Welcome to our bi-monthly Issue of Integra Insights: January and February edition! It’s amazing to think that around 20% of the whole Cayman Islands population is now registered with Integra Healthcare Ltd, so firstly, thank you, thank you, thank you for the trust you place in us to assist with keeping you in tip top shape.
It also means we have to communicate better too. We are frequently exposed to comments like “oh, I didn’t realise you had a fertility specialist”, or “I didn’t realise you treated XYZ” and at the same time, all of our doctors are exposed to the problems caused by patients accessing unreliable, non-evidence-based information and spurious advice, which abounds on the internet. But everybody is different and because we are required by law to retain health records, it means people can remain in our system long after they have left the Island.
Historically, we have struggled with providing preference options that don’t also interfere with operational practice emails like appointment reminders etc. To address this, and provide you with far more choice, quite rightly, we have adopted a new Customer Relationship Management system that allows us to separate operational and functional communications from health information, notices about new services and doctors etc. and this is the second Integra Insights ‘short magazine’ from that system.
We’re excited, because it means we can serve you far better, keep you informed and educate too. It is currently being issued bi-monthly, so it’s enough but not overwhelming, and always close enough to the health issues of the day.
We hope you enjoy this edition. If you have moved on or just don’t wish to be included, simply unsubscribe to these (from the email) and you can do so knowing it won’t affect your normal practice emails and notifications. If you have unsubscribed to a previous system, we obviously apologise but hope you understand our desire to get this permanently right for you, which this is designed to achieve.

Long COVID-19: The Signs and Symptoms
A majority of the people who contract COVID-19 will recover in several weeks’ time. However, there are some that may continue to experience the symptoms long after. This is called ‘long COVID-19’. We want to offer a short overview of what ‘long COVID-19’ is and the signs and symptoms to look out for.
Mayo Clinic offers an explanation of what long COVID-19 (sometimes referred to as ‘postCOVID-19 syndrome’) is stating: “Post-COVID-19 syndrome involves a variety of new, returning, or ongoing symptoms that people experience more than four weeks after getting COVID-19. In some people, post-COVID-19 syndrome lasts months or years or causes disability.”
Some of the main symptoms of long COVID-19 include:
» Fatigue
» Fever
» Shortness of breath, difficulty breathing
» Cough
Some other symptoms reported included:
» Difficulty concentrating
» Headache
» Sleep problems
» Joint/muscle pain
» Problems with digestive system e.g. stomach cramps
» Blood clot/vascular issues
There are several factors which may make individuals more susceptible to long COVID-19 which include:
» A severe case of COVID-19 where you were hospitalized/required intensive care
» Pre-existing medical conditions
» Having a condition such as multisystem inflammatory syndrome which impacted your organs while you had COVID-19 or after you had it
It’s important to note that some of the symptoms listed above might not be from long COVID-19 but another illness (e.g. some symptoms are similar to chronic illnesses such as chronic fatigue syndrome). However, if you have concerns or are experiencing the symptoms above, please get in touch to book an appointment at 745-7450. In the meantime, you can start noting down the symptoms you have been experiencing, when they started and so on to give to your physician.

What Happens During an Annual Physical?
It’s a new year meaning it’s time once again to book appointments, including your annual physical. You might say, why should I go to the doctor if I’m not currently having any problems? However, annual physicals are in fact very important.
An annual physical is required to ensure you keep healthy by having a catch-up chat with your doctor to address any questions or concerns, screen for any abnormalities and look at preventative approaches to help manage any possible conditions in the future.

World Cancer Day and Cancer Prevention Awareness Month
February is World Cancer Day (4th February) and Cancer Prevention Awareness Month. To raise awareness and increase education, we asked some of our physicians for Cancer Prevention Tips & Advice:

Skin Cancer Prevention Advice:
Dr Davide Altamura, one of our Specialist Dermatologists provides some preventative measures for melanoma: “…there is no definitive way to prevent melanoma although there are strategies of prevention that can help to reduce the overall risk:
» Using appropriate sunscreens prior to sun exposure
» Wearing UV protective clothes, hats, and sunglasses
» Avoiding sun lamps and sunbeds”
Skin Cancer Prevention Advice:
Dr Alison Duncan, also one of our Specialist Dermatologists reiterates the importance of sunscreen and UV protection and adds some further recommendations:
» One of the best ways to look after your skin is to wear a broad-spectrum sunscreen SPF 30 and higher when out and about.
» If you struggle with the reapplication of sunscreen every 2 hours, then UV protective clothing is available, and you can look good, and sun protected at the same time!”

15th February was International Childhood Cancer Day
At Integra, we join in with supporting efforts to raise awareness and increase education on childhood cancer.
Cancer in children is rarely due to environmental factors or lifestyle choices but mainly changes in cell DNA either before birth or in early childhood.
In children (0-14 years), the two main types of cancer are leukemia and brain cancer:
» Leukemia (cancers of the bone marrow and white blood cells) can present with: bone and joint pain, tiredness, weakness, pale skin, bleeding or bruising, fever, weight loss and other symptoms.
» Brain tumors can cause: headaches, nausea, vomiting, blurred or double vision, dizziness, trouble walking or handling objects, and other symptoms.
Fortunately, most children (more than 80%) with cancer today survive because of high quality clinical treatment trials and centralization of care in childhood cancer.
However, despite these great advances, cancer remains the 2nd largest cause of death in children, after accidents.
Increasing education on childhood cancer, regular checkups and contacting your physicians for any concerns (particularly if you child has a higher chance of developing a cancer, due to certain genes) is important in helping with early intervention and greater awareness of this disease.

Children’s Rash that Mustn’t be Ignored: Meningitis Rash
Nearly three quarters of patients diagnosed with meningitis are young children ages five and below. Therefore it is fundamentally important for parents to stay aware of ways to check whether their child’s rash or spots is not an indicator of something more serious such as a meningitis infection. This article breaks down how parents can test whether their children’s rash is a sign of meningitis through the ‘rolling glass method’.
Dangerous Plants in the Cayman Islands
There are a range of dangerous plants in the Cayman Islands to be aware of, such as Manchineel which is especially dangerous and possibly life threatening. More recently, the Cayman Islands Department of Environment (DOE) has also warned about a new plot of the Brazilian Peppertree, a poisonous South American plant that has recently been found in Grand Cayman. In this article, we break down four common yet dangerous plants in the Cayman Islands and information on how to properly identify them
from the DOE. We also note how to know whether your skin has come into to contact with these plants based off of the reaction that has occurred and remedies you can use to help ease the symptoms - given from both DOE and our dermatologists here at Integra.

A Guide to Hives and its Symptoms
Hives, also known as urticaria, are red itchy bumps on the skin, usually caused by exposure to an allergen. Additionally, these can also be red and become very painful for some. Hives deem to be a temporary issue that can usually clear up by themselves. But in some cases, they may have chronic hives that can last for months or even years.
Look out for welts as small as a pea or as large as a dinner plate that are round, oval, or worm-shaped. The skin around the welts is normally reddish on lighter skin, and purplish on darker skin.
The causes of hives vary besides being associated with allergies can include:
» Clothing that may be too tight
» Stress
» Working out
» Illnesses
» Infections
The risk factors of hives are common if you have had hives, had other allergic reactions, or had a history of hives in your family. To lower the risk of having flare-ups or experiencing hives, be sure to avoid any triggers, take showers, and change clothing, especially if exposed to an allergen.

Birth Plans and Delivery Choices in the Cayman Islands
Vaginal delivery is the most well-known and familiar way to give birth. Babies are primarily born in the head-down position. The baby moves down into the birth canal when the mother starts pushing on her contractions. A lot of the time, vaginal birth is the safest birthing method for both mother and baby. But there can indeed be risks for some women that vaginal birth is unsafe. Assistance may be needed to deliver the baby vaginally, or a caesarean section may have to be done.

Demystifying Food Allergies in Children
For the past several decades, parents have been encouraged to not give children and babies these particular foods. While in some cases babies do develop allergies and intolerances at a young age to food or drinks such as cow’s milk and may need to avoid these, the majority of parents preventing their babies from trying these foods is what researchers argue has had adverse affects.
Researchers who have studied food allergies in children concluded from the
results that the longer infants are kept away and not exposed to these common food allergies, the more likely they are to have allergies to these foods beyond infancy and into childhood. This article breaks down these findings and explains the importance of exposing children to these at a young age.

Improving Your Teen’s Sleep Quality
School holidays are a time for family, friends, and fun activities; and teens can stay up as late as there’s no school! After holidays like the recent half-term break, our teens become sluggish and low on energy. They go to bed late and must wake up early for school. Yup, a thrown-off sleep schedule. Experts have said that from puberty up until the age of 22 that 9 hours of sleep is required to be mentally, physically, and
cognitively healthy. A teenager being a teenager, will debate this theory.
But paediatricians have declared that insufficient sleep is a public health issue. There are some ways in which sleep schedules could get back on track after the holidays. READ

Fever is a normal response of the body when fighting an infection. All children will experience fever during childhood at some point, especially in pre-school years. Our Specialist Neonatologist and Paediatrician, Siobhan Jaques has created a fever advice sheet for you which offers information and advice on what to do when your child has a fever.

Fever is a body temperature over 38oC or 100.4oF Please use our tra c light system below to guide your response... Facts about Fever Can be caused by bacteria or viruses (most childhood infections are viral and therefore do not need antibiotics). Treat fever with medicine if it is making your child feel unwell- you can use paracetamol (also known as tylenlol/calpol/acetoaminophen) or ibuprofen (also known as nurofen/advil). It is safe to give both types of medicine together, but we recommend giving one, allowing an hour or so to have an e ect and then giving the other one if needed. Always read the directions. Make sure your child is drinking or feeding, little and often to stay hydrated. A low temperature can also be concerning- especially under 6 months. If your baby has a temperature of less than 36.0oC (96.8oF then they need an urgent review. Did You Know? The most reliable way to measure your child’s temperature is using a digital thermometer in the armpit or under the tongue. Try to avoid using forehead thermometers as these are less reliable. Fever is a normal response of the body when fighting an infection. Fever with spots or rashes that do not fade under pressure is a MEDICAL EMERGENCY

Specialist Otolaryngologist (ENT), Aesthetics and Cosmetic Surgeon
Dr Yammine is a Specialist in Otolaryngology (ENT) Head & Neck Surgery, Laryngology & Cosmetic Surgery, with over a decade over experience and is based at Integra’s Eastern Avenue location in the Cayman Islands.
Prior to working at Integra, she ran a successful practice in Ontario, Canada where she specialised in Otolaryngology and Laryngology services along with refined aesthetics and cosmetic surgery. She then moved to the Cayman Islands where she first worked with ENT in Cayman, as one of the clinical leads who helped launch voice and swallowing therapy services in Cayman, a particular area of interest for her.
Dr Yammine now joins us at Integra Healthcare with an aim of optimising patient care and safety to achieve the best natural results and passion for helping you to feel more confident and happier in your own skin!

Insurance: What Happens After You Leave?
Over the coming months, we will attempt to demystify the seemingly opaque world of health insurance. We wish to start with
helping you understand what happens after leave clinic following a visit or procedure.
Many individuals have raised concern about Cayman First insurance, following news articles and social media comments. We also get calls asking if people will have to pay everything up-front, which they are being asked to do in certain practices.
The problems experienced by Cayman First relate to the implementation of new computer systems and provider portal, all designed to enhance the service they offer.
Integra has a strong relationship with and complete confidence in Cayman First and we continue to manage individuals with this insurance in the normal manner. If you have friends and family having the full care bill charged on check out elsewhere, we can assure you that this is not happening at Integra!
