The Community Health Improvement Plan (CHIP) is a way to document, monitor, and evaluate the community’s progress towards meeting set goals and objectives. The objectives noted in this plan are monitored over the three-year period. Annually, the CHIP is reviewed, and objective status is updated to indicate progress being made (or not). The organization, Achieve Healthy EscaRosa is our communities health focused collective impact initiative that works to identify and implement action plans to improve the health and wellbeing for all residents.
