2019 – Collective Intelligence Strengthened by the support of the many relationships within our stakeholders’ and partners’ community EFORT has been able to achieve some major goals as set forth in its strategy 2018-2020.

Particularly encouraging news comes from the attendance of figures for the 20th EFORT annual congress held in Lisbon with a 13% increase on the previous year.

20 years of continuing education and expertise: 2019 has been an anniversary for EFORT with its 20th annual congress.

On this occasion we re-established a regionally themed social event, the so called “Lisbon”-night, to celebrate and network with friends and colleagues from all around the world.

The scientific programme featured numerous symposia dedicated to the main theme of Registries and Impact on Practice. The EFORT Science Committee and the NORE Standing Committee built, together with several world-recognised experts, a specific agenda for joint arthroplasty specialists and health care professionals and/or researchers whose practices are directly linked to implant attributes and techniques.

Exciting developments happening throughout global orthopaedics and traumatology hold promise for continuously improving patient outcomes as they become better understood and refined. One such area is the field of robotics in surgery. A partnership between a key manufacturer and EFORT allowed us to launch the EFORT Robotics Fellowship programme. Both visiting and travelling fellowships have been made available to make this programme as interactive as possible and forming an excellent preparation for clinical practice outside of an academic environment.

We have also established a regular webinar series with 7 live broadcasts on various topics during 2019.

In a collaborative effort with the European Society of Cardiology we ran an event in Brussels focusing on Medical Device Registries. Orthopaedic and cardiovascular devices constitute over 50% of all high-risk implantable medical devices.

These examples hopefully demonstrate how we are meeting tangible objectives within the Orthopaedic and Traumatology profession in delivering purposeful education & training. We have formulated an e-learning strategy and established the concept of the EOTEP, the EFORT Orthopaedic and Traumatology Education Platform. New products were brought to the market through innovative collaboration models with industry. Further, we were able to organise some selected events in partnership, in Brussels, getting closer to politicians and other stakeholders to make the case for a regulatory environment favourable to patients and healthcare professionals alike.

In light of these achievements we believe more than ever in enhancing our impact through partnerships and collaborative environments, be it through our standing committees & taskforces but also experts, and the constantly moving “Brussels microcosm” which we feel is becoming less of a labyrinth and more an open playground to find solutions for issues of European and even global significance.

With revolutions ahead in applying new business models, coupled with the quest towards constant and increased transparency, organisations will need to be able to rely on trusted sources to “continually learn from feedback and to produce just-in-time knowledge for better decisions” 1.

Message from the Presidents and Secretary General

Though the concept is not new, Collective Intelligence, as we refer to, signifies the application of all that is entailed in working in networks and partnerships.

Re-assessing its core value and vectors of dissemination, Education and Congresses in EFORT’s case have certainly made their way into the methodology of the next stepsin strategising the organisation’s activities.

To this end, a congress innovation group was created. It already organised several workshops in which the industry‘s, the surgeons’ and trainees’ views were discussed on how to capitalise on new educational opportunities and create further added value.

Perhaps the start of this should be identifying the needs for competency-based education in Europe: an international needs assessment could be conducted so as to obtain an in-depth understanding of the current gaps and challenges of European O&T professionals, with the aim to provide evidence for the development of needs-driven educational and professional development activities.2 The path for the future seems to lean on agility, indeed!

COOPERATION AT NATIONAL LEVEL EFORT FORA have continued to collaborate with our stakeholders. This series of sessions, launched in 2002, aims to organise specific EFORT symposia during the annual congresses of our member National Societies.

The EFORT Board would like to acknowledge and thank the member societies for their invitations to attend various high level education meetings and congresses, promoting best practice.

In recent years, the EFORT Fora have attained a very high scientific level and have provided an excellent exchange between National Societies and EFORT. Accordingly, the interest in such Fora has grown significantly.

In 2020 EFORT is planning a new format of exchange and collaboration with the larger national orthopaedic societies each year at their annual congresses, in the format of a joint session building on EFORT’s visibility.

All other national societies will continue have the opportunity to continue to apply in September each year for Fora according to their wishes.

SPECIALTY SOCIETIES The Memorandum of Understanding established between EFORT and the Specialty Societies, including an Appendix relating mutual copperation at the EFORT Annual Congress, has been well received by all Specialty Societies and facilitated joint working atr the EFORT Lisbon congress in 2019.

The new methodology for developing the Congress programme, setting up a rolling programme of topics to be covered over a three-year cycle, is progressing. As reported earlier, this will also improve the input from National Associations at the same time as allowing the Specialty Societies to coordinate a specialist education programme and respond to ‘hot topics’ that arise between congresses.

In 2019, bi-annual meetings of the EFORT Specialty Societies’ Standing Committee (ESSSC) took place.

Apart from the specific topics and the three-year rolling programme of the scientific sessions beginning in 2021 the proposed specialty society curriculum was discussed. EFORT has initiated the idea to deliver a ‘minimum requirements’ specialist training curriculum with input from the specialty societies, with curricula in each specialty being based on the same structure, reassuring external reviewers that efforts are being made to ensure a harmonisatrion of standards between our diverse specialty groups. This project will be further developed and a first version could be ready in 2020/2021.

In the future, beginning in 2021 and as mentioned above, in order to improve the structure of the congress scientific content, EFORT’s objective is to introduce a 3-year rolling programme, in collaboration with the Specialty Societies. The specialty groups will specify the topics planned for their own particular session and identify complementary topics for other sessions in each congress, based on the curriculum.

The latter can then direct the work of the Science Committee, incorporating input from the National Societies, to create a comprehensive programme without undue overlap. Moreover an opportunity will remain to leave 10% of the programme free for hot topics and to work further alongside the National Societies.

Japanese Orthopaedic Association (JOA) 92nd Annual Meeting of the Japanese Orthopaedic Association from 09 to 12 May 2019, Yokohama, Japan.

Ass. Prof. Per Kjaesgaard-Andersen (Denmark) had the opportunity to represent EFORT during the Japanese National meeting of Orthopaedics.

Mexican Federation of Orthopaedics and Traumatology (FEMECOT) Joint academic activity at the XXX FEMECOT, from 30 October to 01 of November, 2019 Hermosillo, Sonora Mexico.

The EFORT delegation included Prof. Dr. Philippe Neyret (France) and Prof. Dr. Onder Aydingoz (Turkey).

From this joint programme Sergio Quintero Hernández, President 2018-2020 of FEMECOT said “For us it has been an honor to have EFORT participation, consolidating our friendly and scientific relationship, we are sure of the success of this academic and scientific cooperation between both societies in benefit for the professional development of the orthopedics and traumatology surgeons in Mexico”.

Dr. Prof. Klaus-Peter Günther (Germany) had the opportunity represent EFORT during the 12th International Conference of the Jordan Orthopaedic Association.

International Society of Orthopaedic Surgery and Traumatology (SICOT) 40th Orthopaedic World Congress from 4 to 7 December 2019, Muscat, Oman Prof. Dr. Philippe Neyret (France) presented a keynote lecture on the topic “Management of Meniscal Pathology in Footballers” and had the opportunity to also moderate free paper sessions. This was a great opportunity for EFORT to be represented in this international forum.

Egyptian Orthopaedic Association (EOA) EFORT-EOA Course during the 71st EOA Annual Meeting from 16 to 19 December 2019, Cairo, Egypt Similar to previous years EFORT had the privilege to jointly participate in the EFORT–EOA course which took place in Cairo from 16 to 19 December 2019 during the Egyptian National meeting of Orthopaedics. The EFORT delegation included Dr. Jean-Alain Epinette (France), Prof. Dr. Theofilos Karachalios (Greece), Prof. Dr. Luigi Zagra (Italy) and Prof. Dr. Philippe Neyret (France). The programme focused on hip and knee surgery.

The AIOT was established in 2018 to provide a platform of communication and collaboration between five leading international organisations in the field of orthopaedics and traumatology.

EFORT’s Framework for Educational Structure: the EOTEP

EFORT ANNUAL CONGRESS 2019 IN LISBON The 20th EFORT Congress brought together the Scientific Community devoted to Orthopaedic Care and specifically improved patient outcomes.

The congress’ theme in 2019 was “Registries and impact on practice” but a broader theme of passionate patient care emerges from a substantial gathering of students, practitioners, and industry influencers.

EFORT, true to its vision and as platform organisation provided an opportunity for its member societies to gather together in a more intimate venue this time, in the heart of Lisbon, Portugal to present more than 180 scientific and informative sessions featuring over 2,000 presenters from across the globe. The stated theme of “Registries and impact on practice,” also presented the perfect opportunity to dedicate the entire Volume 4 of EFORT Open Reviews (EOR) to this topic.

Beyond the stated focus on registries another theme emerged amongst presenters and participants alike: an unwavering commitment to improving patient care. The heartfelt commitment to that goal was palpable not only in the topics selected for discussion during the sessions, but also in the bottom line of each session attended, where patient care was uniformly summarised as a key priority of the presenter.

For example, in a discussion on the impact of smoking cessation on post-surgical outcomes on patients, a variety of presenters showed unanimous independent results that cessation programs have a measurable effect on providing better patient outcomes following a surgical intervention. The data were clear but highlighted as part of the discussion were the attitudes and behaviors that practitioners looking for a boost to patient care should undertake.

The concern for patient care was not only evident in open policy dialogues like that of the symposium on smoking cessation but was also clear in more practical sessions such as extremely technical talks on topics such as total ankle arthroplasty versus ankle arthrodesis. For every orthopaedic specialty imaginable there was a detailed discussion from which practitioners could take away new facts and review case studies.

In nearly every session attended, presenters interacted with the audience throughout , whether by general polling or involving the audience in discussions to find best practice in case studies. At times, the input was not what was expected by the presenter, taking the dialogue into new and unforeseen territories.

In the expert review session on pain management in total knee arthroscopy, for example, panelists presented real cases from their own practices. From a practical perspective, the cases and discussion were divided into two key subdivisions: perspectives on acute pain and discussions of chronic pain.

For an organisation so geographically diverse with a variety of members from across cultures, the level of interaction and camaradery amongst participants was impressive throughout. This was true even in moments of disaccord where camps on two divergent preferred practices debated the merits of one practice versus the other.

Polling, the diversity and inclusion of a variety of different presenters, and the background preparation that was obvious to the event kept these discussions scientifically informed and professionally beneficial to all involved.

Industry involvement in annual congresses is a staple of these types of gatherings, and the impressive display of the latest devices and tools for practitioners on show was no exception to this rule. The exhibition floor, however, was kept separate from the scientific discussions in such a way that students, professors and practitioners in all fields could on the one hand benefit from the scientific and data-led discussions with their colleagues, while in a separate moment see the practical application of the collaboration with manufacturers providing the tools to best implement the techniques being discussed in the scientific reviews.

Not to be overlooked was the poster hall where researchers were able to present and discuss their studies and results. Authors could interact directly with mingling congress attendees, increasing interactivity between peers and potentially raising the level of discussion on new research providing fresh perspectives.

EFORT in its own right provided a venue to bring together the top minds, best practitioners, and most influential industry representatives to flesh out new and better ways to provide patient care for those patients seeking better mobility, faster recovery times, and improved quality of life following issues of musculoskeletal health.

This annual forum is supplemented by numerous print and online publications and ever-increasingly involves follow-on e-learnings and interactive discussions throughout the year. Members voiced their requests for standards and continuously improving ways to share best practices and to improve their knowledge of the field and the innovative products and procedures available to them with one overarching thought in mind: improving patient care.

EFORT is recognised as the main European O&T organisation on National, European and International level and organises the EFORT Annual Congress on an annual basis. With its over 15 parallel session rooms, with over 180 sessions, an average of 3800 presented abstracts and a comprehensive exhibition representative of its industry, it is the largest cross-border O&T Congress worldwide.

The Lisbon 2019 Congress gathered just under 6000 participants. Compared to the previous year this represents an increase of 13% of delegates and 6% of attendance overall.

EOTEP ACCREDITED EVENTS EFORT developed a comprehensive new tool, the European Orthopaedics & Traumatology Education Platform (EOTEP), to provide an educational overview in orthopaedics and traumatology for those wishing to advance their professional growth. Now in its 3rd year, the EOTEP has collaborated closely with the Speciality Societies to endorse their specialised courses and act as a reference point for education in orthopaedics and trauma. We are continuously updating the content of the platform and are open to receive education from all areas of the musculoskeletal training in Europe.

The EFORT Travelling Fellowship Programme aims to enhance and unify orthopaedic training, continuous medical education and knowledge transfer, supports and creates new personal bonds to improve the mutual understanding, collaboration and unity of all National Associations of Orthopaedics and Traumatology in Europe.

The fellowship provides a possibility to spread new ideas and introduce fellows to local traditions, social systems and the culture of the host countries.

The fellowship is open to surgeons from European countries selected by their National Association, in order to favour exchanges between the Travelling Fellows and the Orthopaedics of the inviting nation.

This year’s EFORT Travelling Fellowship took place in Tartu and Tallinn, Estonia from 12 to 18 May 2019 and in Hungary from 06 to 13 October 2019.

For the first, twelve participants from Europe were given the opportunity to interact, visit local hospitals and take part in lectures on everything from highly specialised trauma to ortho-geriatrics. Highlights of the presented lectures of the programme in Hungary were the treatment of knee chondral defects and some innovative techniques, such as mosaicplasty and a video lecture on the removal of a sacral chordoma that raised much interest. Not to forget the visit of the National Center of Spinal Disorders in Budapest.

MARK PATERSON TRAVELLING FELLOWSHIP 2019 The fellowship is named after John Mark Hamilton Paterson to honour his work developing Children’s Orthopaedic services in East London.

After an absence of 1 year, The British Editorial Society of Bone & Joint Surgery and EFORT were ready to re-launch the Mark Paterson Travelling Fellowship, which was held from 2 to 14 September 2019 in Padova, Italy (Dept. of Orthopaedics and Orthopaedic Oncology, University of Padova); Lisbon, Portugal (Orthopaedic department of Hospital Cuf Descobertas); and Madrid, Spain (Hospital La Paz).

The opportunity was taken to revamp the programme, reestablishing the periodicity of every second year and the location, which will always be continental Europe. The rotation system has been abandoned, and the fellowship will no longer be organised every other year in the United Kingdom nor will the pool of potential applicants alternate between UK surgeons and those from Continental Europ. The applications are now open to all European orthopaedic and trauma surgeons, including the United Kingdom and Northern Ireland surgeons.

One of the aims of fellowship programmes is the opportunity to broaden clinical experience, make important professional contacts and gain experience in presenting their work and opinions in an international setting. This naturally applies to the Mark Paterson Travelling Fellowship as well.

VISITING FELLOWSHIPS PROGRAMMES EFORT and the EFORT Foundation provide a series of visiting fellowship programmes to support young and active orthopaedic & trauma surgeons in their career paths for continuing personal development and medical education.

This is achieved by enhancing their professional skills through continuous education and providing them experiences on the latest surgical interventions, approaches and new techniques.

We are working with a number of partners to set up the respective programmes and are grateful for their support. ROBOTIC FELLOWSHIP Robotics Fellowship programmes supported by Stryker were launched during the EFORT Annual Congress 2019 in Lisbon.

During the EFORT Robotics Fellowship Program, the fellow will be trained in all aspects of robotic-assisted hip and knee arthroplasty surgery. This program will be an excellent preparation for clinical practice and to learn how to effectively integrate robotics into the day-to-day practice.

CLINICAL RESEARCH FELLOWSHIP SUPPORTED BY MEDACTA One clinical research fellow is chosen for this programme and visits a selected host center according to clinical research availability. The fellow is guided through the research process including research ethics from the development of a hypothesis to data collection, manuscript writing and presentation at national and international orthopaedic conferences.

The focus is around hip, knee or shoulder arthroplasty.

In September 2019, the fellow Dr. Eran Beit-Ner from Israel started his 12-month Clinical Research Fellowship under supervision of Prof. Dr. Leela BIANT in Central Manchester NHS Foundation Trust, Manchester, United Kingdom. His research topic is “A personalised precision approach to surgery of the osteoarthritic knee”.

BRAZILIAN & ARGENTINIAN FELLOWSHIPS The Brazilian & Argentinian Visiting Fellowship is an initiative between EFORT Foundation and the Brazilian Society of Orthopaedics and Traumatology (SBOT) and the Argentinian Association for Orthopaedics and Traumatology (AAOT). Every year, the two organisations are requested to nominate one fellow each who will participate in the fellowship.

EFORT EORN – European Orthopaedic Research Network Interactive map What is EORN? – A user friendly geographical interactive map of Europe reflecting the current European orthopaedic research in Europe, identifying research contributed during the 2019 EFORT annual congress in Lisbon from research Institutions across Europe and included in the EFORT Congress Scientific Programme.

The aim of the EORN is to advocate promotion of research throughout Europe in particular but with a broader view, as in the future it will integrate nonEuropean O & T projects presented during congress.

TRAINING FOR THE UPCOMING GENERATION FORTE SUMMER SCHOOL 2019 The 4th edition of the FORTE Summer School took place for the second time in Milan on 08 to 12 July. It gathered 37 renowned experts and 108 residents from 18 different European Countries. The Summer School includes basic and advanced review courses designed for orthopaedic trainees at all levels, either as an an-nual review/update or as preparation for their Orthopaedic Board exams.

The format of the Summer School provides networking, workshops and exchange of ideas between participants and faculty from different European countries.

European Activities

EBOT EXAM AND INTERIM EXAM Over more than a decade now the EBOT examination has gradually grown in status. Increasing uptake of the examination has led to an increased need for examiners, examiner training and logistical support.

In addition to the written and oral part in English language, the European exam also found acceptance among some of the national societies and has now been run three times in Spanish for the oral part, in collaboration with SECOT, the Spanish society.

This European certification has also been included in French regulations in a degree pursuant to article 59 setting for the organisation of the 3rd cycle of the medical studies (organisation du troisième cycle des études de médecine).

The exam is open to certified specialists from Europe, trainees in their final year of training in a recognised European training scheme programme as well as non-EU citizens working in an EU country.

In 2018, EFORT and UEMS started discussions about the formation of an independent body in charge of the organisation and administration of the examination and an interim board representing both parties was set up. However, during the last meeting of the interim board in Lisbon in November 2019, it was decided not to set up an independent organisation, but rather both parties would enter a cooperation agreement, equally sharing the financial liability. Administration and organisation of the EBOT exam and all related activities remains with EFORT. nnEBOT Interim Exam 2019 The EBOT Interim Exam is an annual assessment for all European Orthopaedic trainees. It gathers information at European level, handed over to both trainees and heads of training, regarding not only the performance of the different residents but also detailed data on how they have been doing in the five different areas assessed at the final EBOT Exam.

In 2019, the interim exam gathered more than 1000 participants from 17 European countries in more than 160 hospitals. nnEBOT Final Exam 2019 The EBOT Final Exam consists of two parts, section I the written part and section II the oral part. The written part of the EBOT final exam is to be completed online and consists of 100 multiple-choice questions. The exam is held in different examination centers in Europe. The oral examination consists of five sessions that last 30 minutes each and will test candidates in the subjects of spine, lower limb, upper limb, children’s orthopaedics and basic science related to orthopaedics.

Over 170 candidates sat the written part on 11 June 2019 with 124 passing part I and moving on to section II, the oral section, which is divided into the oral part in English and the one in Spanish for Spanish candidates only. Section II in English in 2019 took place in Manchester, United Kingdom in October; section II in Spanish was organised in Madrid, Spain in November 2019.

European Activities

REPORT OF THE EUROPEAN AFFAIRS COMMITTEE In this era, in which politicians need accurate intelligence to inform their decisions from a voice representing the widest possible viewpoint of the orthopaedic and traumatology profession, EFORT is positioning itself to represent its National Associations and the Specialty Societies with which we work closely.

As a strategic option to fulfil this aim EFORT decided to become a member of the Biomedical Alliance1 which we joined a little more than a year ago.

This significantly impacted our influence on decision makers. The relationship between EFORT and the Biomedical Alliance continues to flourish. It remains the case that a fundamental benefit of this relationship is that we now have a voice in European politics through the Alliance, which is increasingly recognised in Brussels as representing all medical specialists.

In August, the Biomedical Alliance became a recognised European Medicines Agency stakeholder, allowing its members including EFORT to participate in consultations on medicines relevant to our specialty. We also achieved stakeholder status in a new electronic platform that allows questions to be put to parliamentarians and committee members directly.

In addition to the call for experts to join panels evaluating medical devices that we have previously publicised, EFORT members have also received an invitation to participate in the setting of European Research priorities. EFORT was represented in a Horizon European workshop evaluating funding opportunities for Biomedical alliance members. The alliance, with the support of EFORT, was even recently able to send an open letter to the European Parliament, which resulted in a change in the title of one of the new European College of Commissioners to include ‘research’, bringing science and innovation to the political forefront in the health arena.

EFORT joined the Alliance just as a short life working group on CME reported back to recommend that the Alliance sets up a permanent CME experts committee, which will work to harmonise the standards for assessment and accreditation of CME events and ensure that this is not driven by commercial influences.

EFORT has a seat on that committee.

1 The Biomedical Alliance came into existence to facilitate cross-specialty research cooperation but has evolved such that it has a voice in the European Parliament, which is less than 1km away from the Alliance’s headquarters in Brussels. Whilst the National Medical Associations of each European Country have delegates in UEMS, which also carries political influence, the Biomedical Alliance is formed by delegates representing the Medical Specialties at European level and is based in the premises of one of its founding member associations – the European Society of Cardiology.

European Activities

Separately the Biomedical alliance is concerned about the impact of PlanS on orthopaedic researchers and Specialty Societies that offer a journal as a member benefit. PlanS aims to ensure that all publicly funded research is published only in open access journals, but this horourable aim could have meant the demise of all subscription journals which presently help fund medical education in many specialties.

EFORT has contributed to a delegation interacting with CoalitionS, the instigators of the plan, to find a way through this development in the publication of Science in a way that does not cause unintended damage to medical practitioners and their education.

Events organised and attended: On 04 April 2019, Per Kjaersgaard-Andersen, the President of EFORT at the time, spoke at the European Parliament in a meeting organised by the Panel for the Future of Science and Technology. The subject of the meeting was ‚Technology and Innovation of Human Implants‘ and the EFORT President was able to reach out with a message to European Parliamentarians with his talk on the importance of registry data in assessing the outcomes of implant utilisation and its impact on patient safety.

On 17 June the Biomed Alliance, together with the European Society of Cardiology (ESC) and the European Federation of National Associations of Orthopaedics and Traumatology (EFORT), organised a Joint Workshop on Medical Device Registries in Brussels.

The workshop aimed at exploring what role clinical registries may play in informing regulators’ market surveillance activities, and identifying a possible “common framework” for EU registries responding to the needs of regulators, patients, physicians and industry alike. The event brought together key experts in data collection, review and management, including European Commission officials, healthcare professionals, and patient and industry representatives.

In preparation is also an inaugural workshop in Brussels2 which will be attended by key decision makers and important stakeholders. With the introduction of the new Medical Device Regulations (MDR) having a significant impact on the manufacturers and introduction of new devices into the market this event will address the impact of this concept and discuss issues around development, introduction and clinical application of implants, focusing on hip and knee arthroplasty, to start with.

European Activities

POST-OPERATIVE PAIN MANAGEMENT EFORT partnered with Grünenthal to raise further awareness of the unmet needs in postoperative pain management (POPM) in order to develop solutions that improve POPM across Europe.

For 6 months, we involved experts from across Europe in interviews, debates and other discussions to generate a better understanding of physicians’ and patients’ perspectives on the issues surrounding their practices.

During our discussions, there were a number of consistent themes that continued to arise, which we have addressed throughout various articles. As mentioned above, these articles are part of an ongoing series to raise awareness of a broad set of topics impacting orthopaedic practice to broaden the collaborative discussion around improving patient surgical outcomes, and in particular POPM. The opinions expressed in the conducted interviews are meant as an informal conversation to facilitate dialogue.

In 2019 a strong topic (full of concern) has arisen to be the use of opioids in POPM. Rather than focusing on this sole aspect we engaged in several discussions with our experts, to try and see how from different perspectives of conditions and care, opioids could be best administered. These discussions included: Post-surgical chronification of pain: discussions provided further details on the mechanisms behind the chronification of pain and how better understanding of those mechanisms can provide practitioners with a more effective approach not only to pain management, but also to pain prevention.

Wound closure and infections: Reducing infection has changed practice based upon avoiding dressing changes and standardising on a triple closure method. The more frequently the dressing requires changing, the more opportunities there are for the wound to be exposed to bacteria…The triple closure method seeks to reduce chances of wound leakage providing two subcutaneous layers, a dermal layer, then clips and glue on top. This method looks to give absolute certainty that there are no leaks as the wound heals.

Pain management treatment gap after discharge: There are many reasons to need to find the proper balance between quick mobilisation and early discharge and having a strong coordination with exercise or a rehabilitation program while simultaneously giving patients the opportunity to have access to pain relief.

And one of the key issues for practitioners to keep in mind with the pain management plan is the aim to reduce the opioid load. This can be achieved with the use of multimodal analgesia consisting of the combination of nonopioids and atypical analgesics. While each patient is different, it is rare that a pain plan can avoid 100% the use of opioids as a strong analgesic in the first hours and days following a surgery. In these cases, transitioning a patient from hospital care to home puts the burden and trust of pain management more into the hands of patients.

Outreach

Over the last year we have been progressing on our dissemination channels and improving our presence on the net. We reporting in 2018 that the congress section of the EFORT website was launched successfully.

The next step in this journey is the re-design of the public website and the further development of the Customer Relationship Management (CRM) area.

On this note, EFORT has undergone an extensive audit of its privacy policy in preparation to the European General Data Protection Regulation (GDPR), which will have been implemented by the time this report is published, in May 2020. The new document emerging from this analysis will be reflected in the privacy policy posted on the new website. Further, it guides EFORT in its interaction with providers and contractual agreements in the field of data management.

In terms of marketing, we have been exploring new ways to reach out notably on specific activities like the webinars and the newly created EFORT Robotics Fellowship programme. Indeed, due to the specificity of these programmes, a better segmentation of the audience and enhanced analysis of the behavioral marketing patters were needed. A new social media approach was adopted, with a small investment allocated to Sponsored Ads. The return was conclusive, both in terms of project‘s visibility and global branding.

We continue our progression in social media engagement and have added Instagram as a new channel, which per definition attracts new type of visitors.

SOCIAL MEDIA EFORT has ensured the continuity of its social media presence and the constant growth of its Twitter, Facebook, LinkedIn and Youtube channels up to a regular posting. An Instagram account was also opened in October 2019 to meet the new information channels trends.

EFORT’s social media news strategy reflects the development of new EFORT products and programmes (EFORT Open Reviews, new campaigns such as Postoperative pain management). AUDITOR‘S REPORT

Report of the statutory auditor to the General Assembly of EFORT Rolle Report of the statutory auditor on the financial statements As statutory auditor, we have audited the financial statements of EFORT, which comprise the balance sheet, income statement, cash flow statement, statement of changes in restricted funds and general funds and notes (notes 1 to 7.7), for the year ended 31 December 2019. As permitted by Swiss GAAP FER 21 the information in the performance report is not required to be subject to audit.

Executive Board’s responsibility The Executive Board is responsible for the preparation of the financial statements in accordance with Swiss GAAP FER, Swiss law and with the articles of the federation. This responsibility includes designing, implementing and maintaining an internal control system relevant to the preparation of financial statements that are free from material misstatement, whether due to fraud or error. The Executive Board is further responsible for selecting and applying appropriate accounting policies and making accounting estimates that are reasonable in the circumstances.

Auditor’s responsibility Our responsibility is to express an opinion on these financial statements based on our audit. We conducted our audit in accordance with Swiss law and Swiss Auditing Standards. Those standards require that we plan and perform the audit to obtain reasonable assurance whether the financial statements are free from material misstatement.

An audit involves performing procedures to obtain audit evidence about the amounts and disclosures in the financial statements. The procedures selected depend on the auditor’s judgment, including the assessment of the risks of material misstatement of the financial statements, whether due to fraud or error. In making those risk assessments, the auditor considers the internal control system relevant to the entity’s preparation of the financial statements in order to design audit procedures that are appropriate in the circumstances, but not for the purpose of expressing an opinion on the effectiveness of the entity’s internal control system. An audit also includes evaluating the appropriateness of accounting policies used and the reasonableness of accounting estimates made, as well as evaluating the overall presentation of the financial statements. We believe that the audit evidence we have obtained is sufficient and appropriate to provide a basis for our audit opinion.

Opinion In our opinion, the financial statements for the year ended 31 December 2019 give a true and fair view of the financial position, the results of operations and the cash flows in accordance with Swiss GAAP FER and comply with Swiss law and with the articles of the federation.

Report on other legal requirements We confirm that we meet the legal requirements on licensing according to the Auditor Oversight Act (AOA) and independence (article 69b paragraph 3 CC in connection with article 728 CO) and that there are no circumstances incompatible with our independence.

In 2019 and 2018, EFORT received grants from the Industry to conduct several projects for fellowships, education and communication. EFORT is currently in discussion with a company in order to compensate the negative funds for one project by a transfer from other funds not yet used.

NOTES TO THE FINANCIAL STATEMENTS General information The European Federation of National Associations of Orthopaedics and Traumatology (“EFORT”) is an association located in Rolle, Switzerland.

The aim of the Federation is the promotion of the sciences of orthopaedics and traumatology. To accomplish these objectives, the Federation organises European congresses and training programmes, collaborates with universities and other qualified institutions, stimulates basic and clinical research and undertakes any other activity which might be useful in fulfilling its aims. The funds of the Federation may be used only for the aims outlined above.

In 2019 and in 2018, the Federation employed less than 50 full-time equivalent employees.

1. Summary of significant accounting policies The principal accounting policies applied in the preparation of these financial statements are set out below.

1.1 Accounting principles EFORT prepares its accounts in compliance with the policies described in the following notes.

Accounting policies applied are in compliance with the Swiss GAAP FER (Swiss Accounting and Reporting Recommendations) 21.

As permitted by Swiss GAAP FER 21, the accounts were prepared in compliance with the conceptual framework, the fundamental recommendations and recommendation 21. The accounts also comply with the article 69a of the Swiss Civil Code.

Information required by Swiss GAAP FER 21 on the performance of the Federation, and not disclosed in the financial report, are included in the annual report.

The financial statements have been prepared under the historical cost convention. The income statement is presented using the classification of expenses by function. The statements are based on economic values and present a true and fair view of the Federation’s assets, financial position and results of operations.

1.2 Foreign currency translation a) Functional and presentation currency Items included in the financial statements were measured using the currency of the primary economic environment in which the Federation operates (‘the functional currency’), which is Swiss Francs (“CHF”). The presentation currency is CHF. Annual Report 2019

b) Transactions and balances Foreign currency transactions are translated into the functional currency using the exchange rates prevailing at the dates of the transaction. Foreign exchange gains and losses resulting from the settlement of such transactions and from the translation at year-end exchange rates of monetary assets and liabilities denominated in foreign currencies are recognised in the income statement.

Balances in foreign currency are revalued using the exchange rates prevailing at the dates of closing.

2. Principles of valuation 2.1 Cash and cash equivalents Cash and cash equivalents include cash, balances in bank accounts, and short-term time deposits with a residual term of less than three months. They are valued at their nominal value.

2.2 Accounts receivable Accounts receivable are amounts due from parties for services performed in the ordinary course of business. If collection is expected in one year or less (or in the normal operating cycle of the business if longer), they are classified as current assets.

These receivables are valued at their nominal values. Credit default risks are accounted for by specific allowances.

2.3 Property, plant and equipment Property, plant and equipment are valued at purchase cost less any depreciation required by generally accepted accounting principles. Depreciation is charged on a straight-line basis over the useful life of the fixed asset. The assets’ residual values and useful lives are reviewed, and adjusted if appropriate, at the end of each reporting period.

Gains and losses on disposals are determined by comparing the proceeds with the carrying amount and are recognised in the income statement.

2.4 Intangible assets Computer software Costs associated with maintaining software programs are recognised as an expense as incurred.

Purchased software are capitalised and amortised on a straight-line basis over a period their estimated useful lives, which does not exceed 4 years.

Internally developed software costs are capitalised only if the following conditions are met: • software are clearly identifiable; • costs are reliably determinable; • the group has sufficient resources to complete their development; • software bring measurable benefits to the group over the course of several years.

The amortisation of internally developed software starts when the asset is ready for use.

2.5 Impairment of assets The value of non-current assets is assessed on the balance sheet date for signs of impairment. If there is evidence of any lasting reduction in value, the realisable value is calculated (impairment test). If the book value exceeds the realisable value, the difference is recognised in profit and loss as unscheduled depreciation.

2.6 Provisions Provisions are recognised if an event in the past gives rise to a justified, likely obligation which is of uncertain timing or amount, but which can be estimated reliably. Provisions are measured on the basis of the estimated amount of money required to satisfy the obligation.

2.7 Trade payables and other liabilities Trade payables are obligations to pay for goods or services that have been acquired in the ordinary course of business from suppliers. Accounts payable are classified as current liabilities if payment is due within one year or less. If not, they are presented as non-current liabilities.

Trade payables and other liabilities are stated at their nominal values.

2.8 Employee benefits Wages, salaries, social contributions, paid annual leave, sick leave and other benefits are paid or accrued undiscounted in the year in which the associated services are rendered by employees of the group. Legal or constructive obligations such as bonus are recognised for the amount expected to be paid in the year the services are provided.

Financial statements

EFORT is a member of a collective occupational pension that fully insures the risks with an Insurance Company. This pension scheme is financed by employer and employee contributions.

The economic impact of existing pension schemes on the group is reviewed each year and the related economic benefit or liability recognised. The employer contribution reserves available are recognised as assets. Changes in employer contribution reserves and stated changes in economic impact arising from excess/insufficient cover for pension schemes are recognised in the income statement under personnel expenses.

2.9 Exceptional items Exceptional items are disclosed separately in the financial statements where it is necessary to do so to provide further understanding of the financial performance. They are material items of income or expense that have been shown separately due to the significance of their nature or amount.

2.10 Revenue recognition Revenue is recognised when the services are rendered, if the amounts can be measured reliably and if it is probable that the Federation will receive future economic benefits.

The main revenue streams of the Federation are Membership fees, Annual Congress income, donations and grants for specific projects.

Membership fees are recognised in the accounting period they relate to.

Annual Congress revenue is composed of registration fees and sponsorship income.

Revenue from sponsorship and registration fees are recognised in the period when the services are rendered. Sponsorship invoiced in advance are deferred in the balance sheet.

Grants and Donations received for restricted projects are recognised as an income when expenses are incurred. If they are not used or partially used during the financial year, the unused amounts are recorded in restricted funds. When used in subsequent years, the restricted funds are reduced accordingly. These movements are captured by the variation of restricted funds in the income statement.

2.11 Leasing Leases in which a significant portion of the risks and rewards of ownership are retained by the lessor are classified as operating leases. Payments made under operating leases (net of any incentives received from the lessor) are charged to the income statement on a straight-line basis over the period of the lease.

The Federation leases certain property, plant and equipment. Leases of property, plant and equipment where the Federation has substantially all the risks and rewards of ownership are classified as finance leases. Finance leases are capitalised at the lease’s commencement at the lower of the fair value of the leased property and the present value of the minimum lease payments.

Each lease payment is allocated between the liability and finance charges. The corresponding rental obligations, net of finance charges, are included in the Finance Lease Liabilities. The interest element of the finance cost is charged to the income statement over the lease period so as to produce a constant periodic rate of interest on the remaining balance of the liability for each period. The property, plant and equipment acquired under finance leases is depreciated over the shorter of the useful life of the asset and the lease term rebates and excluding taxes and value-added taxes.

An account with the Raiffeisen bank in Switzerland was opened for interest remuneration reason and to dilute risk between banks.

From June 2019, the EFORT Board members decided to cease the payment of secretarial fees and to transfer the monthly amount on a separate bank account. In 2019, a total amount of EUR 18,900, equivalent to approximately CHF 21,000, was transferred to the Raiffeisen bank account. These funds are available for future projects.

O.I.C. srl (“OIC”) handled the organisation of the annual congress from 2006 to 2011. OIC deducted withholding taxes on Event income by 27.5% for the fiscal year 2006 (for a total amount of CHF 374 thousands) and only 5% for the following years (for a total amount of CHF 546 thousands).

EFORT filed two separate claims of reimbursements for the tax withheld in 2006 of 22.5% and 5% respectively. As Italian Tax Authorities rejected EFORT’s claims, both parties entered into two litigations: 1. Withholding tax of 22.5% - CHF 290 thousands: in this litigation, EFORT lost in first instance, while gained in second instance. In March 2016, EFORT filed defensive memorials with the “Corte Suprema di Cassazione” (Supreme Court). On 27 March 2019, the Supreme Court confirmed that EFORT was entitled to a full refund and ordered the Italian Tax Authorities to proceed with the payment of the withholding taxes and associated legal costs.

2. Withhodling tax of 5% - CHF 83 thousands: the Court did not accept EFORT’s request of reimbursement in both first and second instance. As a consequence, the judgement became final.

In addition, EFORT had not presented other claims of reimbursement for the tax of 5% withheld from 2007 to 2011. Considering the legal limit of 48 months to claim the reimbursement, EFORT wrote-off CHF 630 thousands in 2013, corresponding to the years 2007 to 2011, for CHF 547 thousands, and to the second litigation on the 5% of withholding tax in 2006, for CHF 83 thousands.

The remaining amount of CHF 277 thousands corresponding to the 2013 balance of CHF 290 thousands (EUR 237 thousands), which was revalued at 31 December 2017, was considered as not recoverable based on legal advice obtained. An impairment charge of CHF 277 thousands was recorded in the financial year 2017.

Despite, the positive decision rendered by the Supreme Court on 27 March 2019, delays for the payment may be expected from the Italian Tax Authorities and it may be necessary to action further legal procedures for the decision to be executed. Consequently, EFORT has assessed that the full provision should remain as at 31 December 2019. The decrease of the balance of the receivable and provision to CHF 257 thousands is due to foreign exchange differences..

7.1 Pledged assets The rent of the Rolle offices is guaranteed by a deposit with the UBS for CHF 28,119 (2018: CHF 29,126 at BCV).

7.2 Pension Fund The position with the pension fund at the end of the year was a payable of CHF 36,811 (2018: CHF 35,691).

The pension fund is fully insured with an insurance company. The capital ratio is therefore 100%.

7.3 Benefits to the Executive Committees and to the Management Indemnities to cover secretarial expenses and paid to the Members of the Executive Committee amounted to CHF 20,524 (2018: CHF 50,305). From June 2019, the Executive Committee decided to terminate the payment of secretarial fees. Since then, the amounts are transferred from the main bank account to another bank account considered as a reserve bank account.

These secretarial expenses exclude reimbursements by EFORT of the effective costs incurred by the Members of the Executive Board and other Committees in the normal course of their functions.

As permitted by Swiss GAAP FER 21, benefits paid to management are not disclosed.

7.4 Personnel For the year 2019, total personnel costs amounted to CHF 1,502,970 (2018: CHF 1,463,404).

7.5 Transactions with related parties Receivables and payables to related parties were disclosed in the relevant notes. Transactions with related parties other than salary and indemnities are disclosed below.

The EFORT Foundation was invoiced for CHF 13,320 (2018: CHF 13,925), net of VAT, by EFORT for administration services.

7.7 Subsequent events Due to the pandemic situation caused by the Covid-19, and following the decision of the Austrian government of 06 April to forbid events until at least end of June 2020 the EFORT annual congress 2020 in Vienna, Austria, was finally postponed to 2021 following the decision made by the EFORT Board. The decision was rendered public on 06 April 2020 through an official statement posted on the usual communication channels such as the website of EFORT, personalised mailings, news bulletins and social media.

In order to mitigate the financial consequences, EFORT applied the following measures: • Implementation of partial unemployment for all staff. EFORT will receive indemnities from the Swiss Government to cover for the work reduction.

• Negotiations with the major suppliers and partners involved in the organisation of the congress. This concerned mainly the professional congress organiser, the hotels, Messe Vienna Exhibition & Congress Center and M-events for audio and technology services. EFORT was able to transfer most commitments and expenses to the 2021 congress.

• Launch of a virtual congress taking place from 28 to 30 October 2020 and will propose a comprehensive educational programme, as well as a virtual exhibition.

Based on the latest cash projections, EFORT is confident that the situation is sustainable, and that the entity will continue to operate with no adverse financial difficulties. The financial statements have therefore been prepared based on the going concern principle.
