Established in 1956, FiftyForward enriches the lives of adults 50+ by providing pathways to health, well-being and lifelong learning.

CARING FOR OTHERS LIVING AT HOME SUPPORT SERVICES FiftyForward.org/Supportive-Care/

In fiscal year 2014, FiftyForward Care Team exceeded program projections for private pay customers. FiftyForward Conservatorship (assigned by court appointment) now manages the largest caseload in the 21-year program history. FiftyForward Victory Over Crime served a diverse client base with a median age of 68 and 70% of the cases involved elders who were victims of either domestic assault or financial exploitation.

FiftyForward Meals on Wheels* celebrated special milestones this year: 20+ year volunteer Barry Coggins was a finalist (one of 70 nominees) for the prestigious Mary Catherine Strobel Volunteer Service Award.

FiftyForward Meals on Wheels boasted more than 100 active volunteers who delivered more than 20,000 meals from July 2013 - June 2014.

Additional volunteers delivered 597 meals to 396 individuals on Thanksgiving and Christmas Days.

FiftyForward Adult Day Services*, the first social model day program in Tennessee to serve the community’s older adults and their caregivers, marked 40 years of continuous service helping customers maintain or increase their functioning level and supporting caregivers who work or need a break from caregiving.

The program is the only local adult day care program partially supported by the Tennessee Department of Human Services, through a Social Services Block Grant. This fiscal year, FiftyForward Adult Day Services received a $10,000 increase in funding; in FY15 funding from this source increased by $40,000. * These programs are housed at Second Presbyterian Church, 3511 Belmont Blvd., Nashville.

FiftyForward centers are among the elite 1% in the U.S accredited by the National Institute of Senior Centers (College Grove, Knowles, Martin and Madison Station have been re-accredited by NISC until 2019). Center milestones include:

FiftyForward Bordeaux (3315 John Mallette Dr., 37218), during the 5th Annual Hats Off to Bordeaux fundraiser, Ms. Robbie Williams was named Volunteer of the Year for her daily commitment to serve meals to center members.

FiftyForward College Grove (8607 Horton Highway, 37046) now offers aerobics and yoga classes adding energizing programs from which members benefit. In addition, FiftyForward College Grove gained increased funding this year from the Area Agency on Aging due to the center’s outreach in its rural community.

FiftyForward Donelson Station (108 Donelson Pike, 37214) was fortunate to have Fran Mazzaferro, former FiftyForward Associate Executive Director, come out of retirement to serve as interim director until February (2014) when Angela Bryan was named as center director. Bryan brings many years of experience (program direction, administration, and community development) to FiftyForward.

FiftyForward J. L. Turner Center (in Bellevue Family YMCA, 8101 Highway 100, 37221) launched Turner at Twilight in March expanding its evening and weekend programming.

FiftyForward Knowles (174 Rains Ave., 37203) and the Knowles Choraleers welcomed an average of 50 older adults each week in July and August for the center’s annual Summer Singing Camp.

FiftyForward Madison Station (301 Madison St., 37115) added a wheelchair accessible bus thanks to the efforts of the fundraising committee at Madison Station as well as a match from a generous FiftyForward donor.

FiftyForward Martin Center (960 Heritage Way, 37027) Card Crusaders raised enough money to have the center’s parking lot sealed and striped. In addition, sponsorships brought added visibility and increased revenue to this year’s Music and Memories concert.

FiftyForward centers are among the elite 1% in the U.S. accredited by the National Institute of Senior Centers.

Center members from across the agency participated in a membership satisfaction survey. With a high response rate, results of both the survey and focus groups underscore the personal benefits of center membership.

Community volunteers gave their talents, expertise and time to others through the following programs.

Foster Grandparent Program More than 2,000 letters and drawings of support were mailed to legislators in Washington, D.C. for continued funding of Senior Corps to serve at-risk children who are assisted by this program daily.

Volunteers from the program engaged in the “Talking Transportation” effort led by the Tennessee Department of Transportation (TDOT) to share ideas and discuss needs and concerns regarding senior transportation in Nashville.

Thanks to the Frist Center for the Visual Arts, 70 Foster Grandparents were able to share their Senior Corps Day experience with the children they serve.

Retired Senior and Volunteer Program (RSVP) With a mission to match volunteers age 55+ to demonstrated community need, RSVP focuses on key categories of service to maximize impact.

Food Security: More than 70 RSVP volunteers served with FiftyForward’s Meals on Wheels program this year, spending a combined 2,400 hours bringing food and comfort to homebound seniors.

Veterans: RSVP volunteers transported patients to medical appointments at the Nashville VA Hospital, served lunch bi-monthly at Operation Stand Down through a partnership with The Nashville Food Project, provided Christmas gifts to families at the Nashville Vet Center, a counseling program for combat veterans and served as greeters at the USO Welcome Center at the Nashville International Airport.

Education: 140 Friends Learning in Pairs (FLIP) volunteers tutored 500 children in 29 Williamson and Davidson County schools through this award-winning program. Ninety percent of the children served increased reading or math levels by at least a half-grade level or met grade level benchmarks as a result of the one-on-one tutoring provided by

ENGAGING AND EMPOWERING TRAVEL (LOCAL, REGIONAL, NATIONAL AND INTERNATIONAL ADVENTURES) FiftyForward.org/Travel/

FiftyForward Travel In trips across the country or across the globe, 863 individuals participated in the FiftyForward Travel program during 2013-14. Trips included Alaska and Vancouver, B.C.; Ireland; Asheville, NC; Salt Lake City, UT; Jackson Hole, WY and Yellowstone National Park.

On the Road with Joan hosted day trips via motor coach to regional historic sites and community festivals. Nearly 300 travelers were On the Road with Joan from June 2013 - July 2014 to experience distinct locales within driving distance of Nashville including Monteagle, TN; Edgar Evins State Park, TN; Lawrenceburg, TN; Paris, TN; and Grand Rivers, KY.

The Larry Keeton Theatre (Senior Center for the Arts) The Senior Center for the Arts/Larry Keeton Theatre hosted seven productions at the dinner theatre. In addition, a summer children’s camp delighted kids of all ages and featured many first time performers (young and old) in Seussical and other productions.

Two new fundraising events held at the theatre this year included a wine tasting and a live Tony Awards screening and party.

Music for Seniors Music for Seniors averaged 57 performances monthly, impacting more than 1,400 older adults at venues including FiftyForward centers, Adult Day Services facilities, residential communities and private residences.

Community partners such as Cheekwood, Nashville Jazz Workshop and The Frist helped produce the program’s free Daytime Concert Series. Grants from the Memorial Foundation, the Alzheimer’s Foundation of America, Tennessee and Metro Arts Commissions and others helped support stipends for the participating solo musicians and ensembles.

With this as its final year as a FiftyForward affiliate, Music for Seniors now continues as an independent 501(c)3 arts organization under the leadership of founder and executive director, Sarah Martin McConnell.

Community Collaborations FiftyForward was pleased to present screenings of NPT’s compelling series Aging Matters. We are honored to work together with community partners to provide education and awareness on topics related to the issues and opportunities surrounding aging. As one of the few agencies providing direct service in the community to older adults, we take pride in building awareness, visibility and providing care focused solely on aging issues.

Technology Completed replacement of equipment, software and servers to support the agency. In addition Stringfellow Technology Group is providing IT customer service and hardware support.

Renovations The $896,000 renovation of the Donelson center (to address life safety, ADA, and structural integrity concerns) and improvements to The Larry Keeton Theatre were completed on time and under budget. The renovation funds were provided

by FiftyForward Donelson Station and Senior Center For The Arts “Two Stars/One Stage capital campaign 2006-2012.”
