If you are an adult aged 18+ and need support because of illness or disability, or if you are caring for someone and need help with your caring role, our friendly staff will take time to listen to you. If you contact us we can give you information and advice on what help may be available.

Below are some examples of how we may be able to offer you support and advice.

Adult Services Continued - Day Centres/ support What happens at Day Care Centres?

Day centres are busy places, based in the heart of the community, where a variety of activities take place.

These are some of them: • • • • • •

personal care a hot meal (special diets can be catered for) arts and crafts a chance to meet and chat with others outings and special events gentle exercise.

Not everyone wants to join in, and we respect your wishes if you want to keep to yourself. We also take into account religious and cultural needs. By being with you in this way, we can also keep an eye on your physical health, and work with our colleagues in the health service. This is to maintain your health and well-being.

Through a social work assessment, where a social worker visits and asks questions and listens to you, to fi nd out what help you need. Unfortunately we cannot help everyone, we aim to help those who have the greatest need and we measure this using certain standards, which we call ‘eligibility criteria’. Once it has been agreed a service can be offered, an introductory visit will be arranged.

Emergency Social Care The Emergency Duty Team operates an emergency out of hour's service for professionals and the general public in Bolton. The team consist of four Social Workers with one Social Worker on duty per shift. The Emergency Social Worker works between 6pm and 9.30am every night and between 9.30am and 6.00pm on weekends and bank holidays.

The Emergency Duty Social Worker offers an emergency service for Children, Adults, Older People, Mental Health and homelessness. They can be contacted on 01204 337777.

Pet Care When Hospitalised If an elderly or disabled person is suddenly taken ill and goes into hospital or residential care, pets can be a problem.

It is part of a social worker's role to make arrangements for pets in this case. Relatives, friends and neighbours may be contacted, to see if they can temporarily care for the animal, or, adequate dog walking and temporary arrangements may suffi ce until the owner and the pet can be reunited. The social worker will strive to make the best possible arrangements for the owner and the pet.

Where keeping the pet at home is not possible, the social worker will arrange for the cat or dog (fi sh or bird etc) to be admitted to a kennel.

Callers can request a free call back. Advisers can give you information to help make decisions about your personal support needs and the needs of the person you are looking after. This includes information on assessments, benefi ts, direct payments, individual budgets, time off, work or education. Carers Direct can provide information, advice and supports on carers’ issues but they cannot provide ongoing casework. They can put you in touch with specialist national or local sources of help, including carers’ centres, social care and selfhelp networks.
