EvergreenHealth Partners joins Puget Sound High-Value Network

Specialists join renowned Joint Replacement Learning Community

EvergreenHealth Partners worked with industry-leading medical providers to create Puget Sound High-Value Network in 2014, which gives employers access to a comprehensive network of care at a reduced cost. Participating organizations in PSHVN are united by a shared goal to provide highquality, low-cost care solutions to employers and patients from more than 350 provider locations across the Northwest.

EvergreenHealth was selected to join the Institute for Healthcare Improvement’s (IHI) Joint Replacement Learning Community, supported by Harvard Business School (HBS) faculty, which brings together a small number of highly visible provider organizations to measure and improve costs and outcomes for knee and hip replacements with the aim of ensuring highvalue care.

EvergreenHealth and Virginia Mason continued to provide a collaborative network of care for patients in the Puget Sound region within cardiology, neurosciences, home health and hospice and other specialties. In 2014, providers saw more than 3,500 neurology patients, half of which were first-time visits, demonstrating the partnership’s commitment to meeting the clinical needs of the community. In cardiology, the health systems consistently outperformed state recommendations by emergently transferring patients in half the recommended time between the two organizations.

Best Hospital by 425 magazine Readers of 425 magazine voted EvergreenHealth “Best Hospital” in the 425 area code for the second time in 2014. Voters praised the knowledgeable and friendly staff at EvergreenHealth and the 24/7 Nurse Navigator & Healthline among the reasons they choose EvergreenHealth to serve their health care needs.

Fairfax Behavioral Health approved to provide care in Monroe EvergreenHealth Monroe’s partnership with Fairfax Behavioral Health was approved to move forward with a plan to integrate inpatient mental health services on the EvergreenHealth Monroe campus, with care expected to begin in 2015.

Valley General becomes EvergreenHealth EvergreenHealth Monroe launches patient cost-estimator tool In 2014, Valley General Hospital in Monroe and EvergreenHealth approved the final phase of the organizations’ affiliation agreement that fully integrated Valley General into EvergreenHealth system of care. As part of the alliance, Valley General became EvergreenHealth Monroe and together with Kirkland-based EvergreenHealth, the organizations are dedicated to providing even greater choice and access to the highest quality, most satisfying care for residents of East Snohomish County.

Patients gained access to a free, personalized service to help estimate their out-of-pocket expenses for procedures at EvergreenHealth based on their individual health insurance policy. The initiative, which makes cost estimates available by phone or email and an “Understanding Your Healthcare Costs” web page, is available to patients who have scheduled procedures at EvergreenHealth as well as those who are simply seeking more information about the cost of care.

Halvorson Cancer Center cares for more patients EvergreenHealth and Seattle Cancer Care Alliance provided the safest, highest quality care and exceptional patient experience to 20 percent more patients. Innovative and integrated care is ensured by investment in technology, physicians and specialized treatments. At the 26,000 square-foot Halvorson Cancer Center, support groups, classes, nutritional consultation, a resource center, pain management, social and rehabilitation services, and palliative care complement the expert clinical care.

Recognized among “America’s 100 Best Hospitals” for specialty care Healthgrades ranked EvergreenHealth among the top 5 percent of hospitals in the nation for its clinical excellence in general surgery, gastrointestinal care and pulmonary care.

Top 5 percent of hospitals for the sixth time in seven years EvergreenHealth earned recognition as a Distinguished Hospital for Clinical Excellence by Healthgrades for the sixth time in seven years, placing the organization in the top 5 percent of hospitals nationwide for its quality patient care and superior clinical performance.

As a public hospital district, EvergreenHealth has an important role in the community. Each year, we are entrusted to use levy funds to meet community health needs. It is both an honor and responsibility that we take seriously in caring for those we serve.

Commun EvergreenHealth’s Community Healthcare Access Team (CHAT) links individuals to health care services and essential resources that often are not met because of low income, inadequate access to insurance and other hardships. CHAT’s social workers work to remove barriers, offering assistance at no cost, using a one-on-one, individualized approach to assess needs and connect patients and their families to appropriate services.

ity Healthcare Access Team CHAT Stats: 4,456 total community member contacts In 2014, CHAT met with 507 clients to discuss health care options and the program assisted 473 clients in completing an online application for insurance coverage.

CHAT successfully enrolled 80 clients in a Qualified Health Plan through the Washington Health Benefit Exchange

Maria s Story As an independent housekeeper in Redmond, Maria didn’t have access to employer-sponsored insurance for her and her family. Private coverage was simply too expensive so she enrolled her children in Medicaid. But because of her immigration status, she did not qualify for her own coverage.

EvergreenHealth’s CHAT outreach workers were able to provide fast support, doing so in Spanish, ensuring Maria gained a clear understanding of her various health care options. Though she has few medical needs, Maria was relieved to learn that she was eligible for an affordable, tax credit health plan.

She now lives with the security and peace of mind knowing that she has the support to stay healthy so she can care for her children and provide for her family.

EvergreenHealth Geriatric Care serves the community’s medically complex senior population and their families with a comprehensive approach to care and services. The team approach includes care provided by physicians, mid-level providers, nurses, social workers and pharmacists. From physical to mental health, the team focuses on keeping family, community, independence, function and quality of life central to their care.

The practice achieves low hospital readmission rates among seniors by partnering with patients, their families and other health care providers to ensure a smooth transition to recovery. In 2014, just 11 percent of EvergreenHealth senior patients were readmitted to the hospital within 30 days of discharge, which is significantly better than Washington state’s average readmission rate of 15.5 percent and the national rate of about 18 percent.

Geriatric Care Medical Director Dr. Connie Smith worked alongside experts including Virginia Mason’s Dr. Nancy Isenberg, a cognitive neurologist; Dr. David Likosky, medical director of The EvergreenHealth Neurosciences Institute; and Dr. Michelle Jeannette, a psychiatrist at EvergreenHealth, to create a tool box to

aid primary care providers in memory care evaluation and consultation.

A pilot program was designed to expand the interdisciplinary approach to EvergreenHealth Primary Care practices by providing geriatric consultations to primary care patients with complex needs related to cognitive disorders.

The team also began work to develop a new model of care aimed

at addressing senior health in light of the community’s rapidly aging population. Under the new model, experts in geriatric care will provide consultative services to patients from our primary care practices. Patients, their families and caregivers and their primary care provider can access the interdisciplinary team for the necessary support to provide the right care.

More Services for Seniors Horizons Senior health and wellness resources encourage and support a healthy, happy and injury-free lifestyle at locations conveniently located throughout the community.

Kirkland Senior Council

Care

This dedicated group advocates for community programs and services that meet the needs of adults age 50 and over in the community, often making recommendations to the Kirkland City Council on matters affecting seniors including fitness, pedestrian safety and elder abuse.

Kirkland Steppers Organized walks promoting health and wellness for walkers age 50 and beyond take place in downtown Kirkland twice a week throughout the summer months.

Northshore Senior Center Older adults are encouraged to improve their physical and emotional well-being through the Evergreen Enhance Wellness programs at Northshore Senior Center in Bothell.

EvergreenHealth Expands

RN Care Management Services

In 2014, EvergreenHealth expanded its RN Care Management Program as part of its commitment to partner with all patients to enrich their health and well-being and provide assistance in navigating the health care system.

Dedicated RN care managers support patients with one-on-one outreach to proactively address their health care needs and identify any potential issues based on their personal family history or existing chronic conditions. As true advocates for community health, RN care managers lead patients through the health care system, collaborating with providers to remove any potential barriers to achieving the best outcomes, and ensuring patients receive the referrals, education and coordinated treatment to thrive.

Community Grief & Bereavement Services

Our levy-funded programs provide support to individuals and families anticipating a death, grieving a death, or seeking to understand grief as a process, at no cost to the community. Community Grief & Bereavement Services provides consultations, assessments and referrals along with support groups and community education. Services also include specialty support to families and children around pregnancy loss, sudden and traumatic deaths and those facing loss as loved ones age.

The death of a loved one can be overwhelming and life changing. For some, the death is sudden and unexpected, while others have time to anticipate. However the death happens, people are often not prepared for the varied emotions that accompany loss. Gracious Grandparents After the loss of a young mother, three children were placed in the loving care of their recently retired grandparents. The heart-broken couple had a feeling there would be great challenges along with the many joys of raising their grandchildren, but they knew they couldn’t do it alone. Early into the transition, the children were having significant adjustments to the loss

of their mother, so the couple sought consultation and support from EvergreenHealth Community Grief & Bereavement.

Over the course of several visits, the grandparents received guidance and assurance on how to best support their grieving grandchildren, and they learned to watch for typical signs and symptoms of a grieving child. They came to understand and find hope in the fact that children have a unique and powerful drive to be okay, even in the face of life’s most difficult changes.

Insight into the minds and hearts of their grandchildren’s loss was invaluable to the family’s transition, while at the same time, the grandparents also found support in dealing with the loss of their beloved daughter.

“Since we talked with the grief counselor, we feel better able to understand the children. Her perception and insight into how children grieve has been invaluable to our seeing our grandchildren and their circumstances in a little different light. Our time with the counselor has helped us tremendously in knowing how to handle situations in a more positive way. Thank you for taking time to guide us on our journey.”

Safe Kids Eastside

Safe Kids Eastside is a local coalition anchored by EvergreenHealth that is dedicated to protecting children from unintentional injury—the no. 1 cause of death and disability for children ages 1-14 in this country. The program strives to reach families with injury-prevention information, education and programs through partnerships with community members and local organizations.

In 2014, Safe Kids Eastside reached 3,343 people through 25 injury-prevention and awareness events.

STEM

Education in Local Schools

In 2014, EvergreenHealth celebrated the first class of local high school students who gained experience in health care through the new STEM Global Health program created in partnership with the Lake Washington School District. The students studied STEM subjects (science, technology, engineering and math) through a year-long collaboration that provided hands-on education and learning opportunities at EvergreenHealth and in the classroom.

The partnership began in the fall of 2013 and continues to provide students with opportunities to work closely with health care professionals, thanks to grants from the Lake Washington Schools Foundation in addition to EvergreenHealth’s taxpayer-supported levy funds.

EvergreenHealth Partners Brings

Value to Patients and Providers EvergreenHealth Partners (EHP), the region's first clinically integrated network, launched in 2013 with a vision to transform health care delivery and offer the greatest value to both patients and providers.

Through EHP, independent physicians, specialists and hospitalemployed doctors partner to streamline care, increase cost efficiencies and improve patient access to resources. With these tools, EHP providers have a deeper, more patient-focused conversation and align around quality and cost in a way not achievable in traditional health care models.

In 2014, the network saw significant growth, bringing more than 550 independent-practice physicians and EvergreenHealth-employed providers together to serve patients

across east Snohomish County and northeast King County.

To expand the network’s reach even further, EHP leaders worked in 2014 to establish partnerships with like-minded organizations to create an attractive high-value, low-cost health care solution for employers, in particular. In September 2014, EvergreenHealth Partners joined with a group of other well-recognized health care organizations united in this goal and created the Puget Sound High-Value Network (PSHVN).

EHP staff collaborate with physicians and care teams to strategize ways to address care gaps that aim to promote patient health through active care management, and ultimately save patients and providers unnecessary costs.

EvergreenHealth Parkinson’s Care EvergreenHealth’s Booth Gardner Parkinson’s Care Center offers Parkinson’s patients comprehensive diagnosis, therapy, and rehabilitative services, as well as leadingedge treatment, rehabilitation and access to clinical trials.

Its innovative clinical and educational services, include groundbreaking clinical trials and research and programs such as Dance for Parkinson’s and From the Ground Up that encourage patients with neurological disorders to strengthen their bodies, use their eyes and ears, and incorporate touch to control movement.

Adding to the success of the program are local business leaders and philanthropists, Nick and Patricia Coluccio, who have seen first-hand how the Booth Gardner Center improves the quality of life for patients and families living with Parkinson’s. The proceeds of their company’s annual golf tournament benefit the Parkinson’s Care Endowment Fund in memory of Nick’s father who founded Frank Coluccio Construction Company.

“The care at our Parkinson's Center is both comprehensive and compassionate and we are fortunate to have a world-class facility right in our own back yard,” says Patricia Coluccio, who recently joined the EvergreenHealth Foundation Board of Trustees to help raise funds and awareness for Parkinson’s.

Evergreen Fund Allocation Committee Each year, donors to The Campaign for EvergreenHealth, our staff campaign, choose where they would like to distribute their contributions. Many employees elect to support the Evergreen Fund, which benefits the areas of greatest need within the hospital. The Evergreen Fund Allocation Committee (EFAC) meets annually to allocate a portion of the Evergreen Fund to projects that enhance the services of EvergreenHealth. In 2014, EFAC awarded $50,000 to 22 new staff-presented projects, 100 percent of which came from employee contributions during the 2013 Campaign. Since the program’s initiation in 2003, EFAC has awarded more than $375,000 for projects throughout the EvergreenHealth system.

In 2014, EFAC funded CLiMB, a support program designed for youth ages 6 to 12 whose parents have been diagnosed with cancer. Through art projects and discussion, this structured program helps normalize feelings of sadness, anxiety, fear and anger, while improving positive communication between the children and their parents.

We make health care easy and convenient with 10 primary care practices, two urgent care centers and two 24/7 emergency departments, along with comprehensive primary, specialty and emergency care provided at our sister hospital, EvergreenHealth Monroe.
