From both the histological and anatomical point of view, the alveolar bone is a tooth dependent structure that develops in conjunction with eruption, and its topography is determined by the form of the teeth and their axis of eruption. In the first phase of remodelling of the buccal/lingual walls of extraction sites, the bundle bone is resorbed due to the lack of nutritive support from the periodontal ligament and replaced with woven bone. Consequently, the socket’s walls will reduce in both vertical and horizontal dimensions. This resorption may lead to aesthetic and functional disadvantages that can compromise future implant placement since, in order to have a correct endosseous implant positioning, an adequate volume of the bone ridge is essential. Current knowledge suggests to augment fresh extraction sockets rather that to wait for spontaneous healing and perform further guided bone regeneration.

Different techniques have been presented in literature in order to preserve the post-extraction site, including the use of barrier membranes, 3D matrices and bone fillers. An “open-healing” approach is nowadays commonly used, combining a proper management of soft tissues with biological integration of biomaterials.

This course is designed for practitioners with an interest in expanding their treatment options for extraction sockets.

Extracting teeth is part of everyday clinical practice, we now know that once the tooth has been removed the inevitable consequence is bone loss. Ridge preservation has been scientiďŹ cally shown to maintain ridge volumes once teeth have been removed. This program will look at the scientiďŹ c rationale of preserving existing bone as well as atraumatic tooth extraction by means of Piezosurgery, techniques for hard tissue augmentation to preserve ridges and soft tissue management using contemporary biomaterials. Dr Mahnaz Syed Dr Mahnaz Syed qualified from Guys Dental School in 1992 and obtained her Diploma in general dental surgery from the Royal College of Surgeons England.

Mahnaz commenced her specialist training at the Royal London Dental Hospital in 1998 and gained her specialist M.Clin.Dent with distinction in 2001, she holds a fellowship and membership qualifications from the Royal College of Surgeons England. She worked as a specialist periodontist in London and clinical lead in mucogingival surgery and implant soft tissue augmentation at the Royal London Hospital. Mahnaz continued her research at the Royal London Dental hospital completing a three year treatment study investigating prognostic factors in relation to treatment outcomes in patients with aggressive periodontitis. Mahnaz has been in Perth Western Australia since 2007 and runs a successful specialist periodontal and implant practice. Her specialist interests lie within minimally traumatic extractions, bone surgery and both hard and soft tissue augmentation for teeth and implants.

This is a didactic, clinically-oriented course that will equip the attendee with the necessary knowledge and skills ready to apply on return to the surgery.

Dr Anand Patel BDS (Adel.) , M.Clin.Dent. (Pros. with Dist.), MSc (Oral Impl) Dr Anand, a Fijian achieved a Bachelor of Dental in 1996 from a scholarship at the University of Adelaide. For the next 10 years he worked in private practise, taught at the local dental school and as an accredited dental surgeon at the private hospital in Fiji. In 2008, he obtained a Master of Clinical Dentistry in Prosthodontics, with distinction from The Kings College, University of London.

In 2014, he completed Post Graduate Anatomical Surgical Training at the Anatomy Department of the Medical University in Vienna. In 2015 he also obtained a Master of Science in Oral Implantology, from the Goethe University, Frankfurt am Main, Germany. Dr Anand currently lives in Darwin, Northern Territory and is engaged full time in private practice.

Dr John Giblin BDS, MDS, FRACDS (GDP), Diplomate ICOI In a career spanning 40 years, Dr John Giblin has placed several thousand implants, grafts and sinus lifts. Dr Giblin is a Diplomate of the International Congress of Oral Implantologists, a member of the Ambassadors’ Circle for contributions to the ICOI, a member of the Academy of Osseo Integration, the Australian Osseo Integration Society, the American Academy of Implant Dentistry and the Australian Society of Implant Dentistry. He has published and lectured extensively on Planned Extraction Therapy and Immediate Implant Placement following extraction, both locally and internationally. Dr Giblin has a special interest in the field of Oral Rehabilitation, which includes complex restorative and implant-supported prosthodontics. He elects to remain in general practice in Hornsby (Sydney), with an emphasis on complex oral rehabilitation which includes oral tissue reconstructive procedures.

The A-Z of Tooth Extraction, Ridge Preservation and Guided Bone Regeneration This practical course will provide the participants with an overview of the most important aspects around ridge preservation. To understand the fundamental principles of the biology of extraction socket healing is crucial and will be reviewed under the aspect of current scientiďŹ c knowledge. In order to estimate the degree of surgical diďŹƒculty a pre-extraction assessment is important, which will be discussed in depth.

A further important decision is when and how to use a piezotome and periotome. Dr Dax Calder Dax Calder is a periodontist in full time private practice in West Perth, Western Australia. He completed his BDSc in 1990 and MDSc in 1997 from the University of Western Australia. In 1998 he completed further graduate studies at UCLA. He has lectured nationally and internationally to general dentists, graduate students and specialists; with a particular focus on case selection, risk assessment and skills development in the ďŹ eld of implant-related augmentation techniques.

His current research interests include the use of peptide hydrogels in alveolar ridge augmentation for the optimisation of implant aesthetics.

He currently holds dual Associate Professor appointments in post-graduate surgical implant training programmes at the University of Sydney and the University of Western Australia.

The changing face of bone grafting for orthognathic surgery

Contemporary concepts and grafting techniques for soft and hard tissues in implant dentistry

Traditional grafting of osteotomies used autologous bone from the iliac crest and more recently bone grafts from the mandible. Contemporary techniques now involve the use of xenografts alone or in combination with autologous bone to bridge continuity defects. Xenografts are also now used to aid in contouring defects in the mid face and mandible. These modalities will be discussed with the aid of clinical examples.

This lecture will discuss decision making in regenerative therapy. Ridge preservation techniques, horizontal and vertical bone augmentation, sinus lifting and grafting in combination with simultaneous implant placement will be review and discussed.

Adequate soft tissue management is a further critical aspect to enhance long term predictability of dental implants, which will also be covered in this lecture.

His previous National appointments include Consultant in Oral and Maxillofacial Surgery at Princess Margaret Hospital (PMH), Sir Charles Gairdner Hospital and Fremantle Hospital. Dr. Allan is currently the Head of Oral and Maxillofacial Department at St John of God Hospital Subiaco. He was awarded the Medical and Optical Martin Travel Scholarship in .

His areas of surgical special interest include surgical correction of dentofacial deformity, maxillofacial reconstruction and advanced osseointegrated implant techniques. The possibility to place implant immediately after tooth extraction or within a few weeks later is considered as a reliable technique. Anatomical parameters have to be strictly evaluated to inverstigate the possibility to take advntage of this procedure. Sinus elevation is a common procedure to rehabilitate the atrophic maxilla in the posterior area.

Alternative procedures to the classical approach have to be considered depending on the anatomy and the compliance of the patients. Prosthetically driven implant treatment planning is the gold standard in order to achieve a final implant position that provides the most optimal final restorative result. It is an everyday challenge in implant dentistry, especially when defects are present in the alveolar bone complicating the positioning of the implant and resulting compromise in the aesthetics, function and longevity of our treatment for our patients.

Today we have an overwhelming selection of different implant systems, biomaterials and surgical techniques.

It is not always easy to determine which system or material to choose, based on the reliability of the material and also ease of use in order to achieve our goals. A lack of knowledge or confidence to choose between the available techniques, particularly in compromised situations, can lead the dentist to exclude implant treatment as part of treatment planning options. In the modern Periodontology, Periodontal Plastic Surgery procedures are critical to improve soft tissue conditions at teeth and implants. Among the possible indications, the treatment of gingival recession is very frequent. Several clinical trials have shown the efficacy of different surgical procedures to obtain root coverage and new prognostic factors seem to predict the clinical outcomes of single and multiple gingival recessions. Conversely, the surgical expertise in treatment of mucogingival deformities around teeth has been shift at implant site to improve the final clinical and aesthetic outcomes of dental implants.

The present course will highlight the current evidence and surgical strategies in root coverage procedures at single and multiple gingival recessions, the procedures in soft tissue grafting, the efficacy of biomaterials as a soft tissue substitute and the treatments to obtain soft tissue aesthetics around dental implants.

This course is designed for practitioners with experience in treating periodontal disease and who want to advance their soft-tissue regeneration skills.
