As a public hospital district, EvergreenHealth provides a special benefit to the greater community. Every year, the organization is entrusted as responsible investors of the community’s resources, to use tax levy funds to meet important community health and wellness needs. In 2021, more than $6.8 million in levy funds were invested back into community health through a range of programs designed to provide quality health care, services and resources—often at no charge to the community.

EvergreenHealth 24/7 Nurse Navigator & Healthline
EvergreenHealth’s Nurse Navigator & Healthline continues to offer a levy-funded resource to the community by providing 24/7 on-call nurse support, as well as help with scheduling and referrals and registration for health programs and classes.
In 2021, Healthline served callers in the following ways:

Healthline’s call center team, with resources and support from Microsoft, handled a total of 97,074 calls related to COVID vaccines and information, including: 74,297

Providing Education and Resources for Living Well
EvergreenHealth Community Education Services
EvergreenHealth’s Community Education Services provides hundreds of free and low-cost seminars, workshops and health education classes each year that offer resources to community members for living well. Whether it is cooking demonstrations to inspire nutritious eating, exercise groups, emergency preparedness or parenting classes, the health system offers a wide range of curricula for the community’s benefit to address diverse needs and interests. Among the year-round programs, EvergreenHealth’s Childbirth and Early Parenting Education resources offer dozens of classes to help families navigate the joys and challenges of parenting. Its Parent-Baby Groups provide education and support through facilitator-led discussions that encourage new families to share successes and challenges on a range of topics, including feeding, relationships, postpartum adjustment, developmental stages, sleep and more.

Removing Barriers to High-Quality Health Care & Social Services
IN 2021, CHAT STAFF SUPPORTED THE HEALTH AND WELL-BEING OF THE COMMUNITY BY:
Community Healthcare Access Team (CHAT)
The levy-funded Community Healthcare Access Team (CHAT) serves low-income, underinsured and uninsured residents throughout the Puget Sound region, with the help of a range of community partners, by connecting them with resources for insurance, financial assistance, behavioral health and many other forms of assistance.
CHAT staff are specifically trained in Medicaid and Medicare eligibility, are certified as navigators for Washington’s Healthplanfinder and provide connection and insight into community resources designed to support residents’ well-being. CHAT also continued its partnership with the Lake Washington School District (LWSD) by providing a dedicated social worker employed by EvergreenHealth to support students and their families with mental health resources.

Supporting Eastside Seniors and Caregivers
EvergreenHealth Geriatric Care
EvergreenHealth Geriatric Care is the only practice on the Eastside dedicated to exclusively serving families and patients who are 65 years and older with specialized care without requiring a referral. Just as pediatricians focus on the health needs of children, geriatric care providers focus on the complex health needs of seniors. Working in partnership with an interdisciplinary team of board-certified specialists, Geriatric Care providers include geriatricians, nurses, social workers and pharmacists—all with special training and experience assisting seniors. The Geriatric Care team provides comprehensive assessments, evaluating patients’ medical conditions, cognitive status, functional ability and mental health needs to determine the most suitable treatment for each person. This experienced team also works hand-in-hand with each patient’s family, and coordinates with other health care providers to create an individualized care plan designed to empower seniors with choice and independence.

Understanding Our Community’s Health Needs
King County Community Health Needs Assessment
EvergreenHealth participates in King County Hospitals for a Healthier Community (HHC), a collaborative of 10 hospitals/health systems in King County, that includes Public Health – Seattle & King County. HHC produces a Community Health Needs Assessment (CHNA) that analyzes community inequities, strengths, and health needs. The CHNA presents community-identified priorities, a detailed description of the

Northshore Senior Center
In 2021, EvergreenHealth continued its partnership with Northshore Senior Center (NSC), a Bothell-based nonprofit which provides resources, programs and services for Eastside seniors. After pivoting to a largely all online format in 2020, NSC slowly began to safely return to in-person engagement for many of its classes and programs in 2021. In addition to their well-attended virtual caregiver support groups NSC offers a variety of classes and programs that help keep participants active and connected. Among its programs, NSC offers Enhance©Wellness, an evidence-based, health management program designed to help participants set goals and learn strategies for making positive choices to improve their physical and/or emotional well-being.
IN 2021, EVERGREENHEALTH’S PARTNERSHIP WITH NORTHSHORE SENIOR CENTER:
Served 245 family caregivers through
927 touchpoints via programming, consultations and services.
The NSC Adult Day Health program offered 84 virtual health and wellness classes,
to encourage movement, fine motor skills, strengthening and other rehab services.
Helped 238 older adults maintain and improve their mental and emotional well-being through support groups, personal consultations, meditation, conversation groups and more. As a full-time caregiver for her husband who lives with frontotemporal dementia (FTD), Jeanne Clark has been a long-time participant in many of Northshore Senior Center’s support groups and educational programs. When the active 80-year-old began suffering from significant fatigue and muscle weakness, largely because she was eating fewer meals as she focused on her husband’s care needs, the Social Services/ Wellness team at NSC suggested she explore the Center’s Enhance©Wellness program.

I had struggled with making progress on my own, but my Enhance©Wellness coach gave me the support and structure to follow a plan that works. It is a self-directed program, so I have to put in the work, but Lydia is there to regularly check-in to provide support and encouragement as I work toward my goal of regaining strength and energy. It has been a life saver.”

community, analyses of data on life expectancy and leading causes of death and chronic illness rates throughout King County. The results of the CHNA compiled by HHC informs EvergreenHealth’s community programming. EvergreenHealth has chosen Access to Care, Youth Mental Health, and Chronic Disease Management as areas of focus for our community programs, responding to identified needs and strengths found in the CHNA data.
Resources for Responding to Students in Crisis
Youth Mental Health Programs
EvergreenHealth’s levyfunded Youth Mental Health programs provide support and resources to help community members identify solutions and take action to promote awareness of the mental health challenges facing young adults.



With the support of community levy dollars, EvergreenHealth’s Youth Mental Health Task Force brings together youth, other organizations, and community members who share a passion for supporting young adults. The task force meets regularly for inspiring discussions that encourage the exchange of information and resources among community members and foster grassroots solutions. The task force continues to expand its reach beyond EvergreenHealth, forging partnerships with other like-minded community organizations to develop programming and resources. In 2021, EvergreenHealth continued to partner with NAMI Eastside, the local affiliate of the National Alliance on Mental Illness, to host monthly hour-long forums on educational topics geared toward youth and community members who work with young people. Expert guest speakers presented a range of topics, including mindfulness, suicide prevention, bipolar disorder, childhood trauma and many others.
“With EvergreenHealth’s support, we are bringing needed programs into Eastside schools, including Youth Mental Health First Aid trainings, which provide participants with tools to help adolescents experiencing a mental health crisis or addiction challenge. We are also piloting an evidence-based program, Cope2Thrive, which is designed to help children, teens and young adults deal with anxiety, stress and depression by teaching them how to develop the skills needed to stop negative or unhelpful thoughts and start thinking, feeling and behaving in more positive ways. These are just a few of the things that have been possible to help support the young people in our community thanks to our partnership.”

Support for Northshore School District Students
Grant Funds Full-Time Mental Health Therapists
EvergreenHealth levy dollars continued to provide funding for full-time, onsite mental health therapists for each high school in the Northshore School District who support students referred by high school guidance counselors. In 2021, the district’s mental health therapists met with students individually and in small groups to address their needs.

Advancing STEM Education
Partnering with local schools
With help from the community’s levy dollars, EvergreenHealth partners with local school districts to provide middle school and high school students with information and education related to Science, Technology, Engineering and Math (STEM). EvergreenHealth’s nine-year partnership with Juanita High School’s Global Health STEM program continued in 2021, which provided opportunities for EvergreenHealth leaders and physicians to present virtually as guest speakers on important health care topics. The interactive sessions provide students with the insight into health care-related career fields and encourage them to ask questions and immerse themselves in learning about career paths in science, health care and many more industries.
In 2021, EvergreenHealth speakers joined Juanita High School Global Health STEM for 9 educational sessions.
EvergreenHealth Financial Report
In 2021, EvergreenHealth continued investing in several initiatives to expand and enhance patient care while also responding to the ongoing COVID-19 pandemic. The organization’s investments to upgrade the health system’s infrastructure and facilities reflect EvergreenHealth’s commitment to grow alongside the community in order to meet patients’ and families’ evolving health and wellness needs. EvergreenHealth continued to receive CARES Act funds in 2021 and realized a net income of $11.6 million, while also investing $34 million in community benefit services including charity care and subsidized health services. It also contributed $51.7 million toward significant capital improvement projects across the EvergreenHealth system.

This focus on quality has resulted in notable awards, including being named one of America’s 50 Best™ hospitals by Healthgrades again, placing EvergreenHealth in the top 1% of hospitals in the nation for clinical excellence. This honor comes as we celebrate our 50th anniversary of serving this region. As we embark on our next 50 years, your support has never been more important. The Foundation is a vital partner with the health system in helping to fund critical programs and services. In 2021, donors contributed more than $3 million to help fund the buildout of a new Intensive Care Unit in EvergreenHealth’s Silver Tower. The Heart Care Center benefited from more than $400,000 in support to construct and equip a cardiac catheterization lab, and nearly $500,000 was invested in our Hospice Care Center. Thank you for helping EvergreenHealth meet the needs of our growing region! You may have noticed a new face at the Foundation. This spring, I joined the team as the Chief Philanthropy Officer.
