hen was the last time you walked into your office through the front door?

Look at your office, both front and back, through the eyes of a new patient…does it say leading edge progressive or Marcus Welby?

If your office speaks for you, I suggest you watch your language.

Half-lit neon signs.

Sun-faded cardboard posters in the windows and on the walls.

Vertical blinds that haven’t been dusted since the Leafs last won the Cup. Chipped paint, torn upholstery, tattered, dated magazines.

And that bathroom…

Real ‘retro’ takes money. It isn’t retro because you’re too lazy or cheap to get rid of it.

Lose the fake flowers and plants and please lose the dried flowers, unless your name is Martha.

If it isn’t Christmas, lose the twinkling lights. If it isn’t Thanksgiving, lose the cornucopia. And unless you

still refer to it as ‘Dominion Day’, lose the framed Diefenbaker photo.

If the appliances don’t work perfectly (or if they’re harvest gold)… lose ’em.

If anything on the walls is velvet, it goes in the garage. Preferably your neighbor’s.

And for goodness sake, clean the fishtank!

Lose the hi-fi and invest in Wi-Fi. Update the childrens’ toys.

Indoor air quality…when was your HVAC last checked? When they say change the filters periodically, they didn’t mean between ages.

Retire the frayed electric cords, the overloaded outlets, the non-approved equipment, the combustibles, the space heaters.

Let’s face it, dental offices are among the least favorite places to be.

Outdated office décor and reception areas can leave a negative impression on patients, causing them to wonder whether your equipment and methods are also obsolete. Ensuring your office portrays a sense of warmth and professionalism while also building patient confidence goes a long way toward easing apprehension.

Stale offices also speak volumes to your team members. Are they not worth the investment? Your choices in décor, equipment and technology will keep your staff motivated, productive and happy. Whether you’re buying or leasing, renovating or building, your practice’s success is directly affected by your environmental decisions. Design for now and for the future.

A great dental office serves you, your team and your patients for now and for a while, not forever. Let it be calming, efficient and visionary.

ADA Comments on “A Costly Dental Destination” Report Washington, DC—The American Dental Association (ADA) agrees with much of the report, “A Costly Dental Destination,” released recently by the Pew Center on the States, which points out the burden placed on already strained hospital emergency departments by patients needing dental care. echnology changes quickly and often. This is the one overriding consideration to remember when planning for technology in a dental practice. Technology components will be replaced more frequently than anything else. The wall treatments, the flooring, the cabinetry, the upholstery, and the dental equipment will all last longer than the computers, displays, and other information technology equipment. Ideally, future changes to the relatively short-lived technology components will be made without requiring changes to the more permanent items like cabinets, walls, and ceilings.

Remember that computers will be replaced, on average, every five years. This means that the cabinetry will be used to house several generations of computer hardware. Do not make the mistake of focusing on the size and shape of current computer hardware. Building cabinets which accommodate a very specific computer, monitor, keyboard, printer or other device is shortsighted. Instead, build to accommodate generic sizes.

Current requirements

It is straightforward to design for current technology requirements, especially with the assistance of experts who are familiar with the implementation of technology in a dental practice. Almost every new office design includes provisions for digital radiography, which requires computer workstations in each operatory and a dedicated server located somewhere fairly secure. Operatory workstations are often simply housed in rear or side cabinets to hide the mess of associated wires. Greater thought must be given to the placement of the display screens in the operatories, along with the location of the keyboards and mice. Think carefully about the items that the patient should see (intra-oral photos and radiographs, patient education materials, entertainment) and the items that they shouldn’t (the schedule, treatment notes). Often two displays, an operator screen on the rear wall behind the patient’s head and a patient display in front of the patient, provide the versatility required to display the appropriate information where required. Workstations in the admin areas, consult areas, and private office spaces are also necessary, and are usually easily implemented. Think through the current requirements for the phone system, the audio system, and any entertainment systems and plan accordingly.

Planning for tomorrow’s technology requirements is not so straightforward. While it’s impossible to predict how technology will change, it is possible to design in ways which facilitate future change.

Cabinets Cabinets are often a source of frustration when a change to the technology equipment is required. Space limitations are a fact of life in every dental practice. Unfortunately, it is easy to limit the space available to technology equipment too

(the most popular size for computer workstations installed where space is limited) have not changed in decades. Designing a cabinet to accommodate a box at least 16” tall, 8” wide, and 21” deep, with space on each side for cooling, and appropriate ventilation in and out of the cabinet, will allow for the implementation of nearly any computer hardware now and in the future. Building any smaller to accommodate specific hardware available today will cause unfortunate restrictions down the road. Avoid the temptation to assume that the ongoing miniaturization of electronics will allow for the purchase of ever smaller computers. Computing needs in dentistry are always increasing and future technologies may require proprietary cards, specialized or advanced hardware, multiple network connections, or any number of things which might not fit into a tiny CPU case.

Cabling During the last two decades the speeds that standard ether­net network cables have been required to handle has increased by two orders of magnitude. This means that many practices have had to install new network cables to accommodate new speeds. In addition, many newer networked devices now require cables which were not previously expected or installed. Running new network cables in existing dental practices is a common occurrence. This trend will inevitably continue.

During the design process, include network cables to every point that might eventually require one. Run extra cables to the front desk. Run one to every current or potential pan and TV. Run network cables to the phones, even if the current phone system

requires only telephone cables. The least expensive and least disruptive time to run network cables is during the initial construction.

Regardless of the number of network cables that are run, it is inevitable that newer technology will require new or different network cables. The design of the space determines how difficult this will be. Suspended ceilings offer easy access and allow new cables to be added with ease. If suspended ceilings are not appropriate to the design, consider adding access to the ceiling where appropriate to allow for future cabling. Where possible, also add conduit in the walls, ceiling, and floors to allow for future cabling or changes to existing cabling.

Whether the current requirements demand it or not, run a 2” conduit from the rear cabinet or wall of each treatment room to the toe of the dental chair, as well as a 2” conduit from the same position in the rear wall up to the ceiling above the dental chair. These conduits allow a path for video and data cables that carry signals to and from devices mounted on the dental chair, to the CPU in the rear cabinet, and also up to a ceiling mounted screen or other device.

Without these conduits it will be difficult or impossible to add devices in the future.

Obsolescence Technology items not only become obsolete quickly, but also look obsolete quickly. Today’s newest flat panel display will look outdated in a few short years. It will take much longer for the cabinets, the

wall treatments, and even the flooring to start showing its age. If the technology items are a central part of the design, be prepared to update them often.

Imagine two dramatically different but equally attractive reception areas. The first is warm and comfortable, with wood trim and country accents. A large wooden cabinet fills a corner of the room. A gas fireplace occupies the bottom of the cabinet and bookshelves run up and down either side. Built into the upper half of the cabinet is a small flat panel TV. The second reception area is modern, clean, and white.

Black upholstered chairs with steel frames are reflected in gleaming white tile floors. The room is bathed in light. Nearly filling one wall is a very large and impressive flat panel TV. The entire space screams ‘high-tech’ and ‘modern.’

Both reception areas offer an impressive and inviting introduction to the practice, although each one creates a dramatically different first impression.

In the first example, however, the technology (the TV) is playing only a minimal role in the overall look of the space.

In fact, the TV could be turned off, or replaced with a painting, and the charm of the room would be unaffected. In the second example, the technology (the TV) is perhaps the single most important element in the room, and plays a key role in creating the feeling that the space is ‘modern’ and ‘high-tech.’

Neither design is better than the other, but in the second example the importance of the TV necessitates that the TV be a relatively new model. Regardless of the brand, expense, or specifications, any TV that is purchased today will look dated within three to five years. Consumer electronics manufacturers are very adept at coming up with new designs that render recent designs obsolete, and consumers are very aware of the difference between a modern electronic device and one that is dated.

It is very acceptable, and even advantageous, to include design elements which convey the idea that the practice is ‘modern’ and ‘high-tech,’ but be ready to update any key technology items every few years if they are being used to deliver this message.

ONLINE doesn’t work...

Online marketing doesn’t work for dentists.

There. I said it.

And there are lots of dentists who are nodding their heads right now.

They’ve spent thousands of dollars on a flashy website.

They’ve paid top dollar for Google ads. They’ve hired shady internet companies to do their social media.

But it has not put a single patient in their chair.

So is it all a scam? No. Here’s the simple truth: online marketing doesn’t work for dentists... who commit one or more of the following four sins of online marketing:

SIN #1: No Strategy “This guy is an amazing dental surgeon. He works fast. And his patients comment on how little pain they feel during extractions. But he hates working with kids. So why does he market himself online as a family dentist?”

So what is an online marketing strategy? Simple: “be found by prospective local patients when they search for your services on the Internet,” says Dr. Gary Leung, President of www.Health-Local.com, a national online directory of health professionals.

How can you do that? Well, explains Dr. Leung, “first you need to find out what your prospective patients are typing into search engines (like Google, Yahoo!, Bing, etc.).

Then you need to have those search engines provide those patients with a prominently placed link to your website when the search results are displayed”. This will take prospective patients to your website which should convert some into actual patients. Voila!

So what are prospective patients looking for? Let’s take a look at the most popular searches:

And because of that, it is too difficult for newcomers to try to dethrone the established players for those keywords.

But couldn’t a dentist simply pay Google for an ad to be placed on the first page of results for the keywords “Toronto dental”? Sure! But that would cost the dentist up to $2.27 every time anyone (including your competition) clicked on that link. Ouch! And if you’re willing to throw money away on Google ads, I’m sure a lot of shady online marketing companies would LOVE to talk to you.

So where does that leave the average dentist in terms of an online strategy? Well, as you can see from the table above, prospective patients are looking for dentists in their immediate geographic area (Toronto or Ottawa) who provide particular services (e.g. dental implants or teeth whitening) or practice specialities (e.g. emergency or children). They are not searching for your name (Dr. John Doe), your practice’s name (e.g. “Happy Smile Dental Practice”) or the name of the street your practice is located on. So don’t bother targeting those keywords.

If in doubt, consult with an online marketing expert as to what keywords have relatively HIGH search volume and LOW competition from other websites. Importantly, your keywords should take into account your location (e.g. Spadina and Bloor), type of dental services (e.g. extractions), and perhaps even the type of patients that you already cater to (elderly, young families, children etc.).

Bottom line: have an online strategy that focuses on HIGH search volume and LOW competition keywords that make sense to your practice and prospective patients. Don’t try to target all keywords or you’ll end up nowhere fast.

SIN #2: No Website “I recall a (non-client) dental practice which paid top dollar for Google ads. They paid Google a few dollars every time someone clicked on their ad.

The problem was that, when prospective patients clicked on those Google Ads, they were re-directed to this dental practice’s website — which simply said: ‘Under Construction.’ Ouch!”

A website is your chance to make a good first impression on a prospective patient. But what should be included? Naturally, you should provide the basics (e.g. who you are, where your practice is located, your services, contact us, etc.).

But your website should always be selling your practice’s services. “While Dentists don’t like to think of themselves as sales people,” says Matt Bladowski, President of www.

DentalStrategy.ca, a dental management consulting company, “the truth is that dentistry is only a small part of what they practice in real life. Running a business (managing staff, scheduling patients, collecting receivables, etc.) can take up a lot of time. And marketing your services online has a significant role to play as well.”

So how can you use your website to sell yourself and your practice? Well, you can gain credibility by asking your existing patients for testimonials and then posting them on your website. And writing a few articles (without the dental jargon please!) will make you appear like an expert on your topic.

And how about a professional video that gives a virtual tour of your practice? Nothing wrong with those ideas.

And please don’t forget your strategy! If prospective patients searched for “Oakville family dental” and came to your website, make sure everything (i.e. layout, colours, fonts, images and content) clearly and consistently reflects those keywords.

Bottom line: have a website and make sure it reflects your strategy.

SIN #3: No Content “If you build it, they will NOT necessarily come.

I’ve seen dentists pay thousands of dollars for flashy websites. These graphic-intensive websites are often not found by search engines. Hence, the only people who ever visit them are a dentist’s friends, family and existing patients — not very good for getting new business. Moreover, these sites are typically very difficult for the owner to support and maintain.”

It needs content that is meaningful to prospective patients and which is regularly updated. That content could come in the form of articles, videos, pictures, presentations, etc. Websites with stale information will always be less noticeable to search engines than ones which are regularly updated.

But putting information out there for its own sake is not good enough. Remember the strategy! Your content needs to be presented in a way that targets your keywords. There are various techniques that come into play here — most of which are very technical (e.g. permalinks, meta tags, key tags, titles, headers, keyword density, etc.). But all of these techniques essentially involve repeatedly inserting your keywords in specific areas to help search engines rank you high

for those keywords.

Bottom line: in the online world, content is king. Keep it fresh and optimized for the search engines.

SIN #4: No Backlinks A link to your website is called a “backlink.” It typically appears as a simple set of words in blue which are underlined (e.g. Ottawa emergency dental). It could also be an image. Regardless of what the actual link looks like, once clicked, it will redirect users to your website.

Backlinks are your online relationships with the World Wide Web. The more you have, the more popular you are for your keywords. And the higher your search engine rankings will be. Why? Because backlinks tell search engines that your website is authoritative and trustworthy for your keywords. But it’s not just about quantity; quality counts too!

Those backlinks need to come from other authoritative and trustworthy websites that have content which targets your keywords (or subject area) as well. So a link from your dental association’s website is better than a link from an online gambling website. You get the point. The older the links (and websites those links come from), the better. And here’s a final tip: the text or image of the link should contain those keywords too. For example, the actual text of a link should say “Ottawa emergency dental” instead of “click here.”

So where can backlinks come from? Well, you can find them in health professional directories (free or paid), online articles or blogs, social media websites (e.g. Facebook, LinkedIn, Twitter, etc.), the commentary that appears underneath online videos, and lots of other places.

Bottom line: prominently placed links from older, contentrich websites that are related to your keywords are the best to help search engines rank your website higher for your keywords!

Conclusion Now that you know the four sins of online marketing, here’s one more sin (perhaps the most important one): do online marketing yourself! As Dr. Leung advises: “Dentists should spend their time doing what they are good at: practicing dentistry. Leave the online marketing for online marketers.”

Go with someone who knows what they’re doing, is resultsoriented (i.e. putting patients in your chair), and who has a solid reputation in the dental community.

It was after a series of consultations with Dr. Bhalla that the vision for his office came to light.

The idea was to create an office that was modern, inviting and functional for all aspects of the practice. One of the biggest challenges externally was that the space existed in a strip plaza. This meant that the exterior had a uniform look with the other existing units. This is why it was important that the interior space be unique and visible as to invite in new patients.

Internally, we encountered some architectural challenges because the space was quiet long and narrow. The solution we came up with was to create two very distinct spaces — administrative and clinical. The long linear lines of the reception desk guide the eye visually towards the back of the space into the clinical area. www.oralhealthgroup.com

The clinical corridor was accentuated by creating a wavy feature wall with built in niches in an effort to mimic waterfalls. Customized cabinets were designed with niches to provide storage and are ideal for displaying art and/or decorations.

The repetition of rain drop chandeliers is functional and aesthetically pleasing.

The spacious operatories are equipped with flat screen televisions for patient education and entertainment value. The use of cutting edge technology such as digital radiography, provides precise and accurate dental information. Digital radiography allows Dr. Bhalla to efficiently and effectively provide his clients with information regarding their oral hygiene. From the outset, the doctors’ goal was to create a modern, professional-looking, and technologically current dental facility without sacrificing a friendly and inviting atmosphere. They enlisted the services of Interior Designer Jean Akerman, who coordinated the design elements that achieved their vision: a neutral palette punctuated by parrot green accents, textural contrasts, juxtaposition of transparencies and illusions of transparency, and thoughtful detailing in walls, millwork and ceilings. www.oralhealthgroup.com

The waiting area, subtly differentiated from the reception by glazing, boasts a feature wall with fireplace and TV, a refreshment bar, and children’s area. The Consultation Room feels larger than it is because of its glass entry and floating millwork. Operatories are laid out along window walls, and the central stericentre is prominently placed.

With input from the team at K-Dental Inc., and construction by Bates McLeod Contracting, the finished office has great functionality, good ergonomics, and a handsome aesthetic.

“Patients love the colour choices and are impressed by the digital radiography, and monitors in each operatory,” say the doctors, remarking on how much they enjoy their new office. “We couldn’t have done this on our own.”

Clearly, working with talented and dedicated professionals resulted in a successful outcome for DPM this project.

Performance Architecture for performance orthodontics When his orthodontics practice outgrew his mid-Toronto office, Dr. Bruno Vendittelli seized the opportunity to move into a larger building where he could also share his passion for architecture and mid-century modern design. The result, a gleaming bi-level ergonomic space with vivid accent walls, cheerful art, and contemporary furniture, has garnered a decidedly enthusiastic response. Form, function, and fun unite in the second-floor operatories, which sport colourthemed semi-private nooks. In the downstairs reception area, patients grin at the panda pop-art and bask in the natural light. On more than one occasion, Dr. Vendittelli has heard a patient say: “I wish I could move in!”

While the new location near Yonge and St. Clair is extremely convenient (close to schools, public transit, and parking, and only four blocks east of the previous office), the building itself initially posed some challenges. Noise issues stemming from the abundance of wood, drywall and stone, threatened to disrupt the peaceful space. Acoustics engineers solved the problem by installing white sound panels to inhibit the noise and reverberations of footsteps,

voices, and dental procedures.

With similar perseverance and ingenuity, Dr. Vendittelli’s team (comprised of architects Frank D’Ambrosio and Maxine Shabove, designers Bender and Kid, Mayfair Contracting, and interior designer Connie Braemer) proved that a welldesigned bi-level space can become an office advantage. In order to limit traffic on the stairs, Dr. Vendittelli’s team decided to make the main floor the reception, administration, and records area, while designating the second floor as the clinical area for orthodontic treatment. The building’s steep staircase was transformed into a handsome focal point, with widened steps for patients’ added ease and comfort. The result is a calmer, more intimate experience for clientele.

Judging from the reactions of his patients, Dr. Vendittelli’s new office has given them yet DPM another reason to smile.

You want to secure your income stream from your professional corporation and maximize the value of your professional corporation’s financial assets for your family. You still have a significant amount of cash inside your professional corporation that you cannot take out with paying an exorbitant amount in taxes. The government now wants to be your partner. They want a portion of your hard earned dollars in the form of taxes. You don’t want to pay the increased tax on the interest-bearing investments (GIC’s, Term Deposits, bonds etc.). At the same time, you want to maximize income that you will be receiving, while preserving your investment capital to provide for your family. What is the solution? One of them might be the Corporate Insured Annuity that provides tax efficient retirement income with estate-planning advantages.

A Corporate Insured Annuity provides retirement income with tax and estate-planning advantages. With a Corporate Insured Annuity, your corporation receives a steady stream of payments in return for a lump sum which is deposited into an Annuity. The income is then paid out to the shareholder via dividends. By insuring your annuity, an amount equal to your original deposit is issued to the “Capital Dividend Account” of your corporation when you die, thus enabling the surviving shareholder to distribute dividends tax-free.

How does it work?

The strategy combines a non-prescribed annuity and permanent life insurance policy that is purchased on a single-life basis, or jointly with your spouse. The annuity generates a payment stream that will be used to pay the level insurance premiums of the life insurance policy held in the professional corporation. The remaining payment balance from the annuity can be distributed to the shareholder as a taxable dividend to supplement their income.

icy, or on the second death, if you have a Joint-Last-To-Die policy with your spouse. The PC also receives a credit to its capital dividend account (CDA account) for a portion of the proceeds. Dividends can then be paid — tax free — to your estate out of the capital dividend account.

Big Picture Benefits: • Your professional corporation receives increased guaranteed cash flow while you’re retired; • Dentist’s (shareholder’s) retirement income is supplemented through the distribution of taxable dividends; • Reduces the capital gains tax on your corporate shares; • Life Insurance proceeds are paid out to the professional corporation, tax-free, on death and directly to the professional corporation; • Enhances the estate values to your heirs and reduces the tax bill.

Let’s look at the example of Dr. Jenkins (see chart below).

The $13,000 that Dr. Jenkins will receive in income is tax-efficient, as the payments are treated as a combination of return on capital (return of a portion of the original $500,000 investment) and interest. Also, upon Dr. Jenkins’s passing, his Corporation would receive $500,000 of life insurance proceeds. These proceeds are in turn paid out to Dr. Jenkins’s children by way of the Capital Dividend as mentioned earlier.

Had Dr. Jenkins simply kept the $500,000 of built-up corporate assets in GIC’s, returning 4% annually, this would generate $20,000 a year of interest, which would be completely subject to tax in his corporation. Also, in order for the corporation to distribute the $500,000 investment to Dr. Jenkins’s beneficiaries upon his passing, this can only be done by way of taxable dividends, which would further reduce the value that is ultimately transferred to Dr. Jenkins’s children.

The additional Estate Value preserved for Dr. Jenkins’s children through implementing the Corporate Insured Annuity strategy is significant. Assuming Dr. Jenkins lives to age 91, the Insured Annuity provides for an additional $144,148 of estate value received by Dr. Jenkins’s children, when compared to the status quo; $500,000 kept in GIC’s.
