.

Costs for companion care are covered primarily by private payments.

Home healthcare must also be paid for with private funds, unless it is prescribed by a physician and provided by a Medicare-certified agency. Long-term care insurance policies may cover companion care and home health care. If you qualify for Medicaid, a government program for low-income individuals, it may subsidize some of your homecare needs. Since laws vary from state

to state, you will need to contact your local Area Agency on Aging for more information concerning government benefits.

Skilled nursing care will be paid for

by Medicare as long as it is determined to be “intermittent skilled care,� which includes occupational, respiratory, speech and physical therapy. To be covered, care must be ordered by a doctor. Individuals who meet income requirements may be eligible to receive skilled nursing services through Medicaid. Major medical health insurance will also cover a limited number of skilled home visits. Costs for these services vary greatly depending upon coverage by Medicare, Medicaid and private insurance.

Contact your local Office on Aging or State Department of Health to find out if there are funds from social services block grants available to cover home care costs.

Medicare and Home Care Know the coverage options

When it comes to understanding Medicare coverage, many individuals can find themselves confused by a complex web of information. This especially applies to those who want to know if they are eligible for Medicare coverage when entering a nursing home or receiving home health care. Below is some helpful information on whether you can use your home health care benefits if you have Medicare and what services and products it will cover.

First, you can use your home health care benefits if all of the following applies to your situation. Your doctor must prescribe that you need medical care at home and create a care plan. The care you need has to be intermittent and in the form of skilled nursing care, physical therapy, oc-

cupational therapy or speech therapy. The home health agency must be Medicarecertified and you must be homebound, or unable to leave the home unassisted.

Once you have established those requirements, it is important to understand what Medicare will actually cover, which includes the following: • Home health aide services on a parttime or intermittent basis, as long as there is a skilled need • Medical social services • Certain medical supplies • Durable medical equipment (wheelchair, walker, bedside commode, etc.)

Given the services and products that Medicare does cover, it is also valuable to take note of what is not covered. Care that

is required 24 hours a day will not be covered. Other services not covered include personal care provided by home health aides such as bathing and dressing, along with homemaker services like cleaning and laundry. Also not included are homedelivered meals.

If you require care that goes beyond intermittent visits for a limited amount of time, or if your needs are not covered by Medicare, an important option to consider is private duty home care. For more information on Medicare coverage, visit medicare.gov.

2. A re social security, federal and state taxes, and unemployment insurance paid so the family is not legally responsible?

3. I s there verification that the paid caregivers can legally work in the U.S.?

4. If the hired caregiver is injured, who is responsible?

Remember that many

homeowners’ insurance policies exclude such injuries via “domestic employee exclusions.”

5. A re criminal history background checks, state abuse registry checks, and prior work reference checks completed?

6. Is there bonding and insurance in place that would cover any injury to the client, damage, or theft? When an agency says they are “bonded,” family members should be sure that this covers injury

or damages related to the caregiver(s) working in the home.

7. Assuming family is not available to supervise paid caregivers at all times, will there be documentation that substantiates the completion of services?

If the adult caregiver does not have the time or resources available to answer these important questions, the services of a professional home care agency or adult day care center may be the right solution.

Five Myths About Hospice and End-Of-Life-Care n

1. The patient must have a written referral from their doctor before they can be assessed to see if they are appropriate for hospice care.

NOT TRUE. A patient or their family/ medical POA, can ask for an assessment by hospice at any time. This is an opportunity to learn what supports hospice can provide, and to answer any questions that the patient and family may have. If the patient decides that they would like to elect to use their Medicare Part A benefit for hospice, THEN the physician will need to write a referral to hospice and a hospice nurse will be sent out to do a comprehensive assessment, explain the hospice benefits to the patient and their family, and the patient or their Medical Power of Attorney will sign the admission paperwork.

2. Once you elect to be on hospice, you can never change your mind.

NOT TRUE. At any time, a hospice patient can revoke their hospice status.

There could be several reasons for this, such as falling and breaking a hip, and choosing to have surgery to repair or replace it, as well as rehab services to regain function. You may also wish to try a new curative treatment, or to resume dialysis. You can also be re-assessed by hospice and re-admitted when your condition declines again.

3. Hospice provides 24 hour care for you when you are diagnosed with a terminal illness and have 6 months to live (or less) if the disease runs its normal course.

NOT TRUE. While hospice provides 24 hour support to the patient and their family and they have access to a hospice 52

nurse 24/7, the hospice benefit does not provide 24/7 custodial care. Support is provided to the patient in their home, which may be a private home, an apartment in an assisted living facility or a room in a skilled nursing facility and can include several hours per week of homecare. If an acute level of support is required, all hospices have access to a residential facility where patients can receive respite care or a higher level of pain management. This could be their own facility, or a relationship with a local skilled nursing facility giving them access to beds there.

4. Once you are admitted to hospice, you can remain on hospice until you die, even if you live longer than 6 months.

NOT TRUE. After the initial 90-day admission period, hospice staff are required to assess each patient every 60 days to determine if they are still hospice eligible. During this time, the patient must continue to experience “measurable” decline in order to remain hospice

eligible. The hospice providers’ records are regularly audited and they can have payments retracted if they continued to provide hospice services past the time that the patient is considered eligible.

5. Electing hospice means giving up and hastening death by withholding food and/or medication.

NOT TRUE. A hospice nurse will review all medications that each patient is taking, but it is up to the patient, and/ or their family, which medications they want to remain on. The focus of hospice care is on comfort, so anything that can make the patient more comfortable is done. While on hospice, patients continue to be offered favorite foods/beverages, although the texture may need to be adjusted to help with safe swallowing and to avoid aspiration (for example, pureeing food or thickening fluids).

In the last days and hours of life, it is normal for patients to limit their intake as their bodily functions begin to slow down and prepare for end of life.

Alzheimer’s Behavior Challenges – Tips for Managing Care n

Bobbi Kolonay, RN BSN MS CCM AHN- BC, President of Holistic Aging

Tip 1: Identify causes of problem behavior Problem behavior is often a way the Alzheimer’s patient tries to communicate with you. The progression of the disease means that they may no longer be able to communicate verbally. However, they are still emotionally conscious and will remain so, often until the very end of life.

In many cases the patient’s behavior is a reaction to an uncomfortable or stressful environment. If you can identify the stressor or trigger of their discomfort, you can resolve the problem behavior with greater ease. Remember that the person with dementia is not being deliberately difficult. Their sense of reality may be different to yours, but it’s still very real to them.

Some ways to help identify the causes of problem behavior: • Try to put yourself in the person’s situation. Look at their body language and imagine how they might be feeling and what they might be trying to express.

• Ask yourself what happened just before the problem behavior started? Did something trigger the behavior?

• Are the person’s needs being met?

• Does changing the environment or the atmosphere help to comfort the person?

• How did you react to the behavior? Did your reaction help to soothe the person or did it make the behavior worse?

Tip 2: Create a calm and soothing environment As a caregiver, you may not be able to control the person you’re caring for.

However, if you’re caring for your loved one at home, you can control the environment and the atmosphere you create

while caregiving. These factors play a large part in helping a person with Alzheimer’s feel calm and safe.

• Modify the environment to reduce potential stressors. o Stressors create agitation and disorientation in the person with Alzheimer’s. These include loud or unidentifiable noises, shadowy lighting, mirrors or other reflecting surfaces, garish or highly contrasting colors, and patterned wallpaper.

• Maintain calm within yourself. o Getting anxious or upset in response to problem behavior can increase their stress or agitation. Respond to the emotion being communicated by the behavior, not the behavior itself. Try to remain flexible, patient, and relaxed.

Remember, the person is responding to your tone of voice and body language more than the content of what you’re saying. If you find yourself becoming anxious or losing control, take a time out to quickly relieve stress.

Manage stress in someone with Alzheimer’s: Different stress-reducing techniques work better for some people with Alzheimer’s than others, so you may need to experiment to find the ones that best help your loved one.

• Exercise o Exercise is one of the best stressrelievers for both the person with Alzheimer’s and you, the caregiver. Regular walking, movement, or seated exercises can have a positive effect on many problem behaviors, such as aggression, wandering, and difficulty sleeping. Indoor shopping malls are vast walking opportunities protected from the weather.

Or you may even consider singing and dancing.

• Simple activities can be a way for the person to reconnect with their earlier life. o Someone who used to enjoy cooking, for example, may still gain pleasure from the simple chore of washing vegetables for dinner. Try to involve the person in as many productive daily activities as possible. Folding laundry, watering plants, or going for a drive in the country can all help to manage stress.

• Use calming music o Play a calming song or the person’s favorite type of music as a way to relax them when agitated. Music therapy can also help soothe the person during mealtimes and bath times, making the processes easier for both patient and caregiver.

• Interacting with other people is still important o While large groups of strangers may only increase stress levels for the person with Alzheimer’s, spending time with different people in one-on-one situations can help to increase physical and social activity.

• Pets can provide a source of positive, nonverbal communication o As a result of the playful interaction and gentle touch from a welltrained, docile animal, the person may be soothed and experience decreased aggressive behavior.

• Aromatherapy o The use of Lavender Essential Oil (Lavandula Angustifolia) to calm a person with anxiety has been demonstrated in numerous medical studies to be very effective in decreasing anxiety and improving restlessness.

Untreated Hearing Loss – Our Unrealistic Expectations n

Untreated hearing loss (i.e. not wearing hearing aids) has been linked to numerous health complications such as diabetes, hypertension, depression, anxiety, and even cognitive decline. However, an estimated 28 million people in the United States experience some type of treatable hearing loss. Yet, less than a quarter of those people seek help to enhance their listening lifestyle.

Why One answer is the unrealistic expectation attached to hearing aids: that hearing aids are going to return hearing back to “normal.” Unfortunately, this is not the case. An individual with hearing loss needs to utilize effective communication strategies in conjunction with hearing aids. Combining the hearing and visual senses sets a hard of hearing individual up for less communication breakdown, positive relationships, and ultimately a better quality of life.

You or your loved one should get your hearing evaluated by an Audiologist if you are experiencing any of the following:

• Muffled, mumbled speech sounds •T rouble understanding when a speaker’s face is unseen • Difficulty hearing on the telephone •D ifficulty hearing people speak in a crowd (i.e. background noise) • Trouble

understanding the voices of women or children •F riends or loved ones complaining you turn the volume of your TV or radio too loud

• Be patient if response is slow. Making sense of conversation takes time and is fatiguing.

• Stay positive and relaxed.

EFFECTIVE COMMUNICATION STRATEGIES Communication is a two-way street.

Good communication requires the efforts of at least two people, even when one of them is utilizing hearing aids.

Tips for Communicating as an Individual with Hearing Loss: • Anticipate difficult situations, plan how to minimize them.

• Pay attention. Watch, listen & concentrate to follow conversation.

• Look for visual cues like facial expressions, gestures and body language.

• Don’t interrupt. Let conversation flow to gain more meaning.

• Admit if you are lost.

• Conversation can be tiring. If too tired to concentrate, ask for discussion later.

Tips for Communicating with Individuals with Hearing Loss: • Always face the person you are speaking with • Make your mouth visible • Avoid noisy backgrounds • Get attention first e.g. “Jim, could you pass the vegetables?”

• Don’t shout. Shouting distorts your face and mouth • Speak clearly, at moderate pace • Rephrase if you are not understood & try different words • Use facial expression and/or gestures

Because of the many services and programs they offer, Area Agencies on Aging (AAAs) are one of the single most important retirement resources. Available services vary by locale, and the list below is just a sampling of the ones that are routinely offered by AAAs. Services provided by most AAAs: • Adult Day Care is available for individuals who need daytime supervision.

• Case Management Services are conducted by professionals who can assess needs and arrange for services to encourage continued independence.
