plans. Effective 04/01/26 in partnership with Luminare Health Benefits, we are excited to be the administrator of the Carahsoft Technology Corp. Employee Benefit Plan. This letter contains valuable information about your medical and prescription plans.

Carahsoft Technology Corp is pleased to provide you with Cigna as the PPO Network for your medical plan. Cigna PPO is a national network of nearly 800,000 health care professional and 6,000 hospitals. To verify a provider or facility’s participation, please visit Cigna’s website: https://www.cigna.com.

To verify benefits, you or your provider may call EVHC at 1-800-311-3842. Please note, your plan still requires authorization prior to an elective hospital admission, or within two working days of an emergency admission or prior to receiving home health care. For authorization, please call EVHC at 1-800-311-3842.

Failure to follow this requirement may result in a reduction of benefits.

You will receive a new, combined medical/prescription ID card which contains important information specific to your plan. Please be sure to show this new card to all your medical providers and pharmacists. Your ID card will have a unique medical identification number in place of your Social Security Number. If You Need Care Find a network Medical provider.

How do you find one?

You can search for a medical provider three ways, if you are already registered you can log in to your personal MyEVHC.com portal and select to search Cigna providers under My Links tab. If you are newly enrolling and have not yet received your ID number to register; you can go directly to either www.MyEvhc.com and at the bottom of the home page, click on the Cigna logo. Or you can go directly to www.cigna.com . Once on Cigna’s provider search page, select plans offered through work or school. Enter your City and State, choose your Medical Plan: PPO, Choice Fund PPO and narrow your search filter by: Physician name, facility, specialty, location and other options. You can get additional information about doctors, understand your coverage and learn which services may require approval.

Take advantage of network care.

Network doctors, mental health professionals, hospitals, clinics and laboratories charge discounted rates, which typically saves you money. Even if your plan allows you to receive care outside of your network, be aware that it could cost you more.

Need hospital care?

Talk to your doctor first to determine which hospital can meet your medical or surgical needs. If you are admitted to the hospital for an inpatient stay or having an upcoming in-patient admission, you or your provider will be required to notify EVHC before you’re admitted.

Choose a primary care physician (PCP).

Although your plan may not require you to choose a PCP, it’s a good idea to have one main doctor with indepth knowledge of your health to help guide you on the best path of care. Find one at myEVHC.com or call the toll-free member number on your ID card.

Schedule your preventive care screenings.

Most EVHC plans pay 100 percent of the cost of certain preventive care services with a network provider. Check your heath plan documents for details.

After You Receive Care Know How Claims are Paid When you see a network doctor: Claims are submitted for you, and you may be asked to pay a copay before you leave. Evolution Healthcare will process the claim to: o Determine whether the claim was for eligible preventive care, so it can be paid 100 percent o Make sure it’s an eligible expense under you plan o Make sure the service is charged at the discounted network rate

When you see an out-of-network doctor: If your plan allows visits to out-of-network providers, you may be asked to pay some or all of the bill before you leave. o If the doctor doesn’t submit your claim, you’ll be responsible for doing it. o Find medical claim forms and instructions on myEVHC.com o Remember, discounted rates don’t apply to out-of-network doctors so you may pay more.
