Preface Premessa A critical message is conveyed in this book: implantoprosthesis should be presented as an established science accessible to anyone. In fact, this book is a manual that convincingly explains solutions to problems with prosthetic implants by using notes and aphorisms. Our certainty comes from listening to the experience of dentists and dental technicians using the OT Equator method and applying these insights to develop our knowledge and gain a precise understanding of the key points to build on.

This manual also recognizes many unknown collaborators. Ever since the 1980s various techniques such as small bridges or “Toronto” arches have been used to apply fixed dentures. Rehabilitating a patient by replacing their removable prosthesis with a fixed one is an extraordinary idea yet its success is marred by the fact that these prostheses do not take a maintenance protocol into consideration. Fixed prostheses have to be supported by reliable and uncontaminable implants (current statistics show 40% of implants presenting peri-implantitis). Surgically mounting the OT Equator Biologic abutment is a great guarantee of biological defense during the healing stage preventing the risk of inflammation, mucositis, or peri-implantitis. Our maintenance protocol provides the patient with much improved oral hygiene than they previously practiced with their own teeth. It is essential that the patient periodically returns to the dentist to take care of their oral hygiene or trusts the work of removing, cleaning, and mounting the prosthesis to a hygienist. This can be done in a few minutes using the OT Equator method so a comparison with Formula 1 motoring is very appropriate to this case where the mechanics change the tires during a pit stop in only a few seconds.

This book will show you some of these new concepts in fixed prostheses.

Il contenuto di questo libro ha un senso critico, l’implantoprotesi viene presentata come una scienza affermata e alla portata di tutti. Questo libro invece, vuole essere un manuale che, con convinzione trasmette, tramite appunti o aforismi, alcune idee che risolvono dei problemi che gravano sull’implantoprotesi. Questa sicurezza deriva da parole e idee formulate e realizzate ascoltando tanti operatori dentisti e odontotecnici i quali, sperimentando la metodica OT Equator nel tempo, hanno permesso di formarci una cultura e comprendere con precisione alcuni punti fermi da cui ripartire.

Questo vuole essere anche un riconoscimento a tanti anonimi collaboratori. Dagli anni ‘80 la protesi fissa su impianti, piccoli ponti o arcate tipo “Toronto” vengono costruite con tecniche varie e molto diverse. Riabilitare un paziente portatore di una dentiera mobile con una protesi fissa è un’idea straordinaria, ma in questo senso il successo della protesi fissa è da considerarsi parziale, perché queste protesi non contemplano protocolli di manutenzione.Le protesi fisse inoltre devono essere sostenute da impianti sicuri e incontaminabili (oggi le statistiche parlano di un 40% di impianti che soffrono di perimplantiti). Montando l’attacco OT Equator Biologic Abutment in fase chirurgica, durante la guarigione, si ottengono un’alta garanzia biologica e una difesa che evita molti rischi di infiammazioni, mucositi o di perimplantiti. Per manutenzione intendiamo che il paziente sia in grado di praticare una buona igiene orale quotidiana, addirittura migliore di quella praticata in precedenza sui suoi denti. E’ indispensabile che il dentista richiami periodicamente il paziente e si prenda cura o affidi all’igienista il compito di smontare, pulire e rimontare la protesi che, con la metodica OT Equator, avviene in pochi minuti. Qui regge bene il paragone con la Formula1; dove ai box il cambio delle gomme viene fatto dai meccanici in pochi secondi.

In questo manuale vogliamo esporre alcuni di questi nuovi concetti di protesi fissa.

Introduction Introduzione The World Health Organization defines health as “the complete physical, mental, and social state and not merely the absence of disease”. This is a clear, precise, and challenging indication which we at OT Equator Center strongly believe in.

This project began ten years ago with collaboration between the University of Modena and Reggio Emilia dental clinic and the MIUR research laboratory of Rhein’83.The dental clinic and Enzo Ferrari Engineering Doctorate School planned and carried out the practical development of the OT Equator Biologic Abutment through several trials and specific tests enlisting the collaboration of several dental technicians. It solved some of the remaining problems in the osteointegrated implantology sector and the rehabilitation of total and partial edentulism by using highly biocompatible materials and the neuropathology of stomatological apparatus.

In order to define implantology as the “third biologic dentition”, all future health risks caused by the implant or prosthesis must be avoided once a prosthesis has been mounted on implants in one part of the mouth.

Edentulism is an anatomical condition that not only concerns the oral cavity and its chewing functionality, but can also become a unique opportunity to regain its shape and general well-being.

Seen from this perspective, the prosthesis can become a really significant life experience.

We believe this illustrates the potential of these new techniques better as they are the only ones currently able to provide treatment programs with greater awareness. A Scientific Committee has been established to manage the OT Equator Center program to develop and carry out these cultural programs.

L’Organizzazione Mondiale della Sanità definisce la salute come “lo stato di completo benessere fisico, psichico e sociale e non la semplice assenza di malattie”. Si tratta di una indicazione chiara, precisa, sfidante, nella quale noi di OT Equator Center crediamo fortemente.

Questo progetto è nato da dieci anni di collaborazione fra l’Università di Modena e Reggio Emilia ed il laboratorio di ricerca MIUR di Rhein83. La Clinica Odontoiatrica e la Scuola di Dottorato della facoltà di Ingegneria “Enzo Ferrari”, attraverso prove e test specifici e con la collaborazione di alcuni odontotecnici, hanno realizzato e reso pratico l’utilizzo dell’attacco OT Equator Biologic Abutment.

Si sono risolti alcuni dei problemi che ancora gravano nel settore dell’implantologia osteointegrata e nella riabilitazione degli edentulismi totali e parziali, mediante l’utilizzo di materiali ad elevata biocompatibilità e la pratica di neuropatologia dell’apparato stomatologico.

Per definire l’implantologia “la terza dentizione biologica”, occorre, una volta inserita una protesi su impianti in un settore della bocca, essere certi che a causa dell’impianto o della protesi non si presentino rischi futuri di salute.

L’edentulia è una condizione anatomica che non riguarda solo il cavo orale e la funzionalità masticatoria, la cui risoluzione può diventare un momento unico per ritrovare la propria forma ed un benessere generale.

Vista in questa ottica la riabilitazione protesica può essere veramente un’importante esperienza di vita.

Diciamo questo per illustrare e aiutare a comprendere meglio la potenzialità di tali nuove tecniche che riteniamo, al momento, le uniche in grado di intraprendere percorsi terapeutici con maggiore consapevolezza.

Per curare e gestire il programma di OT Equator Center, è stato formato un Comitato Scientifico che porterà avanti culturalmente questi programmi.

During the meeting between the University of Modena and Reggio Emilia Dental Clinic and the Miur laboratory of Rhein’83, we became aware of the demand for a new stable type of denture anchored onto implants to rehabilitate edentulous patients in a practical, functional, and economical way.

THE IDEA

The design of the OT Equator was conceived by reducing the size of the OT Cap sphere in the upper and lower poles while retaining the retentive part of the sphere. Once assembled together with the Seeger spring inside a cylinder, they form a resilient lock, which gives outstanding retention and passive results when mounted on multiple implants.

Durante un incontro tra la Clinica Odontoiatrica dell’Università di Modena e Reggio Emilia e il laboratorio di ricerca Miur di Rhein’83 si discute il problema dell’esigenza di un tipo di protesi rimovibile, mobile, stabile, ancorata su impianti per riabilitare i pazienti edentuli in modo pratico, funzionale ed economico.

Riducendo la dimensione della sfera OT Cap nei poli superiore ed inferiore, conservando la sola parte ritentiva della sfera sono nati l’attacco OT Equator e la molla Seeger che, assemblati all’interno di un cilindro, formano un bloccaggio elastico che, una volta montato su più impianti, dà risultati ritentivi e passivi eccezionali.

Una volta realizzati tutti i componenti, essendo un prodotto polivalente con il quale si ottiene ogni tipo di protesi Rhein’83 commercializza e distribuisce le componenti per protesi mobile ed OT Implant quelli per la protesi fissa.

Technical Director Direttore tecnico Ezio Nardi - Dental Technician Founder of OT Equator Center President of OT Implant “FROM THE CAOUTCHOUC PROSTHESIS TO IMPLANTOLOGY” He is one of the pioneers of Italian dentistry and has gained extensive experience in the field of precision engineering while working in the industrial sector. Graduating as a dental technician in 1959 at the George Eastman Higher Education Institute he has following and participating in the progress of dentistry ever since. He founded and has owned Rhein 83 for over 30 years. His experience in the field of removable dentures and in fixed combined prostheses is documented in dozens of patents. Over the course of his remarkable career he has received various national and international honors including Knight of the Italian Republic.

During his professional experience he compared the practices of the Italian school of implantology to the protocols of the Swedish, Swiss, and German schools following the presentation of the “ad modum Branemark” protocol. Decades of professional practice led him to the belief that the orthodontic landscape had focused excessively on surgical protocols and that the prosthetic construction technique applied to implantology had not been addressed with the same rigor. In fact, in clinical practice – for decades and even still today – many professionals continue to complain about prosthetic problems associated with the uncontrolled expansion of working hours and the practical difficulties in the management of prosthetic implant clinical cases as well as biological side effects such as peri-implantitis, mucositis, and peri-implant bone resorption. The idea of the OT Equator Center was conceived from these findings.

DALLE DENTIERE IN CAUCCIÙ ALL’IMPLANTOLOGIA UN SALTO IN LUNGO... O AD OSTACOLI? Considerato un pioniere dell’odontotecnica italiana, Ezio Nardi ha maturato esperienza anche nell’ambito della meccanica di precisione.

Diplomato odontotecnico nel 1959 presso l’Istituto superiore George Eastman di Roma, segue i progressi dell’odontoiatria sin dai primi Anni ’50. Per 30 anni, oltreché fondatore, è stato il titolare di Rhein 83. A testimoniare la sua preziosa esperienza nel campo della protesi rimovibile e combinata inamovibile, stanno decine di brevetti ai quali Nardi, Cavaliere della Repubblica italiana, affianca le molteplici onorificenze, anche internazionali, di cui è insignito.

La sua esperienza professionale si è notevolmente arricchita nel confronto tra le pratiche implantologiche di Scuola italiana e i protocolli delle Scuole svedese, svizzera e tedesca proposti dopo la presentazione del protocollo “ad modum Branemark”. In decenni di pratica ha maturato la ferma convinzione che il panorama odontoiatrico sia eccessivamente concentrato sui protocolli chirurgici e la tecnica costruttiva protesica applicata all’implantologia non sia codificata con altrettanto rigore. Molti professionisti continuano infatti da decenni a denunciare problematiche protesiche nella loro pratica clinica associate a dilatazione incontrollabile di tempi di lavoro e difficoltà nella gestione di casi clinici implantoprotesici, oltre agli effetti collaterali di tipo biologico, perimplantiti, mucositi e riassorbimenti ossei perimplantari. Di qui scaturisce l’idea di fondare OT Equator Center.

Scientific Committee Comitato Scientifico OT Equator Center is a special and complex organization unique in its field, drawing on our results in over 10 years of research. Our area of interest is osseointegrated implantology and oral rehabilitation of partial and total edentulism using highly biocompatible materials and the neurophysiopathology of the stomatological apparatus.

OT Equator Center è un’organizzazione particolare e complessa, unica nel suo genere, che riassume i risultati di ricerche ottenuti in oltre 10 anni di lavoro.

Il settore d’interesse è l’implantologia osteointegrata, la riabilitazione orale degli edentulismi parziali e totali, mediante l’utilizzo di materiali ad elevata biocompatibilità e la neurofisiopatologia dell’apparato stomatologico.

We created a scientific committee to manage and look after the OT Equator Center program. It includes highly experienced odontologists working together researching into new technologies who share this method through publications, instructional courses, training courses, and refresher courses in Italy and abroad.

Per gestire e curare il programma di OT Equator Center è stato creato un Comitato scientifico di cui fanno parte odontoiatri di elevata esperienza, che collaborano alla ricerca di nuove tecnologie e che, attraverso pubblicazioni e corsi di alfabetizzazione, formazione e aggiornamenti, divulgano questa metodica in Italia e all’estero.

A series of original research projects have been created over the last 10 years aimed at simplifying reconstructive implantoprosthetic therapies from a surgical and prosthetic point of view, with these projects brilliantly solving important issues using the OT Equator Biologic Abutment, and giving rise to a new philosophy.

Negli ultimi 10 anni è stata realizzata una serie di progetti di ricerca originali, mirati alla semplificazione delle terapie ricostruttive implantoprotesiche, che sia dal punto di vista chirurgico che protesico hanno risolto brillantemente problemi importanti, utilizzando il pilastro OT Equator Biologic Abutment, attraverso il quale è nata una nuova filosofia.

The result is a new philosophy of working that has abolished the use of many of the classic implant abutments such as healing screws, temporary abutments, and straight and angled MUAs (multi-unit abutments), and custom laboratory angled and straight abutments.

Ne è scaturito un nuovo orientamento di lavoro che ha abolito l’impiego di molti pilastri implantari classici, come le viti di guarigione, gli abutment temporanei, i MUA (Multi Unit Abutment) e i monconi personalizzati di laboratorio angolati e dritti.

Dott. Giampiero Ciabattoni Born in Bologna il 07/06/1971, enrolled the faculty of Medicine and Surgery at the University of Ferrara in 1991. Graduated in Dentistry and Dental Prosthesis at the University of Florence. He attended several courses at the “Branemark Clinic” in Sweden and keeps a collaboration with Prof. Stefan Lundgren from the University of Umea - Sweden and Dr. Eric Van Doren Anversa - Belgium. He is partecipating to several studies about immediate function, guided surgery, and post extraction implants. He is the winner of the 1st Price Winners 2006 at the World Tour in Rimini. He is now working as a freelance and he is particularly involved in oral surgery specially implantology and implantoprosthesis.

Dott. Gabriele Vaccaro Graduated in Dentistry and Dental Prosthesis at the University of Palermo in 2003 full marks with honors. He attended Dr. Mario Palmeri’s annual endodontic course He attended Dr. Francesco Amato’s annual implantoprosthesis course. He accomplished the perfectioning Post-Graduate Master on advanced Implant treatments at the University of Modena e Reggio Emilia directed

by Prof. Ugo Consolo. Since 2009 he collaborates as a consultant with OT Dental Studio, and since 2011 he became the health director at the new headquarters in the OT Equator Center in Bologna. From several years he lectures in Italy and abroad on new implantosprosthetis subjects that simplifies all phases from surgery to the application of the prosthesis.

the patient’s rehabilitation of the fixed “Seeger Bridge” prosthesis. The “Seeger Bridge” type prosthetic stands as a class of rehabilitation that combines old and new features to effectively and precisely solve problems such as the exact number of sessions and therefore the costs incurred from the surgical phase to the prosthesis itself. Innovation means demonstrating a completely new technique, developed through serious research and carried out by competent staff, who in addition to having personally tested it, have contributed to sharing the OT Equator Biologic Abutment method with clients in the early stages of promotion, presenting it through courses, conferences, and congresses in Italy and abroad (IADR USA in 2011, Brazil 2012, USA 2013, South Africa 2014, USA 2015) to the point where this technique is being used by many professionals and now has a base in our new headquarters (OT Equator Center) in Bologna, specifically created and equipped for this system.

There are many ways of designing and carrying out research, but some elements are undoubtedly fundamental: a specific awareness of the subject, a significant amount of experience, and new ideas about what you wish to accomplish. Furthermore, it requires the space afforded by a real laboratory, a range of equipment, a team of trained and motivated staff, a lot of time, passion, and funds to invest.These are all the essential ingredients necessary to carry out a project and obtain a result. Indeed, our research group had no lack of experience, desire, motivation, and innovative ideas.We found the missing parts in the landscape surrounding us.

Emilia-Romagna is a region full of professionals. Besides their knowledge and techniques acquired during their studies, they often have the genes of an artisan in their DNA. Our researchers began by spending time with these experienced professionals, followed by discussing our idea with the University, then passing the idea from a planner to a designer to develop the prototype, and finally returning to the University to evaluate our results.

Whenever faced with unsatisfactory results, the cycle was repeated again and again until we obtained a prototype ready to present to the dentist and dental technician who tested it in a patient’s mouth after a thorough evaluation.

These trials allowed us to fine-tune the measurements, dimensions, and tolerances.These recalculations often led us repeat the cycle of experiments and came to be known as our “research years”. With the “finished” product in hand, the next steps could begin: making sure the products met required standards by performing trials and tests at independent laboratories authorized to obtain the necessary certification (ISO, CE, etc.).The years of market research began at this stage in the project in combination with the communication of our product on a wider scale. Based on the favourable opinions of early adopters, a complete line of accessories and components were developed, which when used correctly became a valid prostheticsurgical protocol allowing the dentist and dental technician to plan

Si è partiti, allora, dalle esperienze di questi professionisti, frequentandoli con tempo e pazienza,pensando e discutendo l’idea in ambito universitario,si è passati dal progettista al disegnatore, e poi ancora al meccanico, per ottenere il prototipo e, infine, per tornare all’università e valutare i risultati.

Spesso, a fronte di un esito non soddisfacente, si è ripetuto il percorso; una, due, tre, dieci volte... fino ad ottenere un prototipo da presentare all’odontoiatra ed all’odontotecnico che, dopo un’accurata valutazione, lo hanno provato in bocca ad un paziente.

Questi test ci hanno permesso di apportare le modifiche per la precisa messa a punto delle misure, delle quote e delle tolleranze; anche queste operazioni ci hanno spesso portato a ripetere il giro completo tra gli esperti e così sono trascorsi quelli che possiamo definire gli “anni della ricerca”.

Con il prodotto “finito” in mano sono poi partite le fasi successive: mettere a norma i prodotti, fare i test e le prove presso laboratori esterni autorizzati per ottenere le certificazioni necessarie (norme ISO, Marchi CE, etc.). A questo punto del progetto, sono iniziati gli anni della ricerca di mercato, abbinata ad una diffusione del prodotto su scala più allargata. In base ai pareri favorevoli dei primi utilizzatori, si è messa a punto una linea completa di accessori e componenti che, utilizzati in modo corretto, sono diventati un vero e proprio protocollo chirurgico-protesico che consente all’odontoiatra ed all’odontotecnico di programmare nel paziente una riabilitazione di protesi fissa “Seeger Bridge”.

La tipologia protesica “Seeger Bridge” si pone come una classe di riabilitazione che unisce caratteristiche vecchie e nuove per risolvere efficacemente i problemi, dalla fase chirurgica alla protesi, stabilendo con precisione, ogni volta, il numero esatto delle sedute con il paziente e, quindi, anche dei costi. Parlare di novità vuol dire dimostrare che si tratta di una tecnica completamente nuova, realizzata attraverso una ricerca seria, fatta da persone competenti che, oltre ad averla collaudata personalmente, hanno contribuito nei primi anni a far conoscere questa metodica, definita OT Equator Biologic Abutment, presentandola attraverso corsi, conferenze e congressi in Italia e all’estero (IADR - USA nel 2011, Brasile 2012, USA 2013, Sudafrica 2014, USA 2015), al punto che oggi questa tecnica è usata da molti professionisti e viene presentata e realizzata in questa nuova sede (OT Equator Center) a Bologna, appositamente creata e attrezzata.

Vi sono certamente molti sistemi per progettare e fare ricerca, ma alcuni elementi sono, indubbiamente, fondanti: una cultura specifica sul tema, un’importante dose di esperienza e idee nuove su ciò che si vuole realizzare.

Certo, occorrono anche il laboratorio come luogo fisico, le diverse attrezzature, uno staff di persone preparate e motivate, molto tempo, passione e fondi da investire. Sono tutti elementi essenziali per raggiungere un risultato e realizzare un progetto. Il gruppo di lavoro formato per questa ricerca non aveva a disposizione tutti gli ingredienti necessari, ma sicuramente aveva esperienza, volontà, motivazione e idee innovative; si è cominciato da quello e tutto ciò che mancava, di volta in volta, lo si è cercato sul territorio.

L’Emilia-Romagna è una regione ricca di professionisti che, oltre a possedere le conoscenze e le tecnologie apprese durante gli studi, hanno spesso nel DNA i geni dell’artigiano.

Dental technology and implantology Odontotecnica e implantologia The art of dental technology tries to keep up with the times but it is right that things should change. The industry adapts and accepts important solutions (CAD-CAM, sinterization, etc.) yet we do not believe that a machine will substitute the human hand in dentistry.

Neither do we believe that the technology proposed by the industry attempts to undermine the craft. The traditional prosthesis on natural teeth has followed a protocol for years.Whether it is built using modern technology or by a dental laboratory, the dentist can deliver a fixed or mobile prosthesis to their patient in 4-5 sessions. This is more or less common all over the world.

Dentists seeking to practice implantology in many developing countries find themselves faced with various difficulties of choice other than the considerable initial investment: which implant type to choose (internal or external hexagonal connections, conical connections, etc.)? Catalogues with illustrations of the mechanized components can be found that, although precise and beautiful, offer little technical information helping to construct the dental prosthesis.

We wish to standardize the implant prosthesis using the OT Equator Center prosthetic protocol and make it possible to create and deliver the Seeger Bridge fixed prosthesis to the patient in just a few sessions with a technique that is applicable and repeatable on every type and brand of implant available on the market.

The dentist and dental technician can learn the OT Equator Center protocol technique and the use of its components by participating in a course lasting a single day.After completing this course, they will be able to assess how to prevent peri-implantitis and become familiar with the technical solutions to build a prosthesis on divergent implants. All this without having to provide for new investments and instead relying on standard studio and laboratory equipment.

Research at the OT Equator Center, despite having obtained important results, will continue with the knowledge that many problems remain to be studied and resolved.

Con il protocollo protesico realizzato da OT Equator Center, si è voluto standardizzare anche la protesi implantare e rendere possibile realizzare e consegnare in poche sedute la protesi fissa Seeger Bridge al paziente, come nella protesi tradizionale su denti naturali, con tecniche applicabili e ripetibili su ogni tipo di impianto esistente in commercio.

L’odontoiatra e l’odontotecnico che già praticano l’implantologia, potranno apprendere la tecnica proposta da OT Equator Center e l’utilizzo delle sue componenti partecipando ad un corso di un solo giorno, al termine del quale saranno in grado di valutare come prevenire le perimplantiti e la soluzione tecnica per costruire la protesi su impianti disparalleli. L’arte odontotecnica segue con fatica i tempi ma è giusto che le cose si evolvano. L’industria si adegua e si inserisce nell’odontotecnica con soluzioni importanti e innovative (CAD-CAM, sinterizzazione...), inoltre non crediamo che nel campo odontoiatrico la macchina possa sostituire completamente la mano dell’uomo.

Non riteniamo che la tecnologia proposta dall’industria tenti di scalzare l’artigianato. La protesi tradizionale su denti naturali è protocollata da anni.

Che sia costruita con tecnologie moderne o dal laboratorio odontotecnico, il dentista, in 4-5 sedute, consegna al paziente la sua protesi fissa o mobile.

Questo è ciò che accade più o meno in tutti i paesi al mondo.

In molti paesi emergenti, il dentista che vuole praticare l’implantologia, a parte il notevole investimento iniziale, incontra varie difficoltà: quale tipo di impianto scegliere (connessione ad esagono interno o esterno, connessione conica, etc.). In tutti i casi trova cataloghi con le illustrazioni delle componenti meccanizzate, belle e precise, ma con poche informazioni tecniche per agevolare l’odontotecnico nella costruzione della protesi.

implant. In contrast, the implant remains buried under the gum unable to be contaminated, avoiding inflammation, and any risk of peri-implantitis. By using this technique, the dental technician has solved the problem of the implants’ lack of disparallelism because it exceeds undercuts of 70°/80° in a simple way.

There are hundreds of implants easily found on the market, each presented with well-illustrated catalogues accompanied by designs and shapes with special qualities, with tricks and discoveries ranging from mechanics and biology to design and aesthetics. However, all of these appreciable and quality solutions are always attributed to the implants alone. While admitting that the fixtures are of good quality, many catalogues of these companies have accessories and prosthetic components with very similar technical qualities.

They all present attachments for overdentures,abutments for fixed prostheses, all well illustrated with precise measurements and various angles but few instructions regarding their application in a fixed prosthesis.

This always forces the dentist and the dental technician to invent and design the fixed prosthesis each and every time.

This means everyone pays a high price, including the patient, who undergoes long and painful tests under anaesthesia in each session (often for more than two hours). Frequently, more sessions have to be scheduled and in many cases the execution and application of the prosthesis leaves the practitioner unsatisfied if not concerned.

The OT Equator Biologic Abutment research project was conceived with a commitment to study and solve many of these problems. A protocol can now be presented that by mounting the OT Equator Biologic Abutment on the implant during the surgical phase to the “letter” subsequently reduces all tests in the patient’s mouth to a quarter of the time it has taken up until now.

Work is performed on the sphere of the OT Equator during these in-mouth tests without ever having to intervene inside the implant by removing and replacing the internal parts of the

OT EQUATOR SYSTEM Un odontoiatra che oggi voglia avvicinarsi all’implantologia si trova a dover affrontare diversi problemi, non tanto per quanto riguarda GLI IMpIANTI reperibili, quanto per lA CARENZA DI istruzioni e pROTOCOLLI di utilizzo dei prodotti stessi.

In commercio sono ormai facilmente reperibili centinaia di impianti, presentati con cataloghi ben illustrati, corredati di disegni e forme specifiche con accorgimenti e scoperte che vanno dalla meccanica al design, all’estetica e alla biologia. Tutte soluzioni sicuramente di qualità e apprezzabili che hanno peculiarità che però riguardano sempre i soli impianti. Riconoscendo la buona qualità delle fixture, molti cataloghi di queste case presentano gli accessori ed i componenti protesici con caratteristiche tecniche molto simili fra di loro.

Tutti presentano attacchi per overdenture, monconi per protesi fissa, illustrati bene, con misure precise e di varie angolazioni, ma poche istruzioni d’uso riguardo la costruzione in protesi fissa.

Questo costringe sempre il dentista e l’odontotecnico a progettare ed inventarsi ogni volta la protesi fissa.

Di questo ne pagano tutti un prezzo molto alto. Anche il paziente, che in ogni seduta viene sottoposto con anestesie a prove lunghe e dolorose (anche di oltre due ore). Spesso aumenta anche il numero delle sedute programmate ed in molti casi l’esecuzione e l’applicazione della protesi non lasciano completamente soddisfatti.

La ricerca OT Equator Biologic Abutment è nata con l’impegno di studiare e risolvere molti di questi problemi. Qui presentiamo un protocollo che, se praticato alla “lettera”, montando sull’impianto l’attacco OT Equator Biologic Abutment durante la fase chirurgica, consente di ridurre tutte le prove in bocca al paziente ad un quarto del tempo fino ad oggi impiegato. Durante queste prove in bocca al paziente, si lavora sempre sulla sfera dell’OT Equator, senza più intervenire smontando e rimontando le parti all’interno dell’impianto, che rimane sepolto sotto la gengiva, impossibilitato a contaminarsi evitando infiammazioni e rischio di perimplantiti. L’odontotecnico, con questa metodica, non ha più il problema del disparallelismo degli impianti, perché supera in modo semplice sottosquadri minimi o eventuali divergenze accentuate anche di oltre 70° / 80°.

components. In order to mount on the implant during the surgery stage an exhagonal joint component, MUA like, the surgeon will have to couple the male an female exhagons in a precise direction lined with the other implants, thus will make it a difficult procedure.

2. All clinical experts know that rotational implant abutments are more practical to position and they create a better circumferential closing seal than all other components. The degree of surface polishing of the implant abutment favors hemidesmosomal adhesion in the coronal epithelial component of the biological width and the connective junction in the deep layers of the connection.

3. Defence of the biological barrier by eradicating any screwing or unscrewing abutment manoeuvres capable of iatrogenic damage to the same biological barrier, causing the apical migration of the gingival junction.

4. The application of an OT Equator Biologic Abutment in any implant inclination. Even with angled, divergent, or tilted implants, a straight rotational abutment should be applied.The surgeon will have an immediate net advantage in the reduction of time required to attach the abutments.The prosthetist can manage the following steps with ease, overcoming any degree of divergent implants without having to resort to complex solutions, mesostructures, or angled abutments.

5. The OT Equator Biologic Abutment has led to a momentous change in the operating protocol of prosthetic implants by doing away with the dogma of prosthetically driven implantology. The clinician should feel free to place the implant screw carefully and safely, delegating the solution of the problems of abutment convergence or divergence to the OT Equator Biologic Abutment. Fractures of the OT Equator Biologic Abutment in cases with angled implants with disparallelism up to approximately 80 degrees are negligible.

Hundreds of clinical cases have been documented in the last 10 years, and it is clear that the incidence rate of mucositis and peri-implantitis is so low as to be irrelevant.The presence of a collar of adhering fibromucosa, circumferential to the OT Equator Biologic Abutment, has proved to be a positive prognostic factor. The hygiene maintenance protocol of the OT Equator Biologic Abutment will be described in a dedicated section of this manual.

It is a universal Titanium abutment for any implant type and connection, which provides the clinician and technician with a series of unique operating advantages compared to all of the other therapeutic alternatives available on the market.

The advantages can be summarized in three categories:

biological, mechanical,and prosthetic.

Biological advantage The most fundamental aspect of the OT Equator Biologic Abutment is that it was designed to be screwed onto the implant during the surgical phase without having to be removed.

The OT Equator Biologic Abutment guarantees the accuracy of the screwed connections favoring and promoting the formation of the epithelium connective junctions on its smooth outer surface.

The diameter of the OT Equator Biologic Abutment is less than all other prosthetic abutments and seeks to maintain the very strong fibromucosal adherence to it with the clear aim of emphasizing the formation of the gingival barrier as key in preventing inflammation and ultimately mucositis, peri-implantitis, and peri-implant bone resorption.

The biology of the Equator connection is based on four basic concepts: 1. The accuracy of the abutment implant connection. The OT Equator Biologic Abutment differs from the MUA (Multi Unit Abutment) connections because it is composed of a single component: it is a filled titanium casing with no possibility of microbial contamination in the coronal apical direction. Clinical practice has convinced us that this is a key factor and that its rotational connection on the implant platform guarantees greater accuracy on all of its non-rotational

tipo MUA, sull’impianto in fase chirurgica, dovendo inserire ed accoppiare gli esagoni maschio e femmina in una direzione precisa, in linea con gli altri impianti il chirurgo è costretto ad un’operazione complicata e laboriosa.

2. Tutti i clinici esperti sanno che i pilastri implantari rotazionali sono più precisi e pratici da posizionare e realizzano un sigillo di chiusura circonferenziale superiore a tutte le altre componenti. Il grado di levigatura superficiale (Ra) del pilastro implantare favorisce l’adesione emidesmosomiale nella componente epiteliale coronale dell’ampiezza biologica e la giunzione connettivale negli strati profondi della connessione.

3. Difesa della barriera biologica mediante l’abolizione di qualunque manovra di svitamento e riavvitamento del pilastro, in grado di fungere da danno iatrogeno alla stessa barriera biologica, provocando la migrazione apicale della giunzione gengivale.

4. Applicazione di un pilastro O.B.A. in qualsiasi inclinazione implantare.

Anche nel caso di impianti angolati, disparalleli o tiltati deve essere applicato un abutment rotazionale dritto. Il chirurgo avrà un vantaggio netto immediato nella riduzione dei tempi e delle difficoltà di messa a dimora del pilastro; il protesista potrà gestire le fasi successive con semplicità, superando qualunque grado di disparallelismo implantare senza ricorrere a soluzioni complesse, mesostrutture o monconi angolati.

5. Il Pilastro O.B.A. ha determinato un cambiamento epocale del protocollo operativo implantoprotesico. Il clinico deve sentirsi libero di posizionare la vite implantare in condizioni di sicurezza e di prudenza, delegando al pilastro O.B.A. la soluzione dei problemi di convergenza o divergenza dei pilastri. Le fratture del pilastro O.B.A., nel caso di impianti angolati con disparallelismi fino a 80 gradi circa, non sono fino ad oggi rilevabili.

Negli ultimi 10 anni sono stati eseguiti e documentati centinaia di casi clinici, da cui emerge chiaramente che l’incidenza di mucositi e perimplantiti è assolutamente irrilevante. La presenza di un collare di fibromucosa aderente, circonferenziale al pilastro O.B.A. si è dimostrato un fattore prognostico positivo. Il protocollo di mantenimento igienico del pilastro O.B.A. è descritto in una sezione dedicata di questo volume.

Vantaggio biologico Il primo aspetto fondamentale è che il pilastro O.B.A. è stato progettato per essere avvitato all’impianto durante la fase chirurgica, in sostituzione della vite di guarigione senza dover essere più rimosso. O.B.A. garantisce la precisione della connessione avvitata, favorendo e promuovendo la formazione della giunzione epitelio connettivale sulla sua superficie perimetrale liscia esterna.

Il diametro del pilastro O.B.A. in titanio vuole conservare, attorno ad esso, la maggior quantità possibile di fibromucosa aderente, con lo scopo preciso di enfatizzare la formazione della barriera gengivale come momento chiave per la prevenzione dei fenomeni infiammatori e - in ultima istanza - di mucositi, perimplantiti e riassorbimenti ossei perimplantari.

La biologia della connessione Equator ruota attorno a cinque concetti di base: 1. Precisione della connessione impianto-abutment. Il pilastro O.B.A. si differenzia dalle connessioni MUA (Multi Unit Abutment) perché costituito da un’unica componente: è un cilindro in titanio pieno, senza possibilità di trasmettere contaminazione microbica in senso corono apicale. La pratica clinica ci ha convinto che questo sia un fattore chiave che deve alla connessione rotazionale sulla piattaforma implantare la garanzia di una precisione superiore a tutte le componenti in appoggio non rotazionali. Ot Equator Biologic Abutment consente tre distinte possibilità di connessione protesica, a livello dell’estremità a sfera modificata del pilastro.

Il design dell’estremità ritentiva è ottenuto da una sfera alla quale sono stati rimossi i poli superiore e inferiore, mantenendone solo la componente ritentiva equatoriale. Da ciò il nome: Equator Abutment.

La connessione Equator è disponibile per l’applicazione su qualunque tipologia implantare.

1. La sfera Equator presenta il minor ingombro dimensionale tra tutte le componenti implantari per overdenture, sia in senso verticale che orizzontale.

2. The main advantage of the OT Equator Biologic Abutment system is its multi-functionality. The OT Equator Biologic Abutment has three distinct types of prosthetic connection at the far end of the modified sphere of the abutment.The end is retained by a sphere on which the upper and lower poles have been removed, keeping only the retentive equatorial component. Hence its name ‘Equator Abutment’.

The Equator connection can be applied on any type of implant and in the overdenture technique on natural teeth.

1. Overall the Equator sphere occupies the smallest amount of space of all the implant components used both vertically and horizontally in overdentures.

2. The Equator sphere allows for the anchorage of the nylon retention caps for overdentures, encoded in different colors according to their hold and indicated by both the management of the partial edentulism by means of an RPDI (Removable Partial Denture Implant) and for the OVD (Overdenture) technique for total edentulism.

3. Seeger connection. 4. The acetal material’s lack of hygroscopia also makes it suitable for prosthetic solutions in zirconia.

The Seeger ring is housed at the level of the prosthetic cylinder on the axis of the prosthetic insertion. It is a very simple yet surprisingly effective device able to guarantee a retentive capacity of around 8 kg/F to the prosthetic product and constitutes a self-centering part of the prosthesis on the Equator sphere.

The Seeger ring is available in two interchangeable versions of pink and white: the more elastic pink version is indicated for the laboratory phases and for the prosthetic test manoeuvers while the more rigid white one is indicated for intraoral use.

This is original and unprecedented on the market, and is particularly welcome in the aesthetic area in the frontal regions of the upper jaw, especially for the Seeger Bridge technique.

The Seeger connection also demonstrates great practicality in the intermediate tests of the prosthetic product because the clinician can insert and remove the prosthesis in seconds using the “snap” mechanism without having to unscrew and screw anything.

The test sessions of the teeth assembly, the intermaxillary impressions with wax rims and the intermediate tests of the prosthetic framework of the fixed prostheses can be carried out quickly and effectively even by operators with little implant experience, and without any symptoms for the patient.The click of the insertion is manually perceptible to the operator on both the laboratory model and in an intraoral case, acting as proof of the prosthetic connection.

The remaining gap between the OT Equator Biologic Abutment and the prosthetic cylinder, omitting the use of the through screw and choosing the Seeger connections alone as the retentive solution, is less than 100 microns.

The last connection mode of the OT Equator Biologic universal abutment is the through screw, which crosses the threaded geometric center of the sphere longitudinally. It is a short prosthetic screw that completes the Seeger connection and secures the prosthesis to the Equator abutment.The screwed connection can be applied (at the discretion of the clinician) on all of the Equator abutments or only on some of them, placed in a strategic position on the arch, and generally placed far from the aesthetic area or from the Equator sphere at a higher level of disparallelism.

The screwed connection facilitates complete housing of the implant abutment and reduces the marginal gap between the extra grade prosthetic cylinder and the closing edge of the OT Equator Biologic Abutment to only 25 microns.

Technical features: keep or ignore them?

EVITARE problemi futuri Esclusa la prima regola (rigidità e robustezza) sulla quale tutti concordano, per tutte le altre, pur riconoscendone l’utilità tecnico-scientifica, per molte ragioni si è abituati a venire a patti. Ne sono prova le presentazioni nei vari congressi, le pubblicazioni scientifiche su riviste del settore e tante immagini su libri di autori professionisti.

Grazie anche ai molti dentisti che da anni usano i prodotti e la metodica OT Equator nella protesi fissa Seeger Bridge, si è notato che le procedure di lavoro risolvono in modo standardizzato e ripetitivo qualsiasi progetto protesico. Questo ha permesso di studiare e sviluppare un protocollo di lavoro che va dalla chirurgia alla protesi, tale da favorire l’odontoiatra e l’odontotecnico che nel 90% dei casi evitano molti incontri o telefonate per discutere problemi tecnici di progettazione quali l’estetica, il parallelismo, le viti ecc.

Usando il protocollo OT Equator dopo una prima esperienza si noteranno delle economie di lavoro, sia nelle protesi a carico immediato che in quelle a carico differito, abbreviando di molto il tempo delle sedute con il paziente ed anche il numero delle stesse.

Lavorando con questi nuovi componenti si noterà che questa tecnica consente di ottenere protesi di ottima qualità, robuste ed estetiche.

Caratteristiche tecniche: Ottenerle o trascurarle?

Anche la protesi fissa su impianti ha le sue regole e le sue difficoltà.Tutti gli operatori, odontoiatri ed odontotecnici, le conoscono. Alcune regole devono essere necessariamente rispettate: - la travata dev’essere rigida, non deve assolutamente flettere, dev’essere abbastanza robusta da non rompersi; - la scelta di materiali e tecnologie è fondamentale (titanio, zirconio, cromo cobalto, fresatura al CAD-CAM, fusione a cera persa, etc.); - l’ottenimento dell’estetica: ceramica, composito, resina, denti del commercio o modellati a mano, etc.;

BIOLOGY In a special microenvironment such as the oral cavity the Titanium attachment was designed with features and a shape that keep the implant uncontaminated, isolated, and buried under the gum. The attachment is no longer removed during the healing stage and the surrounding tissues adheres to the titanium surfaces of the attachment creating a barrier that remains a biological defense able to limit any infiltration of bacteria and so avoid peri-implantitis.

BIOLOGIA In un ambiente come il cavo orale, dove vi è un microambiente particolare, si è voluto progettare un attacco in titanio con forma e caratteristiche tali da mantenere l’impianto incontaminato, sepolto e isolato sotto la gengiva. In order to build the Seeger Bridge fixed prosthesis and lock it onto the implants, all of the procedures, the impressions, and the tests will have to be carried out by beginning from the hemisphere of the OT Equator Biologic Abutment attachment, which is exterior to the gingiva and will never be removed so that the biologic barrier is preserved.When designing and then building the prosthesis, the dental technician will not face any problems with any possible divergences between the implants as any bridge or connecting bar can be placed on both the model or in the patient’s mouth using the Seeger Bridge method even if the implants diverge over 80 degrees.

Trials are performed using the same cleaned resin plate with the teeth fitted over it and the retentive caps inserted. Due to the stability of the plate, the patient is able to judge the aesthetic and phonetic result, and the dentist can check the product technically.

At the OT Equator Center we are convinced that the immediate-load prosthesis is a therapy of the future. After several years’ experience and a series of cases carried out by various dentists with satisfactory results, OT Equator is a surgical-prosthetic technique that has been protocolled without any clinical or scientific conclusions being made yet. The diagnosis and the project are important for obtaining functional, aesthetic, and therapeutic results using the very simple immediate-load prosthetic technique presented here.A previously made semi-finished resin bridge is used in an initial in-mouth test on the patient before the surgery is started. During this trial, the vertical and centric articulations are registered and locked with a self-curing resin key. After the surgical insertion of the implants, this semi-worked bridge will be used as a custom made impression tray.The tray is filled with impression material (elastomer), positioned in the patient’s mouth, which by closing the mouth is guided by the resin key taken beforehand,providing the gnathological data and allowing the dentist to achieve a precise impression to be able to complete the threadreinforced resin bridge. In OT Equator Center siamo convinti che la protesi a carico immediato possa essere una terapia del futuro. OT Equator, dopo l’esperienza di alcuni anni e una serie di casi eseguiti da alcuni dentisti con risultati soddisfacenti, propone una tecnica chirurgica-protesica protocollata; senza volerne trarre al momento conclusioni cliniche o scientifiche. La diagnosi ed il progetto sono importanti al fine di ottenere i risultati terapeutici, estetico e funzionale. Qui viene presentata questa tecnica molto semplice di protesi a carico immediato. Si comincia utilizzando un ponte in resina semilavorato costruito in precedenza, che viene provato in bocca al paziente prima di iniziare la chirurgia. Durante questa prova si rilevano l’articolazione verticale e centrica, che vengono bloccate con una chiave di resina autopolimerizzante. Dopo la chirurgia e l’inserzione degli impianti, questo ponte semilavorato verrà utilizzato come portaimpronte individuale che, riempito di materiale da impronta (elastomero), viene posizionato in bocca al paziente il quale, chiudendola, viene guidato dalla chiave di resina rilevata in precedenza e fornisce i dati gnatologici permettendo di ottenere l’impronta precisa per poter finire il provvisorio in Resina-Filo-Armata. Il ponte R-F-A verrà fissato agli impianti e non sarà più rimosso fino ad osteointegrazione avvenuta. Questo ponte provvisorio è robusto, preciso ed estetico, grazie ai monconi in titanio a fori passanti collegati fra loro da due fili ortodontici di diametro 0,8 mm che formano un rinforzo metallico inglobato nella resina. Il paziente lascerà lo studio dopo una seduta durata 5-6 ore (di cui 2 passate a riposo).

Fixed implant prosthesis and comparison with Seeger Bridge Implantoprotesi fissa tradizionale e Seeger Bridge a confronto There is considerable demand for fixed prostheses in implantology nowadays. Of course, it is for a dentist to suggest this. The industry sometimes confuses itself by presenting many types of implants, each accompanied by their own infinite variety of accessories, each one differing from the other.

Surgical techniques vary in dentistry. The result is guaranteed in more favourable situations where the bone is prepared. Where the bone is deficient and predispositioning is impossible, the dentist has to resort to a variety of techniques using bone graft tests directly on the patient and grafts taken from the iliac crest or the cranial theca.

Otherwise grafts are taken from the bone bank or bovine bone grafts and other methods are still used not to mention the more established surgical techniques of enlargements of the alveolar ridge, surgery with open air flaps, or flapless surgery.

After many adventures, we developed a prosthesis using models and implants that are extremely divergent at times and in positions where building the prosthesis would not be anywhere near as possible even though it should be. Dental technicians are not capable of judging these surgical practices as they do not have the culture or knowledge to discuss them.What is more, the surgeon is unaware of all the problems a dental technician faces. So in creating prostheses they are forced to resort to technical tricks and design innovations.

Sometimes an implant cannot be used because it is impossible to insert it in a framework bridge (the surgeon placed it where there was bone but technically it cannot be used). This may sound absurd but those who practice implantology know these situations all too well. Unfortunately, practical and functional problems arise from here on in with the casting of the impression, the long and difficult test sessions, the pollution of marginal tissues, gingival infiltration and inflammation, and lastly problems with halitosis following the application of the prosthesis on the patient.

In short, it opens up a can of worms risking peri-implantitis.Without

going any further, we are confident that the OT Equator Biologic Abutment technique solves these problems since, technically speaking, it is a very simple method. It goes without saying that analytical evaluations like this is subjected to a competent public authority operating in the field which is able to consider and judge appropriately.

OT EQUATOR SYSTEM Oggi, in implantologia, vi è una notevole richiesta di protesi fissa, anche perché viene di frequente proposta dall’odontoiatra. L’industria a volte disorienta presentando innumerevoli tipologie di impianti, ognuna corredata dai propri accessori, di varietà infinite e molto diverse e assortite fra loro.

Nell’implantologia le tecniche chirurgiche sono varie. Nelle situazioni più favorevoli, dove l’osso del paziente è predisposto, il risultato è scontato.

Dove l’osso è carente e non esiste la predisposizione, si ricorre a tecniche molteplici, con prelievi di osso e innesti diretti sul paziente, prelievi dalla cresta iliaca, teca cranica; oppure innesti di altro tipo: prelievi dalla banche dell’osso, innesti di osso bovino e altri metodi ancora, senza parlare delle tecniche chirurgiche più affermate: allargamenti di cresta alveolare, chirurgia con lembi a cielo aperto, chirurgia flapless.

Dopo tanti passaggi arriviamo alla protesi, con modelli e impianti a volte molto disparalleli e in posizioni dove costruire la protesi non sarebbe quasi possibile (ma lo deve essere!). L’odontotecnico non può giudicare le pratiche chirurgiche; non ha la cultura, né la conoscenza per discuterle. Ma d’altronde anche il chirurgo non conosce tutti i problemi dell’odontotecnico. Per realizzare le protesi si è costretti a ricorrere a stratagemmi tecnici, invenzioni progettuali, a volte addirittura non si riesce ad utilizzare un impianto perché è impossibile inserirlo in una travata (il chirurgo lo ha posizionato dove c’era l’osso, ma tecnicamente non è utilizzabile). Quanto detto potrebbe sembrare assurdo, ma coloro che praticano l’implantologia conoscono queste situazioni. Purtroppo da qui in poi insorgeranno problemi pratici e funzionali: il rilievo delle impronte, le sedute con prove lunghe e difficoltose, la contaminazione dei tessuti marginali, le infiltrazioni e le infiammazioni gengivali, i problemi di alitosi dopo l’applicazione della protesi al paziente... Insomma, si aprirà una strada piena di rischi verso le perimplantiti. Senza dilungarsi ulteriormente, se con la tecnica OT Equator Biologic Abutment, si è già fatto un cammino su di una strada che può risolvere alcuni di questi problemi, riteniamo che sia il caso di proporla, anche perché,

tecnicamente parlando, OT Equator Biologic Abutment è una metodica molto semplice. Ovviamente valutazioni analitiche come questa si possono proporre ad un pubblico competente, che opera nell’ambiente ed è in grado di riflettere e giudicare.

The OT Equator attachments have a spherical diameter of 2.5 mm. The spheres are covered during the insertion of the prosthesis with the cylindrical abutments inserted into the same prosthesis (they have a 3 mm diameter plus the Extragrade eccentric indented rim of 0.17 mm). Some movement is created by these spaces during the insertion of the prosthesis on the divergent implants that means the prosthesis can be placed over the attachments in a precise and passive way.The Seegers are slightly elastic and are contained in a groove inside the cylindrical abutments.

In order to overcome the undercuts, the Seegers at some point tend to squeeze themselves. However, they resume their original shape in a short time thanks to their elastic memory. This is how the “Seeger + screw” double seal is obtained. Instead of creating a problem, the undercut of the implants becomes an advantage in many cases since with the prosthesis inserted it creates an interlock between the OT Equator attachment and the abutment.

Consequently, it secures the prosthesis with the Seeger alone without needing any screws. Using their experience, this opportunity can be exploited by dental

In other cases, parallel abutments are built on each abutment in the laboratory or using CAD-CAM. This entails long and additional tests with the patient. Even after the application of the prosthesis on the patient, dentists sometimes remain perplexed or concerned. Many techniques require a difficult and long session to remove and mount the prosthesis, the use of an anaesthetic, the unscrewing of the abutments from the inside of the implants, and the tagging of each abutment so that they can be mounted again. The Seeger Bridge method is simple and eliminates all of these problems in fifteen to twenty minutes maximum

technicians in any type of prosthesis providing practical and aesthetic advantages.

I Seeger, per superare i sottosquadri, in qualche punto tendono a schiacciarsi ma, grazie alla loro memoria elastica, riprendono la forma originale in poco tempo; si ottiene così una doppia tenuta “Seeger + vite”. In molti casi, il sottosquadro degli impianti anziché costituire un problema diventa un vantaggio perché, a protesi inserita, si crea un incastro fra l’attacco OT Equator ed il moncone, che in questo modo blocca la protesi anche con il solo Seeger, senza la vite. Questa opportunità, in base all’esperienza, viene sfruttata dall’odontotecnico in ogni protesi con vantaggi pratici ed estetici.

La mancanza di parallelismo e la divergenza degli impianti è un problema sia per il dentista che per l’odontotecnico. In certe protesi si ricorre a progetti laboriosi utilizzando monconi prefabbricati tipo MUA, in altri casi si costruiscono in laboratorio o al CAD-CAM monconi paralleli su ogni impianto. Questo comporta prove lunghe e supplementari con il paziente.

Il metodo Seeger Bridge elimina tutti questi problemi in modo semplice, operando sempre senza anestesia con tempi massimi di 15-20 minuti e offrendo la massima sicurezza. The implant treated patient has surely much more difficulties to correctly perform domestic and studio hygene because of the more or less extended prosthesis base and the complexity to remove it.

All of these aspects are resolved in a practical way using the OT Equator method.You can see from these images (using the extragrade abutments) how you can avoid exceeding the edges of the implant with the prosthesis and therefore allow the patient to use dental floss, brushes, or an irrigator. Another significant advantage for hygiene is that you can offer more frequent checkups to the patient since the disassembly and reassembly of a Seeger Bridge prosthesis (carried out without anaesthesia) is a simple operation requiring only a few minutes and is harmless to the patient. We remind you that neither plastic nor metal curettes should ever be used during this dental hygiene session.

The ideal solution for many implantology patients is a fixed prosthesis instead of dentures. Using the latest surgical techniques and implants, the dentist is able to satisfy the patient by swapping the denture for a fixed prosthesis but there are still some problems which are not discussed enough. It is essential that a fixed prosthesis on implants is easily removable.There are many reasons why you may need to take action, for example, to repair a broken tooth, or retouch or modify the gingival base.

Even periodic hygiene in the dental studio can be problematic. In order to remove and replace certain prostheses in many cases, it is necessary to remove the connection screwed into the implant, an operation that can take up to two hours. An anaesthetic is needed to avoid the patient suffering pain.These problems limit call backs for this most effective form of periodical hygiene.

USING THE SEEGER BRIDGE FIXED PROSTHESIS, DENTISTS SIMPLY NEED TO UNSCREW A MAXIMUM OF 2 TO 3 SCREWS SINCE THE OTHER ATTACHMENTS HAVE SCREWLESS SEEGERS WITH ONLY A “SNAP� RETENTION. A hammer blow is enough to remove the prosthesis and then the dentures are washed using a toothbrush. With the OT Equator attachment emerging from the gingiva and the gingival tissues adhering to the titanium of the implant, plastic curettes must not be used at all for any studio hygiene.

Il paziente implantare ha indubbiamente più difficoltà a praticare una corretta igiene domiciliare e ambulatoriale a causa della protesi più o meno estesa e per impossibilità di rimuoverla.

Un altro notevole vantaggio a favore dell’igiene è che si possono proporre al paziente visite di controllo più frequenti, perché smontare e rimontare una protesi Seeger Bridge, sempre senza anestesia, è un’operazione semplice che richiede pochi minuti e non comporta particolari fastidi al paziente. Va ricordato che per questa igiene ambulatoriale non si usano mai le curette, né di plastica né di metallo.

Nell’implantologia la protesi fissa al posto di quella mobile sarebbe la soluzione ideale per molti pazienti. Con la chirurgia e gli impianti di ultima generazione l’odontoiatra è in grado di soddisfare il paziente cambiando la dentiera con una protesi fissa, ma esistono ancora alcuni problemi di cui si parla poco. La protesi fissa su impianti è indispensabile che sia facilmente smontabile. Sono tanti i motivi per cui è necessario intervenire, per riparare un dente rotto, per ritoccare o modificare la base di appoggio gengivale.

I controlli periodici possono essere problematici, in alcuni casi per smontare e rimontare certe protesi si è costretti a rompere il dente per poter togliere la vite; questa operazione richiede del tempo e una complessa riparazione del dente.

Per evitare dolori al paziente, alcune volte, si è costretti a ricorrere all’anestesia, queste difficoltà limitano molto i richiami per i controlli ambulatoriali periodici.

Nelle protesi fisse Seeger Bridge, con molti impianti, è sufficiente svitare al massimo 2 o 3 viti perché negli altri ATTACCHI vi è una ritenzione “snap” con i soli Seeger senza vite.

Tolte le poche viti con colpo di martelletto si può rimuovere la protesi; basterà lavarla con uno spazzolino facilitando l’esecuzione della seduta di igiene Con la sfera OT Equator che fuoriesce dalla gengiva e con i tessuti gengivali aderenti al Titanio dell’impianto, per l’igiene ambulatoriale non si usano ASSOLUTAMENTE più le curette di plastica.

The dental surgeon can continue mounting the same brand and type of implants in the patient’s mouth without requiring any initial investment as they already own the necessary tools and burrs.

Instead of the healing screw the OT Equator Abutment can be screwed on top of the implant using the screwdriver and the OT Equator Abutment will never be removed.The screwdrivers are used to screw the OT Equator Abutment onto the implant and they can also be used to connect the prosthetic components to the Equator sphere during the trials and application of the prosthesis.

Dental technicians will also not have to make any significant purchases.

They only require two screwdrivers. All of the accessories needed to build the prosthesis (analogues, abutments, screws, etc.) have to be bought each time or they can arrange for a small stock order.

Any laboratory working for a lot of dentists will be able to provide a better service by using the OT Equator method and this small assortment of accessories since the prosthesis can be built immediately on any brand of implant.

Il chirurgo odontoiatra, senza fare nessun investimento, seguiterà a montare in bocca al paziente la marca ed il tipo di impianti che già usa, di cui già dispone, delle frese e degli attrezzi necessari. Utilizzando il solo cacciavite Equator a innesto quadrato, monterà in testa all’impianto, al posto della vite di guarigione, l’attacco OT Equator Biologic Abutment che non verrà più smontato. I cacciaviti servono per avvitare l’attacco OT Equator sull’impianto, ma anche per connettere alla sfera Equator le componenti protesiche durante le prove e l’applicazione della protesi.

Anche gli odontotecnici non debbono fare spese importanti, acquisteranno come strumentario solo due cacciaviti Equator a innesto quadrato. Gli accessori per costruire le protesi (analoghi, monconi, viti...) vanno acquistati di volta in volta. Per comodità si può tenere un piccolo magazzino di accessori assortiti, per avere una scorta di riserva.

Knowing all too well the discomfort this situation carries, the OT Equator seeks to solve all of these cases of the Seeger Bridge fixed prosthesis and Seeger bar combined prosthesis, eliminating many current problems such as divergences between the implants or the screws that unscrew and in consequence break.

These are situations which at times require complex and costly work using other techniques to solve them.

These reflections are the result of our first few years of experience with the O.B.A. technique. None of our early adopters have abandoned us. Many are grateful and are very satisfied. Therefore, we have developed something that will become increasingly more viable and enriched by new ideas.

It may seem “risky” to propose a fixed prosthesis system applicable to all types of implants in use around the world to resolve problems such as the difficulty of overcoming any disparallelism using a simple and repeatable technique for every prosthetic case.

However, drawing on the experience of Rhein 83, which has been successfully applying the same concepts with attachments for overdenture prosthesis for many years, it seemed natural for us to apply the same procedures on a fixed prosthesis too. This method begins with the world of the dental surgeon, who while continuing to apply their preferred type of implant on the patient, will be able to apply the OT Equator Biologic Abutment attachment on the implant at the end of surgery instead of using healing screws.

TheSeeger Bridge fixed prosthesis system simplifies the procedure and becomes a protocol that standardizes the design and execution of the prosthesis. These concrete results are recognized and used by many professionals. Following the appreciation gained in our early years, the new OT Equator Center headquarters will host national and international courses to accelerate the acceptance and spread the use of this method.

OT Equator Center is a multi-purpose complex that includes the OT Implant sales office for the OT Equator attachments and components. OT Equator Biologic Abutments (O.B.A.) are made to measure and produced for every type of implant in use in the world. Furthermore, attachments for the most famous brands of implants are available from our warehouse. The attachments for all other implants are built specially and shipped to the customer within 4-5 working days.

In order to guarantee the construction of O.B.A. attachments on all types of implants in the world, OT Implant has a database of more than 12,000 technical drawings.

The O.B.A. attachment and its components can be defined as a kind of “Lego system” by which it creates a completely new prosthetic implant method based on the OT Equator Biologic Abutment attachments fitted onto the cylindrical abutments with the Seegers inside. There are various rehabilitation possibilities available today to create every type of prosthesis on implants: the cemented or screwed fixed prosthesis, overdenture prosthesis on attachments, or double structure prosthesis. All these types of implants are made using components produced by various implant houses that produce components and accessories suitable for their own implants only and which require the technician to understand

Gli attacchi OT Equator Biologic Abutment O.B.A. vengono prodotti su misura per ogni tipo di impianto in uso nel mondo. Inoltre sono disponibili a magazzino gli attacchi per le marche d’impianti più note. Per tutti gli altri impianti, gli attacchi vengono appositamente costruiti e spediti al cliente entro 4 / 5 giorni lavorativi.

Può sembrare “azzardato” proporre un sistema di protesi fissa, applicabile a tutti gli impianti in uso nel mondo che risolva alcuni problemi, come la difficoltà di superare i disparallelismi, con una tecnica semplice e ripetibile per ogni caso protesico. Rifacendosi all’esperienza della Rhein 83, che da anni applica con successo gli stessi concetti con gli attacchi per protesi overdenture, ci è sembrato naturale applicare la stessa procedura anche in protesi fissa. Questa metodica parte dall’operato del chirurgo odontoiatra, che pur continuando a montare al paziente il proprio tipo di impianto preferito, al termine della chirurgia, al posto della vite di guarigione si troverà sull’impianto l’attacco OT Equator Biologic Abutment.

Questo sistema di protesi fissa Seeger Bridge semplifica la procedura e diventa un protocollo che standardizza la progettazione e l’esecuzione della protesi. Questi risultati concreti sono riconosciuti e utilizzati da vari professionisti.

A seguito dell’apprezzamento ottenuto in questi primi anni di divulgazione, la nuova sede OT Equator Center ospiterà corsi nazionali ed internazionali per allargare il consenso verso questa metodica.

Precautions and maintenance Accorgimenti e manutenzione When a dentist applies the Seeger Bridge prosthesis to the patient at the end of the therapy, he will organize a checkup program for the first year as well as a series of instructions on appropriate maintenance. The patient will have to respect them to guarantee themselves any future assistance. A card bearing the reference number of the medical records of the dental practice where all of their data is stored can be given to the patients with OT Equator Biologic Implant Abutment connections. In the first year the patient will have to follow the recommendations and follow-up examinations planned to maintain good maintenance. Prostheses fixed to implants can be considered to be mechanisms that operate in a very different environment. Some patients chew soft foods while others chew different kinds of foods. There will be bruxist patients and patients who have vices and acquire new habits. These situations may require maintenance over time. Many professionals share the opinion that teeth should be made in adaptable and therefore wearable materials (resin or composite). Using the Seeger Bridge system in these cases, old and ruined teeth can be replaced by new ones from the same series after a few years. The teeth can be swapped in one day, perhaps with a touch of extra color.The same operation can be carried out calmly in two or three days while the patient uses the temporary thread-reinforced-resin bridge that was previously stored away. This and other measures help (but are not a guarantee) and can act as a safeguard for the dentist and the patient to help a peaceful and trusting relationship develop between patient and dentist.

With this card and the attached contra-angle screwdriver, any dentist anywhere in the world can remove and replace this prosthesis.

Moreover, the studio guarantees both an employee responsible for help and assistance and an appropriate odontostomatological performance.

OT EQUATOR SYSTEM Quando l’odontoiatra, a fine terapia, applica la protesi Seeger Bridge, oltre ad una serie di istruzioni per la buona manutenzione, darà al paziente un programma di controllo che dovrà essere rispettato per avere assicurata una futura assistenza. Volendo, ai pazienti con connessioni OT Equator Biologic Abutment può essere consegnata una tessera di riferimento che riporta il numero della cartella clinica conservata nello studio dentistico, in cui sono riportati tutti i dati clinici del paziente. Il paziente, durante il primo anno, dovrà seguire i consigli e le visite di controllo programmate per mantenere, anche successivamente, una buona assistenza. Queste protesi fissate agli impianti possono essere considerate dei meccanismi che, a loro volta, funzionano in ambienti molto diversi. Alcuni pazienti masticheranno cibi morbidi, altri cibi di ogni tipo; vi saranno pazienti bruxisti ed altri che prenderanno vizi o abitudini nuovi. Queste situazioni con gli anni potranno necessitare di manutenzione. È opinione di molti professionisti che i denti debbano essere di materiale adattabile, quindi usurabile (resina o composito). Con il sistema Seeger Bridge, se sono stati utilizzati i denti del commercio, è possibile sostituirli con denti nuovi della stessa serie, cambiandoli tutti in una sola giornata, magari con un tono di colore in più. La stessa operazione la si può fare con calma in due o tre giorni, montando al paziente il ponte provvisorio in R-F-A che si era precedentemente conservato. Questo ed altri accorgimenti sono un’assistenza – non una garanzia – ma, per il dentista ed il paziente, sono una tutela che porterà avanti, con serietà, un sereno rapporto di fiducia.

Con questa tessera e l’allegato cacciavite per manipolo contrangolo, qualunque dentista, in qualsiasi parte del mondo, è in grado di smontare e rimontare questa protesi in pochi minuti.

Inoltre, lo studio dentistico che ha fornito la protesi assicura per iscritto una responsabile assistenza e un corretto risultato delle prestazioni odontostomatologiche.

The OT Equator Center has created a program of theoretical and practical courses for the structural understanding of the use of the OT Equator Biologic Abutment. The educational training program for the use of the OT Equator Biologic Abutment has separate sessions reserved for dentists, dental technicians, retailers, and sales agents of dental materials and biomedical equipment.

The OT Equator Biologic Abutment method is a new and completely different technique from systems practiced so far. Features and organization of courses The Center organizes three types of courses:

Any family members accompanying the students can go on an organized tour of the city accompanied by a tourist guide to visit the historic center, museums, and go shopping.

Course “A” Theoretical course lasting 1 day. It can be taken off-site and summarized in a conference of 90 minutes or 2 hours.

Course “B” Theoretical and practical course lasting 2 days (only at the Bologna headquarters).

Bologna is a historic medieval town a few kilometres from other major tourist cities easily reached by public transport and taxis: Venice 140 km; Florence 90 km; Ravenna 75 km; Modena and Ferrara 35 km;

Course “C” Surgery and prosthesis on patients course lasting 2 days (at the Bologna headquarters or also off-site with prior agreements and organization).

Anyone wishing to stay longer can obtain information from our reception.

Our center has an agreement with the AC Marriott Hotel, 200 meters from our headquarters.

In addition to the scheduled courses every year, other meetings are available such as refresher courses designed to be taken at exterior locations such as universities, retailers, etc.

OT Equator Center operators speak in English during the international courses. If a mother-tongue interpreter is required for courses with more than 8 foreign participants, it should be communicated in time to be able to find an interpreter and, if necessary, give them the time needed to acquire the technical terms of the dental sector.

All those who have participated in previous courses will be constantly updated about any future educational and cultural activities at the Center.

Lunch of a light Italian cuisine is offered by the organization at 12:30.

At the end of the first day of work on 2 day courses a dinner is offered at a restaurant known for its traditional Bolognese cuisine. If the course does not run longer than expected, a short sightseeing trip of the city of Bologna will be made before dinner.

Everyone who parecipated to the course will be updated with all the news, cultural and educational activities organized by the Center.

Per gli eventuali familiari che accompagnano i corsisti, è possibile organizzare un tour della città, con l’accompagnamento di una guida turistica, per visitare il centro storico, musei o fare shopping.

Bologna è una città storica medievale e dista pochi km da alcune città turistiche importanti e raggiungibili facilmente con mezzi pubblici e taxi: Venezia 140 km; Firenze 90 km; Ravenna 75 km; Modena e Ferrara 35 km;

Corso “A” Corso teorico della durata di 1 giorno. Può essere svolto fuori sede e sintetizzato in una conferenza di 1 ora e mezza o 2 ore.

Corso “B” Corso teorico e pratico della durata di 2 giorni (solo in sede a Bologna).

Corso “C” Corso di chirurgia e protesi su paziente della durata di 2 giorni (in sede a Bologna o anche fuori sede, in centri adeguatamente attrezzati, previo accordo con l’organizzazione).

Chi volesse prolungare il soggiorno può ricevere informazioni presso la nostra reception.

Il nostro centro è convenzionato con l’Hotel AC Marriott, che dista 200 metri dalla nostra sede.

Oltre ai corsi programmati annualmente, sono allo studio altri incontri, tipo conferenze di aggiornamento, da tenersi presso sedi esterne, come Università, rivenditori, etc.

Durante i corsi internazionali, gli operatori di OT Equator Center parleranno in lingua italiana e inglese. Se per i corsi con un numero superiore agli 8 partecipanti stranieri occorresse un interprete di madrelingua, va segnalato in tempo per poter trovare l’interprete e, se necessario, consentirgli di acquisire i termini tecnici del settore dentale.

Tutti i giorni il pranzo delle ore 12.30 è offerto dall’organizzazione, con cibi leggeri della cucina italiana.

Nei corsi di 2 giorni, al termine della prima giornata di lavori, sarà offerta una cena in un noto ristorante di cucina tradizionale bolognese. Prima della cena, se il corso non si protrarrà oltre i tempi prestabiliti, si farà un piccolo giro turistico nella città di Bologna.

Tutti coloro che hanno partecipato ai corsi precedenti saranno costantemente aggiornati su tutte le future attività didattiche e culturali del Centro.

The building is a multi-purpose Oral Rehabilitation implantoprosthetic centre divided into three areas: The industrial area, dedicated to the production and distribution of micromechanical components for fixed and removable oral implantology; The clinical area, composed of: - Dental laboratories; - Dental surgeries dedicated to providing private, freelance care The educational area, dedicated to theoretical and practical training about the head and neck region.

The center is the only facility in the world able to manufacture and deliver a Titanium implant component, built and machined to spec during a surgical procedure.

OT Equator Center is home to the International Academy of Implantology, Biomaterials, and Prosthodontic Rehabilitation.

OT Equator Center was set up to spread – from an educational and clinical point of view – a new philosophy of oral rehabilitation based on diagnosis, the simplification of minimally invasive operating techniques, innovative scientific research, and the training of professionals in the sector.

The building La struttura

OT Equator Center si estende su un’area di 2600 metri quadri. È una struttura polivalente di Riabilitazione Orale Implantoprotesica distinta in tre settori: Settore industriale: dedicato alla produzione ed alla distribuzione di componenti micromeccaniche per implantologia orale, fissa e rimovibile; Settore clinico: composto da: - Laboratori odontotecnici; - Ambulatori odontoiatrici, dedicati all’attività assistenziale in regime libero professionale privato; Settore didattico: dedicato all’attività formativa teorica e pratica del distretto testa-collo.

Il Centro è l’unica struttura al mondo in grado di realizzare e consegnare un componente implantare in titanio, costruito e macchinato su misura, durante un intervento chirurgico.

OT Equator Center ospita la Scuola Internazionale di Implantologia, Biomateriali e Riabilitazione Orale Protesica ed è nato per diffondere dal punto di vista didattico e clinico - una nuova filosofia di riabilitazione orale, basata sulla diagnosi, sulla semplificazione delle tecniche operative mini invasive, sulla ricerca scientifica innovativa e sulla formazione continua degli operatori del settore.

Lo scopo del Centro è di ottenere estetica, funzionalità e durata nella riabilitazione orale.

La filosofia di lavoro, condivisa dagli operatori, è concentrata su tre punti: conoscenza scientifica, abilità manuale, umanità.
