Itisadifferentkindoftextbookofpsychopharmacology,notmeanttoreflectthe commonconsensusofWhatmostcliniciansbelieveorpractice.ItisnotamerecompendiumofcommonlyacceptedViewsInthatsense,itisnotatextbook,astypically conceived,butachallengeTruthisnotamatterofmajorityvote,HenryDavidThoreau saidVVhatisthestatusgun,orthemainstreamView,isnotnecessarilytrue,includingin clinicalmedicineThistextbookseekstopresentwhatistrue,asbestasitsauthorcandi vine,irrespectiveofpopularityorcommonality.Doubtlesstherewillbemistakesthatthe futuremightcorrect,buttheywillnotbedrivenbypreferenceforconventionalWisdom Itishopedthatreaderswillseeksuchanapproach,evenifitchallengesorconflictswith someoftheirownviewpoints,andthattheymightusethistextbookaccordingly,not merelytoacceptitasawhole,nortorejectitasaWhole,butrathertouseittothinkabout theirassumptionsandbeliefs,soastobecomebetterclinicians

Clinicalpsychopharmacologyispracticedtodayinamanneropposedtotherestof themedicalprofession.Psychiatricpracticetendstoanon~Hippocraticapproach(the Galenicapproach),whichtherestofInedicineputasideacenturyago(Chapter 1 6 ) . Othertextbooksofpsychopharmacologyunconsciouslyandwithoutapologyteachthe Galenic,nonHippocraticapproach.Inthisview,biologicaltheoriesarethebasisfor decisionmaking.Oneofthebestaspectsofrelianceonbiologicaltheoryprimarilyisthat factscanbedistortedtofitthetheoryItmatterslittlewhetheroneusesfourhumors,or threemonoamines,ortheegoandtheidmanyfactscanbedistortedtojustifyafavored theory

Incontrast,theHippocraticapproachprioritizedclinicalobservationoverbiolog icaltheory.Itfocusedonsymptomsinrealclinicalpractice,notspeculationsabout

bodilycausesOvertilne,thisclinicalapproachinspiredthefoundersofclinicalre searchinmedicine,leadingtotheuseofstatisticstorevolutionizeclinicalmedicine, aswiththediscoveryofthemethodofrandomizationtoproveordisprovetreatment efficacy.

Yetbiologyalwayshasbeenseductive,andclinicalresearchcanappearpedestrian ResearchpublicationsandfundingaremuchgreaterintheformertOpicthanthelatter Intheend,biologicalresearchstillneedstobetranslatedintoclinicalpractice.Biological speculationitselffailstoachievethattranslation.

TruthsoftheoryaretransientNotonlyisGalenoutofdate,butsoaremanybiolog icaltheoriesofrecentvintage,likethecatecholaminehypothesisofdepressionorthe dopaminehypothesisofschizophrenia.Clinicalobservationandresearch,incontrast,is steadier:thesamemelancholiathatl-lippocratesdescribedcanbediscernedindepres siontoday,-thesamemaniathatArateusofCappadociaexplainedinthesecondcentury ADisvisibleincurrentmania

Clinicalresearchisthesolidgroundofmedicine,-biologicaltheoryisanecessary butchangingsuperstructureIftheserelationsarereversed,thenmerespeculationtakes over,andthemoresolidgroundofscienceislost.Thistextbookwilltakethatclinical approach,explicitlyeschewingthecommonstandardbiologicaltheoryapproachto teachingpsychopharmacology.

Innon-Hippocraticmedicine,manytreatmentsareprovidedaggressivelyathigh dosesandforindefinitedurations,andallsymptomsaretreatedinthisaggressivemanner equallyInHippocraticmedicine,thereisascientificallyandclinicallysoundrationale forwhentobeaggressivewithdosingandtypeofmedication(fordiseases)andwhento bemoreconservativeandcautious(fornondiseases).HencetheWisdomofParacelsus, Whocanbeparaphrasedtosaythat:allmedicationsaretoxic;itisonlytheirdosingand indicationwhichmakesthemtherapeutic.

Thistextbookwilltrytodotwothings:Oneidentifyanddiiferentiatepsychiatric diseasesfromsymptomsandfrom“clinicalpictures,”which,althoughreceivingpsychiatricattention,arenotdiseasesTwoteachthepracticeofpsychopharmacologyof diseasesbasedonexaminationoftheclinicalresearchevidence,notthroughbiological speculationaboutneurotransmittersystems

“MoodStabilizers”:Secondh/lessengerModifiers

Thephrase“moodstabilizer”hasnoscientificmeaningitwasusedcolloquiallyinthe 19505forthecombinedprescriptionofan"upper,”dextroamphetamine,anda“downer,” phenobarbital,Thisamphetamine/barbituratecombinationobviouslyhasnothingtodo withlithiumandbenefitsforbipolarillness.Rather,thetherapeuticconceptinvolved raisingandloweringmoodssimultaneouslyandsymptomatically(Table1.5).Thisidea doesn’tcapturelithium’seflectofpreventingfuturemanicanddepressiveepisodes,while havingfewimmediatesymptomaticeffects.

Infact,formanyyears,theterm“moodstabilizer”wasn’tusedmuchinpsychiatry,nor appliedtobipolarillness,sincelithiumwastheonlyagentavailableforthisconditionfrom the1960sintothe19803.Whencarbarnazepinewasdevelopedinthe19803,andthen valproatewasmarketedformaniaintheearly19905,theterm“moodstabilizer”beganto beusedfortheclass,drivenprimarilybymarketingofvalproatebyitsmanufacturer.This usageobfuscatedthefactthatlithium’smainefficacywasinpreventionoffutureepisodes, nottreaUnentofcurrentsymptoms,asthephrase”moodstabilizer”implied.

Later,pharmaceuticalcompaniesobtainedindicationsformostnewantipsychoticsfor acutemaniaaswell.Sincemostsuchagentswerewidelyusedinbipolardisorder,theterm “moodstabilizer”wasappliedtothem,despiteabsenceofevidenceofprophylacticefficacy.

Ifagentsinthisclassarelimitedtothedrugsthathaveefl'icacyinpreventionofmanicor depressiveepisodes,thenincludedagentswouldbelithium,valproate,carbamazepine,and lamotrigine.Thedopamineblockerswouldbeexcludedsincetheirmaintenancepreven~ tionstudiesmaybeinvalidduetoabiased“enriched”design,Whereonlydrug responsive patientsarepreselectedtoentermaintenancestudies,asdescribedinChapter6.
