From Me To You... Have you heard of the Sandwich Generation? It sounds like a great place to be, right? Grilled cheese, or ham and cheddar on wheat bread with a pickle on the side? Actually it is none of those. What it does involve is having children who are growing up (or have grown and fled the nest) then all of a sudden, your reliably independent parents slip and break a hip, or they don’t seem to remember like they used to, or they just need you to help them get by with daily tasks. You are sandwiched in between both generations, taking on the burden - sometimes all at once - of caring for everyone. It is a tough spot to be in, not one that most of us prepare for, and not one that most of our parents desire either. The awkward conversations arise. Who tells Dad he should not be driving anymore? Is someone able to help Mom make sure she takes her medication? Did anyone mention plans for later life? Is there money put aside for long term care? Is there a will? Did anyone talk to Mom and Dad about funerals, or cremation?

I have had most of these conversations with my parents and my in-laws in the last few months. Some of them out of necessity and some so we can all be prepared. Let me tell you, it is not easy. Then, just when you think you have it all locked in place, the future you were preparing for may end up not being the one that comes about. My mother has always planned on aging in place. She has a chair lift so she can get to her room, a life alert device so she can call for help if necessary, a cleaning service to keep up with the housework and a local meal delivery service so her freezer is fully stocked. Then there are the friends who pop by to check on her and keep her spirits up should she feel lonely. Sounds perfect, right? Well all of this was premeditated on her declining physically. The problem that is beginning to arise is that due to Long Covid (the lasting, nasty effects of the disease), she is facing other challenges. My sharp-as-a-tack mother is losing words, and it is driving her crazy. She is most likely having mini strokes causing the memory and coherence issues, and that is creating a situation we did not anticipate. How long can she stay in her perfect home? That is a question I am having to come to terms with. The future we all planned together has gone and a new alternative is before us. We will navigate it, sandwiched between other responsibilities and the need to parent our parents. What I would like you to come away from this letter with, is the understanding that planning is key. Not just for the obvious things, but for other eventualities. If you have a resolution this year that you follow through on, it should be this. Plan your estate. I don’t care if you are 24 or 74, you need to let people know what your wishes are- in every eventuality you can imagine. Start a conversation with your loved ones and take some of that burden away from them - you can even bring sandwiches.

Dental Care It’s no secret, the holidays are a sweet time of year with seemingly endless sugary snacks, drinks and desserts. That, coupled with a long break from school can mean grazing in-between meals is at an all-time high for families across the nation. Dr. Jeannie Beauchamp, president of the American Academy of Pediatric Dentistry, shares her top teeth tips. * Schedule (or reschedule) those dental appointments: Like many other medical appointments, pediatric dental check-ups have been pushed off during the pandemic, causing cavity-prone kids. Dr. Beauchamp has noticed an increase in cavities, tooth decay and longer cleaning times in patients. Preventive dental appointments not only catch compromised teeth but offer children educational reinforcement on proper dental cleaning and habits. Leaving cavities untreated can result in larger, lasting health issues. * Teeth are NOT package openers: Teeth make for excellent tools when it comes to eating, drinking and enunciating. But spare those tiny

teeth when it comes to opening packages, snacks or presents. This can lead to weakened enamel and expensive and painful repairs. * Avoid constant snacking: It’s important to limit in-between meal snacking to prevent unwanted wear and tear on teeth that can cause the tooth enamel to weaken and lead to tooth decay or cavities. Dr. Beauchamp also warned, many children might have fallen into a pattern of grazing throughout the day due to digital learning and encourages families to set boundaries when snacking. * Keep a routine: Make brushing and flossing a priority and ensure kids brush thoroughly for two minutes at least twice a day. Keeping a routine means preventive dental appoints as well, shared Dr. Beauchamp. With regular pediatric dental appointments, there are fewer dental emergencies. * Water, water, water! Drinking water in between meals encourages saliva, washes away bacteria/ food debris, and neutralizes plaque acids. This includes drinking fluoridated water, proven to keep teeth strong and reduce cavities. Covid Tests

When you think about efforts to fight the spread of COVID-19, vaccinations, hand washing and masking likely come to mind. However, one important effort that must remain the first line of defense is effective testing. Testing, combined with other defensive measures, is essential to keeping ourselves and our loved ones safe. The two main types of tests authorized for detecting COVID-19 are polymerase chain reaction (PCR) tests and antigen tests. Here is what you need to know about both: Antigen tests Also referred to as rapid tests or rapid antigen tests, these tests work by quickly identifying protein fragments from a virus and providing test results in about 15 minutes. Despite being fast, antigen tests can be less accurate than other testing options because they are less sensitive. Because no special equipment is needed, antigen tests may be performed in various community settings or at home. This convenience has made antigen tests, particularly at-home options, popular when quick results are needed, as when returning to an office or preparing for an event like a vacation. However, there is more room for error when self-testing, as opposed to having a test administered by a trained professional.

these tests are mostly available in healthcare environments, such as hospitals. Why would someone use a test that takes longer to get results? The answer: accuracy. PCR tests have remained a touchstone in diagnostics due to their heightened accuracy over antigen tests. They are highly sensitive and can detect very small amounts of virus genetic material in a sample, such as viral DNA. These tests have a sensitivity of about 98%, much higher than comparative data for antigen tests. This is why PCR tests can detect active infections nearly every time, and it is rare for someone uninfected to receive a positive test. These require special equipment and are typically sent to a lab, with results often available within 24 hours, depending on location. Some PCR tests provide results in just 20 minutes, however, 12
