Accessing your Form 1095-C

Logging in to View your 1095-C Form While in your internet browser, type the following into the address bar https://benefits.plansource.com/logon/endeavor_air

✓Name: Jane Anderson

✓Your birth date in the format YYYYMMDD.

✓First initial of your First Name — j

✓Birth date of February 7, 1975 would look like this: 19750207.

✓ First six characters of your Last Name — anders ✓ Last four digits of your SSN — 1234

✓Password: 19750207

✓Username: janders1234

Please Note: After the you log on for the first time, you will be prompted to change your Password for security reasons. Best practice is to keep your new password in a secure location so they can access PlanSource easily in the future.

Logging in to View your 1095-C Form PlanSource may also require Multi-Factor Authentication (MFA) the first time you log in.

You will be prompted to select the telephone number, if one is on file, or add a new telephone number to receive the verification code to access your account.

Please Note: You should add your telephone number including area code without dashes.

You will be directed to the verification page to enter the verification code that has just been sent to the telephone number selected.

Choose Tax Documents

If a current year tax form exists, it will show here. If not, expand Prior Calendar Year
