SUMMER 2014 EARLY ENROLLMENT APPLICATION MONTGOMERY This application provides you with an exclusive opportunity to enroll your child with ESF for the summer of 2014. Space is limited.

Applications will be accepted on a first-come, first-served basis, and enrollment is determined by postmark date. Please complete both sides of this application, sign the back of the form and return it with a $300 deposit per camper. Early bird tuition rates expire January 10, 2014.

If you need to cancel your child’s enrollment in camp, you must notify us in writing by March 1, 2014 in order to receive a refund. Cancellations on or prior to this date are subject to a $50 processing fee per child. No refunds will be given out after March 1, 2014 regardless of your registration date. Changes in enrolled weeks are based on availability and must also be made on or before the March 1, 2014 deadline.

I understand that my child should not attend the camp if he/she is not healthy. I understand that my child must abide by camp policies and the instructions of the camp staff.
