FAITH360 NETWORK | NCCARE360 PARTNER GUIDE
What makes a good case note? A good case note should summarize the client’s situation and basic information for every interaction. This helps fellow CC’s determine the client’s needs and unique circumstances at a glance. Example: Jane Doe called in requesting assistance with a past due Duke utility bill in the amount of $300. She indicated that she lost her employment 2 months ago and has yet to obtain a new job. The Veteran expressed interest in getting help with employment resources. She receives 40% VA SCD and was able to use her savings to make her rental payment. Veteran is a single mother of 3 and expressed interest in local food pantries as well. CC Action: Sending food pantry resources via text. Referral submitted to employment support. Contacted VFW to request financial assistance with utilities.
Receiving Referrals The Coordination Center receives referrals from multiple sources. These include referrals that are sent directly from providers as well as “self-referrals” submitted by clients online. These referrals can be found on the main Dashboard and are labeled as Inbound referrals and Assistance Requests
Faith360 Network • 20
FAITH360 NETWORK | NCCARE360 PARTNER GUIDE
Inbound Referrals Submitted by a community provider for assistance with services. The referral information is given by the provider. The upper portion of the referral will identify the referral’s status, what organization sent the referral along with the service type and description. Below the description, there is an area to attach any documentation that may be needed for the referral. For example, a Veteran’s DD-214 to verify eligibility for Veteran specific programs.

Responding to Referrals Once we have received a referral for a need, we will need to contact the client to complete an Intake. This assessment helps determine what programs may be a good fit and if there are any co-occurring needs the family may need additional help with. By clicking on the Take Action Button in the referral below the uploads section we are given options on how to proceed with the referral. • Hold for Review - Placing a hold on the referral to give the CC extra time to request documents, contact providers, or follow up with the client. This moves the referral from “Needs Action” to “In Review” on the dashboard. • Send - To send the referral to another Unite Us provider or to send the referral as an Off-Platform case. • Accept - To accept the referral and begin working on the case. • Reject - The option to reject a referral back should be used sparingly. If a referral has no contact information at all, reject it back and request the referral be resent with updated information. Faith360 Network • 23

In the table below, you’ll see the primary role and features that are available for that role. Below the table, you can find descriptions of each feature. Every user needs one of the following roles and can be assigned only one:
