by The Chartis Center for Rural Health. This annual award honoring rural hospital performance is determined by the results of iVantage Health Analytics’ Hospital Strength INDEX, which is celebrating its

10th anniversary in 2020.

Based entirely on publicly available data, the INDEX is the industry’s most comprehensive and objective assessment of rural hospital performance.

Utilizing 50 independent indicators, the INDEX assesses performance across eight pillars of performance that span market-, value- and financebased categories. Hospitals recognized as a Top 100 facility had one of the 100 highest overall scores among all Critical Access Hospitals nationally.

minds us that rural providers haven’t lost touch with their mission and are committed to delivering better quality, better outcomes and better patient satisfaction,” Michael Topchik, National Leader, The Chartis Center for Rural Health said.

In 2020, PMC was also named one of America’s Best Hospitals for Emergency Care by the Women’s Choice Award. The award signifies that Providence Medical Center is in the top eight percent of 4,191 U.S. hospitals offering emergency care services.

He’s my Godwink Marcia Johnson is one tough lady. During the Christmas Blizzard of 2009, Johnson fell.

Even though she was in a significant amount of pain, she didn’t go to the emergency room. That Christmas, much of Nebraska was experiencing whiteout conditions that upended holiday plans and stranded many people far from home.

So she toughed it out, went to bed, hobbled around for a few months and, eventually, the pain started to dissipate.

Years later, Stephen Smith, MD, a local orthopedic surgeon, discovered that Johnson had actually broken her hip in that fall. But Johnson pushed on. Then, last year, she began experiencing consistent pain.

“Slowly the pain got worse and worse, until it became unbearable,” she says.

In July, she went to the doctor again. “The pain was too much,” she says.

Johnson consulted with all the orthopedic surgeons in Norfolk, but her situation was very complex. She’d had both of her hips replaced in 2001.

When she fell and fractured her pelvis, a fibrous scar tissue formed, which initially provided her stability, but, over time, failed to fully heal. The cup of her hip replacement moved past the bony stop inside her pelvis because of significant

bone loss, putting it near her bladder and other vital structures.

For a case like hers, Johnson would need a surgeon trained in advanced hip revisions with experience in dealing with bone loss and achieving stability, if needed.

The final consensus was that Johnson would need to go out of the area for this type of operation. Her orthopedic team first tried Nebraska Medicine, but Johnson wasn’t going to be able to get in, even for a consultation, until Sept. 18. It was only the second week in July.

“The orthopedic team at Faith Regional tried everyone they could think of: Grand Island, Omaha, Lincoln,” she says.

At one point, the pain grew so severe that Johnson traveled to the emergency room at Nebraska Medicine. “They kept me overnight, did a CT scan, and tried to help with the pain, but I was released the next day,” she says. “I cried all the way home.”

Johnson was at the orthopedic office in Norfolk, getting an injection, when Dr. Smith let her know that there was a new doctor in town. This doctor had completed an adult reconstruction and hip arthroscopy fellowship during his training.

“He said, ‘Marcia, I think he can help you,’” she says. The

following Monday, Johnson met Michael Woodbury, DO. Johnson remembers the moment vividly. “He entered the room, sat down and looked at the x-rays. His eyes opened up. I could tell he knew what he was looking at, and he knew what he was doing. Just watching him gave me so much hope.”

“He said, ‘Marcia, I can fix this, and you’ll only need one incision,’” she says. “I can’t describe the feeling in my stomach -- this huge realization of letting go: This is actually going to happen.”

“When we walked out, I looked at my aunt who was with me and said, ‘This is a God thing,’” Johnson says. “’I will be able to have the surgery done in Norfolk, and it’s going to be sooner.’ She said, ‘I know.’”

Johnson’s surgery was scheduled for a week and a half later.

“I can’t tell you the relief I felt. I can’t describe the pain I was in,” she says.

In the time leading up to the surgery, Johnson kept coming across this term: Godwink.

“It’s when God knows where you are, what you are and what you need,” she explains.

Johnson had the surgery on Oct. 3. Today, she is no longer in pain.

“The hospital staff was all wonderful,” she shares. “They were knowledgeable, caring – honest-to-God, genuine caring. This is what it means to have care at home. It’s wonderful. I can’t believe my luck.”

When it comes to Dr. Woodbury, “He’s my Godwink,” Johnson says.

Faith Regional Foundation provides businesses, schools with Automated External Defibrillators Faith Regional Health Services recently delivered an Automated External Defibrillator (AED) to Burrows Family Dentistry and Kaler Orthodontics in Wayne.

Staff at the facility submitted an application for the AED earlier this year and were notified that they were among those who received the lifesaving devices.

Faith Regional Health Services’ cardiac care coordinator Jenny Simmons, RN and manager of cardiac services, Kim Long, RN have been traveling the region, gifting Automated External Defibrillators (AEDs) to businesses, schools and organizations.

“We’ve really enjoyed being able to give back to our communities and the surrounding

area,” said Simmons. “Especially since it’s a gift that could save a life.”

AEDs are, by design, easy to use. By following an AED’s simple, clear voice, bystanders can perform the crucial steps that can save a life.

Sudden cardiac arrest occurs when the heart stops beating suddenly and unexpectedly. Often, this is caused

by ventricular fibrillation, an abnormality in the heart’s electrical system. When this occurs, blood stops pumping to the brain, heart and rest of the vital organs. Bystanders who promptly begin CPR and defibrillation can keep oxygenated blood flowing through the body and preserve life.

However, the biggest hurdle for many schools and businesses who don’t have an AED, is cost. The minimum cost for an AED is around $1,200 which is why Faith Regional launched its AED Community Initiative in 2019. The goal of this initiative is to raise funds to equip schools, businesses and organizations across the region with an AED if in need.

After funds were raised, any business, school or organization could submit an application to Faith Regional. All 26 of those that applied received a free AED. Burrows Family Dentistry/ Kaler Orthodontics in Wayne was among those on the list of recipients.

“Every two years we (staff) have to be CPR certified. An AED is often talked about at these certifications and demonstrations given on how to use one. That is one reason we chose to apply for one for our office. It is nice to have, but we hope never to have to use it,” Dr. Rob Burrows said.

This AED will be kept at the office, located at Seventh and

Pearl Streets, and is available to whomever needs it during regular business hours.

Signs you might be allergic to your medication Medicine helps billions of people across the globe stay healthy. Medicines are used to treat existing illnesses and reduce a patient’s risk of developing others. As effective as medicines can be, no two humans are the same, so medications that may help one person could prove harmful to another.

The American Academy of Allergy, Asthma & Immunology notes that all medications can cause side effects, but only 5 to 10 percent of adverse reactions are due to allergies. Allergic reactions begin in the immune system, which controls how the human body defends itself. If a person is allergic to a certain type of medication, his or her immune system identifies the drug as an invader or allergen and it can respond in various ways. The AAAAI notes that the most common im-

mune response to medication is a result of the expansion of T cells. These cells identify the drug as foreign and create a delayed immune response that most often affects the skin.

While allergic reactions to medications may not be very common, the AAAAI says that such reactions can be lifethreatening. That highlights the importance of learning the signs of allergic reactions.

Symptoms of drug allergies The American College of Allergy, Asthma & Immunology advises anyone who experiences these symptoms while taking medication to speak with their physicians, including an allergist, immediately: • Skin rash or hives • Itching • Wheezing or other breathing problems • Swelling

• Anaphylaxis Anaphylaxis is a rare but severe allergic reaction that can occur suddenly and quickly worsen. Symptoms of allergic reactions to medication are not often life-threatening,

PMC working to meet community needs Providence Medical Center has always been there for Wayne and the surrounding communities. As a way to continue that commitment, PMC joined the Northeast Nebraska Health Department, Pender Community Hospital and the Winnebago Tribal Health Department to perform an area health assessment with the goal of identifying health needs in the service area with a goal to improve the health of those living in the area. After surveys and townhall-style meetings, behavioral health was identified as one of the key concerns in Northeast Nebraska.

Providing Access to appropriate, quality mental and behavioral health services is one way PMC is prepared to serve the healthcare needs of the community. In response to the community health needs assessment, PMC announced a partnership with Senior Life Solutions in January.

Senior Life Solutions is an

intensive outpatient group therapy program designed to meet the unique needs of older adults typically ages 65 and over struggling with symptoms of depression and anxiety, stress from age-related health concerns, or difficult life transitions such as the loss of a loved one. Individuals may benefit from the Senior Life Solutions program if they are experiencing any of the following common indicators or triggers of depression and anxiety: •Recently experienced a traumatic event •Lost a spouse or close family member •Loss of interest in previously enjoyed activities •Changes in appetite •Difficulty sleeping •Loss of energy •Feelings of sadness or grief lasting more than two weeks •Feelings of worthlessness or hopelessness.

(continued from page 10) people who have already administered epinephrine, the drug used to treat severe allergic reactions, should visit an emergency room immediately at the first sign of anaphylaxis, which may include trouble breathing, tightness of the throat, hoarse voice, nausea, vomiting, abdominal pain, diarrhea, and dizziness. Rapid heartbeat, low blood pressure, a feeling of doom, cardiac arrest, and fainting are some additional signs of anaphylaxis.

According to the AAAAI, most anaphylactic reactions occur within one hour of taking a medication or receiving an injection of the medication. However, such reactions also can take place hours after a medication has been taken.

Antibiotics are the most common cause of anaphylaxis, but the AAAAI notes that recent research has shown that chemotherapy drugs and monoclonal antibodies also can induce anaphylaxis.

People take medicine to feel better, but sometimes their bodies and certain medications are not a match. As a result, people must be vigilant when taking medications for the first time, paying particular attention to how their bodies react. Mohlfeld is a Licensed Clinical Social Worker with over 20 years of experience. She specializes in helping people manage their stress, increase their coping skills and process their experiences. Mohlfeld said seeking Behavior Health support can be daunting and those who need it can be at a variety of stages in processing their needs.

“We’re here for those people who are at a stage where they’re ready to work on an issue,” Mohlfeld said.

Brookdale Wayne becomes Kinship Pointe After a change in facility management, Brookdale Wayne became Kinship Pointe effective June 1.

Still struggling with hearing aids?

How to maximize their benefits

A good hearing aid fitting can improve quality of life by keeping us connected with loved ones through better communication. Also, well-fitting hearing aids can increase a person’s safety with the ability to monitor environmental sounds. Hearing aids have come a long way since the days of wheeling up a volume dial on the aid until it squeals with feedback and then dialing it back. Hearing aids now have computer chips that can sample the sounds in the environment and make decisions about which sounds to de-

crease and which to increase in order to offer the wearer the best chance to understand speech while keeping sound in a comfortable range. Many hearing aids today can be paired with smart phones. The wearer can make volume and setting adjustments with the phone and stream calls, music, and audiobooks or podcasts directly to the hearing aids.

With this amazing technology, why do people still struggle?

First, the process used to diagnose and fit the hearing aids should be evaluated. The American Academy of Audiology has developed a comprehensive list of evidence-based best practices. A patient will have the best chance of a positive result if their audiologist

follows these best practices.

Unfortunately, not all do. For example, hearing aids can easily be programmed to the manufacturer’s settings for a particular hearing loss without accounting for the physical fit and needs of the patient.

While this method of fitting is easy and requires little expertise for the audiologist or hearing instrument specialist to perform, generic programming is not optimal for most patients. Incomplete assessments, generic recommendations, and generic fittings may be to blame for poor hearing aid benefit.

Best practices should be patient centered. A patient should expect the following care: • a complete audiological

evaluation including a case history, otoscopy, air and bone conduction testing, word recognition testing, and speech in noise testing • an explanation of the results and appropriate medical referrals if needed • determination of hearing aid candidacy and a hearing needs assessment • appropriate hearing aid recommendations for the patient based on audiological data and patient needs assessment • hearing aids fit using Real Ear verification measures • subjective measurement of hearing aid benefit • instruction on care and

This ensures the devices are specifically programmed for the patient and optimizes the speech sounds needed to improve conversational speech.

With appropriate hearing aids properly programmed, some individuals may find significant benefit while others may still struggle with communication especially in challenging environments with background noise and reverberation. The simple reason for this is that we are sending a signal to the brain through a system that has been damaged. It is the brain’s duty to interpret that imperfect signal. Until we achieve scientific advancements that allow us to physically repair the damage and restore the auditory system to normal function, this will be the case.

For individuals continuing to experience difficulty, hear-

ing aids are only one part of better communication. Communication strategies are integral to hearing aid success.

The listener, the speaker, and the environment are all factors. Controlling background noise when possible, getting the listeners attention before speaking, and speaking at an easy distance to see facial expressions and cues are examples of these strategies. The audiologist will provide more techniques tailored to the patient’s needs.

The current technology in hearing aids is designed to reduce background noise, but not eliminate it. Situations in which the background noise is still an issue or distance from the speaker is too great, hearing aid accessories can be an asset by improving the speakers voice above the noise and overcoming distance. A remote microphone can accomplish just that. It can be worn by the patient’s communication partner, picking up his voice and sending it directly to the listener’s hearing aids.

They work great in a car where there might be road noise and it is difficult to see the speaker’s face. Table microphones work in much the same way but have the benefit of picking up multiple voices at small table groupings. They are very effective for dining out and for conference meetings.

Finding a comfortable television volume can be a struggle between couples. A TV accessory can send the audio signal directly to the listener’s hearing aids and can be adjusted for comfort, while the partner can then adjust the volume on the tv for his or her comfort.

Individuals with hearing loss commonly struggle in places like churches or auditoriums because of sound reverberation. Hearing Loops are an

excellent solution. A Hearing Loop is a band of cooper wire professionally installed around the perimeter of a space and is connected to the sound system. When voices and music are sent through the sound system, the signal is also sent through the Hearing Loop. Many hearing aids are equipped with a telecoil which is a special program to receive the signals from the Loop. When the hearing aid wearer is in the

Loop, he activates the telecoil program on the hearing aids, and the sound will be sent directly to the hearing aids. This is a great technology because it works for multiple brands and technology levels of hearing aids. Facilities equipped with a Hearing Loop post a sign with a blue ear and a ‘T’ to notify the public of the Loop.

Migraines differ from other types of headaches Migraine sufferers are adept at recognizing when migraines begin to form. Many people only find relief after spending several hours lying down in pitch-black rooms.

Calling a migraine a headache is somewhat misleading, as migraines produce an assortment of symptoms and may not feature pain at all.

The Mayo Clinic says sinus headaches, which occur with sinus infections or symptoms like congestion and facial pressure, sometimes are mistaken for migraines. Penn Medicine states that migraines occur in four different phases.

1. Prodome phase: This preheadache phase includes painless symptoms that can occur days or hours before the migraine. Mood swings, stiffness in the neck and food cravings are some of these symptoms.

2. Aura phase: Auras are sensory disturbances that occur before or during a migraine.

Auras can affect vision, hearing, touch, or speech.

3. Headache phase: If pain appears, sufferers are in the headache phase. Pain may range from mild to debilitating. It’s important to note people can have a migraine with-

out head pain. The American Migraine Foundation notes that silent or acephalgic migraine is a type of migraine that can be very alarming. During these types of migraines, one experiences dizzying aura and other visual disturbances and nausea, but no head pain.

4. Postdromal phase: This is when the migraine pain has subsided. People may feel tired and unwell during this phase.

Migraines generally have triggers. Finding the cause can help sufferers avoid these triggers and tame the severity or frequency of migraines. Post-intensive care syndrome: Why patients may face issues after recovery A cancer diagnosis can be a life-changing moment marked by uncertainty. As patients and their families prepare for treatment, they may not know what to expect, though it’s important to remember that the

condition called post-intensive care syndrome, or PICS. PICS is a series of physical, mental and emotional symptoms that persist after a patient leaves the ICU. Medical professionals have recently begun to delve

The SCCM says 33 percent of patients on ventilators, 50 percent of all patients admitted with severe sepsis and up to 50 percent of patients who stay in the ICU for at least one week will experience PICS. Some with PICS may have memory issues, trouble solving problems, issues speaking, and difficulties working on complex tasks. Anywhere from 30 to 80 percent of those who have been in the ICU may experience these kinds of problems. Health professionals believe that PICS occurs due to the intense, often stressful level of treatment in the ICU. Doctors will use various cognitive impairment screening tests and physical diagnoses to uncover any physical, mental and cognitive issues associated with PICS. Get to know mental health like the back of your hand Monitoring one’s mental health is now more important than ever. With a global pandemic causing numerous disruptions to the normal ebb and flow of life, some are finding coping with the changes difficult. Donna Wolff, president of Northeast Nebraska Suicide Prevention Coalition, said people need to think about their physical and mental health as the two sides of their hand.

“The top of your hand is our physical health. We talk about this all the time, we need to exercise, get enough sleep, drink more water. We teach our kids physical health; shampoo your hair, use deodorant, cleanse yourself,” Wolff said.

“If you put your hand up on the other side you have your palm, that can be your mental health. It’s the very same hand. We need to start talking about mental health just

like we do physical health. We know, it’s proven scientifically, that with most mental health disorders, there’s a chemical imbalance in the brain that (with) talk therapy and medication, it can be lived with. It can be managed.”

Unfortunately, there is still a negative stigma surrounding treatment for mental health issues. Wolff said it’s important to break that down, and the first step to doing so is through education, preferably, the earlier the better.

“We need to normalize this and let (children) know, it’s okay if you’re not okay and it’s okay to talk about it with anybody and everybody,” Wolff said. “If we need to take diabetes pills or insulin, if we need to take heart medication or we have COPD, no big deal to go to the doctor and get medication for that. So if a kid

needs an antidepressant, no big deal.”

Especially during this time of social distancing, working from home and the various other impacts COVID-19 and social unrest, it’s important to check in with loved ones and friends as well.

“As human beings, we need that human contact,” Wolff said.

If changes in someone’s sleep patterns occur, whether that be sleeping too much, or too little, it can be a sign of mental distress. Lack or self care and lack of desire for social interaction can also be something to look out. Wolff said that if someone notices a friend or loved one has a dramatic shift in mood, even if that person has quickly gone from being down in the dumps to overly happy, they may need help.

“If they’ve been in a funk for so long and all of a sudden everything’s better and they’re so happy, that can truly be a sign to watch out for as well because they’re getting enough energy to follow through with a plan (to selfharm),” Wolff said.

More warning signs to look out for include someone showing apathy for things they used to care a lot about, not eating and a lack of self care.

If these behaviors are noticed or if someone is feeling suicidal, Wolff suggests calling the 1-800-273-8255.

New equipment helps Providence Therapy patients take ‘steps’ to recovery Providence Medical Center Therapy Department announces the addition of an innovative piece of therapy equipment. In mid-October, the Solo Step overhead track and harness system was installed, helping patients go through therapy more independently and reducing fall risk for patients with stability issues.

According to the company’s website, “Solo Step is designed to protect patients from falling during the rehabilitation process while giving them the confidence they need to break through their barriers in a safe environment.” The Solo Step consists of a fitted harness, which attaches to a 30 foot ceiling

track that allows them to be hands free. The harness provides weightbearing support for those that need more assist with standing.

“The Solo System assists in supporting the patient. We are able to try more challenging balance exercises sooner with having the harness system to assist. Patients can be less afraid of falling and be walking sooner,” Michele Albers, physical therapist said.

From a patient’s perspective, the sense of independence is a huge benefit.

“It makes you feel like you can do it yourself,” Harlan Thompson, a Providence Physical Therapy patient said. “Even though there is assistance beside you (the therapist), you

have some freedom.”

Our ancestors may have held the secret to better brain health

By Karen Longe, MPT, DPT KEL Physical Therapy & Wellness It turns out that while our early ancestors spent their days out in the wilderness hunting and foraging for food, they may also have been keeping their brains sharp. Researchers at the University of Arizona have found a connection between the highly active lifestyle that was prevalent in those ancient days and the prevention and treatment of Alzheimer’s disease and other age-related diseases.

Some two million years ago, our ancestors covered long distances—mixing moderate and intense bouts of aerobic activity throughout the day— as a means of survival. In sharp contrast, today’s average American starts his day by getting ready for work or school, then travels by car or train to an office where he’ll likely spend hours in meetings or in front of a computer, only to retire home to catch up on the latest series on Netflix. For the most active among us, exercise might consist of an hour at the gym three or more days a week and perhaps a few short walks sprinkled in throughout

the day. But contrary to popular belief, that level of activity doesn’t completely erase the damage we do to our bodies during the other 23 hours in the day—which are largely spent sitting in a desk chair or at the dining room table, reclining on the family sofa or sleeping.

An obvious connection can be drawn between today’s sedentary lifestyle and a host of prevalent modern-age health problems like obesity and chronic illness, but how do low levels of activity affect our cognitive abilities? The study’s co-authors hypothesize that the amount of exercise that our ancestors engaged in reduced the burden

of a genotype that leads to a high risk of Alzheimer’s disease, cardiovascular disease and increased mortality. As a result, the researchers stress the importance of exercise as a potential disease prevention and intervention tool.

Beyond reducing the risk of disease, the role that movement and exercise play in a person’s cognitive performance cannot be underestimated. Many research studies have proven that engaging in regular physical activity keeps our brains in top form, helping us to focus, recall and think more clearly. And while it’s not exactly feasible to adopt a hunter/gatherer way of life in today’s world, perhaps the takeaway is that moving— more frequently and for longer periods of time—can help

stave off disease and boost our brain function. Just some food for thought next time you settle in to binge watch Game of Thrones or pass up an opportunity to bike to the park.

Parkinson Voice Project awards 2020 SPEAK OUT! & LOUD Crowd Grant to PMC – Therapy Services Parkinson Voice Project, a 501(c)(3) nonprofit organization, has named Providence Medical Center as a recipient of its 2020 SPEAK OUT! & LOUD Crowd Grant Program.

Grant recipients include hospitals, university speech therapy clinics, private practices and nonprofit organizations. Each clinic receives free training for their speechlanguage pathologists and graduate students, as well as speech therapy supplies.

Some grant recipients will also receive funding. Providence Medical Center is committed to offering Parkinson Voice Project’s effective speech therapy program at the Providence Therapy locations.

“Up to 90 percent of people with Parkinson’s are at high risk of losing their ability to speak, and aspiration pneumonia caused by swallowing issues accounts for 70 percent of the mortality rate in this patient population. Parkinson Voice Project’s vision is to replicate its speech therapy program across the globe to help individuals with Parkinson’s preserve their speech and swallowing abilities. More than 2800 speechlanguage pathologists have been trained to date, including clinicians in Australia, Canada, Finland, Israel, Italy, Greece, Japan, Korea, New Zealand, Taiwan, and the United Kingdom. PMC adds telehealth capabilities Providence Medical Center’s highly trained medical staff and state of the art equipment are always readily available for patients who enter the Emergency Department. Providence Medical Center offers large trauma rooms equipped with the right supplies and expertise at the right time. This last year, services were also expanded to include Avera eCare Emergency telehealth which allows for a Board-Certified Emergency Medicine Physician to be in immediate contact with the PMC staff and provide support throughout the care of critical patients who are experiencing unique needs.

a patient is having a myocardial infarction (heart attack).

The eCARE physician can also prescribe medications for emergent situations, such as pain medication for traumas, blood thinners for heart attacks and other critical care medications for things such as strokes. If needed, they can assist in setting up bipap and ventilator (respiratory support) machine settings. Pharmacists, behavior health specialists and transfer coordination assistance is also available through eCare Emergency services and has been utilized by the medical staff at PMC. This additional support from the eCare Emergency services ultimately provides that extra back up to medical staff and allows them to focus and address the patient’s urgent needs.

Prediabetes can be a serious precursor to Type 2 diabetes, but can be managed According to the American Diabetes Association, over 7 million Americans have undiagnosed diabetes. Before most diabetics contract Type 2 diabetes, they experience prediabetes.

Prediabetes is caused when the body cannot properly process glucose anymore.

Because of this, glucose, or sugar, builds up in the blood instead of fueling energy in the cells of muscles and tissue.

Moving sugar throughout the bloodstream to the cells creates the hormone insulin. Insulin comes from the pancreas and is released when a person eats. As insulin circulates, energy is delivered to cells. Prediabetes disrupts that process, and the pancreas is no longer

able to make enough insulin, or cells become resistant to insulin.

A prediabetes diagnosis means a person has elevated blood sugar, but not elevated enough to be in the Type 2 range. There usually aren’t distinct symptoms for prediabetes, but knowing family history and monitoring weight and fat build up around the abdomen is important. Those with a family history of Type 2 diabetes is one risk factor, according to the Mayo Clinic, other risk factors include: Weight - Being overweight is the primary risk factor for prediabetes according to the Mayo Clinic. The more fat tissue there is between muscles and skin around the abdomen,

the more resistant cells become to insulin.

Waist Size - Men with waists larger than 40 inches and women with waists larger than 35 inches have an increased risk for insulin resistant cells.

Diet - Diets heavy in red meat, processed foods and sugary beverages can lead to a greater chance of prediabetes.

Inactivity - Those who are less active stand a greater chance of developing prediabetes.

Age -P rediabetes can develop at any age, but those 45 years old or older have a greater risk of development.

Race - Certain groups, including Black Americans, Hispanics, Asian Americans, Pacific Islanders and Native Americans

have a greater likelihood of developing prediabetes.

Gestational Diabetes - Women who developed diabetes while pregnant (gestational diabetes) and their children are more likely to prediabetes.

Polycystic Ovary Syndrome (PCOS) - Women who suffer PCOS, a condition that causes irregulat menstrual cycles, excess hair growth and obesity, have an increased risk for developing prediabetes.

Sleep Apnea - It’s estimated that 22 million Americans suffer from obstructive sleep apnea. Sleep apnea can also contribute to a higher risk for prediabetes.

Northeast Nebraska Health Department, partners join forces to create Northeast Nebraska Rural Health Network In 2018, Northeast Nebraska Health Department (NNPHD), Pender Community Hospital, Providence Medical Center and Winnebago Tribal Health Department started collaborating to answer an important question, according to Julie Rother, Health Director at NNPHD. “(We) came together and talked about how do we work together on community assessment and working together on developing a plan that will address some of these health issues for the public in the area without duplicating efforts and, by working together to support each other,” Rother said.

After coming together for a healthier northeast Nebraska, the entities applied for a federal grant to carry out a community needs assessment and to develop a community health improvement plan. As

a result of those assessments and thanks to the grant, those partners were able to officially establish the Rural Health Network, and quickly, the Rural Health Network got to work.

Through those discussions, a community needs assessment was carried out and two areas of concern were picked to bring citizens and health providers to work on - behavioral health and obesity.

Throughout 2019 and into 2020, real momentum was beginning to build up in both groups. In November 2019, the Rural Health Network applied for a received a three-year $300,000 grant from the U.S. Health Resources and Services Administration (HERSA). With the grant, the group gained a new partner and a new project.

Prediabetes can also lead to high blood pressure, low levels of HDL (or “good” cholesterol) and high levels of triglycerides, according to the Mayo Clinic.

Though the thought of developing insulin resistance can be scary, those at risk have the power to lessen the effects if not delay the disorder all together. Through simple lifestyle changes, if caught early enough, some people may be able to return their blood sugar levels to normal range before prediabetes becomes serious. According to the American Diabetes Association, weight loss of 5-7 percent and 150 minutes of moderate exercise can go a long way to help blood sugar levels. It also helps create better blood glucose levels. Speaking to a doctor about the right type of diet to counteract prediabetes will also be beneficial.

The American Diabetes Association provides a quick 60-second test to assess prediabetic risk. While signs and symptoms may not make themselves known, anyone with risk factors should monitor blood sugar levels do that prediabetes does not lead to Type 2 Diabetes.

They had worked on a research project in northeast Nebraska, not just in our health districts, but we worked closely with them on the project and we had community health workers stationed here that did quite a bit of the legwork with participants, but their focus was on men in rural areas of Nebraska to use this technology to manage their weight,” Rother said. “Their focus was really weight reduction and then hopefully stability from when they reach their goal.

They had a lot of success with that.”

The Rural Health Network hopes to include women in their re-creation of the project as well as bringing the Pender Clinic and Wayne Family Medicine into the group as well. The HERSA grant will also allow Rural Health Network to hire a director. Working together as a cohesive unit could have long lasting positive health outcomes for northeast Nebraska.

“Especially in rural areas, we’re really limited on the number of resources that we have whether that be human resources, financial resources, whatever it might be,” Rother said. “We know that if we are working together and complementing or supporting what each other is doing, then we can actually be more efficient and more effective and have better outcomes than we can if we’re trying to do something alone.”

Work plans for both the behavioral health and obesity

coalitions were underway and projects being planned when COVID-19 hit and Rural Health Network partners were forced to focus on pandemic response. Though the coalition work had to slow down, collaboration and open communication from regional and local health partners has not. Rother and NNPHD have held a series of regular meetings with a number of partners whether it be local governments, school districts or other community leaders, to help pass on information.

“What I’ve seen on those calls is that there’s a willingness to share ideas, there’s a willingness to share questions, and just be very open with each other and say ‘we don’t know how to do this’ or ‘does anybody have any suggestions on how to do that.’ People are willing to share,” she said. “I think it’s that network of people that are trying to do the right thing and figure it out together and maximize our resources. Of course we’ve had a lot of bumps in the road and new turns and everything along the way because it’s just all so new, but I feel like people have been very flexible and very adaptable.”

NNPHD has been busy working through the pandemic to track and chart coronavirus in the Cedar, Dixon, Thurston and Wayne county areas. Rother said they’ve also recently developed a risk dial that shows people if coronavirus exposure is low, moderate, high or severe in the four-county district. Sleep apnea more than just excessive snoring Excessive snoring may be more than an inconvenience for those who share a room, it may actually be a warning sign of a deadly sleep disorder, obstructive sleep apnea.

According to the Sleep Foundation, “sleep apnea is a disorder in which breathing is briefly and repeatedly interrupted during sleep.”

Apnea refers to a breathing pause that lasts at least 10 seconds. Sleep apnea is caused by the muscles at the back of the throat’s inability to keep the airway open. While obstructive sleep apnea causes disruptive sleep, it can be more serious than that. For the over 18 million Americans diagnosed with sleep apnea, and the many more who go undiagnosed, obstructive sleep apnea can also lead to hypertension, heart disease and even mood problems.

According to the Sleep Foundation, a number of factors can indicate an increased risk for obstructive sleep apnea (OSA). Those include having a small upper airway, large tongue, tonsils, or uvula. Being overweight, having an overbite, recessed chin or large neck, especially if over the age 40 can also be risk factors.

Smoking and drinking increases the chance of developing OSA as well. There is some indication that OSA may also be genetic.

Sleep apnea has several symptoms that should be monitored by health care professions. For people who experience chronic snoring, sleep deprivation, irritability, difficulty concentrating, depression, memory difficulty or chronic tiredness, a sleep test may be ordered by a doctor.

A CPAP is a machine that utilizes a mask or nasal cover that blows air into the users airway, keeping it open all night.

Other treatments may include surgery to remove airway blockages, dental work to reposition the lower jaw and/ or tongue, and more. A doctor will help determine the best form of treatment for OSA. Lifestyle choices like losing weight and giving up smoking will also have a positive outcome for those seeking to alleviate the sleep disorder.

Diet checkup tool, counseling can help heart health By: American Heart Association News A quick, simple questionnaire in the doctor’s office can help fight one of the leading causes of cardiovascular disease – a poor diet, a new report says.

What and how much you eat can affect other heart risk factors, such as high cholesterol, high blood pressure, diabetes and being overweight. About 11 million deaths – and about half the heart disease deaths globally – can be blamed on a poor diet, according to the 2017 Global Burden of Disease Study, a look at the health impact of diet in 195 countries.

The new scientific statement from the American Heart Association, published Friday in its journal Circulation: Cardiovascular Quality and Outcomes,

calls for routine health care visits to include some form of dietary assessment and counseling. The diet screening tool could be integrated into electronic health records across all health care settings, the statement said.

“Dietary patterns and quality are not sufficiently prioritized when addressing modifiable risk factors during regular health care office visits,” nutritional epidemiologist Maya Vadiveloo, chair of the statement writing group, said in a news release. “Given the evidence that diet contributes to disease and mortality, it is a risk factor worth screening for continuously.”

Even though thyroid cancer may not be the most common or aggressive cancer, it does have a 30 percent recurrence rate.

Papillary carcinoma is the most common form of thyroid cancer, accounting for approximately 80 percent of all cases, says Cancer Treatment Centers of America. These carcinomas are slowgrowing and can develop in one or both lobes of the thyroid.

While it may spread to nearby lymph nodes in the neck, papillary carcinoma is generally treatable and patients typically are given a good outlook for survival.

Follicular carcinoma is the second most common type of thyroid cancer. Even though it does not typically spread to lymph nodes, follicular carcinoma is more likely to spread to other organs, like the lungs or bones. This type of cancer is found more frequently in countries with inadequate dietary intake of iodine.

Hürthle cell carcinoma, medullary thyroid carcinoma, and anaplastic carcinoma are other, more rare types of thyroid cancer.

Early diagnosis is the key to successful treatment of thyroid cancer. Should any signs or symptoms present themselves, individuals should speak with their doctors right away. The ACS says lumps in the thyroid are common and are usually benign.

“However, these barriers can be overcome,” said Vadiveloo, an assistant professor of nutrition and health sciences at the University of Rhode Island in Kingston. “We want a valid, reliable way to assess diet that reflects the best science, and most of the tools assessed take under 10 minutes to use.”

Depression may complicate recovery from peripheral artery disease By: American Heart Association News People with a vascular condition that commonly affects arteries in the legs could have a tougher road to recovery if they also have depression, according to a new study.

The research, focusing on 1,243 people in the U.S., the Netherlands and Australia treated for peripheral artery disease, or PAD, showed women were more likely to be depressed than men. The research was published Wednesday in the Journal of the American Heart Association.

PAD causes a narrowing of the arteries serving the legs, stomach, arms and head. It is associated with a high risk of death and illness from cardiovascular diseases and may cause excruciating pain while walking, impacting mobility

and quality of life.

“A major goal of PAD treatment is improving patients’ health status and quality of life,” the study’s lead author Dr. Qurat-Ul-Ain Jelani said in a news release. She is an interventional cardiologist in training at Yale University School of Medicine in New Haven, Connecticut. “Not recognizing or treating depressive symptoms may stand in the way of realizing optimal recovery. … A multidisciplinary approach that includes mental health supports for patients with PAD would be beneficial.”

Researchers assessed depression symptoms at the beginning of the study using an eight-item survey that included questions about their energy, appetite, sleep pattern and interest or pleasure in participating in activities.

Their medical conditions were

assessed via phone interviews four times over a year.

The results showed: – At the start of the study, 21% of women had symptoms of depression compared to 13% of men.

– Men and women with depression symptoms had worse health status at each time point, compared to patients who did not report such symptoms.

“This is the first study to document how depressive symptoms may complicate PAD recovery even among patients receiving specialty care,” senior study author Kim G. Smolderen said in a news release. She is a clinical health psychologist and co-director of the Vascular Medicine Out-

Because the study focused on people treated at vascular specialty clinics, the findings may not apply to the general population of PAD patients who may not have access to specialty care. In addition, the study excluded other mental health issues, such as anxiety and stress.

“We need awareness for the patient as a whole in order to provide patient-centric care.”

Seven myths about sunglasses that could damage your vision By: American Academy of Ophthalmology Did you know that too much sun on unprotected eyes increases the risk of eye disease?

The good news is that prevention is simple: Wear sunglasses that block 99 to 100 percent of UVA and UVB radiation.

But there are a lot of common myths about sunglasses and eye health. That’s why the American Academy of Ophthalmology is working to debunk the myths and offer tips to promote smart eye health choices.

Myth #1: Sunglasses are only for sunny days False. UV light gets through clouds and haze. Regardless of the season, sun exposure can increase the risk of developing cataracts, growths on the eye and cancer. Sun reflecting off ice and snow can also lead

to photokeratitis, a painful eye condition sometimes referred to as snow blindness.

Myth #2: Babies and children don’t need to wear sunglasses False. Children are just as susceptible to the sun’s harmful rays as adults. Start them on healthy habits early.

Myth #3: Cheap sunglasses don’t offer protection False. Sunglasses don’t have to cost a lot of money to provide adequate eye protection.

Less expensive pairs marked as 100 percent UV-blocking can be just as effective as more expensive options. Labels can sometimes be confusing. Some indicate sunglasses offer 100 percent protection from UVA/UVB radiation, others offer 100 percent UV 400 protection. Both block 100 percent of the sun’s harmful radiation.

Myth #5: Polarized lenses offer more protection from the sun False. Polarized lenses do not block more radiation; however, they can cut down on glare reflecting off cars, water or pavement, which can make activities such as driving or boating more enjoyable.

Myth #6: Darker lenses offer more protection from the sun False. The darkness of the lens isn’t indicative of protection. Make sure the label on

the lens says 100% UV-blocking.

Myth #7: Tinted lenses offer more protection from the sun False. Amber, green and grey lenses do not block more of the sun’s harmful rays.

However, tinted lenses do offer increased contrast for those who play sports, making a softball or a golf ball easier to see.

“Your eyes need protection from the sun’s damaging ultraviolet rays just like your skin,” said Dianna Seldomridge, MD, clinical spokesperson for the American Academy of Ophthalmology. “Sunglasses are your prescription for eye health. They are effective, inexpensive, and provide protection year-round.”
