PRIORITY HEALTH UPDATE- NEW PROGRAM AVAILABLE

Welcome To Gtc Judge Hamlyn

A new program will become available to GTC’s employees that are enrolled in Priority Health beginning 04/01/2023. Priority has partnered with Livongo to provide its members with a best-in-class program to assist with managing their type 2 diabetes Any member who is at least 18 years old and has a diagnosis of type 2 diabetes will be eligible to enroll. Pregnant members will be excluded from eligibility. Members may be contacted through direct mail, or a referral from their provider or Priority Health Care Management

Once enrolled, Livongo will provide members a connected blood glucose meter, unlimited test strips, and lancets, access to coaches at any time with a personalized approach, and much more with the overall goal to reduce A1c levels, all without extreme dieting The Livongo member journey currently exists within the Livongo member website or Livongo app. Priority is working towards a solution that would bring this into the Priority Health member portal and they will communicate that progress as it becomes available Members will have access to share functions within their website to share the success of the program with their providers or other advocates that are a part of their support team To provide a better visual of what you will be experiencing if you enroll, please see below the Welcome to Livongo video geared toward the member journey Click here
