Today’s older individuals have more options than ever, but the rules and terminology can be tough to wrap your head around.

Deciding where and how to live is a major decision for anyone. As we grow older, the choice can become even more critical.

Some of us will continue to live independently, with few changes in our daily lives. Others may prefer a

living arrangement that is smaller, more maintenance-free or economical. Some will need increasing help with personal and domestic tasks, while others ultimately will need the complete care and supervision found in assisted-living communities and nursing homes.

Many options have emerged in recent years that suit older persons’ housing and healthcare needs and lifestyle preferences. Yet too many people don’t know all the choices that exist.

They may have outdated images of dreary “old folks’ homes” — which is not what today’s modern senior community environments provide.

There’s a new focus on “personcentered” care and services, in which individual preferences are honored (aka, the culture-change movement). For more details, please visit the Culture Change Network of Georgia at www.

Options today range from homes designed to help older adults who favor independent living, to retirement communities for those whose need an increasing degree of care.

At some point, almost every aging person can benefit from some type of supportive Home and CommunityBased Service, known in the industry as HCBS. These services help with personal activities and/or daily living chores, provide healthcare assistance and/or make social opportunities more accessible. The most common types of HCBS services include: • Home care. For people living at home. Health aides can assist with personal hygiene, prescribed exercise, meal preparation, and some shopping and light housekeeping under the supervision of a licensed

nurse. Homemaker or concierge services can help with laundry, shopping, housekeeping, errands, driving and companionship.

• Home healthcare. Skilled nursing services as well as physical, occupational and speech therapies can take place in a person’s home with a doctor’s order.

Outside of staying in your own home, housing options range from independent-living communities and personal-care homes to nursing homes.

Here’s a look at each.

Retirement housing Sometimes called senior, or independent living. These communities provide a secure home environment but not the skilled healthcare associated with nursing homes.

Retirement communities offer privacy in safe, well-maintained buildings that often are designed to alleviate physical limitations that can come with growing older. Bathrooms can be equipped with handrails and grab bars, for example, or electrical outlets can be placed higher on the wall.

Support services such as meals, recreation and daily phone reassurance may be offered.

Fees and monthly charges vary, depending on the size of the apartment, the services offered and the income group the building is designated to serve. Government subsidies allow

senior-housing managers to offer lower rental rates to people who qualify based on income. Average fees: zero to $1,000 per month.

P ersonal care homes Also known as assisted-living communities. These are best suited for basically healthy aging adults who need daily personal assistance.

These communities may stand alone or be part of a retirement community.

Accommodations vary from single or double rooms to suites and apartments. People can receive personal-care services in their own homes as a supplement to family and neighborhood support.

Typical services include reminders or physical assistance with meal preparation, eating, bathing, dressing, toileting and other daily activities, medication, transportation, shopping, laundry and housekeeping.

Whatever the setting, individuals have a sense of contributing and controlling their own lives.

Nursing -home care This care falls into one of two categories: skilled care or intermediate care on a long-term basis.

Skilled nursing is an intense level of physician-authorized care on a 24-hour basis. Intermediate care is physician-authorized, 24-hour care on a less intensive level.

PAYING FOR IT Older adults and their families can pay for home and community-based services (HCBS) in several ways: • From their own private funds.

• Through Medicare, which covers a few services. This makes checking your eligibility guidelines worth your time.

• Through Medicaid, which pays for health-related community services for qualified clients.

• Through Medicare and Medicaid, which offer limited coverage for hospice care.

• Through Medicare, Medicaid and some private insurance, which provide limited coverage for home-healthcare services.

• Through local governments or other organizations that run such programs as phone reassurance, senior care activities, and information and referral programs, with little or no cost to participants.

All homes in this category offer a full array of personal, dietary, therapeutic, social, spiritual, recreational and nursing services as well as meals, laundry and housekeeping.

Good nursing homes help all residents capitalize on their strengths and compensate for their weaknesses in an atmosphere that is as home-like as possible.

Some residents and/or their families pay for this care through private funds or with private long-term care insurance.

Others rely on Medicaid. It’s important to know that Medicare is very restrictive in reimbursement, has considerable co-payments, and covers a maximum of 100 days.

Also available You or your older loved one can take advantage of services ranging from A to T. Here’s a rundown, in alphabetical order.

• Adult day services. Enables people to spend their daytime hours in organized, supervised social activities that can include physical and mental activities, exercise and more.

• Caregiver support or training.

Family caregivers learn techniques that help them better assist an older loved one.

•C ongregate dining programs.

Low-cost, nutritious meals served in group settings.

• Health education and wellness programs. Teaching, counseling, etc., that help a person maintain health.

• Home-delivered meals. Often known as Meals on Wheels. These bring nutritionally balanced meals to those unable to prepare their own.

• Hospice care. Nursing care for the terminally ill and counseling for family members. Nurses and social workers can visit regularly wherever the patient’s home may be.

• Phone reassurance. Regular phone contact that often gives those living at home a feeling of security.

• Respite care. Gives family caregivers a few days’ or weeks’ rest by providing temporary, overnight care for the ill or disabled.

• Senior centers. A place for daily visits and social/recreational services that reduce isolation and keep life interesting.

• Transportation. Services that help individuals keep appointments and take advantage of recreational opportunities.

IS ‘LIFE PLAN’ THE RIGHT FIT FOR YOU? In 2011, Georgia passed legislation that established and defined assistedliving communities (at least 25 apartments/units) that offer such services as administering medications and more opportunities to age in place.

Here’s a closer look at the terminology.

Continuing Care Retirement Communities (CCRCs) are also known as life-plan communities. They generally offer an innovative lifestyle that combines housing options with a continuum of care (nursing on one site to meet residents’ needs).

The three most common types of CCRCs are retirement housing, assisted living/personal care and skilled/intermediate nursing care.

CCRCs provide comprehensive services tailored to an individual’s needs, abilities and preferences. Services and amenities can include nursing and other health services, meals and special diets, housekeeping, transportation, emergency and personal assistance, dining services, wellness programs, and recreational/educational activities. Payment is generally through one of three long-term contracts: 1. U nlimited nursing care for little or no substantial increase in the usual monthly payment.

2. A specified amount of nursing care that, if exceeded, must be paid for by the resident.

3. Long-term nursing care, that if required, must be paid for at the full daily rate.

CCRCs often require a one-time entrance fee and monthly payments thereafter. Fees vary, depending on the type of housing and services offered. CCRC residents have a wide variety of activities and services available to them. Many continue to work, travel, volunteer and enjoy life.

When considering whether continuing care is affordable, review your current expenses, income and assets, noting that many expenses would no longer be incurred, and compare this to what the retirement communities that interest you would cost.

HEARING CARE IS HEALTH CARE

If you were sick, would you grab a friend’s pill bottle to treat what ails you? If you had poor vision, would you use your spouse’s eyeglass prescription? If your car needed tires, would you buy them for a cheaper price even if they didn’t fit your vehicle?

Obviously, the answer is no. So why would you buy a hearing aid that hasn’t been programmed to meet your specific needs? The answer is you shouldn’t.

That’s the message doctors at North Georgia Audiology and Hearing Aid Center want to make clear.

“It is important to consult with a licensed doctor of audiology,” says Dr. Christa Nelms. “We have eight years of education in hearing healthcare and are specialized in assessing and treating hearing loss and tinnitus [ringing] as well as other ear-related issues.”

According to a recent study reported on the Hear-It website (www.hear-it. org), temporary loss can, in many cases, be recovered if treated quickly enough.

Hearing affects everything between the ears, which relay meaningful sound to the brain. When sound signals from the ears are compromised, the brain works harder to fill in the gaps, and that can take a toll.

Doctors at North Georgia Audiology Center work to eliminate excuses often used for not getting help.

The top excuse: It’s too expensive.

Initial costs can be reduced by 30-40 percent at the center because patients can choose a pay-as-you-go plan versus the industry norm of paying a higher “bundled” price that covers services, supplies and office visits. Multiple financing options are available.

Another big excuse: Fear that the device will be visible and embarrassing.

MYTH BUSTERS Myth: I hear what I need to hear and get along fine.

Fact: You only hear what your hearing capacity permits. Reality, for you, is now altered. If you can’t hear it, you have no idea what you’re missing.

Myth: If I wear a hearing aid, it’s a sign that I’m old and frail. act: You’re just as likely to find F 40-year-olds with devices as you are older adults. According to a recent U.S. Centers for Disease Control and Prevention study, 1 in 4 adults age 25 or older has a correctable hearing loss.

That includes 40 million Americans under age 70.

Myth: I’ll save money if I buy online or at a retail outlet.

Fact 1: You get what you pay for.

Don’t be fooled into thinking you can pay one low price up-front for your device(s) and be set for life. Buying a device is just the beginning. It must be properly programmed by a healthcare provider, which, in reality, takes consultation, assessment, fitting, programming and follow-up for longterm success.

Fact 2: Warehouse clubs and hearing aid “stores” don’t accept or file your insurance. North Georgia Audiology’s staff checks eligibility for each patient. Complete diagnostic hearing testing is typically covered once a year and most plans include hearing aids.

DID YOU KNOW? Emory Johns Creek Hospital (EJCH) is a 110-bed acute-care facility located near McGinnis Ferry and Medlock Bridge roads. It’s staffed by 328 Emory faculty members, 386 private-practice physicians and 42 Emory Specialty Associate physicians. Services include emergency care, a birth center with Level 3 neonatal intensive care, adult intensive care, the Winship Cancer Center of Emory University, an infusion center, breast imaging with 3D tomography, a certified bariatric center, advanced cardiac and stroke care, sleep medicine, a wound care center and a pain center. In 2016, the hospital admitted 7,165 patients and treated another 71,487 as outpatients.

The hospital employs 859 staff members — clinical and nonclinical — in nursing, the pharmacy, administration, environmental services, physical therapy, facilities management, food and nutrition services, and more. Emory Johns Creek Hospital partners with churches, police, fire and community organizations to offer health and wellness screenings and educational events that are open to the public.

Diets Are Out, Mindful Eating Is In

Mindful eating is taking a psychological or emotional approach to eating — thereby enjoying the food and not wasting intake on empty calories. The practice of mindful eating has been around for more than a decade.

According to a “What’s Trending in Nutrition” survey from Pollock Communications and Today’s Dietitian 2016, more than 1,700 registered dietitians polled reported clients are practicing mindful eating instead of dieting as a path to healthier living.

Healthful eating is commonly considered important to achieving success and maintaining health and body weight.

Due to the popularity of weight-loss

diets, however, healthful eating can be confused with adhering to diet rules.

Conventional methods of healthful eating often involve restrictive eating, classifying foods as good or bad, and influencing how much and when to eat.

Mindfulness focuses on a slower and more thoughtful approach.

Mindful eating can help clients depart from restrictive eating by learning how to listen to and respond intuitively to internal cues from the appetite-regulating system.

It’s an extension of the practice of mindfulness.

A person who eats mindfully acknowledges that there is no right or wrong way to eat but also considers such factors as the effect of their

food choices on themselves and the environment.

All in all, the increased attention to mindful eating — or eating with purpose and care — is commonly used as an essential component of a non-diet approach to health and weight control.

This approach opposes restrictive eating techniques to control body weight. It includes self- and size-

acceptance to support self-care.

Perhaps this is the year you’ll want to adopt a mindful eating approach toward your own better health.
