Good Shepherd has been providing care to the Lehigh Valley community and beyond since 1908. Last year, Good Shepherd provided services to more than 60,000 patients. Our goal at Good Shepherd is to put patients on the path to independence.

Good Shepherd Information Good Shepherd was founded in 1908 when The Rev. John and Estella Raker invited a disabled orphan named Viola Hunt into their Allentown home. Their son, The Rev. Dr. Conrad W. Raker, carried on their vision, and Good Shepherd has advanced many new types of rehabilitation care in its more than108 year history.

NO Tobacco Policy: Good Shepherd is a tobacco-free organization. Smoking and the use of tobacco products are not permitted anywhere on the campus, which includes the parking deck.

NO Weapons Policy: For your safety, and the safety of others, weapons are not permitted on the Good Shepherd campus or any of its satellites.

Parking: Good Shepherd oers free parking in its parking deck located on S. 5th Street.

Free valet parking also is available at the Health & Technology Center, as well as at the front of the Rehabilitation Hospital. When you arrive, simply come to the reception desk at either facility, and our security oďŹƒcers will park your car.

Please make sure to lock your car and keep any valuables out of sight.

If it is dark or you would like assistance getting from your vehicle to the hospital (and back), please feel free to call our security desk at 610-776-3541, and they will escort you to or from your vehicle.

To avoid crossing the street, particularly during busy times, you can get to the Rehabilitation Hospital and the Health & Technology Center by taking the elevator or stairs to the 3rd level of the parking deck and come across the bridge. Once you cross the bridge, you will be on the middle level of the Health & Technology Center. To reach the hospital, take the elevator (which will be on your left) to the lower level. Upon exiting the elevator, go through the double doors to the first floor of the Rehabilitation Hospital.

Pet Visitation Policy: Should you want to bring your pet into Good Shepherd, the following guidelines must be met: • A request needs to be made to the director/manager of the patient care unit to determine appropriateness, time, place and length of visit.

• The animal must be immunized against diseases common to that type of animal.

• Vaccinations must be current, and proof of vaccination must be presented prior to the pet visitation.

• The person bringing the animal to the unit is responsible for the animal’s actions at all times.

• Animals must be kept on a short leash or in a cage.

• No food for the pet should be brought in to the hospital.

• Visitor Trays: We can provide visitors with “visitor trays” from the same menu as the patient, allowing both the visitor and patient to dine in the patient dining rooms on the units. To order: - During normal visiting hours, visitors can purchase a meal voucher from the receptionist located at the main hospital entrance.

- At the time of the meal service, present the voucher in the dining room to receive a meal.

- Dinner meals must be ordered by the receptionist no later than 4:00 p.m.

Gift Shop: The Good Shepherd General Store is located in the lobby of the Rehabilitation Hospital beside the main reception desk.

Phone: 610-776-3554 Optimal Fitness: Good Shepherd has a fitness center located on the lower level of the Health and Technology Center that is open to the community. There is no contract or initiation fee required, so you can pay on a monthly basis. The fee is $35.00/month. The gym has cardiovascular equipment, free weights, and Nautilus equipment. Much of the equipment is fully accessible.

A Typical Day in Rehab: Your rehabilitation program begins on the first day of admission.

Your program will be designed for your individual needs by your team.

A typical day in rehab may include: 5-7 a.m.: Your day begins with our nursing staff giving you your medications (depending on when they are scheduled to be given) and taking your vital signs.

6:30 - 9 a.m.: Our nursing and occupational therapy staff will help get you out of bed, dressed and groomed. You may receive medications at this time, lab work or respiratory treatments. You will receive your therapy schedule for the day.

8 a.m. - 9 a.m.: Breakfast in the dining room. We encourage you to come to the dining room and meet others.

8:30 a.m.-12 p.m.: Your physician may come and visit you to see how you are doing. You will go to therapies, which may include physical therapy, occupational therapy, speech therapy and/or recreational therapy. You will receive three hours of therapy a minimum of five days a week.

12-1 p.m.: Time for lunch in the dining room. After lunch, you will be able to rest until your afternoon therapy sessions start. If scheduled, you will be given your medications.

1-4:30 p.m.: You will return to therapies, receive any needed nursing treatments and have your vital signs taken.

5-6 p.m.: Time for dinner in the dining room 6-9 p.m.: You may receive a shower. You will be given any scheduled medications and have time to spend with your family and friends. Some patient/family education may take place during this time to help your family understand your diagnosis and care. The nursing staff will then help you get to bed to enjoy a well deserved night’s rest.

Visiting Hours: Daily—10 a.m. to 8 p.m.

10 a.m. to 4 p.m. between therapies and at lunch 4 p.m. to 8 p.m. any time • Please limit the number of visitors to two per patient. We do have areas (lounges or dining areas) on each floor if you have more than two visitors.

Patient/Family Education: You, as a caregiver, play a very important role in the recovery of your loved one. Good Shepherd considers you an important part of the rehabilitation team. To prepare you for your role and to increase your comfort level, as well as the comfort level of your loved one, there are educational/training sessions that you will be required to attend. These patient/family education sessions will include: • Observation • Demonstration • Hands-on practice For best results, these sessions will be scheduled during treatment hours, Monday through Friday, with your primary rehabilitation team.

Your Care Manager will call you to schedule these sessions once the team has completed its evaluation and determined that it is time to begin the training.

Your Treatment Team: Depending on your diagnosis and individual needs, a treatment team will be established for you. Your team may include: • Physiatrist: This is a physician who specializes in physical medicine and rehabilitation. Your physiatrist will supervise your care and direct your therapy plan. We also have internal medicine physicians available, if needed.

• Rehabilitation Nurse: The rehabilitation nurse will care for your medical needs, as well as facilitating recovery of functional abilities, preventing complications, providing education on disease and disability management and restoring optimal wellness.

• Physical Therapist: Your physical therapist will work with you to increase your strength, endurance, flexibility, muscle tone, posture and coordination and help you transfer and ambulate safely. He/she will use state of the art therapies and equipment.

• Occupational Therapist: Your occupational therapist will design individualized programs to help you regain the skills needed to perform daily activities at home and work, such as dressing, grooming, bathing, etc.

• Speech/Language Pathologist: Your speech pathologist will provide therapy to improve your speech, language, cognitive and swallowing abilities. He/she will work on auditory comprehension, communication skills, attention, memory, problem solving, etc.

• Psychologist: Rehabilitation can be an emotional, as well as physical strain for some patients and their loved ones. The psychologist can be consulted to help with cognitive, behavioral and emotional needs.

• Respiratory Therapist: Your respiratory therapist will assist you in breathing techniques, energy conservation and the proper use of breathing medications.

• Recreational Therapist: Your recreational therapist will work with you to either get you back to the leisure activities that you have done in the past or develop new ones. Outings to the mall, dinner or movies are held to allow you to try out your skills in the community.

• Dietitian: The dietitian will review your diet and develop a plan for you to meet your dietary needs.

• Care Manager: Your care manager will work with you and your family to ensure a smooth transition to your discharge destination. He/she also will provide progress reports to your family, physicians and insurance company.

• You and Your Family: You and your family are the most important members of the team. We will work with you to establish and accomplish your goals.

Pastoral Care: • Good Shepherd has chaplains on staff who can provide you with individual pastoral support.

• We welcome your minister, priest or rabbi to visit.

• A meditation chapel is located on the third floor of the hospital and is available 24 hours a day. Devotional materials and Bibles are provided in the chapel.

• We provide ecumenical services on Sundays, and Eucharistic ministers visit with the Sacrament every Saturday morning.

Bringing Food to the Hospital for Patients: Before bringing any food into the hospital, please check with the nursing staff for any dietary restrictions. If you have any food preferences, please let your ambassador know when he/she calls you, and we will pass them on to our dietitians.

Discharge:

The discharge process begins at the time of admission. The team will work with you to plan a safe discharge.

Patients are usually discharged by 11 a.m. You will receive a full discharge packet, which will include instructions for taking your medications, treatments, a home exercise program, safety tips and follow-up appointments. Please take your discharge packet with you to your discharge destination and share the information with your caregivers.

Ordering Durable Medical Equipment: Depending on your needs, your therapists may order durable medical equipment for you, such as a walker, cane, tub seat, etc. We will contact vendors in your area in order to obtain the equipment that you need.

Should you need to be fitted for any custom equipment (e.g. wheelchair or bracing), the therapist will discuss with you whether it would be better to use Good Shepherd services or make contact with a company closer to your home to set up these services for you.

Outpatient Services If you are coming to Good Shepherd for outpatient services, these services will be provided on our Allentown campus at the Health & Technology (H&T) Center or at another GSRN location. The H&T Center offers a variety of advanced therapies, nursing care and technology under one roof.

When you arrive at Good Shepherd for the first time, the staff at the reception desk on the middle level of the H&T Center will be able to direct you to where you need to go.

• Wheelchairs are available at the entrance to the H&T Center on the middle level.

• Neurorehabilitation services are located on both the middle and upper levels of the H&T Center.

• Treatment sessions are individualized and can last from 45 minutes to one hour.

• Should you need to cancel your therapy session, please call 610-776-3247.

• If needed, and prescribed by your doctor, laboratory and radiology services are available on campus.
