Whether you need to be connected with community resources, trusted in-home caregivers to help you live independently or access to programs to monitor and improve your wellness, EverTrue’s Community Impact programs are here for you.

EverTrue Answers
EverTrue Answers offers guidance on how to access community resources that may be able to assist with housing, food, transportation and much more. You can access a wide range of advice and support through one of the many EverTrue Answers programs:

EverTrue Answers is a free service that connects people with services and resources available at little or no cost.

Care Management
Services are available to you at no cost if you meet financial eligibility requirements. Services include a holistic in-home assessment to identify your needs and goals as well as resource education and connection to EverTrue services and other community resources.
Services include:
• Identify your needs and goals through a holistic in-home assessment
• Address concerns about remaining independent at home
• Evaluate the safety of your home and discuss ways to promote independence
• Connection to spiritual support services if interested
EverTrue Answers is a free service that connects people with services and resources available at little or no cost.
Service Areas: St. Louis City and County, Jefferson County, St. Charles County and Warren County
In-Home Assistance
If you wish to live independently at home, safely, but require a little extra help, you can access assistance at a lower cost, making independence more affordable.
Good Neighbor Program
The Good Neighbor Program makes it easier for older adults to access the support they need at home. Through financial assistance, the program helps cover non-medical services provided by EverTrue Home Help, including personal care, laundry, light housekeeping and meal preparation. In addition to these essential services, clients also benefit from person-centered care — ensuring that any changes in their needs are promptly identified and addressed.
Circle of Care
The Circle of Care program identifies each participant’s needs including medical diagnosis, treatment and follow-up plans as well as coordination of medical resources and comprehensive care plans for short and long term care needs. Based on this, caregiver services are provided to tend to your daily needs such as personal care, escorted transportation, light housekeeping, medication management, diabetic management, nutritional support, clinical oversight and companionship.
Benevolent Care
EverTrue’s Benevolent Care program is here to help ensure older adults continue to live with dignity as you navigate life’s changing needs.
Benevolent Assistance
Provides financial support for uncovered needs such as medical equipment, incontinent supplies and nutrition supplements. Utility assistance may also be available. You work with care management services to evaluate opportunities to be connected with other assistance programs for further support.
Benevolent Care
Contributions serve residents in EverTrue life plan communities who, through no fault of your own, have outlived your financial ability to pay.
Eliezer Fund
You dedicated your life to church or mission work. Now we want to reciprocate that love and care with financial assistance through the Eliezer Fund. Assistance with in-home services, durable medical equipment, medication, and more can be provided. Income and asset guidelines do apply, and award amounts vary depending on the services needed.
Named for Abraham’s most trusted servant, the Eliezer Fund honors your dedication to the Word, including if you served as clergy or a lay employee. This fund helps subsidize the costs of goods and services for church workers and missionaries, both active and retired from all Christian denominations.
Wellness Resources
Having access to wellness resources can be the difference in managing your health and getting care before a medical issue becomes a medical crisis.
Homer Schmitz Memorial Health Clinic
The Homer Schmitz Memorial Health Clinic is saving lives by managing the care of high needs residents at specific EverTrue Affordable Housing communities. If this applies to you, this clinic provides on-site, walk-in healthcare services at no cost, so you can get medical services before major health issues develop.
Project Wellness
Project Wellness empowers older adults within our Affordable Housing communities about their health. Through a combination of having vital signs checked and education/support from a Wellness Nurse, you can be better informed of and in more control of your wellness.
The Healthy Foods Program
This program provides eligible older adults with healthy groceries, nutrition education and community resources. The Healthy Foods Program offers grocery deliveries that are specially curated to your nutritional needs. When groceries are ordered, you also receive nutrition guidance and simple, healthy recipes to help you make the most of this program. In addition, The Health Foods Program offers access to benefit counselors who can help you apply for SNAP and other programs to support you.
