ugust was a bittersweet month as Areawide Media said “best wishes” to our two interns. JR Schaufler and Sophie Rossitto were amazing young individuals to work with and will be greatly missed. Before leaving, Sophie was able to help out with one of this month’s stories.

I interviewed Larry and Angela Burns, owners of a Thayer business many might have questions about, CPI 340B. It’s an interesting business which was created to help healthcare entities benefit and operate their 340B programs correctly and efficiently.

Renee got to sit down with Dr. Scott Ream and discuss the work he does outside of our country to help patients in developing countries be able to see. Though COVID has put a damper on his trips, he has travelled all over in the decades of work he’s done.

I was so excited to feature a home tour this month, and very appreciative to Josh and Melissa Pierce for allowing Avenues in their home. They live outside of Thayer in an amazing home the couple had built. We thought who better for our medical themed issue than two individuals who work in healthcare. Josh and Melissa are employees of Fulton County Hospital in Salem.

I hope by the time I’m writing to our loyal readers in a month that the weather has cooled off and we are starting to see signs of fall.

Gardening in the Ozarks Fall is here, but spring is coming…

he summer is slowly leaving and the fall is just around the corner.

Your vegetables are finishing up.

Your flowers are giving you the final show of the season. All in all, gardening season has been very productive. But wait. It is not time to stop. Remember that promise you made to yourself way back in the spring? When you walked into a beautiful garden filled with all kinds of spring blooming bulbs and you said, “I am going to do

this next year.” Well next year is NOW. Well, those bulbs will need to be planted in Arkansas in September till early October. They will soon be showing up (if they have not already) in your “Big Box” stores. If you are going to get some, do it as soon as they show up to ensure you are getting good healthy bulbs.

Once they show up in the stores, they will slowly dry out because of the heat in the stores. Keep three things in mind when picking out your bulbs for purchase. Weight, firmness and clean.

First your bulb should have some weight to it. Pick out the larger and heavier bulbs if possible. Next, make sure the bulbs are firm. Gently squeeze the bulbs in your fingers. It should be solid, not dry or squishy. Then third,

make sure your bulbs are “clean”. By clean I mean free from scars, blemishes, cuts and mildew. All of these things can cause the bulb not to produce properly.

Bulbs like well-draining soil to keep them from rotting.

If you have a spot that does not seem to drain well, try mixing in some compost or organic material. The depth of planting the bulb is generally three times the width of the bulb. There is no wrong way to plant bulbs or any garden.

Some people like to have straight lines, although in nature there are no straight lines. I prefer to grab a handful and toss them over my shoulder into the bed. Where they land is where they will be planted.

Other things to keep in mind is the mature heights of the bulbs. You would not want your two to four inch tall

crocus’ hidden behind the 16-20 inches tall daffodils and tulips. Also, if you have a squirrel problem, try placing some chicken wire on the ground above the bulbs and cover with dirt. This will deter them from digging them up.

At the time of planting, use Bone Meal in the holes below the bulbs to promote root growth. You can also use a slow-release fertilizer like a 10-10-10. Either way, make sure to water the area well to encourage good soil contact and good growing conditions.

Now just sit back and relax and you will be thanking yourself when the “fruits of your labor” begin to grow and bloom as if they are welcoming the Spring after the Winter.

A

fter completing a renovation as newlyweds, Josh and Melissa Pierce, along with their children (Avery, Matt and Riley) have been enjoying their new construction home since 2018.

“We moved in in October and started building in March. It was roughly seven months,” said Josh. Melissa wanted to build for a long time, and seven years before they were able to build, they found the perfect property for their forever home. However, there was a slight set back. “We had the housing crisis right about that time,” explained

Josh. The home they had first bought together and renovated took longer to sell than the Pierces had planned.

Not only did it take them a while to sell their home, it had taken a while for them to find the ideal land. “It took forever to find anywhere that was a couple minutes from town, with privacy, so it took a while to find that. We bought these five acres and then it took forever to sell our other house,” recalled Josh.

When asked if they preferred the new build over a renovation, Josh said they enjoyed planning the details of the new construction home, which is also their forever home.

“We were pretty young when we did the renovations, so it was kind of more generic, more what the builder suggested.

This was a lot [more] custom.”

“We had to modify the floor plan and change some things around. We knew we wanted a single floor home. We had lots of time to plan. We weren’t worried about the kids’ rooms being too big as they were getting older. We were more worried about the living space, and focusing on common areas,” added Melissa.

Jim Pace built the home. David Nelson, of West Plains, did the trim and finish work. John Wilson, of Alton, completed all cabinetry in the home. The interior is 2,300 square feet with four bedrooms, three bathrooms, laundry area, master suite, two car garage and an open concept living room, kitchen and dining area. The home also features a detached shop, which the family uses as a hang out space.

Melissa doesn’t consider the home to fall into a certain style category. She decorates the home with family photos, special pieces and items she likes. It does have a contemporary and modern feel with an open layout. When you first walk in the home, you have views of the living room, kitchen and dining area. A unique accent wall is located near the

entry that is covered with photos of the family.

The dining room is to the left and features a large custom built table. “Our kitchen table, Josh and I built that with the help of my mom. Danny Tyson cut the wood for us. He does sawmill work in Thayer. The barn doors, Josh and I built those, and that wood is from his grandparents’ farm cabin near Spring River in Fulton County. The barn wood in our shop we used is from my grandparents farm in Couch,” said Melissa about various projects in the home.

The kitchen and living room are opposite each other, with views of the country life and beautifully designed pool in the backyard. The kitchen boasts a large island, quartz countertops and a tile backsplash. The wall color of the

home was chosen to be subtle and color is added through décor.

Planning and building the home definitely came with some obstacles. “She definitely didn’t want a cookie cutter plan, which obviously created challenges. I was dead set on having the angled garage, you get tired of seeing the same thing. That created an odd shape roof,” said Josh. “I wanted a large dining area because we are a family of five, and when you have anybody come to visit and eat with us, at our other house, we didn’t have any room for them to sit. So I wanted a large dining table. I started pricing tables and thought this is crazy, so I was just like, let’s build one so I got on Pinterest. Luckily my mom was familiar with building things,

so she directed us,” added Melissa. A lot of their family and friends helped on projects.

She also bragged on Josh for the work he completed on the pool. Though he hired various jobs for the project, he planned the whole process and assembled the pool.

Each family member was asked about their favorite feature in the home. Josh: “Mine is easily the shop area. Any guy who hasn’t had a place to keep their stuff, work on their stuff, you know, hang out, that isn’t even close for me. No contest, the shop.”

Matt’s favorite feature of the home is that he has his own bathroom, a part from his sisters.

Avery said: “My bedroom, because I used to share with Riley and she’s messier than I am, and I have a really big closet.” Her mother added, “The girls had to share a room before. They got their own room and style [in the new

house].” Melissa also added her favorite feature was the accent wall with collage of photos in the entry. “I love my accent wall. I just love the collage of pictures up there. Josh wasn’t too pleased with that project because he isn’t a fan of holes in the wall. I just love looking at a different picture every time I walk by it.”

The two girls’ bedrooms are located off of the kitchen, near the laundry room and garage entrance. They share a “jack and jill” bathroom. Also in this section of the home is a hallway leading to the laundry room which features family photos from years’ past.

ACNA’s Center Stage Thursday is back

he Arts Center of North Arkansas is pleased to announce the return of Center Stage Thursday this coming fall. The four consecutive Thursday events will occur from 6 to 7:30 p.m. at the gazebo in Cherokee Village Town Center on Sept. 23 and 30 and Oct. 7 and 14. Be sure to bring your lawn chairs and prepare to be entertained. A concession stand will provide drinks and snacks, or you may bring your own. This event would not be possible without the support of its sponsors. Opening the Center Stage Thursday season on Sept. 23 will be the local band Stillwater Hardy which plays a mix of classic rock and blues. The leader of the band, vocalist and lead guitarist Bobbi Saxton, refers to it as “swamp pop.”

Bobbi has been playing for well over twenty years and has had her own band for more than 12 years. She plays three piece usually. She is fortunate to have played with some of the best musicians from Northeast Arkansas to Southeast Missouri. She also does a solo act using acoustics, electric guitars, and backing tracks. She plays standard tunings as well as alternate tunings with a little bluesy slide guitar. Stillwater Hardy’s drummer, Ron Yow, is also a great chiropractor. The bass player, Bill Worley, is from Lake of the Ozarks.

Guitarist Danny Dozier will perform on Sept. 30. Born and raised in Batesville, Ark., he started playing the guitar at an early age and has never stopped. His style has been influenced by some of the greats: Merle Travis, Chet Atkins, Doc Watson, Jackie Phelps, James Burton, Eric Clapton, Jimi Hendrix, Jerry Reed, and James Taylor.

During the mid-1960’s through the early ’70’s, Danny played in his high school and college bands. After college, he moved to Hattiesburg, MS, where he formed a band called the Howlers. The group moved to Austin TX, in order to showcase themselves in search of a record deal.

The Howlers are now known as Omar and the Howlers.

Returning to Arkansas in the late ’70’s, Danny played the nightclub scene throughout the state for several years. In the early ’80’s, he became the band leader for The Country Music Story, a country music show at the Mid-America Amphitheatre in Hot Springs. Later, he worked at The Grandpa Jones Family Dinner Theatre in Mountain View. For six years in the mid to late ‘80’s, he was in the staff band for a syndicated radio program called At Home in the Country, which was broadcast over public radio.

Danny won the Merle Travis Guitar Contest held at The Ozark Folk Center in 1991 and 2009. In recent years, he was the band leader for a seven-piece band called The Lockhouse Orchestra. Currently, he performs with an ol’ time string band in Mountain View called The Leatherwoods, plays solo instrumental guitarist gigs, and performs with a guitar trio from Little Rock called Finger Food.

Performing on October 7 will be the McMickle Brothers, Tony and Harold. Their experimentation with several different styles of music over the years has resulted in an eclectic repertoire, ranging from the early roots of country music to Tony’s original songs that paint emotional landscapes.

With musical roots that run deep in the Arkansas Delta, their early influences range from the gospel and blues music that surrounded them while growing up to such modern singer-songwriters as James Taylor and John Prine. The brothers spent their youth attending every concert they could, seeing, among others, guitar greats JohnnyWinter, Albert King, and Leo Kottke. Tony has personally met Stevie Ray Vaughn, Alvin Lee, Kenny Wayne Shepard, Steve Cooper, and Duck Dunn.

Having been raised in Wynne, Ark., Tony now resides in Cherokee Village and Harold in Jonesboro. A “people person,” Harold has a huge online presence, being friends with hundreds of musicians not just in Arkansas but also nationwide. An active member of the Memphis Songwriters Association, Tony won an online songwriting contest in 2017 which resulted in a free trip to California to audition for America’s Got Talent. He has also made it to the finals in another contest.

Performing at the Fall 2021 Center Stage Thursday’s finale on Oct. 14 will be singer-instrumentalist Grace Stormont, who was born and raised in Heber Springs, located in the foothills of the Ozarks. Grace’s main musical genre could be described as Americana as it consists of a mixed flavor of folk, jazz, blues, and old-time music. She plays several instruments, including guitar, banjo, upright bass, fiddle, ukulele, and dulcimer.

Grace’s musical influences include Sarah Jarosz, Gillian Welch, Willie Watson, Abigail Washburn, Jason Isbell and many others.

Grace has been entertaining audiences around the Southern United States for the past eight years. She has performed with several bands, including The Claney Ferguson Band, Armadila Holler, and most recently Taller Than You, which won New Group of the Year at the Ozark Folk Center State Park in Mountain View, AR. She has also been privileged to open for several people, including The Steel Wheels, Suzy Boggus, Malcom Holcombe, Levia McCalla, and Carlene Carter, daughter of June Carter-Cash.

This year, Grace won two Arkansas Country Music Awards: Acoustic Act of the Year and Song of the Year for her original song “On Your Own.” Currently, she is working on an all-original album which should be released later this year.

Hometown Strong is a way for GTS as therapists and member of community to give back to the communities that we love and live in. Each month employees of GTS donates a portion of their paycheck and GTS rounds that amount up to make a donation to a local charity. Each staff member that donates to this fund is entered into a drawing and once per month we draw a staff member name. Angie Cadenbach chose the Viola Backpack Program to receive $1,000.

The Viola School Backpack Program started in 2016, when two students were looking to address a community/school need for a FCCLA competition. Since then the program has tripled in size. The program has solely been funded by community donations, both food and monetary. High school students in Mrs. Walker’s Leadership class are in charge of packing backpacks, shopping and organizing the food room. Backpacks are sent home with students in need each Friday.

Batesville Montessori is a nonprofit private school for children 6 weeks to 6 years of age. At Batesville Montessori we focus on child-led learning.

Olivia’s Journey Olivia Jane is a 3 year old happy bundle of joy. She started her journey 2.5 years ago. Since day 1, she has had: 6 tumors, unable to remove the most recent, 4 brain surgeries, 5 treatment regimens, 3 stem cell transplants, 10 different chemos, 50 doses of radiation; 30 proton & 20 whole brain & spine, and 1 trial drug. And with all of this, none of it has slowed her down. She has handled all of this with so much bravery and strength.

The week before Mother’s Day, Olivia started having minor left sided weakness and over the last couple weeks, it’s gotten worse. She’s not having pain in that side, she just days “it’s not working”. On June 14, her family leaned that, unfortunately, the trial medicine isn’t working. Olivia’s tumor has grown to be about double the size over the last two months. So, they stopped the trial medicine & discussed their options.

Option one was to do another surgery that will almost guarantee left sided paralysis. They would place a catheter into her ventricles that would give chemo straight to her brain. She would then have a G-tube placed because of how much medicine she would need. This would have Olivia spending the majority of her time being inpatient at the hospital.

Option two is to not do anything.

With the rate it’s growing, her doctor predicted her family would have a few months left with her with hopes they would be able to make to her birthday on September 22.

On July 25th, Miss Olivia was dedicated at her church and her mom was baptized. What’s Cookin’ in the Ozarks?

Easy Lasagna Courtesy of Cooking ClassyTM 1 lb. lean ground beef 1 (25 oz) jar marinara sauce, preferably Rao's tomato basil** 1/2 cup vegetable broth 6 uncooked lasagna noodles (not no boil, preferably use Barilla) 1 (15 oz) container ricotta cheese (whole or part skim) 1 (8 oz) pkg. shredded mozzarella cheese 1/2 cup grated or finely shredded parmesan cheese 2 Tbsp Chopped fresh basil or parsley (optional) Preheat oven to 375 degrees. Spray a 12 by 8-inch baking dish with non-stick cooking spray.

Heat a 12-inch non-stick skillet over medium-high heat. Crumble in beef and cook and break up beef until cooked through. Remove from heat and stir in marinara sauce and vegetable broth.

To assemble lasagna, spread 1/3 of the marinara sauce (about 1 1/2 cups) into bottom of baking dish. Top with 3 uncooked lasagna noodles.

Spread 1/2 of the ricotta over the noodles using the back of a spoon. Top evenly with 1/2 of the mozzarella (1 cup) and 1/2 of the parmesan (1/4 cup). Top with another 1/3 of the sauce.

Add 3 more uncooked lasagna noodles then spread evenly with with remaining 1/2 of the ricotta.

Pour and spread over last 1/3 of the sauce. Top with remaining 1/2 of the mozzarella and parmesan.

Cover tightly with foil (you can grease first if the top of the lasagna will be touching the foil so cheese doesn't stick), bake 40 - 45 minutes until pasta is fairly tender. Remove foil then continue to bake uncovered 10 minutes, or until lasagna is just tender (not soggy. There will seem to be a little liquid on the sides still). Let rest 5 minutes before slicing. Garnish with fresh basil or parsley if desired.

oing above and beyond the call of duty, Dr. Scott Ream, has not only utilized his optometric skills for patients in Thayer and West Plains, but has applied these skills to make a difference to those in developing countries.

Ream, who has practiced optometry for more than three decades, has dedicated a portion of each year to travel to third world countries to provide eye exams and eyeglasses.

Ream grew up in Thayer, graduating from Thayer High School in 1978 as valedictorian. He attended University of Missouri Kansas City where he graduated with honors with a Bachelor of Science in 1982. Then in 1987, he graduated from University of Missouri - St. Louis School Optometry.

Ream moved back to Thayer and practiced with his father at his practice, eventually buying it from him in 1988.

Throughout pursuing his optometry career, Ream began volunteering with Volunteer Optometric Services to Humanity (VOSH).

“When I was in school, I went on my first VOSH trip to Mexico and just really enjoyed it,” said Ream, stating he also went to Guatemala the same year.

Since then, collectively, he has been on about 35 trips traveling to poverty-stricken areas to provide eye care.

“Last year was the first year we were not able to go due to COVID,” said Ream, stating they are trying to plan their next

trip, but they are unsure as to when they will get to go due to COVID -19 here and in other countries.

“We really enjoy the trips. We give a lot to different charities but this way we get to see the smiles on the faces of the people we give the eyeglasses to,” said Ream, stating they make these trips because approximately 15 percent of the world’s population cannot learn or work because they cannot see clearly. “Most of them are in developing countries. We see the kids that can’t do their schoolwork because they can’t see the board or read their lessons.

We see people that need to work but they can’t see to do their work anymore. Sometimes, because of genetics and nutrition, they start needing reading eyeglasses about age 28 where it’s usually around 40 to 45 in the United States. Those are the people we see, even if they could afford eyeglasses, they just don’t have the access to eye care,” said Ream.

“Most people we see, they earn around $50 to $150 yearly income, and most of them just need eyeglasses,” said Ream, stating cataract surgery receives a lot of press coverage, but approximately 61 percent of those who are functioning blind just need a pair of eyeglasses to see. Approximately seven million need cataract surgery and 250 million are functionally blind because they need glasses.

“Our group is a little bit different. We try and go somewhere where no other group has ever been. That kind of makes it hard on us, but others can follow in our footsteps and hopefully continue the care there,” said Ream.

With VOSH, he has made seven trips to Central America

including Mexico, Honduras, Nicaragua and two times to both Guatemala and Panama. Ream has traveled nine times to South America, which included visits to Paraguay, four visits to Ecuador, Chile and three trips to Bolivia. His four trips to Africa have included Uganda, Morocco, Kenya and Tanzania.

Making one visit to the Caribbean to the Dominican Republic and numerous trips to Asia including India, Nepal two times, Cambodia, Vietnam, Thailand two times, Bhutan, Sri Lanka, Indonesia, Mongolia and Myanmar two times.

“We were the first ones to go to Asia. We have been to all kinds of remote countries including Mongolia and Bhutan…A lot of times it is very limited that they even let foreigners in the country through, a lot of times Rotary Club can kind of get us into these countries and get us through customs.”

Customs can be a problem because sometimes they want a share of what the eyeglasses will be sold for. However, the eye exams are free as well as the glasses. The first-time visiting Mongolia was not an issue, but the second time, customs wanted money to bring the glasses into the country. In Nepal, they spent two days getting their glasses out of customs.

They have given eye exams in the airports as well as glasses.

“We have really enjoyed all the time we have done this,” said Ream. Although, some situations they have encountered or came close to may have scared some away from taking such trips.

“In Guatemala, we were taken off the bus at machine gun point by men in black hoods.

We have been to villages where all the men had disappeared the night before. Rebels had come in and killed all the men. You could still

see the bullet holes and the blood. In Paraguay, we were there four days before there was a revolution. They overthrew the government, and they killed the vice president. In Bolivia, one of the towns we worked, they had a revolution right after we had left. They had sent pictures of the whole square just covered with dead bodies. You try and go where there is need and to where you are also going to be safe. You kind of have to pick and choose some of these places, but some of the third world countries governments can kind of turn on a dime,” said Ream.

The trips have also included flying into five of the world’s most dangerous airports. In Bhutan, the short runway is surrounded by 18,000-foot peaks and only eight pilots are certified to land there. In Nepal’s airport, there were no lights or air traffic controllers.

The airport in Bangkok they flew into has the highest crash to landing ratio in the world and has an 18-hole golf course in the middle of the airport. In Honduras, there is a 45-degree bank followed by a quick drop in altitude and in an airport in Quito, where a building has been hit nine times by active volcanoes.

However, none of this stops Ream and his colleagues from continuing to provide the invaluable services to people around the world.

“People kind of wonder why I do this, but you just kind of get hooked on it. I mean, we just love doing it. Each doctor sees usually between 125 to 150 people a day, basically work sunup to sundown. A lot of times we can’t rely on electricity. So, we can’t really work after dark. We have worked in churches and schools and civic organizations where they have their meetings. We have worked outside in some places. It is just whatever we can do to find any place to work there. It’s great.

We just enjoy the people. When we go there it’s not like being a tourist. A lot of these places you would never go as a tourist,” said Ream.

When visiting, they are often treated different than if they were tourists.

“They invite us into their homes. We eat meals with them. It is a totally different experience than you would ever get being a tourist. They are very appreciative of all the work that we do,” said Ream.

Eating locally has included food such as tarantulas, fertilized duck eggs, water bug gland secretions, cobras, crickets, fish stomachs, guinea pig and dog.

Accommodations have included staying in a concrete building in India while it was snowing with no heat and a blanket so thin it could be seen through. In Nicaragua, they stayed in an orphanage’s laundry room with roach infested baby blankets to cover up with.

In many of the places, hot showers are nonexistent. For instance, in Nepal, they had a choice of two motels to stay at, one costing five cents and the other 10 cents. Visitors could pay five cents for a cold-water shower, consisting of a bucket of cold water to pour over your head or pay 25 cents for hot water.

Eyeglasses are considered precious in the places they have gone.

Ream stated in some places where they have gone back after a couple of years, the eyeglasses had been passed from one person to another.

The individuals will be so happy, they will put the eyeglasses in their pocket. To which the doctors will tell them to wear them, and they will receive the response that they are too precious, and they do not want to break them.

After patients receive their eyeglasses, they are provided with reading material or cloth. Ream stated women that were needing to sew will start picking at the thread on the cloth. People that were not able to read their Bible are shown one.

“It is said that eyeglasses are the most effective poverty alleviation tool in the world because if you can’t do your job. If you can’t see, you can’t do your job. If you can’t see, you can’t learn. You can’t do any of this. This can make an enormous effect on people,” said Ream.

They have had people crawl in to where they are working because they could not see. They have also had people drop to their knees thanking them for the eyeglasses and stay there all day looking around.

“We had one family we had given eyeglasses, they were very high prescription, and as we were all pulling out, they were all standing underneath a tree looking at a branch like two feet above them.

They couldn’t even see that far without the eyeglasses. That just makes it all worthwhile,” said Ream.

The eye exams have also led to finding undiagnosed brain tumors and they would all “chip in to send them to the hospital to have them taken care of.”

The doctors will also diagnose other diseases during the exams.

“A lot of times, we will work with local organizations like Rotary, which will work with the hospitals to try to get these people taken care of after we have been there as well. So hopefully our care doesn’t end when we leave but continues on after that.”

There are approximately 3,000 diseases that can be detected through an eye exam. Ream stated they are not just checking if an individual needs a pair of eyeglasses but diseases as well as pieces of wood or metal in their eye. They have also treated pink eye,

glaucoma, high pressure inside the eye, etc.

“We are doing all of that while we are examining for the eyeglasses as well,” said Ream.

“A lot of times, this is the only eye exam they will ever have. The only pair of eyeglasses they will ever have. We are going out pretty much in the middle of nowhere when we are doing these eye exams,” said Ream. Sometimes, they will have a two-hour bus ride or stay closer to where they are examining people.

“It is usually about a two-week trip. A lot of times, when we are going to Asia, it is sometimes a 36-hour plane ride with layovers.

So, we get there, we usually have about 12 hours to kind of recover and then start working. So, we are jet lagged. It is a midnight to noon difference. We just want to utilize all the time we have to be able to see the people. It’s a rough trip. It takes about two to three days to adjust to that big of a time lag. I don’t sleep on the plane well anyways. It takes a couple of days to recover from all of that,” said Ream.

Of the size of the team that goes on the trips, Ream said, “There are about three of us that have been going together for 35 years.”

Other doctors have through the years as well. Sometimes they have had two to three additional doctors go or it has been just them three and other times it has been up to 12 doctors. “It just depends on different times,” said Ream.

Including the doctors; opticians, who adjust the eyeglasses, and others who help with screening, there are usually 10 to 12 on the trips.

Ream is also active in the optometric field serving on many boards and organizations.

He is currently President of Missouri State Board of Optometry and current chair of the Missouri Optometric Association (MOA) insurance committee.

In addition, he previously served as president of MOA as well as past membership services chair of MOA and has served on affiliated organizations, public relations, governmental affairs, and administration committees. Summer time Rivers By Dewayne French Rivers are one of the most common fishing adventures we have around the Ozarks.

We cross them every day on the way to work or school by crossing over the bridge and hurrying up to get to our destination. Most don’t even give it a second thought, but if you’re a fisherman you constantly look at the flows, color, and level of the rivers we cross.

In the hot summer time, like we are in, most rivers are at their lowest level of the whole year. We can almost step across some without getting our ankles wet in some areas. This can lead to some awesome wade fishing and kayak fishing opportunities.

Fishing a river in the summer can be one of the most productive times of the year. A rivers water level will be lower and most fish holding structures in the water will be prime

real estate. Just simple and basic fishing will get the job done almost every trip out.

With less root-wads and big rocks in the water which provide hiding places for the fish, these “should” be your prime targets. A well placed lure in the right location next to a fallen tree that still is in the water will generally be met with a bite from a fish. At normal water levels there are lots of hiding places for the fish to be around and just like we talked about earlier it is all about location, location, location, this time of the year. Lower water equals less fish holding structure and what is left in the water is prime fish holding habitat.

Big rivers are also slower moving and have less water, so they are somewhat easier to fish around also. Our main rivers around here, in the Ozarks, region like the spring, current, and 11 point have a lot of pressure on them. Mainly from people just wanting to cool off in the outdoors and float or boat around our beautiful Ozarks. People come from hours away just to enjoy what we have in our back yards. These rivers along with some of

our lesser known rivers can offer great summer fishing.

We just have to look out for other people enjoying our great waterways. If you have time to wade a stream or kayak a section of river always be aware of your surroundings. Even though the water is lower there can still be slipper rocks and fast moving water over rocky shoals.

Give someone your locations and times you plan to be off the water.

Most of our deep off road rivers areas don’t have good cell service and if you do get in trouble you may not be able to reach a tower for service. The high bluff lines and beautiful areas that we all love in nature are just that, and not great with a big cell tower in the middle of them. So pay attention and have fun but use caution and common sense in the river.

Summer fishing still has its cautions so just heed to what Mother Nature is telling you and you will be fine. Simple lures and an ultralight rod/reel combo are all that’s needed to have a great day with your fishing buds. or Thayer residents, many may wonder what exactly is going on in the former Thayer Drug Store, located on South Sixth Street. Though the business model of CPI (which stands for Contract Pharmacy Insight) is complex and doesn’t provide a product or services to individual consumers, the business is certainly making a difference in the healthcare industry. The business is also helping locally, as CPI has created nearly 20 professional jobs for people in the area.

The 340B program itself is defined as follows from the Health Resources and Services Administration. “The 340B Program enables covered entities to stretch scarce federal resources as far as possible, reaching more eligible patients and providing more comprehensive services. Manufacturers participating in Medicaid agree to provide outpatient drugs to covered entities at significantly reduced prices” Areawide Media intern Sophie Rossitto also researched 340B. “Congress created the 340B drug pricing program in 1992 to help vulnerable patients receive medicines at affordable prices,” said

the 340B Health website. “The program requires pharmaceutical manufacturers to offer discounts on outpatient medicines distributed to qualifying hospitals, clinics and other health organizations.”

The Office of Pharmacy Affairs, which is part of the Health Resources and Services Administration, administers the drug pricing program, said the 340B Health website. Both the OPA and HRSA are within the Department of Health and Human Services.”

With that explanation in mind, owners Larry and Angela Burns, then share how their business is helping hospitals, clinics and healthcare centers with 340B. “We didn’t invent anything. The government already had these money saving mechanisms for hospitals and health centers. It turns out some of those mechanisms are complex. It is very difficult for them to figure out how to realize income, or their savings as it is called, from 340B. So when you start going to these rural areas where there are hospitals and health centers, they don’t always have access to the expertise they need. They may not have an on-staff attorney, accountants, pharmacists, IT, and nurses who possess specialized 340B knowledge,” said Larry.

CPI is benefitting these healthcare facilities, as well as the area by providing unique jobs. “We are a rare business for our area,” said Angela. “We are able to provide high-tech and professional jobs,” said Director of Compliance, Jennifer Crask, “but it also creates curiosity in the community due to not having a set of products or services to offer people off the street.”

Larry said there may not be another business exactly like CPI in 500 miles.

While CPI does partner with other consulting companies in specialized areas, CPI is unique in that it offers a large breadth of 340B expertise.

“We have Jennifer who is an attorney, two pharmacists, we have several nurses, accountants, IT and other operations staff; we basically outsource what a small or mid-sized hospital needs in order to have access to the same expertise as very large hospital systems,” Larry explained.

The federal laws for these programs were already in place, but figuring out how to do it is complex. “If you are a hospital and you are thinking you would like to participate in 340B, but are going to need five people who are specialized, you may be wondering where are we going to find all those people? We can go there and already have the expertise; the legal, accounting, operations, IT; we can do staff augmentation, we can take a slice of our

time and we can build this thing and we can make it operate like you are Mercy or UAMS.” “We have helped health centers build programs that allow them to expand. They’ve been able to expand into new rural, underserved areas,” said Larry as Angela added, “Not even just clinic expansion, but service expansion as well because a lot of these health centers offer dental. Dental can be a financial underperformer so they use some of the funds from the 340B savings we help them generate to support their dental services.” Some of CPI’s clients also offer opioid abuse counseling, mental health services and transportation services for patients. “There is no money in transportation,” said Larry. It is CPI’s job to help generate 340B savings to help these entities. “Our clients are non-profit organizations, and many have to reinvest their federal funds back into their service offerings.”

Larry and Angela saw the struggle entities were having with 340B and wanted to make it a better experience. “We realized there was a gap there five or 10 years ago and started thinking, ‘is there is a business model to help those people in rural areas like Salem, Harrison, or Mountain Home’; so we started building that team,” said Larry.

CPI was incorporated in 2015. In the beginning, CPI renovated existing office space in Mammoth Spring, but then quickly outgrew it. The Thayer Drug Store building became available, so they remodeled the facility for their needs.

Remnants of the old pharmacy and cafe can still be seen throughout the office space, which is modern and well organized in a fashionable manner. There is a break room and a recently added conference room for virtual meetings. Fast forward to 2021, CPI now employees nearly 20 individuals in the Thayer headquarters.

When asked why creating a business to help hospitals and health centers was important they said, “We were involved in this early

on from the pharmacy perspective … There were many pharmacy-related things hospitals just didn’t have the expertise to manage. The 340B programs were not optimal.

Pharmacies were dropping out, revenues weren’t where they needed to be, there were compliance issues and other problems; what we realized as 340B grew, and it was going to grow, was that there were some hospitals and health centers that were going to get left behind and so we saw an opportunity with an interesting problem to solve.

I don’t think we set out to build what it became,” replied Larry. “The hospitals and health centers in our small communities, they need the resource more than anyone else does and have the hardest time getting access to the expertise,” Angela added.

Congress’ initial intent was to create a mechanism so that recipients would have an additional funding source that was not coming from the government but from the drug manufacturers, who ultimately fund 340B. Another portion of the program

is that some health centers and hospitals can take their drug pricing and pass it on to patients that are uninsured. “If you were an uninsured patient and needed insulin, which is expensive for no good reason, and you go to a 340B participating health system and you are uninsured, then they can use this same mechanism we are talking about to provide insulin to you a much lower cost. They can provide $1,000 of insulin for much, much less through the local pharmacy in that town.

You don’t have to go through a bunch of paperwork, you don’t have to mail it off somewhere.

When we saw that, we said that was a huge win. We help health systems build these 340B programs, so now if you live in rural communities and are uninsured, you have a safety net to get expensive prescriptions cheaper,” said Larry. “There are a lot of moving parts to make that system work. You have to be tightly integrated with the local pharmacies,” said Angela.

In general, CPI saw that the relationships between pharmacies and hospitals were not the greatest due to neither one understanding the others’ business model. “They are completely separate universes, when you try to converge them, it sometimes doesn’t work well,” added Larry. “So we help bridge the gap and facilitate that whole process,” explained Angela.

What does the future hold for CPI? The team has had great success in Arkansas and Missouri and are continuing to expand both there and in other states; however, according to Crask, “We carefully manage our growth so that we don’t dilute our expertise or services to our existing client base. And we are always looking for highly qualified people to add to our team as we grow. It is a professional setting and we have a team environment where we collaborate closely with one another.”
