Why routine checkups are vital to overall health Special to the advance Regular visits with a medical professional are an important part of a healthy lifestyle. Too often people visit the doctor only when they are ill, as they may not realize just how essential well visits and physical exams are.

Routine checkups are the smartest way for people in all age groups to stay on top of their health, but they can be especially valuable for those age 50 and older. Regular checkups enable physicians to check current health against past visits, ensuring that any anomalies can be investigated and treated efficiently and promptly. This can make the difference in slowing down the progression of a disease that has already developed or prevent something from becoming a fullfledged issue.

The Mayo Clinic says there are no hard and fast rules about how often seniors should visit health care providers. Those who are in generally good health may only require one medical checkup a year. At this point vital signs will be checked, medications reviewed and lifestyle topics discussed.

Doctors may even recommend or discuss tests. Patients also can bring up any issues they may be experiencing, however insignif insignificant they may seem. Anything from sleep disturbances to memory loss to unexplained fatigue or pain can be addressed. Sometimes getting everything out in the open and being reassured that there's nothing to worry about can be helpful.

General care and geriatric doctors also are adept at asking questions to get a sense of how patients are faring in the world. This

may include topics that seemingly have no relevance to health but can be quite important. A provider may ask about topics such as bathing or dressing. Questions about social interaction or typical routines can paint a better picture of both physical and mental health.

The recommended frequency of doctor visits may change as health issues arise or if follow-up is needed after a treatment plan or injury, according to the caregiver company Home Care Assistance.

Some seniors may have to visit a provider once a week or once a month. Doctors, nurses and therapists will design a regimen based on a patient's current health needs.

Personal safety tips to live by Special to the Advance The fight or flight mechanism is an important tool in safeguarding well-being, but there is much more a person can do than relying strictly on gut reaction. Taking personal safety seriously means having a realistic view of the potential dangers in the world, and employing various strategies that promote safety.

• Put down the phone. Distractions are a major cause of injury and putting oneself at risk. Texting while walking or driving increases the risk for injuries. Predators also look for signs of distraction or weakness, and eyes that are glued to a phone make it hard to remain aware of your surroundings.

• Lock your doors. Everyone wants to believe theirs is a safe community. But crimes like door push-ins or break-ins happen in every community.

Locking home and vehicle doors puts an added barrier between a would-be criminal and your belongings or person.

• Stick to well-lit areas. The dark can hide people and ill intentions. Try to run as many errands or tasks during daylight hours as possible. When you must go out at night, walk or park in well-lit areas.

• Move to a public place. If you think you are being followed, either on foot or in the car, go to a public place or a police station.

• Don't overshare on social media. People have a tendency to throw caution to the wind on social media, revealing personal details they likely wouldn't share in other settings. Posting you are out of the house or away on vacation is an invitation for trouble.

• Avoid isolated places. Waiting for a bus, taxi, ride share, or even to meet another friend in a place without a lot of people around can put you at risk. It's better to wait inside of a store or a building with others, if possible.

• Check visitors first. Whether at home or in a hotel while traveling,

confirm who is knocking on the door before opening it. If someone states they are from a particular business or service, ask for identification. Call the front desk of a hotel to confirm identities and purposes.

• Watch your drink. Never leave a drink unattended in a public restaurant or bar, as something can be slipped into it. Similarly, don't accept a drink you that didn't see being poured.

Beware of skimmers and shimmers Special to the advance Personal safety refers to physical and mental well-being, but individuals also must take steps to safeguard their financial well-being.

Thieves have various ways to steal individuals' personal data — everything from hacking into banking or shopping accounts to stealing physical mail. But thieves also have more sophisticated, often hidden ways to steal private financial data. Consumer Reports says the public should be aware of skimmers and shimmers, which are sophisticated devices that steal data when people use public credit and debit card readers at ATMs, gas pumps and other locations.

A card skimmer is placed on top of the point-of-sale terminal. When a person swipes his or her credit or debit card, the device collects data from it. That data can be used to make fraudulent purchases and to produce counterfeit cards. Among the newest tools scammers use are shimmers, which are tiny types of skimmers that are capable of reading data from new chip-based cards, according to the ATM Industry Association, a nonprofit trade organization. Criminals can even retrofit hidden cameras at ATMs and other locations with card skimmers. Skimmers and shimmers are often Bluetooth-based circuit boards that can transmit stolen card data wirelessly. A thief can drive by a terminal to download card data remotely from a mobile device or laptop.

Alert others in emergencies Special to the advance Emergencies occur every day. Data from the 2016 National Hospital Ambulatory Medical Care Survey found around 145 million emergency department visits are made in the United States each year. Of those visits, 42.2 million are injury-related. The Canadian Institute for Health Information says Canadians make about 14 million visits to hospital emergency departments annually.

While no one wants to imagine themselves in dangerous situations, being realistic about the likelihood of being in an emergency can help people safeguard themselves against such situations.

One way to prepare for emergencies is to make sure you and potential first responders or medical personnel know who to call should you be unable to communicate. Likely many things in the digital age, the process of indicating emergency contacts has changed.

Emergencies may arise due to life-threatening medical conditions.

If a person is in a situation in which they are unconscious or unable to speak, a medical alert card or bracelet can help first responders provide medical care more quickly. Bracelets and other jewelry, like those offered from the MedicAlert® Foundation, are engraved with critical information for conditions like autism, allergies, dementia, diabetes, Alzheimer's, or epilepsy. They can provide peace of mind and speak for the person wearing one in an emergency.

All people are urged to designate an “in case of emergency” contact on their mobile phones. This way a first responder or other emergency personnel can notify the appropriate people in emergency situations. In 2004, after research from Vodafone found that fewer than 25 percent of people carried any emergency contact details on their persons, a campaign was started by Bob Brotchie of the East of England Ambulance Service in the United Kingdom to start providing emergency contact information. Brotchie urged mobile phone manufacturers to support the campaign by adding an ICE heading to phone number lists of all new mobile phones. Emergency contact information also should be provided for school children and with a person's employer and updated regularly.

People should consider naming a health care proxy. This can be any whom they trust. The proxy can make health care decisions for another if he or she is unable to speak. Naming a proxy ensures that one gets the healthcare he or she prefers in the event they cannot communicate their wishes. Sometimes healthcare proxies are merged with living wills as one advanced directive document, according to the Medicare Rights Center.

Preparing for emergency situations, however unlikely they may seem, includes employing various strategies to help emergency responders and medical personnel in the event you are incapable of communicating with them.

Being prepared for emergencies means understanding basic first aid procedures that can help a person navigate a crisis — or at least stabilize the situation until paramedics arrive. The following are some first aid principles that individuals should know and practice, courtesy of the American Heart Association, American Red Cross and CPR Certified Indigo Medical Training. They should not replace formal first aid training, but can assist a person when he or she is providing medical care to someone in need.

Bleeding can almost always be controlled until more help is available. Severe bleeding can lead to shock and death, so bleeding needs to be addressed immediately. Cloth should be used to cover the wound, and direct pressure should be applied to stop the blood flow. Do not remove the cloth; add more layers as needed. Easy ways to keep your immune system strong Special to the advance The immune system is a powerful component of the human body. The immune system recognizes when viruses, bacteria and other foreign invaders enter or compromise the body, and then takes action to prevent illnesses from taking over. The average person can help his or her immune system do its job more ef effectively by making the immune system as strong as it can be.

Harvard Medical School says that diet, exercise, age, and psychological stress may affect immune system response. Certain lifestyle choices can promote a strong immune system.

• Get adequate sleep.

Doctors believe sleep and immunity are closely tied.

A study of 164 healthy adults published by the National Institutes of Health found those who slept fewer than six hours each night were more likely to catch a cold than people who slept for more than six hours.

Aim for adequate rest each night to keep your body in top form.

• Increase your intake of fruits and vegetables. Fruits and vegetables supply the powerhouse antioxidants that are essential for protecting a body against free radicals. Free radicals may play a role in heart disease, cancer and other diseases.

Serve fruits and/or vegetables with every meal to ensure you're getting enough antioxidant-rich foods.

• Consume fiber and fermented foods. Fiber can

help feed the gut microbiome, which is linked to a robust immune system.

The microbiome also may prevent harmful pathogens from entering the body through the digestive tract.

Data also suggests that eating more fermented foods can further strengthen and populate healthy bacteria in the gut.

• Exercise regularly. Aim for 30 minutes of moderate exercise per day, advises the American Heart Association. Thirty minutes of exercise each day can go a long way toward keeping the body healthy. The U.S. National Library of Medicine says physical activity may help flush bacteria out of the lungs and airways.

How to be safe in the sun Special to the Advance A relaxing day outdoors soaking up some of the sun's rays is how many people prefer to spend their free time when the weather allows.

While the very vision of a warm summer afternoon spent outdoors can invoke positive feelings, it's important that people take protective measures before going outside and continue to do so while they're out there.

According to the American Cancer Society, most skin cancers are the result of exposure to ultraviolet rays in sunlight. UV rays are a type of radiation that do not have enough energy to penetrate deeply into the body. As a result, they primarily affect the skin. Overexposure to these rays can lead to skin cancer.

PROTECTION FROM UV RAYS

The ACS notes that there are no safe UV rays, so it's imperative that people take UV protection seriously. The following are some of the many ways to protect yourself while still enjoying sunny days outdoors.

• Go out at the right times of day. The ACS notes that UV rays are at their strongest in the middle of the day between the hours of 10 a.m. and 4 p.m., so staying inside during these hours can protect your skin.

This is especially important in the spring and summer, as the ACS says UV rays are stronger during these seasons than other times of year.

• Employ the shadow test when going outside. It may not seem especially scientific, but the shadow test is a simple way for anyone to gauge how strong UV rays from the sun are at any given moment. According to the ACS, if your shadow is shorter than you, that means the sun's rays are at their strongest. This simple test can help people immediately determine how strong the sun's rays are, compelling them to be extra cautious if necessary.

• Apply sunscreen early and reapply often. The ACS recommends using sunscreens with broad spectrum protection that protect the skin from both UVA and UVB rays, and applying them before leaving the house and reapplying often while outdoors. When choosing a sunscreen, choose one with a minimum sun protection factor, or SPF, of 30. Understanding SPF can help people recognize the importance of reapplication. Sunny summer afternoons may be especially cherished in 2020, when people across the globe have been forced to spend more time indoors as they adhere to social distancing guidelines implemented to prevent the spread of the COVID-19 virus.

Parents of infants may be extra excited to get outdoors this summer. Chances to get out and about don't come around very often for parents of infants, and those chances may be even more

the equivalent of one minute of UVB ray exposure for each 30 minutes he or she spends in the sun. So one hour in the sun wearing SPF 30 sunscreen is the same as spending two minutes totally unprotected. Reapplying SPF 30 sunscreen often can ensure you are protected at all times.

• Wear a hat. Hats with a brim that is at least two to three inches all around protects vulnerable areas such as the eyes, forehead, nose, ears, and scalp. Choose a hat with a dark, non-reflective underside, as such a hat can lower the amount of UV rays that reach the face from reflective surfaces such as water.

• Avoid exposing infants to the sun. Instead of applying sunscreen to their infants, parents should keep them out of the sun entirely. Seek shade between 10 a.m. and 4 p.m., when the sun's rays are especially intense. When taking infants out in their strollers, make sure they are shaded at all times and avoid walking on the sunny side of the street.

• Dress babies for the sun. The SCF advises parents to dress their babies in brimmed hats and lightweight clothing that fully covers their arms and legs. Dressing babies for the sun may seem like common sense, but the SCF notes that researchers at the University of Miami found that only 43 percent of parents consistently cover their babies with hats, and even fewer (40 percent) cover babies with long-sleeved shirts and pants when going out in the sun.

• Consider sunglasses. Sunglasses are another way parents can protect their children from the sun's harmful ultraviolet rays. Sunglasses can be an alternative for parents whose infants keep taking off their hats, as many baby-sized sunglasses are equipped with elastic straps to keep the glasses on. Signs you might be allergic to your medication Special to the advance Medicine helps billions of people across the globe stay healthy. Medicines are used to treat existing illnesses and reduce a patient's risk of developing others. As effective as medicines can be, no two humans are the same, so medications that may help one person could prove harmful to another.

The American Academy of Allergy, Asthma & Immunology notes that all medications can cause side effects, but only 5 to 10 percent of adverse reactions are due to allergies. Allergic reactions begin in the immune system, which controls how the human body defends itself. If a person is allergic to a certain type of medication, his or her immune system identifies the drug as an invader or allergen and it can respond in various ways. The AAAAI notes that the most common immune response to medication is a result of the expansion of T cells. These cells identify the drug as foreign and create a delayed immune response that most often affects the skin.

While allergic reactions to medications may not be very common, the AAAAI says that such reactions can be life-threatening. That highlights the importance of learning the signs of allergic reactions.

tion are not often life-threatening, but anaphylaxis can be deadly. The ACAAI notes that anaphylaxis occurs when an over-release of chemicals forces a person to go into shock. Even people who have already administered epinephrine, the drug used to treat severe allergic reactions, should visit an emergency room immediately at the first sign of anaphylaxis, which may include trouble breathing, tightness of the throat, hoarse voice, nausea, vomiting, abdominal pain, diarrhea, and dizziness. Rapid heartbeat, low blood pressure, a feeling of doom, cardiac arrest, and fainting are some additional signs of anaphylaxis.

According to the AAAAI, most anaphylactic reactions occur within one hour of taking a medication or receiving an injection of the medication. However, such reactions also can take place hours after a medication has been taken.

Antibiotics are the most common cause of anaphylaxis, but the AAAAI notes that recent research has shown that chemotherapy drugs and monoclonal antibodies also can induce anaphylaxis.

People take medicine to feel better, but sometimes their bodies and certain medications are not a match. As a result, people must be vigilant when taking medications for the first time, paying particular attention to how their bodies react. Older adults, especially those with underlying medical problems are at a higher risk for sever complications from COVID-19 infections. So, it is important for family members and care givers of older adults to know what to do to keep the elderly safe during the pandemic and what to do if they become infected.

Keeping oneself healthy: As a caretaker of an older adult, keeping oneself healthy is extremely important to reduce the risk of passing on the infection and to avoid the older adult no having you there to care for them due to you becoming ill. You will want to follow guidelines such as washing your hands frequently, especially before providing care to the older adult, before preparing food and after using the restroom or touching surfaces in public places. You will want to avoid crowds and maintain social distancing. You will also want to clean frequently touched surfaces in your home often, including mobility and medical equipment used by your loved one, such as walkers, canes and handrails.

Distance but don't isolate: You will want the older adult to practice physical distancing, but not isolation. Loneliness in older adults can lead to significant mental health issues including depression and worsening of demential symptoms and can have detrimental consequences to physical well being as well.

That will mean limiting in-person visits, but still allowing for social contact through use of technology such as smartphones, tablets and computers. You may need to assist the older adult in using the devices to stay in touch with loved ones, friends, neighbors and faith/ church groups. Many older adults also enjoy cards or letters as a way to remain in touch as well.

Minimize the risk: You can minimize the risk of infection by having the older adult avoid travel, order groceries or other goods rather than have them go shopping for them and checking into the availability of telemedicine appointments when possible with their physicians.

Make a plan: You will want to decide on a plan for what to do if the oder adult or someone else in the family becomes sick. It would be helpful to talk this through with the older adult to help reduce their stress and help them feel more involved and prepared. You'll want to designate someone you trust to care for the older adult if you become ill. It is also a good idea to stock up on medications, food and essentials in order to make sure the oder adult has everything they would need for at least a two week period in case of the need to quarantine.

cough, fever, shortness of breath, you will want to call your primary care physician. You will want to follow their instructions. It is important to also be aware that older adults, at times, are showing unique symptoms of COVID-19 including seeming “off”- not acting like themselves- early on after being infected by the coronavirus. They may sleep more than usual or stop eating. They may seem unusually apathetic or confused, losing orientation in their surroundings. They may become dizzy and fall. Sometimes, seniors stop speaking or simply collapse. If the older adult has Alzhimer's disease or another form of dementia, they may exhibit increased agitation, increased confusion and sudden sadness in addition tot he other usual symptoms of COVID-19 such as coughing, difficulty breathing, fever, chills, muscle pain, sore throat, loss of taste or smell and headache. Of course, in a medical emergency where the older adult is experiencing severe shortness of breath or a high fever, call 911.

Reflections is staffed by a multidisciplinary team including registered nurses with psychiatric experience and expertise, licensed social workers, licensed counselors and others – such as occupational and physical therapists – working as a team under the direction of a board-certified psychiatrist. Additional physicians may be consulted, as needed, for special medical needs of the patients. Through this coordinated effort, the program provides the support, structure and professional expertise needed to help patients stabilize their condition and achieve their optimal level of functioning.

Medical director Dr. Althea Conley began her path to Reflections by earning her undergraduate degree in psychology at Hendrix College and from there went on to complete her graduate studies and earn a doctorate in clinical psychology from the University of Arkansas - Fayetteville. She completed medical training at the University of Arkansas, followed by an internship at John L McClellan VA Medical Center in Little Rock. Her second internship was with the University of Rochester, where she also completed a residency. Schools shifted to remote learning, nonessential workers were asked to work remotely and families were told to engage in internet-based chats in lieu of in-person gatherings.

Heightened use of the internet and digital products puts users at risk when they go online. When to use soap and water, and when to use hand sanitizer Special to the advance In the wake of the global COVID-19 outbreak in early 2020, millions of people across the globe found themselves scrambling for hand sanitizer. While the Centers for Disease Control and Prevention note that cleaning hands at key times is one of the most important steps people can take to avoid getting sick and spreading germs, there are differences between washing with soap and water and washing with alcohol-based hand sanitizers.

The CDC notes that preventing the spread of sickness through handwashing is most effective when people know which method to use when cleaning their hands.

WHEN TO USE SOAP AND WATER

The following are common situations when the CDC advises using soap and water to clean hands.

• Before, during and after preparing food • Before eating food • Before and after caring for someone who is sick • Before and after treating a cut or wound • After using the bathroom, changing diapers or cleaning up a child who has used the bathroom • After blowing your nose, coughing or sneezing • After touching an animal, animal food or treats, animal cages, or animal waste • After touching garbage

• If your hands are visibly dirty or greasy When washing with soap and water, the CDC advises people to wet their hands with clean running water (warm or cold) and applying soap.

Lather the hands by rubbing them together with the soap, making sure to scrub all surfaces of the hands, including palms, backs, fingers, between fingers, and under the nails. Scrub for 20 seconds before rinsing hands clean under running water and drying your hands, be it with a clean towel or air drying.

WHEN TO USE ALCOHOL-BASED HAND SANITIZER

Hand sanitizer should not be applied to hands that are dirty or greasy.

Hands that become dirty or greasy after activities such as gardening or fishing should be cleaned with soap and water. The CDC advises using alcohol-based hand sanitizer: • Before and after visiting a friend or a loved one in a hospital or nursing home, unless the person is sick with Clostridium difficile (if so, use soap and water to wash hands).

• If soap and water are not available, use an alcohol-based hand sanitizer that contains at least 60 percent alcohol, and wash with soap and water as soon as you can.

Five big health threats for men Special to the advance Many men do not feel the need to visit a doctor unless there is something that is causing problems that can no longer be ignored. The Centers for Disease Control and Prevention says men are 33 percent less likely than women to have visited a doctor within the past year. By the time men see a physician, the window of opportunity to screen for early health problems may already have passed, and now it becomes essential to treat complications of a disease.

Men are at a higher risk for certain health conditions than others. Among the 15 top causes of death, men lead women in all of them, except for Alzheimer's disease, according to WebMD. 1. Cardiovascular disease: The American Heart Association says more than one in three adult men have a form of cardiovascular disease. High blood pressure is a major concern and stroke affects more than three million men. Changing one's diet, exercising and getting routine health

examinations can go a long way toward preventing the onset of heart disease.

2. Liver disease: High levels of alcohol and tobacco use among men can put them at a risk for diseases of the liver, such as cirrhosis and alcoholic liver disease. Globally, cirrhosis caused more than 1.3 million deaths in 2017, two-thirds of which were men. And experts at the Institute for Health Metrics and Evaluation at the University of Washington School of Medicine say hepatitis B and excessive use of alcohol are notoriously high in men, contributing to liver issues.

In addition, nonalcoholic fatty liver disease, which is especially prevalent among obese individuals, can contribute to cirrhosis.

3. Respiratory diseases: COPD and other respiratory conditions can lead to life-threatening conditions. The American Lung Association says more men are now being diagnosed with lung cancer than in years past. Smoking remains the leading cause of lung See MEN Page 21

Recognize signs of heart attack in women Special to the advance Many people are familiar with the image of a heart attack sufferer clutching his or her chest or feeling surprising, tingling sensations in his or her left arm. While those symptoms are common, heart attacks can produce a wide array of symptoms, and some of them may actually be much less apparent than chest pain or tingling in the left arm. That's especially so for women.

The organization Go Red for Women, which highlights women's heart health during the month of February, advises that many symptoms women can experience when suffering from heart disease may be overlooked or misunderstood as signs of less threatening conditions. However, jaw pain, nausea, pressure, and sweating all may be indicative of a heart attack. A failure to recognize that and act quickly could prove fatal.

The American Heart Association says that heart disease is the foremost killer of women in the United States. The Heart and Stroke Foundation says heart disease and stroke kill 31,000 women in Canada annually. Despite those figures, many women are unaware of the threat of heart disease and its symptoms.

Simple ways to save after being laid off or furloughed Special to the advance The outbreak of the novel coronavirus COVID-19 had a dramatic effect on the global economy. Businesses were shuttered seemingly overnight as public health organizations across the globe embraced social distancing as a means to preventing the spread of the potentially deadly virus.

Few, if any, businesses proved immune to the economic effects of the COVID-19 outbreak, which forced many employers to lay off or furlough employees.

According to the U.S. Department of Labor, the week ending April 18 marked the fifth consecutive week that more than three million Americans filed initial unemployment claims. During that period, which for many areas of the United States coincided with the first five weeks of social distancing guidelines going into effect, roughly 26.5 million workers had filed jobless claims. The picture was not any rosier in other parts of the globe.

For example, Statistics Canada noted that more than one million jobs were lost across Canada in March, increasing the country's unemployment rate from 5.6 percent in February to 7.8 percent by the end of March.

Many workers were furloughed in the wake of the COVID-19 outbreak, and while furloughs differ from layoffs, each situation left people looking for ways to save money. The following are some simple ways to save that can help furloughed or laid off employees make it through the financial uncertainty associated with the COVID-19 outbreak.

• Contact your lenders imme-

diately. Many lenders, including Bank of America and Chase, announced breaks for homeowners and other borrowers in the wake of the COVID-19 outbreak.

Lenders may vary regarding the extent of the breaks they offer as well their eligibility guidelines, but borrowers may be able to defer mortgage and auto loan payments for several months.

Consumer advocacy groups warn borrowers to learn what the long-term effects of deferring payments may be. In addition, borrowers should know that they must contact their lenders before ceasing monthly home or auto loan payments.

• Consider cutting the cord.

Many people have long advocated cutting the cord as a means to saving substantial amounts of money. Cutting the cord refers to forgoing traditional cable television service in favor of less expensive streaming options like Netflix and Amazon Prime. The cut the cord mantra seemed to resonate in the immediate aftermath of the implementation of social distancing, as Netflix reported adding nearly 16 million subscribers in the first quarter of 2020. But before cutting the cord, consumers are urged to see if doing so would actually save them money. Customers who currently bundle their cable with internet and/or phone service may end up paying the same amount of money, if not more, even after canceling their cable subscriptions.

Do your homework before cutting the cord, speaking with your provider to learn the true cost of cutting the cord. You may end up saving more by maintaining your cable but removing premium channels and downgrading to

basic packages.

• Develop a new budget. Budgeting is an essential component of responsible money management at all times, and especially so in the wake of a layoff or furlough. Do not hesitate to develop a new monthly budget,

examining all monthly expenses to determine where costs can be trimmed. The longer you wait, the less you may save during a layoff or furlough.

Saving money in the wake of a lay off or furlough can be simpler than people think.
