2015 was another remarkable year, built upon the 42 years that preceded it and paved the way for 2016 and beyond. While we are proud to have been named again among the top 5 percent of hospitals in the nation for the seventh time the last eight years in 2016—a truly remarkable recognition—we are even prouder of the difference we make in the lives of more than 3,000 patients every day.

Keeping Promises: Partnering With the Community to Live Our Healthiest Best In 2015, EvergreenHealth providers, staff and volunteers had the privilege of serving the nearly 850,000 residents of King and Snohomish counties with a breadth of clinical care and service that is among the most comprehensive in this region.

Caring for our community is a profound responsibility and we think it is best achieved through collaboration. This past year, we continued to expand our reach through our partnerships with the Puget Sound High Value Network, Virginia Mason, Seattle Cancer Care Alliance, our sister hospital EvergreenHealth Monroe, and our clinically integrated network EvergreenHealth Partners. Together with these like-minded organizations, we can achieve both the greatest impact and the greatest value for our patients.

As we’ve continued to grow with our surrounding communities, EvergreenHealth has maintained a special relationship with our patients and residents as your hospital and as a public hospital district. Your levy funds, while they represent a small portion of our revenue sources, help us meet the community’s health needs where they are greatest, and 2015 brought significant services to our friends and neighbors who count on us for health care and beyond.

2015 Highlights eHomeHealth Platform Helps Keep Patients Healthy at Home EvergreenHealth Home Care, thanks to funding from donors at the annual Evergreen Gala, launched an eHomeHealth service—a new software platform that helps patients track health information at home and share it virtually with their care teams and family members. This technology complemented the 129,000 in-home visits by 265 clinicians in 2015.

Treatment Options Expand at Halvorson Cancer Center Patients at the Halvorson Cancer Center, a partnership with Seattle Cancer Care Alliance (SCCA), benefit from the installation of a new Varian True Beam linear accelerator for radiation oncology; increasing the number of stereotactic body radiation therapy (SBRT) treatments received. SBRT delivers targeted radiation doses to tumors with millimeter precision – meaning less healthy tissue is damaged by radiation.

Fairfax Behavioral Health Opens in Monroe EvergreenHealth Monroe formed a partnership with Fairfax Behavioral Health for a dedicated 34-bed inpatient adult psychiatric center on the EvergreenHealth Monroe campus.

Board Approves $425 Million, 10-year Expansion and Renovation Plan In July 2015, the Board of Commissioners formally approved a 10-year long-range master facilities plan that includes the build-out of existing infrastructure and opening new facilities to allow EvergreenHealth to better accommodate and care for patients, while serving the increasing needs of our community.

Best Hospital by 425 Magazine

Readers of 425 magazine voted EvergreenHealth “Best Hospital” in the 425 area code in 2015 for the third time.

The annual “Best of 425” poll recognizes organizations that are voted as the most preferred by local consumers.

Puget Sound High Value Network (PSHVN) Selected by State The Washington State Health Care Authority, which is responsible for purchasing health care for more than two million Washington state residents, selected the PSHVN (of which EvergreenHealth was a founding member) as one of just two Accountable Care Programs (ACPs) for eligible state employees’ health coverage.

Top 5 Percent of Hospitals For the Sixth Time in Seven Years

EvergreenHealth earned recognition as a Distinguished Hospital for Clinical Excellence by Healthgrades for the sixth time in the past seven years, placing the organization in the top 5 percent of hospitals nationwide for its quality patient care and superior clinical performance.

Valley General Hospital Officially Becomes EvergreenHealth Monroe In March, EvergreenHealth Monroe cemented its alliance with Kirkland-based EvergreenHealth, sharing a commitment to enhancing care and service for east Snohomish County residents and the greater Sky Valley community. Among its 2015 accomplishments, EvergreenHealth Monroe partnered with Emergency Medical Services Group, the same ER physicians that serve EvergreenHealth’s emergency departments in Kirkland and Redmond; expanded needed clinical services; restored its financial viability so that it can serve the community; and reinvested into its programs and people.

10th Primary Care Practice Joins the EvergreenHealth System In 2015, EvergreenHealth Primary Care—Sultan became the 10th practice in EvergreenHealth’s primary care network, which is the largest on the Eastside. Together with the affiliated independent practices in its clinically integrated network, EvergreenHealth Partners, the organization serves patients from nearly two dozen primary care locations.

Promoting STEM Education in Local Schools

EvergreenHealth’s 24/7 nurse consultation and referral service connects patients and community members, at no cost, to registered triage nurses who can help alleviate concerns for callers and determine the appropriate level of care for a range of clinical care needs and service.

EvergreenHealth partners with the Lake Washington School District through its Signature STEM Global Health program to provide hands-on education and learning opportunities at EvergreenHealth and in the classroom within STEM subjects (science, technology, engineering and math).

Beyond on-call triage services, Healthline helps community members register for EvergreenHealth’s Community Health Education classes, and our experts also help callers identify providers who meet individual needs based on specialty care, location and insurance.

EvergreenHealth also entered its third year of partnership with Juanita High School’s Global Health STEM Program in 2015, providing guest speakers from all aspects of health care and job shadowing opportunities for students.

Community Healthcare Access Team (CHAT)

EvergreenHealth Geriatric Care

EvergreenHealth’s Community Healthcare Access Team (CHAT) connects individuals to health care services and essential resources that often are not met because of low income, inadequate access to insurance and other hardships. CHAT’s social workers remove barriers and offer assistance at no cost, using an individualized approach to assess needs and connect patients and their families to appropriate services.

EvergreenHealth Geriatric Care serves seniors and their families with a comprehensive team approach that focuses on caring for the whole person. The practice’s consultative model, supported by an interdisciplinary team of senior health specialists, nurses, social workers and pharmacists, provides tailored care and service from its practice on the EvergreenHealth campus in Kirkland, in addition to expert consultation with primary care providers across the EvergreenHealth network.

In 2015, CHAT served 4,332 community members with assistance accessing appropriate resources for insurance, financial assistance, behavioral health services, information and help with general health care questions.

Emily’s Story In her eighth month of pregnancy, Emily found herself without health insurance when her husband was let go from his job and his family plan was eliminated. With time being of the essence, the family needed to find a care plan that would allow her to get the treatment she needed in the final critical month of pregnancy.

EvergreenHealth’s CHAT team met with the couple to assist them in enrolling in Apple Health and selecting a health plan which enabled Emily to continue receiving care from the same treatment providers she had been seeing. Following their son’s birth, CHAT also helped Emily with access to baby supplies—diapers, clothes and blankets—at no charge through the Eastside Baby Corner.

“CHAT helped us during an inopportune time for our family when we unexpectedly lost our health coverage when we needed it most. Thanks to their help we were able to find replacement coverage that allowed us to continue seeing the same care providers I’d been seeing throughout my pregnancy, and after the birth of my son they made sure my family was well supplied. I cannot imagine what we would’ve done without them and I am thankful every day for their support. I hope that they are around to help others for years to come.”

In 2015, EvergreenHealth Geriatric Care continued providing critical care and services to the community’s senior population through its memory care assessments and fall prevention program.

EvergreenHealth Community Grief & Bereavement Services EvergreenHealth’s levy-funded Community Grief & Bereavement Services have provided support to individuals and families anticipating a death, grieving a death or seeking to understand grief as a process since 1988. Today, the complimentary service offers consultations, assessments and referrals, along with access to support groups and community education. Specialty support is also available to couples, families and children following a pregnancy loss, and those facing sudden and traumatic deaths.

Group Support that Lasts a Lifetime EvergreenHealth’s six-week Adult Bereavement Series provides a safe, supportive environment for those coping with the death of a loved one. The series is held five times per year and has become a primary resource for bereavement support on the Eastside.

The program also has continually offered a group environment that builds bonds and friendships that last for years after the initial bereavement period ends. One group of women in particular, who attended the support group over five years ago after the death of their spouses, continues to meet monthly and provide friendship and support to one another.

EvergreenHealth Partners Delivers on Quality and Value Achieving Value and Quality Metrics

EvergreenHealth Partners EvergreenHealth Partners is a clinically integrated network of independent physicians, specialists and EvergreenHealth doctors who are committed to implementing initiatives that improve quality, service and patient outcomes, as well as increase efficiency and control the cost of care for patients.

Participants may also be eligible for additional reimbursement from contracts based on EHP's ability to achieve our quality and cost outcomes.

When EvergreenHealth Partners (EHP) launched in 2013, it became the region’s first clinically integrated network, pursuing a vision to transform health care delivery in the Northwest and to offer the greatest value to both patients and providers.

In 2015, EHP continued on a strong growth path, expanding its provider membership base and increasing the number of lives covered by health plans that have selected EHP as its preferred network.

We were looking for a quality organization with a mission and values similar to ours that we could partner with—and still remain as an independent provider. As its reach has grown, EHP providers continue to focus on value and cost efficiencies achieved by collaboration across member providers, all working toward shared goals measured by value and quality.

In 2015, the network established value and quality metrics within three focus areas including breast cancer screening, colorectal care and diabetes treatment. EHP provider members, nurse care managers and EvergreenHealth program directors worked together to identify the most at-risk patients within the focus areas to connect them with the resources to treat and monitor their condition and engage patients with education to promote their best health.

Launching Wellcentive In 2015, EvergreenHealth and EvergreenHealth Partners launched a new population health management platform, Wellcentive, to help nurse managers identify gaps in patient care and prioritize patients within the network’s focus areas. The digital platform was introduced to select EHP practices in 2015, and continues to complement work across member practices toward achieving value and quality metrics, which ultimately saves patients and providers unnecessary costs.

EvergreenHealth Financial Stewardship In 2015, EvergreenHealth achieved a net income of $21 million after expenses, to invest in the improvement of services and meet community needs.

EvergreenHealth contributed $15.7 million in funds to meet community needs and to foster the health and well-being of all those we serve. Of that total, $1.9 million supported charity care, $7.1 million covered subsidized Medicaid costs, $5.9 million made EvergreenHealth’s levy-funded programs possible and $800,000 was directed to subsidized health care services, which include EvergreenHealth’s complimentary 24/7 Nurse Navigator & Healthline.

EvergreenHealth Monroe Invests in its Patients and Community

In 2015, EvergreenHealth Monroe committed to increase access to the highest quality care and the most exceptional experience and service for the residents of East Snohomish County.

As part of its alliance with the Kirkland-based EvergreenHealth system, the 72-bed Monroe hospital renewed its commitment to the community, pledging to provide critical health services delivered by Monroe’s trusted providers.

In its first year as EvergreenHealth Monroe, the hospital recorded its first profitable year in nearly a decade, and reinvested in tools, technologies and infrastructure upgrades to deliver on its commitment to the community.

Expanding Primary Care

Fairfax Behavioral Health Opens in Monroe

In 2015, new primary care practices and providers in Sultan and Monroe joined the EvergreenHealth Medical Group, providing an important entry point for Snohomish County residents into the comprehensive care and services offered by the EvergreenHealth system.

EvergreenHealth Monroe and Fairfax Behavioral Health partnered to open a 34-bed psychiatric unit operated by Fairfax to provide critically needed inpatient mental health care services, utilizing a newly renovated wing on the EvergreenHealth Monroe campus.

New Emergency Department Providers

Foundation Campaign Dedicates Funds to Recovery Center Upgrades

Board-certified emergency physician leaders from EvergreenHealth’s Emergency Department joined the team in Monroe, assuming medical oversight of the EvergreenHealth Monroe emergency department to create a more seamless integration between the sister hospitals and enhance the continuity of care and consistency in service for patients seeking emergency care.

The EvergreenHealth Monroe Foundation led a mini-campaign to raise $150,000 to invest in structural upgrades for The Recovery Center at EvergreenHealth Monroe. The funds help the Recovery Center in its mission-driven work to provide inpatient drug and alcohol treatment for patients in King and Snohomish counties.

EvergreenHealth Introduces Rehab App Thanks to the Schultz Family Foundation Patients use their own device to follow rehabilitation instructions and report progress to their provider.

In 2015 EvergreenHealth introduced a new mobile app-based rehabilitation tool, Wellpepper, thanks to a generous donation from the Schultz Family Foundation.

Wellpepper provides an interactive platform for digital treatment plans that patients can use on their own smartphone.

Patient notifications with task-based instructions help patients navigate through their care and rehabilitation while the app tracks patient-reported outcomes. Physicians and their care teams can follow real-time patient engagement with their treatment plans while patients track their progress from diagnosis through recovery.

With great appreciation to the Schultz Family Foundation, we have been able to share this new technology with patients across several departments, with plans to expand its use and introduce additional interactive features in 2016.

Marcella, an 82-year-old knee replacement patient, was able to install the app on her iPhone and raves about its ease of use.

“I am no longer seeing my physical therapist in person, but have videos of myself doing all of my exercises so I can continue working on my PT from home,” she said.
