CUSTOMER RESPONSIBILITY CHECKLIST BEFORE INSTALLATION

COLLECTIVE

RESPONSIBILITY

Ensure the job site and structure are OSHA compliant. For more information regarding OSHA compliancy, please refer to the Formetco safety letter.

Communicate any changes to the schedule with to Formetco’s Project Manager.

All field work performed should be done as per stamped drawings.

CUSTOMER

RESPONSIBILITY

Report any site access issues to the installer and Formetco Project Manager (if needed Formetco can provide a site visit for an additional cost).

Confirm and receive proper insurance from installer/electrician, naming your company and Formetco (must meet our requirements) as additionally insured.

Ensure content is scheduled to player prior to the time of turn on.

Document and report any damages to the display to the onsite Formetco Technician and Project Manager before install crew leaves to avoid additional expenses on a return trip.

Inform Formetco Project Manager when power to structure will be established.

Allow 3-4 days to dispatch a tech back to the site for any diagnostic errors after turning on (If power is not available at time of install).

Any subcontracts working on display will need to be added into Formetco service system.

INSTALLER RESPONSIBILITY CHECKLIST DURING INSTALLATION

Receive, unload, and prep digital display and equipment for install, must unload using crane and Formetco supplied pick points.

Brace display crates with 2”x4” legs while prepping sections for install.

Do not let faces of display touch while lifting from crate.

Provide and install 4"x4"x3/8" continuous ledger at bottom of digital face.

Confirm ledger is level across structure to avoid alignment issues.

Install digital display with all upright clamps and Unistrut splice plates.

Align and bolt sections together with Formetco supplied 3/8x16 stitch bolts. Vertical and horizontal seams must be bolted.

Mount control box.

Install camera arm, mounting bracket, and position properly.

Install trim (if applicable).

Install spare parts box, securing to catwalk with Formetco supplied rachet strap.

Install imprint (if applicable).

Remove any trash, excess hardware, and crates from structure and surrounding job site.

ELECRICIAN RESPONSIBILITY CHECKLIST DURING INSTALLATION

Adhere to National Electric Code.

Mount meter and main breaker panel at base of pole.

Main breakers installed per riser drawing.

Install conduit from main panel at base to the Formetco contactor box.

Install designated 120V leg to power control box.

Install conduit and 240V circuit from contactor box to Formetco breaker panels.

Install conduit and 120V circuit from contactor box to control box.

Cut to length and wire preinstalled power harnesses from display to Formetco supplied breaker panels.

Wire imprint to contactor box (if applicable).

All conduit needs to be free from obstructing horizontal safety lines.

Confirm and sign off on as built drawings provided from Formetco PM. Clean up all electrical supplies and scrap.

Communicate date of electrical inspection or turn on.

TECHNICIAN RESPONSIBILITY CHECKLIST DURING INSTALLATION

Inspect structure to ensure it is ready for digital installation.

Insure all power and data interconnections within display are made.

Confirm antenna, photocell, and camera were installed properly.

Confirm A & B chain data cable from control box to display were installed properly.

If back-to-back display, confirm shared comms data cable is installed properly.

Assist installer on all lifting, bolting and alignment of sections.

Technical assistance to electrician’s installation work.

Complete all Task in Formetco Field App. Power up and test display (once power is installed).

Maintenance training (if applicable).

Report any issues with install to Project Manager.

FORMETCO’S AUTOMATED WIRELESS AGREEMENT (FAWA) Your Formetco digital display includes a 1-year automated wireless service agreement paid by Formetco unless otherwise indicated in your sales agreement.

YEAR 1 - PAID BY FORMETCO Formetco pays for the wireless service for the first year from the date of turn on.

BEFORE YEAR 1 ENDS You will receive a renewal notice from Formetco 90 days prior to your anniversary date.

CUSTOMER RESPONSIBILITY After the first year the customer is responsible for all wireless service charge.

CONTINUOUS CONNECTIVITY IThis service is essential for remote monitoring diagnostics and content communication
