LOCATION: Greater Belfast area
Funded by Department for Communities, Reach Out supports those living within the Greater Belfast area whose lives have been impacted by emotional health and well-being issues, alcohol/substance use and self-harm. We offer a range of tailored supports that include oneto-one support, drop in acupuncture clinics, systemic practice, counselling, complementary therapy and specialist therapeutic interventions. This service is open to anyone on a drop-in or personal contact basis.

CRISIS INTERVENTION SERVICE
LOCATION: Belfast
Crisis Intervention is for people who have current and ongoing suicidal thoughts and behaviours and who have been assessed as moderate to high risk. The intervention will focus on de-escalating risk and putting safety measures into place. Crisis Intervention is a face-toface meeting, which will usually take place at the same time each week, offering continued support to reduce the suicidal risk to a low level. Once risk has been lowered, staff at the Extern Crisis Team will liaise with other agencies and refer you for further support, and/or to a GP if necessary. The service is suitable for those who have been assessed as being at moderate to high risk of suicide or serious harm, and is suitable for those aged 14 years and over.
The service is available via self-referral, or referrals from GPs or Community Mental Health Teams.

LOCATION: Belfast
The Family Support and Suicide Prevention Response Project works across the Greater Shankill area supporting people after a loss to suicide. The project is the lead in managing the local Community Response Plan which responds to families and community needs after a suicide has occurred. It delivers accredited and bespoke training and awareness-raising sessions across the local community, as well as home visiting, listening ear sessions and delivery of complementary therapy sessions.

COMMUNITIES IN TRANSITION
LOCATION: Belfast, Larne/Carrickfergus
The Communities in Transition (CiT) project works collaboratively with existing community-based projects to deliver a health and well-being project across Larne and Carrickfergus and Belfast’s Greater Shankill area. The project recruits and trains Community Health Champions, offers one-toone intervention to those impacted by drugs, alcohol and poor mental health, raises awareness of available services in the local area and develops resources and training for the local community. The CiT project accepts referrals from all sources, including self-referrals, from anyone aged 18-plus living in the Larne, Carrickfergus or Greater Shankill areas.

COMMUNITY CRISIS INTERVENTION SERVICE (CCIS)
LOCATION: Derry-Londonderry & Strabane
The Community Crisis Intervention Service (CCIS) is a community-led initiative which responds to individuals who are observed to be in distress and potentially vulnerable, and who may be at risk of suicidal behaviour. The service provides a timely (within approx. 30mins), non-clinical, community response to individuals experiencing social, emotional or situational crisis over a timeframe of 8pm on Thursdays through to 8am on Sundays. The service provides support and offers a short-term solution to manage immediate crisis.

EARLY YEARS

Early Years is a group-based programme for young people, aged between 8 and 12, who are experiencing difficulties within home, school and community. The ethos of our Early Years Programme is to enable each young person involved with the groups to become empowered to make informed choices, which will positively influence both their present situation and their future. For this purpose, the Early Years Programme encourages young people to work in conjunction with our group leaders to devise a programme which meets their individual needs in a group environment. PASSPORT
LOCATION: Derry-Londonderry
Passport is a community-based intervention programme offering support to young people aged 8-12 who are deemed at medium to high risk of experiencing difficulties at home, in school and in the community. The programme operates in a group setting and offers individual work, focused group work, daytrips and residential overnights. The purpose of the programme is to provide all the young people with the opportunity and support to develop essential life skills, experience new opportunities, participate in group activities, gain independence, and build resiliency and confidence in a safe and nurturing environment. KINNAHALLA YOUTH VILLAGE
LOCATION: Hilltown, Co Down
Kinnahalla Youth Village, sitting amid a picturesque landscape at the foot of the Mourne Mountains, creates a peaceful atmosphere ideal for the interventive work we undertake with individuals, families and groups. The site was acquired in 2001 and since then has been developed continuously in order to provide high quality accommodation and complementary support facilities. Young people visiting the youth village can avail of a bespoke residential programme specific to their needs. Our qualified and experienced staff deliver programmes aimed at developing social and interpersonal skills and independent living skills. These programmes are delivered in a fun and engaging environment, alongside a range of outdoor and indoor activities.

LINX PROJECT
LOCATION: South Eastern Trust area
The Linx Project is a community-based programme which builds upon the strengths of young people and their families to encourage healthy development, safety, stability and a positive future. We help young people and their parents/carers improve their relationships and we support parents to help their children develop confidence and resilience. ROSCOR YOUTH VILLAGE
LOCATION: Co Fermanagh
Located on the shores of Lower Lough Erne, Roscor Youth Village caters for children and young people through a variety of residential programmes. While at Roscor, children are given the opportunity to enjoy more than 50 fun water and land-based activities, which act as a conduit for the delivery of life-changing psychological and social care interventions by Extern’s experienced staff. Each young person attending is encouraged to develop social and interpersonal skills and, depending on age and ability, they take part in independent living skills development. I was referred to Extern’s FACES project at the age of 11 along with my siblings. We had just started a fostering placement, so this was to help us to settle into our new situation and get some extra support.
A few years later I joined the Pathways project to complete my education, and gained a range of qualifications including English, Maths and ICT.
As my journey with Extern progressed I was given a mentor to work alongside and they helped me look at the options I had. I decided to return to my former school and I am now starting to do my A-Levels. I even did work experience in the summer alongside the Extern staff which really boosted my confidence!
Throughout my time with Extern I struggled with my mental health and other issues I was facing in my family. But every time I needed help and support Extern were there to help and provide a listening ear.
Extern has given me so many special memories and allowed me to try new things in a safe environment. Working with Extern was a brilliant opportunity and helped me through some hard times in my teenage years.

SUMMER CAMP
LOCATION: Western Health and Social Care Trust (WHSCT)
Summer Camp is provided to young people aged 8-13 who live within the Western Health and Social Care Trust region and have been identified as socially excluded, live in an area of high deprivation or are deemed a child in need. Based at the Roscor Youth Village in County Fermanagh, children can participate in a four-day 'holiday' where they can engage in a range of recreational activities available on the site, whilst also developing social and recreational skills. The Summer Camp runs throughout July and August and supports over 200 children every year.

Strength 2 Strength is a community-based intervention provided to children, aged 8-13, and their families, who are assessed as being in need. The Janus Project is a community-based programme with a residential element, which offers intensive support tailored specifically for young people, who may be experiencing crisis or have been impacted by trauma. The aim of the service is to nurture healthy development, safety and stability, and a positive future for the young people involved. It also builds their capacity and resilience, provides emotional and practical support and develops life and social skills. The project provides individually-tailored support packages to young people aged 11–17 years in the South-Eastern Trust area. INTENSIVE SUPPORT FOSTERING
LOCATION: South Eastern and Belfast Health & Social Care Trust Areas
Intensive Support Fostering is a specialist fostering scheme for young people aged 10–18 who present with highly complex needs and challenging behaviour. Difficulties often stem from early developmental trauma, and as such they cannot be placed within mainstream fostering or residential facilities. We have a multi-disciplinary team consisting of social workers, Extern project workers and a clinical psychologist. Our project worker works with the young person to develop an individual and goal-oriented programme of support, designed to provide opportunities to build on their strengths and aspirations. TIME OUT PROGRAMME

Down
The Time Out programme offers respite, safety and support to children aged 8-17 and their families. Working in partnership with social workers, the service responds to crisis and offers a sanctuary for young people who need time in a safe place where they can reflect and feel valued. Time Out provides the social worker and carers with time to identify the stressors and to connect with support. Time Out staff are trained in a range of skillsets and are highly attuned to what works when providing support to children who have experienced trauma. All referrals are via social work departments.

The Youth Engagement Service (YES) is funded by the Public Health Agency. Extern's Belfast and Bangor YES projects have successfully been engaging with young people on behalf of Extern since 2016. Both projects offer a safe social space for young people aged 11-25 within which a confidential health and well-being service is offered. The service provides advice, information and support on any youth-related issue. Key areas of frequent support include mental health and well-being, sexual health, problem substance use, education, employment and advice and signposting. Each project has its own weekly programmes, including age specific drop-ins, events and needs-led activities. There is also an outreach element to the service, which is carried out through engagement with other youth and community groups and a variety of educational settings. Engagement in the service is voluntary and we welcome contact from anyone who wants to know more about the service or to get involved with either project.

I was really struggling in my life when I first heard about Extern. I was facing feelings of anxiety, having low moods and was just so stressed out about things.
A friend put me in touch with Extern and from the very beginning I felt safe and supported. My first meeting with the team had a real sense of energy, of understanding and importance, which helped immensely.
The guys at the Youth Engagement Service provided a place for me where I could speak honestly about what was going on in my life.
Just having that space to share and talk helped me feel heard and less alone in my problems. This helped me come to terms with my own feelings and get advice on what I needed.
I understand now how to better handle issues in my life and have more of an insight into healthier coping mechanisms. Reaching out to Extern, even if it is just for a chat, can make all the difference if you are struggling like I was. I would recommend it to anyone.

Extern’s Alcohol Housing Support Team works with adults who are at risk of homelessness as a result of their chronic/severe alcohol use. Referrals to the service can be made by any person who wants help, a family member or a concerned professional. Engagement is entirely voluntary and the person can withdraw from the service at any time. Extern will work together with the referred person to agree a plan to address alcohol-related issues. With agreement, there may be liaison with other agencies e.g. addiction, housing, benefits etc. to assist.

BDACT CONNECTIONS

The Public Health Agency funds a Connections Service in each of the Health Trust areas in Northern Ireland, whose role is to take forward local campaigns and initiatives in relation to drug and alcohol issues. Extern hosts the Connections team for the Belfast area. The Connections Service is the operational arm of the Drug and Alcohol Co-ordination Teams (DACTs), which are multi-agency partnerships that set local priorities and deliver a drug and alcohol implementation plan across each Trust area. DRUGS & ALCOHOL CONSULTANCY SERVICE (DACS)
LOCATION: Northern Ireland
Extern DACS builds on Extern’s expertise in supporting people living with problem drug or alcohol use, gained in over 20 years of delivering services in the homelessness and addictions fields in Northern Ireland. Extern is now the largest provider of drug and alcohol support services in Northern Ireland. DACS provides consultancy services, alongside specialist training commissioned by the Public Health Agency, for particular groups. These courses include Naloxone Administration: Opioid overdose response; Naloxone: Training for trainers; Reducing Injecting-Related Harms and Needle and Syringe Provision. We provide drug-related information resources such as on naloxone, pregabalin and an assessment of injecting drug use.

DRUGS ACCOMMODATION SUPPORT PROJECT (DASP)

The Drugs Accommodation Support Project (DASP) supports IV drug users to stabilise their lives by accessing temporary supported accommodation with a specialist worker. Through the programme, Extern supports active drug users in moving from the streets and into stable supported accommodation and addiction treatment. This service is provided with the help of partners in the Housing Executive, Belfast Health and Social Care Trust, Public Health Agency, PSNI and DPP. Referrals to this service can be made directly by individuals or from any source. Service users must have an injecting drug issue and be homeless to access the specialist support.

NEEDLE AND SYRINGE EXCHANGE
LOCATION: Belfast, Lurgan, Ballymena
The Needle and Syringe Exchange is a free, confidential service for people who inject drugs. Those engaging with the service can avail of new, sterile injecting equipment which reduces the risk of catching viruses, like HIV and hepatitis B and C, which are spread by contact with infected blood. The service also disposes of used equipment safely, which reduces the risk of other people coming into contact with these viruses via used needles or syringes. Those who inject drugs of any sort – including opioids, steroids, stimulants and tanning solutions - can use a needle and syringe exchange service. The service also offers advice, information and support to help reduce risk of harm from injecting, as well as assisting with access to other areas, such as housing, alcohol use, mental or physical health problems.

This drug and alcohol service works with people at whatever level they wish in a flexible service user-led way. Support can be short-term or longer term depending on the individual needs, reducing the harm that alcohol and/or drugs are having on the person. The service’s open referral criteria is deliberately wide to allow people easy access to support; it works with people at their point of need and keeps expectations of engagement realistic. The service offers support in the home, or in a venue that people feel comfortable in.

A community-based early intervention service for adults and family members affected by substance misuse. For individuals who are not alcohol dependent but wish to reduce their intake, we offer:-
Extended Brief Interventions – a prevention approach that supports at-risk drinkers to make an informed choice to change their alcohol consumption and learn skills that will allow them to consume alcohol in a safer way.
Therapeutic Interventions - This is for harmful substance and alcohol users and delivers Enhanced motivational interviewing; Cognitive behavioural therapy; Relapse management therapy.
Family Support - Any adult affected by a family member’s alcohol and/or drug use can avail of this one-to-one service independently, whether or not the family member has engaged in counselling or other services.

STEP 2 SERVICE
LOCATION: Northern Trust Area
The Step 2 Service offers a therapeutic outreach service supporting people impacted by substance misuse and their families. This service is designed for adults (18+) who are impacted by patterns of alcohol or substance misuse. Individuals will be supported by a highly-trained and dedicated team to achieve agreed goals in relation to substance misuse, with the aim of reducing or abstaining from substances. In addition to this, it provides support to family members impacted by an individual’s substance use. The service operates on an open referral system, meaning anyone can refer in.

STREET INJECTORS SUPPORT SERVICE (SISS)
LOCATION: Belfast City Centre
The Street Injectors Support Service (SISS) uses experienced staff to assertively engage with street injectors in Belfast and to collect and dispose of inappropriately discarded injecting equipment. Funded by the Public Health Agency and Belfast City Council, the project was established in 2017 by Extern in response to a rise in discarded injecting paraphernalia and people who utilise the Belfast City Centre area to inject heroin. The service also provides a vital link to vulnerable drug users by assisting with access to temporary accommodation and healthcare services.

EXTERN PROBLEM GAMBLING

The Extern Problem Gambling project supports people who have been negatively impacted by problem gambling, through providing supports, resources and referral pathways for problem gamblers and their families. The project advocates on behalf of those suffering the consequences of problem gambling, and highlights gambling-related issues in the media. It also actively lobbies for the introduction of fit-for-purpose gambling legislation and regulation in Ireland.

MUST HOSTEL

Mid-Ulster and South Tyrone (MUST) Hostel offers accommodation, support and help, tailored to the specific needs of single people who are homeless and aged 18-65. The hostel aims to deliver a modern supported housing service which is flexible and has a holistic approach to the individual in order to meet service users’ needs. The hostel has 20 beds and accepts both males and females. MUST Hostel is also a Probation Approved Hostel and works with PBNI to supervise those who are subject to supervision under licences or other restrictions.

SAIL PROJECT

The SAIL (Supporting Adults for Independent Living) Project is a Supporting People-funded floating support service and was established by our MUST Hostel in Cookstown, with the aim of enabling people to live independently within the community and to help people to sustain their tenancies. The project offers support and advice to those experiencing homelessness and to those who are at risk of becoming homeless within the Mid-Ulster and South Tyrone area. It offers housing-related support tailored to the specific needs of each client, adopting a flexible and holistic approach. EXTERN HOMES
LOCATION: Nothern Ireland
Extern Homes provides safe and stable places to live for people who are homeless, or who are facing homelessness, by purchasing and refurbishing houses, through which it offers a tenancy period of up to two years. During that time tenants are offered and provided with tenancy support and engagment with local advice services, as well as access to training and employment opportunities, in order to empower them to move on with their lives. Following this, Extern helps tenants to secure longer term accommodation, based on having developed a positive tenancy history, and having been encouraged to save for a future tenancy deposit.
Potential tenants are nominated via statutory and/or voluntary sector organisations e.g. social services, homelessness organisations, support services, housing agencies.

REFUGEE FLOATING SUPPORT
LOCATION: Greater Belfast
The Refugee Floating Support Service offers support to individuals and families who have been granted refugee status and who have been given leave to remain in the UK. Operating since 2013, the project works intensively with refugees during the 28-day notice period given to move out of NASS accommodation, and provides medium to long term specialist and practical support and advice to resettle in the community. The service aims to prevent homelessness, and to maintain tenancies, by directly supporting individuals who have housing issues and practical problems settling in the local community. This service can be accessed through open/self-referral.

FLOATING SUPPORT
LOCATION: Northern Ireland
Floating Support delivers a flexible and responsive service to medium to high risk ex-offenders living in the community. The service is funded by Supporting People and aims to enable these individuals to make the transition from prison to the community, obtain suitable accommodation and maintain their tenancies. The service also supports individuals to maintain positive mental health, counter social isolation, and, importantly avoid further offending. Interventions are based on the Good Lives model, building on strengths and tailored to the individual’s needs. HOMELESS TRAVELLER PROJECT
LOCATION: Greater Belfast
This project provides intensive support to members of the Traveller Community, both individuals and families, in relation to housing and homelessness, tenancy sustainment and link-up with statutory health and social care services around multi-complex needs including mental health, family and childcare issues, addiction, offending, and physical health. This project operates on a multi-agency collaborative model and is part of the Traveller Support Network for Greater Belfast. The service can be accessed through open/self-referral.

ALCOHOL HOUSING SUPPORT
LOCATION: Belfast
Extern’s Alcohol Housing Support works with adults who are at risk of homelessness as a result of their chronic/severe alcohol use. Referrals to the service can be made by any person who wants help, a family member or a concerned professional. Engagement is entirely voluntary and the person can withdraw from the service at any time. Extern will work together with the referred person to agree a plan to address alcohol-related issues. With agreement, there may be liaison with other agencies e.g. addiction, housing, benefits etc. to assist.

ORMEAU CENTRE
LOCATION: Greater Belfast
Operating since 1978, the Ormeau Centre offers support to men and women (18+) experiencing homelessness. It operates an open, non-judgmental referral policy and is available 24 hours a day, 365 nights a year. The centre offers person-centred support, individually-tailored support plans, crisis intervention around complex needs, specialist IV drug accommodation, and referral and signposting to other agencies, both statutory and voluntary. Referrals are made from the Northern Ireland Housing Executive.

MDHST is a Belfast-based outreach team working in conjunction with the Northern Ireland Housing Executive and Belfast Health & Social Care Trust to provide assistance, support and advocacy, to adults and families who are homeless or at risk of homelessness and have complex additional needs, including diagnosed and undiagnosed mental health, substance misuse, physical health issues, offending backgrounds, learning disability and family issues. The team also provides a complex needs floating support service.
Since 2018, MDHST has expanded into the South Eastern Trust area, offering an expansion of the service delivered in Belfast. It also offers support through dispersed projects in the following areas: North Down and Ards, Newcastle and Enniskillen. (Referrals for these dispersed projects are a closed pathway for NIHE.) MDHST can be accessed through open/self-referral.

Extern currently operates two facilities for those who have been through the criminal justice system. The Innis Centre and Dismas House provide accommodation, supervision and monitoring, for people within the criminal justice system, so that they can be positively reintegrated into the community. Working closely with Probation Officers, staff provide support and supervision until appropriate, permanent accommodation has been identified and agreed. Staff in both Innis and Dismas provide housing support and assist service users to access services and improve their lives with regard to independent living skills, finance, physical and mental health and substance abuse. Referrals to this service are made via Probation Board for NI.

Due to depression and anxiety I found I couldn’t cope with keeping on top of things at home. I have difficulty doing even everyday things like cooking, shopping or budgeting. I wasn’t coping well at all, so I had to give up my tenancy.
I went to a hostel but found it too chaotic, so I applied to go to Extern’s MUST hostel in Cookstown. Things were much more structured there for me. There is a lot more support from the staff, and they spend a lot of time just talking to you and helping you get the support you need.
I feel a lot more stable and safe at MUST. There is always someone around to talk things through with or get you the help you need in order to move forward.
I always feel understood and not judged by the staff. They’ll always provide the help and support you need if you are willing to work with them.

FLOATING SUPPORT
LOCATION: Northern Ireland
Floating Support delivers a flexible and responsive service to medium to high risk exoffenders living in the community. The service is funded by Supporting People and aims to enable these individuals to make the transition from prison to the community, obtain suitable accommodation and maintain their tenancies. The service also supports individuals to maintain positive mental health, counter social isolation, and, importantly avoid further offending. Interventions are based on the Good Lives model, building on strengths and tailored to the individual’s needs. I had survived domestic, sexual, and mental abuse. Then I lost both my parents in traumatic ways, and my life fell apart. I froze, as a person, in mind, in living, I was barely surviving. I couldn’t speak.
I tried everything, even suicide attempts. I was begging for help, yet no one could hear me. My own self destruction was all I could focus on. Depression suffocated me, blinded and deafened me, for those years.
I committed a crime, and was given probation for a year. I was asked when was the last time I had spoken to anyone. I was shocked that it had been almost four years.
Then I was introduced to Extern. They had to take a person who didn’t have hope, who didn’t know how bad or mentally ill she was, and break 30 years of abusive behaviour. At that time, I was being evicted from my family home. Extern found me a place to live, yet stayed with me to make sure I could sustain it.
I owe my life, my self-worth, my health, my hope, my beliefs – and most importantly I owe my home, my journey to recovery and my journey to living - to Extern.

REFUGEE FLOATING SUPPORT
LOCATION: Greater Belfast
The Refugee Floating Support Service offers support to individuals and families who have been granted refugee status and who have been given leave to remain in the UK. Operating since 2013, the project works intensively with refugees during the 28-day notice period given to move out of NASS accommodation, and provides medium to long term specialist and practical support and advice to resettle in the community. The service aims to prevent homelessness and to maintain tenancies by directly supporting individuals who have housing issues and practical problems settling in the local community. This service can be accessed through open/self-referral.

NI REFUGEE RESETTLEMENT SERVICE
LOCATION: Northern Ireland
The service provides key workers to support individuals and families arriving in Northern Ireland who have been forced to flee their homes and who have been granted refugee status and given leave to remain in the UK. Staff work intensively with individuals and families from point of arrival, providing holistic assessment of needs, practical support and link-up with statutory services to help families resettle in NI. This service is provided in partnership with Barnardo’s NI, Bryson Intercultural and the Red Cross. Referrals to this service are made via the UK government, who identify families for resettlement.

HOMES FOR UKRAINE
LOCATION: Northern Ireland
Homes for Ukraine is a Government scheme developed in 2022, in response to the Russian invasion of Ukraine. Extern is one of the delivery partners responsible for screening potential ‘hosts’, who will offer accommodation to individuals who are fleeing conflict. This service is offered in partnership with Barnardo's and supported by other external agencies including government departments within Northern Ireland. Applications can be made via NI Direct, and allocations are made via local government agencies.

MOVING FORWARD, MOVING ON (MFMO)
LOCATION: Belfast
MFMO works to support the economically inactive and those not in education, training or employment.The project provides a mentoring support service for those who have previously been through alternative education projects, such as Extern’s Pathways Project, as well as ensuring that young service users have employability skills. Referrals to the programme can be made from alternative education projects, statutory agencies, and organisations including social services, the PSNI or the Youth Justice Agency. Self-referrals are also possible.

PRACTICE DEVELOPMENT UNIT (PDU)

Three services make up the external service provision of the PDU, funded by the NI Social Care Council. The Voluntary Organisations Communication and Learning Service (VOCALS) represents and advocates for the voluntary sector in all aspects of social work education. The Extern Practice Learning Centre provides 12 social work placements per year to students. The Extern Professional in Practice (PiP) service provides post-qualifying training and assessment to social workers working outside of statutory agencies.
