Quiénes somos About us

EL INSTITUTO FERNÁNDEZ-VEGA ES UN HOSPITAL MONOGRÁFICO DE OFTALMOLOGÍA El apellido Fernández-Vega está ligado a la práctica de esta especialidad médica desde hace más de 125 años. Hoy por hoy, en el Instituto se dedican todos los esfuerzos a la prevención, diagnóstico y tratamiento médico y quirúrgico de las enfermedades que comprometen la visión.

Las instalaciones del Instituto en Oviedo ocupan una superficie de unos 12.000 m2, donde trabajan alrededor de 200 personas.

Cada año se atiende a más de 100.000 pacientes y se realizan 8.000 cirugías.

Para mantenerse a la cabeza en innovación y devolver así a la sociedad parte de lo recibido de sus pacientes, el IOFV ha impulsado la Fundación de Investigación Oftalmológica (FIO), donde se realiza investigación biomédica traslacional en oftalmología y ciencias de la visión y que cuenta con la generosa colaboración de otras entidades y fundaciones comprometidas en la lucha contra la enfermedad.

Toda esta actividad genera una gran capacidad docente para

Además, el Instituto atiende a personas desfavorecidas a través de la Fundación Fernández-Vega, presente tanto en España como en misiones internacionales, de las que son buena muestra las llevadas a cabo en los últimos meses a Angola, Perú, Liberia o Camboya.

THE INSTITUTO FERNÁNDEZ-VEGA (IOFV) IS AN EYE HOSPITAL

The surname Fernández-Vega has been linked to the practice of this speciality for over 125 years.

At present, the Institute devotes all its efforts to the prevention, diagnosis and medical and sur‑ gical treatment of those diseases that jeopardise vision.

Currently, the facilities of the Institute in Oviedo occupy a surface area of approximately 12 000 square metres, in which close 200 people work. Every year over 100 000 patients are seen and 8 000 surgeries are performed.

In order to stay ahead in innova‑ tion and to return to society part of what its patients have given to the Institute, the IOFV has launched the Fundación de Investigación Oftalmológica (FIO) (Eye Research Foundation). The Foundation carries out biomedi‑ cal translational research in oph‑ thalmology and vision sciences in conjunction with the generous contribution of other entities and foundations committed to the fight against this disease.

All this activity generates a great teaching capacity for health

professionals devoted to vision (ophthalmologists, opticians-op‑ tometrists, nurses, students of Medicine) that materialises itself through different pre and post graduate programmes in col‑ laboration with Universidad de Oviedo, which has led the enti‑ ty to be classified as a University Institute.

Furthermore, the Institute cares for disadvantaged people through the Fundación Fernández-Vega, present in Spain as well as in international missions, such as the ones carried out in the past few months in Peru, Liberia and Cambodia.

Luis Fernández-Vega, profeta en su tierra Los miembros del jurado reconocieron, el pasado diciembre, la aportación e identificación con su ciudad de Luis Fernández-Vega a la hora de elegirle “Ovetense del año”, distinción que recogió el pasado 10 de marzo, en el curso de una cena a la que asistieron más de 400 personas y en el que hubo que “colgar” el ‘no hay billetes’, dadas las más que numerosas solicitudes para acompañar al director del Instituto que lleva el apellido de una familia con ya cinco generaciones de oftalmólogos.

De la saga familiar a la que pertenece y de los méritos que le han hecho acreedor de esta distinción, habló Víctor García de la Concha, director del Instituto Cervantes, asturiano ilustre y autoridad indiscutible en lo que se refiere a nuestra lengua. Lo hizo con la brillantez que acompaña todas sus intervenciones, apoyándose para ello en algo tan nuestro como es la ola que, en palabras de Gustave Flaubert, cuando viene y llega a nosotros, lo hace recogiendo y sumando la belleza de la que la precede.

Paciente él mismo de tres generaciones de Fernández‑Vega, su intervención estuvo trufada de anécdotas y costumbres a lo largo de 130 años, para lo que se apoyó en varias ocasiones en el libro del que es autora Victoria Cueto-Felgueroso sobre los Fernández-Vega.

Luis Fernandez-Vega, prophet in his own land The members of the jury recognised last December Luis Fernández‑Vega’s contribution and identification with his city appointing him “Oventese del año”. Luis Fernández-Vega explained that his love of Oviedo has its origin in his birth and childhood, but also in the fact that he chose to come back to the city when he was already a head of department in his specialty and had promising prospects elsewhere.

“Oviedo me proporciona raíces sólidas, me ancla a la tierra y me rodea de afectos”, destacó el profesor. Sentimientos estos que personalizó en su mujer e hijos, sin olvidar a los numerosos amigos que quisieron acompañarle en una noche tan especial y a la que no faltaron muchos otros venidos desde distintos lugares de España.

Se mostró satisfecho por lo alcanzado en el plano profesional y personal sin por ello olvidar momentos difíciles y afirmó que recibe la distinción más como estímulo para seguir "haciendo" que como reconocimiento por lo conseguido, y reafirmó su compromiso de seguir vinculado a la ciudad pese a las reiteradas

ofertas de desplazar la actividad a otras áreas con una teórica mayor potencialidad.

El recuerdo para quienes le precedieron estuvo muy presente - el de sus padres de manera muy especial - sin olvidar a su tío Álvaro, y quiso compartir el reconocimiento con un equipo excepcional en el que junto a sus primos, encabezados por un oftalmólogo del prestigio de Álvaro Fernández-Vega Sanz, ocupa también un lugar muy destacado el Dr. José F. Alfonso. Un espejo en el que debe mirarse la quinta generación que ya se está incorporando de forma paulatina al Instituto.

El también catedrático de oftalmología de la Universidad de Oviedo y jefe de servicio del Hospital hizo llegar su afecto a quienes le acompañan en estos otros cometidos, antes de finalizar con la sentida gratitud que le embargaba al verse rodeado de amigos y representantes de la vida económica, universitaria, social y política de Oviedo y Asturias.

Quizás por ello, sus últimas palabras fueron: “Si siempre he querido ser un buen ciudadano, a partir de ahora estoy obligado a ser aún mejor”.

Asimismo, también acudió como invitado al programa “Saber vivir”, donde repasó cómo puede afectar a la salud ocular la llegada de la premenopausia, la utilización de la insectomía refractiva para operar la vista cansada sustituyendo el cristalino por una lente intraocular multifocal o cómo si viviésemos lo suficiente todos

El Prof. Fernández-Vega recordó también en el programa el fármaco familiar que inventara su padre, la CICACOA, acrónimo de ciencia, cariño, comprensión y ánimo, “porque la técnica y

la tecnología son importantísimas, pero es la personalización de cada tratamiento con la dosis adecuada de CICACOA la que no se puede perder y con la que podemos curar muchas enfermedades”.

Eso sí, el estricto protocolo del Instituto obliga a los médicos de la familia a hacer la especialidad en otro centro porque, recuerda el Prof. Fernández-Vega, el apellido no basta cuando se tiene la excelencia como marca.

“In my case, I hesitated on whether to become an economist and I even enrolled in Economics and Medicine at the same time without my parents knowing. I went to class in both fa‑ culties. Maybe it was just the typical rebellious act of youth, but the hesita‑ tion lasted only a few days. I wanted to know that I did it because I wanted to, and not because I was following the family tradition”, explains Prof Luis Fernández-Vega Sanz, medical di‑ rector of the Institute, in the article. A conviction that is also present in the new generations. “Our family does not pressure or prompt, although it is difficult to withdraw from a profes‑ sion so beautiful that it returns one of the most valuable senses and which we have seen our parents enjoy so much”, he adds.

Entre las novedades presentadas en FacoElche 2016 destacan también los nuevos modelos de lentes trifocales para dar solución a defectos oculares en los tres rangos de alcance, los últimos avances para tratar el glaucoma y un innovador colirio para retrasar la progresión de la miopía.

Amongst the most important presentations was that of the nanosecond laser, which aims to perform cataract surgery even more safely. This laser, which does not release heat but cold energy, was presented in a surgery performed live in the operating rooms of the University Hospital of Vinalopó.

El Instituto, en Catarata + Actualización de los criterios médicos, docencia e investigación se mezclaron un año más el pasado 28 de noviembre en Catarata +, cita habitual en el calendario de nuestra especialidad organizada por el servicio de Oftalmología del Hospital San Rafael, en Madrid, que dirige el Dr. Mariano Royo y enfocada sobre todo a los residentes de Oftalmología de toda España en la seguridad de que su formación es una de las claves que define cualquier especialidad médica.

El Instituto Fernández-Vega, comprometido desde siempre con esta filosofía, también participa en el desarrollo del programa aportando enseñanza práctica y teórica a través del Dr. José F. Alfonso, que cuenta con una amplia experiencia en este campo.

En concreto, en esta ocasión se tuvo la oportunidad de exponer temas de actualidad en relación con la cirugía de la córnea y del cristalino, área en el que el Instituto es una clara referencia, ya que el impulso tecnológico que

se ha dado a la especialidad en estos años, y que hemos tenido la oportunidad de poner en práctica, nos permite facilitar información actualizada a nuestros colegas.

El láser de femtosegundo en la cirugía de la catarata, la utilización de las lentes multifocales para mejorar la visión de cerca, el implante de las lentes intraoculares epicristalinianas para la corrección de la miopía y los trasplantes de córnea lamelares (por capas) fueron los temas que se

abordaron durante una reunión cuyas actividades están -como no podía ser de otro modo- avaladas por la Sociedad Española de Cirugía Ocular Implanto-Refractiva (SECOIR), que presta su colaboración, y en la que se tiene la firme intención de seguir participando en el futuro tanto por interés profesional como por la estrecha relación que se mantiene con el Hospital San Rafael.

The Institute, in Catarata + The updating of medical, teaching and research criteria merged once again last 28th of November in Catarata +, a regular event on the calendar of our specialty. The meeting is organised by the Ophthalmology Depart‑ ment at Hospital San Rafael, in Madrid, and is headed by Dr Mariano Royo. Aimed especially for Ophthalmo‑ logy residents from all over Spain, it assures them that training is key in any medical specialty.

The Instituto Fernández-Vega, which has always been commit‑ ted to this philosophy, also par‑ ticipated in the programme deli‑ very by providing theoretical and practical teaching with the help of Dr José F. Alfonso, who has ample experience in this area.

In particular, on this occasion we had the opportunity to present topical issues regarding corneal and crystalline surgery, an area in which the Institute is a clear benchmark. The technological

boost that has taken place in the last few years in the specialty, and that we have been able to implement, enables us to provide updated information to our colleagues.

Femtosecond laser in cataract surgery, the use of multifocal lenses to improve near vision, the implantation of intraocular epicristalinian lenses to correct myopia and lamellar corneal transplants (in layers) were the issues debated during the mee‑

ting. The Spanish Society of Im‑ plant-Refractive Ocular Surgery (SECOIR in Spanish) –not surpri‑ singly – endorsed the event’s ac‑ tivities. The SECOIR collaborates and has every intention of conti‑ nuing to participate in the future both due to its professional inte‑ rest and to its close relationship with the Hospital San Rafael.

IV Jornadas de Enfermería Oftalmológica sobre el polo posterior “Back to the future” EL INSTITUTO OFTALMOLÓGICO FERNÁNDEZ-VEGA ORGANIZÓ EL PASADO SÁBADO 27 DE FEBRERO LAS IV JORNADAS DE ENFERMERÍA OFTALMOLÓGICA SOBRE EL POLO POSTERIOR, “BACK TO THE FUTURE”, EN LAS QUE SE DIERON CITA PRESTIGIOSOS ESPECIALISTAS DEL PANORAMA NACIONAL E INTERNACIONAL PARA TRATAR LAS ÚLTIMAS NOVEDADES DEL SECTOR. Inauguradas por el Dr. Álvaro Fernández-Vega Sanz, subdirector médico del Instituto y director de la unidad de Retina y Vítreo, se abordaron asuntos como la investigación básica en enfermería o la evolución de las inyecciones intravítreas y de la cirugía de retina. En esta última se trató el cuidado postural en los postoperados con desprendimiento de retina para favorecer una óptima recuperación.

Asimismo, la Dra. Eva Villota, de la unidad de Retina y Vítreo del Instituto, una vez finalizada su conferencia, respondió a las preguntas de los asistentes y, para

finalizar, la presidenta de la Sociedad Española de Enfermería Oftalmológica, (SEEOF), Aurora Egido, presentó los proyectos que esta sociedad tiene organizados a lo largo del año.

Chaired by Dr Álvaro Fernández-Vega Sanz, deputy medical director of the Institute and director of the Retina and Vitreous Department, the meeting addressed issues like basic research in nursing or the evolution of intravitreal injections and retinal surgery; the latter also addressed the importance of postural care during post-ops with retinal detachment to foster an optimal recovery.

La importancia del procedimiento en la cirugía de cataratas Entrevista al Prof.

Fernández‑Vega y al Dr. Alfonso La cirugía de cataratas es uno de los procedimientos quirúrgicos más comunes, y cada año se realizan en el mundo casi 22 millones de intervenciones1.

Actualmente, para optimizar resultados y agilizar la intervención, las últimas tecnologías en guiado intraoperatorio y láser de femtosegundo dan apoyo al cirujano durante la intervención para diagnosticar y analizar en detalle el ojo del paciente, planificar con esa información la cirugía y hacerla más predecible.

El número de intervenciones de catarata está creciendo rápidamente debido a los cambios demográficos y al acceso creciente a la atención médica en todo el mundo. La Organización Mundial de la Salud calcula que hacia el año 2020 se realizarán en el mundo más de 32 millones de cirugías de cataratas al año2. Además, los estándares en este tipo de cirugía están avanzando, dando lugar a una demanda de innovaciones y nuevos equipos2.

The importance of procedure in cataract surgery

Interview to Prof Fernández-Vega and Dr Alfonso Cataract surgery is one of the most common surgical procedures; every year, approximately 22 million interventions are performed in the world1. Currently, to optimise results and speed the intervention, the latest technologies in intraoperative guidance and the femtosecond laser support the surgeon during the procedure in diagnosing and analysing the patient’s eye in detail, planning the surgery based on that information and making it more predictable.

The number of cataract surgeries is growing rapidly due to demographic changes and the increasing access to healthcare around the world. The World Health Organisation estimates that by 2020 over

32 million cataract surgeries will be performed every year worldwide2. Likewise, standards in this type of surgery are progressing, creating a demand for innovation and new equipment2.

It is estimated that 11.1% of the Spanish population (not taking into account ametropic patients) undergoes some type of surgery to correct their refractive errors3; in 2012 this meant approximately 125.000 refractive surgeries. The fact is that 77.6% of the Spanish population needs vision correction and the most common refraction errors amongst the Spaniards are myopia (31%), presbyopia (31%), astigmatism (24%) and hyperopia (12%)4.

¿Cuál es el papel de la tecnología durante una cirugía de cataratas?

LFV. La tecnología ofrece al cirujano un extra de seguridad durante el procedimiento; seguridad que beneficia, como es lógico, al paciente. Los pasos quirúrgicos de la cirugía del cristalino están muy bien definidos y cada avance tecnológico incorporado, mejora los resultados de la técnica. ¿Es posible corregir los defectos refractivos durante la cirugía de cataratas?

LFV. Actualmente podemos corregir miopía, hipermetropía, presbicia y astigmatismo cuando intervenimos la catarata, ya que las lentes intraoculares que sustituyen al cristalino pueden llevar incorporada la corrección refractiva que necesita el paciente.

JFA. El desarrollo de la tecnología óptica ha permitido que dispongamos de lentes de todo tipo. Hoy distinguimos entre lentes bifocales, para visión de lejos y de cerca, y las lentes trifocales, que añaden un foco de visión intermedia. Este avance en óptica ha venido a mejorar nuestras indicaciones y nos está permitiendo realizar una verdadera cirugía personalizada: podemos escoger la lente más adecuada para cada paciente. ¿Cuál es la importancia del procedimiento durante una cirugía de cataratas?

JFA. En cualquier técnica quirúrgica, es necesario seguir un procedimiento normalizado. Es decir, seguir un protocolo de actuación que garantice el éxito. El procedimiento de la cirugía de cataratas y de presbicia incluye la valoración preoperatoria, la técnica quirúrgica y el control

LFV. España es el país de nuestro entorno que presenta un mayor índice de implante de lentes trifocales. En la actualidad, el porcentaje de implante de lentes intraoculares trifocales es del 10% aproximadamente sobre el total de lentes implantadas; lo que se traduce en unas 50.000 anuales.

En países como Alemania, Francia o el Reino Unido, el porcentaje no supera el 5%, por lo que nos consideramos líderes dentro de esta parcela de la Medicina.

What role does technology play during cataract surgery?

LFV. Technology offers the sur‑ geon added safety during the procedure, safety that benefits, logically, the patient. The surgi‑ cal steps of crystalline surgery are very well defined and every technology advance incorpora‑ ted improves the results of the technique.

Is it possible to correct refractive errors during cataract surgery?

LFV. Currently we can correct myopia, hyperopia, presbyopia and astigmatism when we are performing cataract surgery as the intraocular lenses that subs‑ titute the crystalline can incorpo‑ rate the refractive error required by the patient.

JFA. The development of optical technology has provided us with all kinds of lenses. We nowadays make a distinction between bi‑ focal lenses, for far and near vi‑ sion, and trifocal lenses, which add an intermediate focus. This optical advance has improved our indications and is enabling us to perform a truly customised procedure: we can choose the most appropriate lens for each patient.

What is the importance of procedure during cataract surgery?

JFA. A standardised procedure must be followed in every sur‑ gical procedure. This means to follow an action protocol that

guarantees success. The proce‑ dure of presbyopia and cataract surgery includes the pre-opera‑ tive assessment, the surgical te‑ chnique and the post-operative monitoring, and will enable us to identify the characteristics of the patient and plan a customi‑ sed surgery. Likewise, one must highlight that the election of the intraocular lens in one of the most important aspects.

Is Spain a leading country with regard to this type of procedures?

LFV. Spain is the country with the highest index of trifocal lens implantation in Europe. Current‑ ly, the percentage of trifocal lens implantation is approxima‑ tely 10% of the total of implan‑ ted lenses, which translates to approximately 50 000 annually.

In countries like Germany, Fran‑ ce or United Kingdom, the per‑ centage does not exceed 5%, we therefore consider ourselves lea‑ ders within this field of Medicine.

Así lo afirma el doctor Jesús Merayo, investigador del Instituto Universitario Fernández-Vega y profesor de la Universidad de Oviedo.

La premenopausia es el periodo de transición natural hacia la menopausia. “Es la etapa de la vida de una mujer en la que empieza a disminuir la reserva ovárica y aparecen irregularidades en el ciclo menstrual. Suele presentarse alrededor de los 45 años de edad y su duración media suele ser de 4 años”, explica el doctor.

El síndrome de ojo seco afecta a casi al 30% de la población, en su inmensa mayoría mujeres, y si bien está muy relacionado con los cambios hormonales de la menopausia y suele aparecer coincidiendo con esta etapa de la vida, también puede deberse a otras circunstancias. De ahí la necesidad de que, sobre todo a partir de los 45 años, la mujer revise periódicamente su salud ocular. “Esto ayudará a verificar si se padece un Síndrome de Ojo Seco y a descartar posibles patologías concomitantes que, como algunas enfermedades tiroideas y otras autoinmunes como las reumáticas o patologías como la fibromialgia o la fatiga crónica, puedan precipitar o provocar esta alteración ocular”, explica el Dr. Merayo. Estas enfermedades provocan por sí mismas que quienes las padecen tengan menos secreción de lágrima, lo cual, unido a la etapa perimenopáusica en la mujer, “puede provocar que pacientes que nunca antes habían tenido síntomas de ojo seco los tengan a partir de los 45 años”.

ALGUNOS CONSEJOS PARA PREVENIR EL OJO SECO Además de la revisión periódica de su salud ocular, y de cara a prevenir el ojo seco, el doctor Merayo aconseja que la mujer se proteja con gafas del viento y el sol siempre que realice actividades al aire libre, cuidar la posición del cuerpo, descansar la vista de vez en cuando y no olvidarse de parpadear para estimular la secreción de la lágrima si se trabaja delante de una pantalla de ordenador, así como resguardar los ojos de forma adecuada cuando se tengan que utilizar productos como el amoniaco o la lejía “de igual forma que nos ponemos mascarillas o protegemos otras partes de la piel ante la presencia de tóxicos”.

También es muy importante cuidar la cantidad y calidad del sueño. “Si no tenemos un sueño de calidad y una cantidad de horas de descanso suficientes no entramos en la fase REM, que actúa como masaje regenerativo de la superficie ocular para disminuir el riesgo de enfermedades oculares”.

Para finalizar, cabe destacar la importancia de cuidar también la delicada piel de la zona de los párpados frente a enfermedades como la rosácea o la psoriasis. “La mitad de los pacientes que se quejan de ojo seco tiene problemas en la piel de los párpados. Por ello es muy importante cuidarla de forma especial cuando se vaya a entrar en la fase de la menopausia. Este cuidado consiste fundamentalmente en desmaquillarse de forma correcta o hacer una buena limpieza de cara y párpados incluso aunque no se maquillen. El proceso de higiene de los párpados se puede protocolizar con calor local, masaje y limpieza de la superficie ocular al levantarse y al acostarse.

Esto debe ser así sobre sobre todo en las usuarias de lentes de contacto y las pacientes que tengan algún tipo de enfermedad dermatológica”, concluye el especialista del Instituto.

It is also paramount to take care of the quantity and quality of our sleep. “Without quality sleep and enough resting hours, we do not enter the REM sleep, which acts as a regenerative massage to the ocular surface in order to decrease the risk of eye diseases” To finish off, it is worth stressing the importance of taking care of the delicate skin of our eyelids to avoid diseases such as rosacea or psoriasis. “Half the patients who report dry eye present problems in the skin of their eyelids; therefore, it is very important to take special care when entering menopause. This care consists mainly of removing make-up appropriately or performing a good face and eyelids cleansing even if no make-up is applied. The process of cleaning the eyelids involves applying local heat, massaging the area and cleaning the ocular surface when waking up in the morning and before going to bed. This is specially important in those individuals that use contact lenses and in patients that suffer from some kind of skin disease”, concluded the specialist of the Institute.

Al introducir sus datos para darle cita, allí está su historia. Se quedan muy sorprendidos”, explica Conchi García sin poder evitar una sonrisa.

Víctor García de la Concha Filólogo, asturiano, profesor y, sobre todo, académico, Víctor García de la Concha dirige desde 2012 el Instituto Cervantes, institución encargada de la promoción y la enseñanza de la lengua y la cultura española e hispanoamericana en todo el mundo y presente en 43 países de los cinco continentes. La verdad es que ahora mismo estoy un poco sobrecargado de trabajo. Estamos terminando de montar un proyecto de enorme trascendencia a iniciativa de la Universidad Nacional Autónoma de México: un certificado internacional iberoamericano del español para medir el conocimiento y dominio del idioma, tanto para estudiantes de español como lengua extranjera como para un nativo que quiere conocer su nivel.

Víctor García de la Concha Philologist, Asturian, teacher and, mainly, academic, Víctor García de la Concha has been head of the Instituto Cervantes since 2012. This institution is responsible for the promotion and teaching of Spanish and Latin American language and culture all over the world, being present in 43 countries within the five continents. Absorbed by the last preparations for the seventh International Meeting of Spanish Language, where he will inaugurate an ambitious project, he takes some time in his busy schedule to talk to VEGA.

His agenda is really busy…

The truth is that I have an overload of work right now. We are finishing the preparations for a project of immense significance based on the initiative of the Universidad Nacional Autónoma de México: an Iberoamerican international certificate of Spanish Language to measure the knowledge and command of the language, both for students of Spanish as a second language and for natives who wish to know their level.

What does it entail?

It is a scale test, from cero to one thousand points that certifies the four skills in Spanish: oral comprehension, written comprehension, oral expression and written expression. The Universidad Nacional Autónoma de México, the Instituto Cervantes, the Universidad de Salamanca and the Universidad de Buenos Aires are the

titleholders, but we have a wide network of associated universities both in Spain and America.

This is the last project I have promoted at the Instituto Cervantes and, naturally, it is taking a lot of time. The kick-start will be in the Meeting in Puerto Rico.

Another important appointment.

Yes. We have been preparing it for the last two years. The idea came about in a meeting on Spanish Language in 1992 aiming to establish an annual appointment in which to gather all the Spanish-speaking representatives to reflect on a given matter and on general issues regarding the situation of the language in the world. It is a very important challenge. When I accepted to head the Instituto Cervantes, the Government put me in charge of making it Ibero-American, as the Institute, in its 25 years of exis-

tence, had been up until now a Spanish creation. However, there are 553 million Spanish speakers and the Spanish language is a heritage shared by Spain and the Hispanic America, so we started to solve it. When I was appointed director of the Real Academia, His Majesty the king Juan Carlos called me and said: “You just need to focus on one thing, America.

All academies must become one, work as one. You need to visit all countries, I will pave the way for you”. It was the Crown’s idea that I carried out and which served to strengthen the Academies.

de que la lengua sea un elemento vivo facilita la crítica?

El diccionario no fabrica palabras ni las promueve. El diccionario levanta un acta notarial o registral del uso que los hablantes hacen de la lengua y, por tanto, surge una nueva acepción de una palabra que ya existía o surge una nueva, porque la dicen los hablantes. El uso de la lengua la rige el pueblo hispanohablante.

Esto lo dijo Horacio hace siglos.

Una palabra es aceptada por la Academia cuando ve que es utilizada con ese sentido por gran número de hablantes no solo en el lenguaje oral, sino en el escrito. Y puede gustar más o menos, pero es la que utiliza el pueblo.

El español está en un momento de verdadera expansión. En EE.UU. hay 53 millones de hispanohablantes y se espera que en 2050 sea el primer país hispanohablante del mundo. Y en Brasil, en Italia, en Francia hay un auge del español, incluso en los países del Este. Nuestro reto ahora son los países emergentes de Asia: China, Japón, la India, Corea, donde está creciendo, y en mí ejerce un gran reclamo África, porque no tardará el día en que países emergentes se vayan incorporando a la vida democrática y en ese momento, cuando entre en nuestro equipo, ¿qué lengua se va a hablar? Está la presencia colonial del inglés y del francés. Habría que buscar el espacio para la presencia del español. En Costa de Marfil hay 150.000 estudiantes de español, pero no damos abasto. El Instituto Cervantes, desde luego, solo no, y digamos que es tanto lo que hay que hacer y tanta la demanda que no llegamos. make it. We have barely survived the crisis, but we have survived without closing any centres, wi‑ thout undertaking any lay-off pro‑ ceedings. I will give my successor the same number of Cervantes centres and it will be necessary to work jointly with America to serve unity an expansion.

Tengo 52 años y necesito gafas de lejos y de cerca. Tengo astigmatismo e hipermetropía y por mi trabajo me resulta muy incómodo cambiar constantemente de gafas, por lo que he utilizado lentes progresivas, pero no me adapto a ellas. Me han hablado de una operación que sirve para prescindir de las gafas, ¿es así?

En efecto, existen varios tipos de intervenciones quirúrgicas para prescindir de las gafas o de la lente de contacto. La técnica empleada depende del defecto a corregir y de la edad del paciente. En su caso, al defecto de visión lejana se suma la presbicia o defecto en visión para cerca. En estos casos, se implantan unas lentes intraoculares que proporcionan una visión adecuada en todas las distancias (TRIFOCALES). Como además tiene astigmatismo, tendría que ser una lente TRIFOCAL TÓRICA, que además corrige el astigmatismo. No obstante, hay que personalizar cada caso con pruebas adicionales. Algunos pacientes necesitan un ajuste final con técnica lasik (láser que actúa sobre la córnea) para corregir el defecto residual.

Hace años que tengo infecciones por Herpes en un ojo que ha afectado a la visión y me produce muchas molestias. Estoy muy preocupada porque veo poco de ese ojo y tengo molestias con frecuencia, ¿qué podemos hacer?

La queratopatía herpética es bastante frecuente y es causa de pérdidas importantes de la agudeza visual por afectar a la córnea y produce molestias oculares por la gran inflamación que se produce en el ojo. Además, las recidivas son frecuentes. El tratamiento consiste en antivirales específicos contra los herpes virus, tanto por vía local como sistémica.

En determinados momentos del proceso, se asocian corticoides y midriáticos, entre otros. Además, hay que cuidar la superficie ocular con lágrimas artificiales. Una vez se ha conseguido controlar el proceso infeccioso e inflamatorio, se procederá a plantear un trasplante corneal lamelar, es decir, no de todas las capas de la córnea, se preservan el endotelio y descemet del paciente, se trasplantan las capas de la córnea que están más opacificadas (DALK). Muchas veces hay que tomar antivirales por vía oral durante meses para evitar que vuelva a aparecer. Recomendamos ser muy cuidadoso y constante con los cuidados y tratamientos prescritos.

Soy una auxiliar de administrativo de 50 años, trabajo muchas horas con el ordenador y desde hace unas semanas noto los párpados muy pesados, los ojos cansados y molestias, noto sensación de roce a veces y también veo borroso por momentos. No he tenido enfermedades de la vista, pero hace tres meses me diagnosticaron un síndrome ansioso depresivo y me pusieron tratamiento, pero estos síntomas oculares me preocupan.

Es muy frecuente que en la menopausia se produzcan cierta sequedad de mucosas por los cambios hormonales. Además, el trabajo con el ordenador hace que disminuya la frecuencia del parpadeo, lo cual produce un aumento de la evaporación de la película lagrimal. Asimismo, hay varios grupos de medicamentos que reducen significativamente la secreción lagrimal, entre ellos los ansiolíticos y los antidepresivos. Por todo ello, usted puede padecer un síndrome de sequedad ocular. En estos casos hay que poner el tratamiento oportuno para restablecer una película lagrimal abundante y sana.

En ciertos casos se requiere hacer estudios mediante una analítica de sangre y otras pruebas médicas. Finalmente, el mayor avance para los casos moderados y graves consiste en elaborar un colirio con la sangre del paciente, que es muy rico en factores estimulantes y regenerativos de la película lagrimal (PRGF), técnica de la que disponemos en el Instituto Oftalmológico Fernández-Vega.

I am 52 years old and I need glas‑ ses for near and far vision. I have astigmatism and hyperopia and due to my profession I find it very uncomfortable to switch glasses constantly, I have therefore used progressive lenses, but I cannot adjust to them. I have heard that there is a surgery that removes the need for glasses, is this true?

Indeed, there are different types of surgical procedures to get rid of glasses or contact lenses. The technique used depends on the error that has to be corrected and the age of the patient. In your case, on top of the long distance error we have to add presbyopia or error in near vision. In these cases, intraocular lenses that provide an appropriate vision in all distances are implanted.

(TRIFOCAL). As you also suffer from astigmatism, you would need a TORIC TRIFOCAL lens, as it also corrects astigmatism.

However, each case has to be customised by performing additional tests. Some patients need a final adjustment with lasik technique (laser that works on the cornea) to correct any residual error.

I have been suffering from her‑ pes infection in one eye for years, it has affected my vision and it causes a lot of discomfort.

I am very worried because I see poorly with that eye and it often hurts. What can we do?

Herpes keratitis is quite common and it causes significant loss of visual acuity as it affects the cor‑ nea, it also causes eye pain due to the high inflammation that occurs in the eye. Furthermore, recurrences are common. Treat‑ ment consists of specific antiviral medications against herpes virus, both in local and systemic regi‑ mes. In certain steps of the pro‑ cess, corticoids and mydriatics are associated - amongst others.

Likewise, the ocular surface must be cared for with artificial tears.

Once the infection and the inflammation are under control, a lamellar corneal transplant can be contemplated, i.e. not invol‑ ving all the layers of the cornea, the endothelium and Descemet of the patient are preserved; only those layers of the cornea with more opacification are trans‑ planted (DALK). There is often the need to take antiviral agents orally for months to prevent its recurrence. We recommend being very careful and diligent with the care and treatment prescribed.

I am a 50 years old administrative assistant, I spend many hours in front of the computer and for a few weeks now I have been fee‑ ling heavy eyelids, tired eyes and discomfort, sometimes I feel gra‑ ting and at some given points I also have blurry vision. I have ne‑ ver had eye problems, but three months ago I was diagnosed an anxiety depressive disorder and was prescribed a treatment, however, these eye symptoms have me worried.

In the Mediterranean, its raise goes back to the origins of agriculture. They have played a key role in the diet of our ancestors and have left a profound mark in Spanish traditional cuisine.

Each region offers us its parti‑ cular version of stews, vegeta‑ ble soups, “ollas podridas”, and many other dishes. Currently, chickpeas, lentils and beans are the most consumed and popular pulses.

Amongst the great variety grown we can highlight those from Amuña, Salamanca, very tasty and big.
