Ebola: Are you Prepared?

At the beginning of October, I sat through my fire departments Bloodbourne Pathogens Refresher Class. The yearly class, if taught correctly, is designed to make you think about the procedures that are in place to keep you safe from disease when you respond on your ambulance. Oh, you don't respond on the ambulance? How about the motor vehicle accident that you have to extricate a seriously injured occupant? Keep in mind that at any moment, you can find yourself in the middle of chaos.

Has your department addressed the protocol that will be used for the latest disease known as Ebola? I am sure the city of Frisco, Texas, didn't expect to find themselves in the middle of a firestorm of media coverage that questioned every aspect of how the call was handled. The call resulted in a deputy being hospitalized as a precaution, the CareNow facility was closed and its employees were checked for symptoms of the disease. Hopefully your department has reviewed procedures to handle unknown diseases and has reminded everyone of your protocols.

Is the glass half empty or half full? I have listened to numerous press conferences on Ebola. Truthfully, I will make sure I follow what I was taught in all of my classes. Follow procedures like not raising your hands above your shoulders to avoid touching your face with a gloved hand. Take time to properly decon the ambulance and yourself before you go back into service.

The Center for Disease Control and Prevention has a detailed Emergency Medical Services (EMS) Checklist for Ebola Preparedness. Download the checklist at www.cdc.gov/vhf/ebola/pdf/ems-checklist-ebola-preparedness.pdf. The CDC website also has a comprehensive article on the Interim Guidance for Emergency Medical Services (EMS) Systems and 9-1-1 Public Safety Answering Points (PSAPs) for Management of Patients with Known or Suspected Ebola Virus Disease in the United States.

In the end, our health system is well aware of this disease. It is up to you to follow protocols that are designed to keep you safe. According to the Center for Disease Control, EMS personnel who develop sudden onset of fever, intense weakness or muscle pains, vomiting, diarrhea, or any signs of hemorrhage after an unprotected exposure (i.e., not wearing recommended PPE at the time of patient contact or through direct contact to blood or body fluids) to a patient with suspected or confirmed Ebola should: Not report to work or immediately stop working and isolate themselves; Notify your supervisor, who should notify local and state health departments; Contact occupational health/supervisor for assessment and access to post-exposure management services; and Comply with work exclusions until you are deemed no longer infectious to others.

I encourage you to download the CDC articles and checklist. Read it, discuss it and follow it!

BILL BEING PRESENTED TO NJ ASSEMBLY AND SENATE I began work on this bill almost three years ago after the passing of Patrolman Joseph Wargo of the Mt. Arlington Police Department. Officer Wargo was killed in a Line of Duty Death on Route 80 in October of 2011.

The bill would protect any roadside memorial erected at the site of a LODD of any police officer, firefighter or EMS personnel.

Currently only the state of Oregon has such a law.

Any memorial existing now on the site of a LODD is not protected from being removed. This would be of no cost to taxpayers, and any memorial put up would be paid for by individuals, such as family, co-workers or the department to which the deceased belonged, with the approval of the state DOT. I am asking if you could please get word out to support this bill. It now goes before the State Assembly and the Senate, then on to the Governor.

MORGANVILLE FIRE CO. ROAST BEEF FUNDRAISER The Morganville Volunteer Fire Company will hold a roast beef fundraiser on October 25, 2014, at its house at 78 Tennent Road, Morganville (by the Little League fields). Doors open at 1700, serving begins at 1800.

Tickets will be drawn at 1930 and the dinner will end 2000. Mays Landing Dryer Fire The volunteer fire companies of Mays Landing, Cologne and Laureldale responded to Princeton Place on August 30, 2014, for a report of a structure fire.

Mays Landing Chief John Saurwald arrived and established command. Upon investigating, the fire was confined to the home’s clothes dryer and its contents.

Firefighters removed the dryer and ventilated the structure.

Why Do They Put Fences Around Cemeteries …?

Engine 5, Rescue 1 and DC 3 responded to Hackensack Avenue and De Voe Place for a two-car MVA with a person pinned in after it crashed through the fence of St. Joseph Cemetery. The person was removed by Rescue 1 and turned over to Hackensack UMC BLS and ALS and transported to Hackensack UMC. Several tombstones and the fence were damaged.

Hackensack police are conducting the investigation.

Church Fire in Hackensack On September 28, 2014, members of the 4th Platoon operated at a second-alarm fire in a church on Hamilton Place. First arriving companies were met with a smoke condition coming from the front.

Engines 5, 2 and 4 quickly stretched multiple 2-½ inch handlines to begin to contain the fire on the exterior and interior.

Ladder 1 and Rescue 1 reported smoke on the interior and fire extending into the structure via the floor joists. Companies worked quickly to open up and extinguish fire in the floor joists and conducted overhaul. A large salvage operation was conducted by all companies to protect items in the church. After an investigation by Hackensack Fire Prevention, it was determined that a heat gun being used on the exterior by workers to remove paint caused the fire. Teaneck operated as the FAST unit and other mid-Bergen units provided city coverage.

AC Apartments Burn

A stubborn three-alarm fire displaced 17 people and destroyed four buildings on Atlantic Avenue in Atlantic City on September 30, 2014.

Residents of the apartment building started smelling smoke and started evacuating the building. The fire eventually destroyed the buildings at 1112-1118 Atlantic Avenue before being brought under control, with

firefighters still on location at noon October 1 . Extensive mutual aid was brought to the scene from fire departments and EMS squads from throughout the island and the mainland. No injuries were reported, and the fire is under investigation.

2-Alarmer in Piscataway

Early on August 26, 2014, the ArborHose Co.#1 was dispatched for a general fire alarm activation on West 7th Street in Piscataway at a large grocery store and butcher shop. Upon arrival Arbor Engine 642’s crew found heavy fire conditions throughout the large commercial structure. A five-inch supply line was dropped to establish a water supply and firefighters pulled a 21/2 inch and two 1-3/4 inch handlines. A second alarm was struck bringing in additional Piscataway companies.

Gas + Sparks = Boom

The City of Somers Point Volunteer Fire Department responded to the Waterfront Marina located at Bay Avenue at the Route 52 Causeway for a report of a boat fire. Two civilians were fueling their boat when one hooked up the boat’s battery and was blown overboard. He was airlifted to the Chester-Crozier Burn Center in Pennsylvania. Firefighters used foam to control the fire.

Fairview Battles Pallet Blaze

On the evening of September 7, 2014, the Fairview Fire Department was dispatched for a report of a warehouse fire on the west end of Fairview Avenue. On arrival it was determined that the fire involved numerous pallets in a pallet manufacturer’s yard, stacked over 20 feet high and covering an area approximately 150 x 150 feet.

Some tractor trailers and one building were also threatened. A sec-

ond alarm was requested as firefighters went to work with three ladder pipes, several deck guns and numerous handlines. The fire was contained after about 30 minutes and declared under control in about 90 minutes. No injuries were reported and the cause was under investigation.

Readington Home Fully Involved

Recently, a home located on Ryland Road in the East Whitehouse section of Readington Township was destroyed by fire. North Branch Fire Company, working as a mutual aid partner with East Whitehouse Fire Company, was dispatched along with Whitehouse Fire Company. Firefighters were greeted with a well involved dwelling with large flames coming from the front of the home. As the firefighters started battling the blaze, more fire companies from the County’s Tanker Task Force brought water to the scene. After approximately three hours the fire was eventually fully controlled and contained with foam. There was one minor injury to a Firefighter who was treated at the scene.

Hotel Fire in Longport

Volunteer fire departments from the City of Longport, the City of Somers Point, Scullville and Bargaintown responded to the Bay Island Inn on Longport Boulevard early on August 31, 2014, for a report of a structure fire. The Somers Point Fire Department arrived first and quickly knocked down a fire in one room, but the fire had extended into the roof. Somers Point Engine 47 laid an

LDH to Longport Boulevard and fed their Squad 43 and 2 Truck.

Firefighters then laddered the roof and began opening up, holding the extension of the fire to the area of origin. One occupant was transported by EMS to the local hospital for burns to the leg, and a total of eight people were displaced.

Medford Lakes Works School Bus MVA Posing after a job well done are members of the Medford Lakes Volunteer Fire Company of Burlington County who, while attending Union Fire Company’s 200th Anniversary on September 20, 2014, left the festivities to assist in a motor vehicle crash involving the bus of the local high school band in which 16 students were injured, six with neck and back injuries. Numerous EMS and fire departments responded, and all worked in treating the injured and getting them transported to area medical centers.

The Union Volunteer Fire Company of Medford in Burlington County celebrated their 200th Anniversary on September 20, 2014, with a muster of antique and modern firefighting apparatus with an inspection by the New Jersey State Parade Judges Association.

Apparatus from throughout New Jersey, Delaware and Pennsylvania turned out for a well-attended and well-organized event. Congratulations on 200 years of dedicated service to your neighbors and community.

3 Injured in Blackwood MVA On August 2, 2014, the Blackwood Fire Company, along with the Deptford Fire Department, responded to Route 42 at the 10.5 mile marker for a reported motor vehicle accident. Gloucester Township BLS 806 arrived and reported one vehicle into the woods.

New Jersey State Police on scene reported two people trapped in an overturned vehicle with one ejected. Upon arrival Assistant Chief Maschke took over operations. Crews worked over 30 minutes to extricate both victims from the car via removal of the passenger side doors and multiple trees.

MVA in Bargaintown The Bargaintown Volunteer Fire Company and the Egg Harbor Township Ambulance Squad were dispatched to Mill Road in the vicinity of the Bargaintown Volunteer Fire Company's Station #1 for a motor vehicle fire over the Labor Day weekend. Units arrived and found the fire was the result of a crash after a car went through a roadside farm market and caught fire. EMS transported the victims from two vehicles to local hospitals, while firefighters extinguished the car fire and stood by while utility crews disconnected the market’s gas and electric. The crash was under investigation by the Egg Harbor Township Police Department.

East Windsor MVA On August 30, 2014, East Windsor Police responded to a reported two-vehicle motor vehicle accident on the Route 33 at the intersection of Lake Drive. One driver became entrapped and required extrication from her vehicle by responding emergency medical services: East Windsor companies 1 and 2, Hightstown and East Windsor Rescue 2. One driver was transported to Capital Health-Fuld Campus via ambulance for treatment of her injuries. The operator of the other vehicle sustained non-life threatening injuries and declined medical treatment at the scene.

Somers Point MVA with Ejections The City of Somers Point Volunteer Fire Department responded to Shore Road at Groveland Avenue on September 20, 2014, for a report of a motor vehicle crash with entrapment. Firefighters arrived to find one vehicle on its side with numerous ejections with pediatric traumas. Two Medevacs were requested along with four ambulances to transport the injured. One was flown to Cooper Trauma in Camden. The cause of the crash is under investigation by the Somers Point Police Department.

MVA with Fire in Cardiff The Cardiff Volunteer Fire Company responded to the Black Horse Pike in front of the Shore Mall early on August 31, 2014, for a report of a motor vehicle crash with fire. Assistant Chief Steven Sear arrived and established command to find a single vehicle off the road into a tree and well involved. Cardiff Engine 1516 arrived and pulled two lines and extinguished the fire. The lone occupant of the vehicle was out of the vehicle upon arrival and was examined by the Egg Harbor Township Ambulance Squad. The cause of the crash was under investigation by the EHT Police Department.

Car Fire in Hackensack On September 9, 2014, Hackensack Engine 4 and Rescue 1 operated at a car fire involving a Bergen County Sheriff’s detective’s car. The officer stated she went over the railroad tracks on Union Street heard a noise, then noticed smoke coming from the trunk and called for assistance. Companies quickly knocked down the main body of fire in the trunk area and conducted overhaul.

New Apparatus for Carlstadt On September 20, 2014, the Carlstadt Fire Department held a dedication ceremony for their new ladder and engine. During the ceremony both apparatus received a blessing from the Fire Department Chaplain Donald Pitches. The following day, a wetdown was held to allow neighboring towns to welcome the two new apparatus into operation. Carlstadt Ladder 1 is a 2014 Pierce Velocity 100-foot Ladder and Engine 2 is a 2014 Pierce Velocity 2000 GPM - 500 GWT Pumper.

The Firehouse Kitchen As I arrived at the Dix Hills Fire Department, I was greeted by Chief Bill Steo and Firefighter Chloe Dubin. “There are not too many female firefighters,” I said to Chloe, “and you are the first one to be cooking on Firehouse Kitchen.” With that said, Chief Steo said, “Not too many female firefighters? We have six here at Dix Hills.” In Bayport, there is only one female firefighter and she has just joined. In the New York City fire department, out of 12,000 firemen, maybe 50 are women. Therefore, this ratio at the Dix Hills was phenomenal.

Onto the cooking! Firefighter Dubin has been cooking for Dix Hills since she joined. This day, she made a beer braised, stuffed chicken breast, which was mouth-watering. I never had chicken breast with beer and brown sugar in the recipe, but the flavor was out of this world. It had a little kick in the end from the mustard seeds (see recipe). For something with such an explosive taste, it was fairly easy to prepare. Basically stir fry up vegetables, add brown sugar to carmelize, stuff it in chicken with some cheese, bake for 45 minutes and voila! You have a new take on baked chicken!

Another interesting bit in this episode of Firehouse Kitchen, involved my Fire Fact. We spoke about and reenacted the bail-out system. At the end of the reenactment, I had the firefighter remove helmet and mask. To the audience’s surprise, it was Chief Steo’s daughter performing a perfect bail-out!

My day with the Dix Hills Fire Department came to a close and left me mulling back over all the great “firsts” of the show. One, the first female firefighter guest cook; two, the first I ever had

Stir fry mushrooms and onions, add brown sugar to caramelize. When carmelized, add mustard seeds. Lay chicken out - season with salt and pepper. Place cheese on chicken and add stir-fry vegetables on top. Roll chicken. Heat olive oil and butter in frying pan. Sear chicken in pan, add beer, chicken broth and rest of vegetables/ingredients. Bring to boil.

Place chicken and contents of frying pan into cooking pan. Bake on 350 degrees for 45 minutes. When plating, remove bay leaves. Use extra juice as gravy and enjoy!

NJ EMS Task Force Formed in the aftermath of the September 11, 2001, terrorist attacks, the New Jersey EMS Task Force has become the organization called in when disasters strike and a model for other states, officials of the organization said on Sept. 20, 2014, during a daylong event to mark the 10th anniversary of the statewide group.

“It is what everyone calls when it hits the fan and the creek rises,” NJ EMS Task Force founding member Mickey McCabe, and president of McCabe Ambulance, told attendees.

“It is like nothing anyone could have imagined and amazing how you do it for free,” McCabe added later.

More than 150 emergency medical service professionals from around New Jersey gathered for the event, which was held at the New Jersey Convention and Expo Center.

The NJ EMS Task Force is a team of career and volunteer EMS professionals that respond to natural and man-made disasters. The organization has been integral to the emergency responses for Superstorm Sandy, Hurricane Irene, wildfires, flooding in Bound Brook, a fire on the boardwalk in Seaside Heights, and the crash of US Airways Flight 1549 and has also provided EMS resources at such planned events as Super Bowl XLVIII, the New Jersey Marathon, the Warped Tour concerts, the New Jersey Balloon Festival and Bamboozle.

“It is the human resources that make this project a success,” said Task Force Chairman John Grembowiec, Sr. Grembowiec noted how the NJ EMS Task Force had evolved since its inception into a major force nationally in EMS planning and response.

In addition to marking the 10th anniversary, the event was also held in conjunction with National Preparedness Month.

Nancy Kelly Goodstein, acting director of the Office of Emergency Medical Services, said the members of the NJ EMS Task Force made it easier for everyone at NJ OEMS to carry out their jobs. “You’ve impacted countless lives and influenced other states,” Kelly-Goodstein said.

The event included a display of major NJ EMS Task Force assets deployed around the state including Medical Ambulance Busses, trucks equipped for mass care situations and other specialty response equipment. Members also participated in a helicopter landing drill, which

included a team from the NJ State Police helicopter unit. Commander Andrew Chen of the United States Public Health Service delivered a luncheon keynote address.

Christopher Rinn, assistant commissioner of the NJ Department of Health Division of Public Health Infrastructure, Laboratories, and Emergency Preparedness told attendees that the group was, “borne out of the cauldron of the 9/11 attacks,” and that 10 years was quite a milestone.

Individuals were the key to emergency response, Rinn said. “These areas, planning and response, is where the Task Force really shines,” he said.

Since it was created, the NJ EMS Task Force has also been responsible for developing more than 15 major disaster plans covering such issues as hurricane and tropical storm response, rail disasters and for the ports and harbors. Indeed, the organization’s planning and response to Superstorm Sandy had garnered international attention and was being implemented elsewhere.

“Many states around the country are looking at New Jersey as the model,” Rinn said. “The question is what will be next, ebola, or some other infectious disease, or terrorism? I know you will be ready for the next event, no matter what form it takes, no matter what the job.”

Breast Cancer Awareness Month

The Egg Harbor Township Ambulance Squad supported Breast Cancer Awareness month by wearing pink t-shirts for the month of October. The ambulance squad has 35 Emergency Medical Technicians under the guidance of new director William Higbee.

Under the direction of Director Higbee, the squad does four to five thousand calls per year and participates in community events.

Bergen Fire Academy Garduates 146

On September 19, 2014, County Executive Kathleen A. Donovan was presented with an official “Transcripts for Graduation” by Chief Larry Rauch of the Bergen County Law and Public Safety Institute’s Fire Academy. The 146 recruits present represented volunteer and career fire departments from across Bergen and five other northern New Jersey counties who have completed a combination of 157 hours of class work and physical skill testing. This was the academy’s second graduation ceremony for 2014, bringing the total number of Firefighter 1 graduates to 371. County Executive Donovan congratulated the graduates on behalf of the 905,000 residents of Bergen County and thanked the graduating class, “Not just for what you do, but for what you are willing to do.”

Clark Scout Builds Memorial On September 7, 2014, Life Scout Samuel M. Guskind, of Boy Scout Troop 145 of Clark, dedicated his Eagle Scout project. Sam chose to do his project — build a monument dedicated to the victims of the 9/11 attack — at the Clark Fire Department headquarters. Sam sent three American flags to Capt. Mike Sogka, of the New Jersey National Guard who had them flown over Camp Phoenix in Kabul, Afghanistan. They were then placed over a piece of WTC steel and then folded by members of the Clark Fire Department, Boy Scouts and firefighters from Elizabeth and Kenilworth departments.

Bordentown Scores $8000 Grant Bordentown Township Fire District 2 was awarded an $8,000 grant that funded life-saving personal safety harnesses and firefighter bail out bag escape systems for each member of the department. .

Bargaintown 9/11 Memorial On September 11, 2014, the Bargaintown Volunteer Fire Company #2 dedicated their 911 Memorial that was erected at the rear of Station #1 on Mill Road.

Fire Company President Dennis Hoffman obtained the steel from the City of New York. The memorial took two years to complete as all the work was done in house by the members of the fire company. The ceremony was emceed by Egg Harbor Township Mayor James “Sonny” McCollough with a blessing of the memorial by Atlantic County Firefighters Associations Reverend Thomas Sweeney. The fire company would like to thank Greenerside Landscaping for all their design help and guidance in completing the brick work.
