Well child visits are required with all vaccinations.
Space and vaccine stock are limited. Children covered by insurance, Medicaid or CHIP get FREE vaccinations and a well child visit through the insurance carrier. Please bring your insurance card and a parent/guardian's ID. If your insurance requires a PCP (Primary Care Physician), please call your insurance to change your PCP to Lone Star Family Health Center.
IMPORTANT: All patients must bring a list of required vaccinations from their school and a shot record.
