At Arab Health 2020, the largest annual gathering of healthcare professionals in the MENA region, American Hospital Dubai outlined steps taken to expand its technological advancements: 1. AI in Radiology: The addition of such a system optimises

radiologists' workflows, directing studies to the dedicated radiologist, setting system pre-sets, limiting diagnostic errors caused by the eye-strain, and complementing their work by providing data analysis too large for a human to process.

2. AI in Gastroenterology: The software is designed to detect colorectal polyps. It provides our physicians with a powerful new solution in the fight against colorectal cancer. It is extremely precise in identifying lesions in the colonic mucosa that can be difficult to detect.

3. AI in Early Detection of Breast Cancer: American Hospital Dubai has acquired Computer-Aided Detection (CAD) software that highlights suspected microcalcifications in the breast and along the chest walls. This software

enhances the doctor's diagnostic abilities and improves outcomes as the physician may approve or disapprove the findings as malignant.

4. AI in Neurosurgery: With this technology, the software loaded on the physician's workstation enables the neurosurgeon to upload the patient's information before surgery and plan the surgical approach safely.

Smarter Healthcare With Malaffi The Chief Executive Officer of Abu Dhabi Health Data Services, the operator of Malaffi, Atif Al Braiki, discusses the benefits of a universal Health Information Exchange (HIE) platform.

Healthcare providers usually record patient medical information in a linear patient file or on their internal electronic medical record (EMR) system, available only within the same organisation; however, this kind of record-keeping does not necessarily provide a holistic view of a patient’s medical history or treatment chronology. What’s more, it is unable to present health indicators on a broader scale across a population. In the absence of this kind of centralised real-time information, it is more challenging for the government to drive and implement critical health directives.

Unified health information Created as a public-private partnership between the Department of Health - Abu Dhabi (DOH) and Injazat Data Systems, Malaffi, which means ‘my file’ in Arabic, is the region’s first Health Information Exchange platform.

“The DOH has initiated Malaffi as the key component in the digital transformation of the healthcare system in Abu Dhabi, and to improve the healthcare quality and patient outcomes”, explains Atif Al Braiki, Chief Executive Officer, Abu Dhabi Health Data Services.

Through Malaffi, healthcare professionals in Abu Dhabi have access to patients’ longitudinal medical files. To date, Malaffi stores more than 81 million unique clinical records and provides access to the platform to over 24,500 end-users, from more than 500 facilities. The list of connected facilities currently includes all the big healthcare groups in Abu Dhabi, like Abu Dhabi Healthcare Services Company (SEHA), Mediclinic, Mubadala Healthcare, United Eastern Medical Services, Capital Health, NMC Healthcare and

VPS Healthcare, and the majority of the stand-alone hospitals.

Malaffi has recently marked a milestone by starting to connect the medical centres and clinics.

“

We are aiming to connect all public and private healthcare providers in Abu Dhabi.

“With many more providers in the onboarding pipeline, through our secure and safe digital platform, we are aiming to connect all public and private healthcare providers in Abu Dhabi,” he continues. “Ultimately, clinicians will have more information about their patients, enabling them to improve the delivery of healthcare”, states Mr. Al Braiki.

Better healthcare Allowing the real-time exchange of patient data, the centralised platform streamlines the unification of the medical records.

Having access to this information simplifies care coordination and transition of care between different healthcare providers.

“Coordination of care is ever more critical as doctors are facing challenges due to the increasing incidence of chronic and communicable diseases, as well as the growing and ageing population,” says Mr. Al Braiki. Local trends also show that patients visit multiple doctors in different healthcare facilities, many times for the same problem. This often results in unnecessary duplication of services, varying standards of care and suboptimal patient experience leading to significant strain on the healthcare system, in addition to fuelling the rising costs of care.

“By having access to the patients’ Malaffi file, healthcare professionals have access to data about previous patients’ visits, recorded allergies, diagnoses, chronic conditions, laboratory results, and radiology reports.

Patients with underlying medical conditions are at higher risk of experiencing severe symptoms from COVID-19. By having access to real-time test results and the relevant information from their medical history, front-line medical workers are empowered to make quicker and more effective first-response and treatment decisions, when a patient is in the ER for urgent care for example. To strengthen the response to the pandemic, Malaffi has allowed urgent access to its platform for providers that are not yet connected to Malaffi and need to have access to this information to treat COVID-19 patients.

Malaffi also helps the Department of Health - Abu Dhabi by providing invaluable population health information. With these vital statistics, the government can monitor the quality of healthcare in the emirate, as well as drive important health policies.

The data in the platform also helps identify public health risks and gives an effective means to measure the emirate’s emergency preparedness and response management. “As part of the COVID-19 response efforts, building on the advanced platform, Malaffi centralised the

Future Developments Additionally, the DOH is in talks with the UAE Ministry of Health and Prevention and the Dubai Health Authority to create a parallel system to centralise patient information across the entire country. Mr. Al Braiki adds that efforts are underway to launch an app to give patients access to their Malaffi file, which will empower them to better engage in managing their health.

General Medical Supplier Hamad Saleh Alhawas Est. Single-Handedly Meets Bahrain’s Covid-19 Challenges The Hamad Saleh Al-Hawas Est. (HSH) is a Bahrain-based healthcare supplies and food import and distribution company.

Only five years old, it has already built solid relationships with the country’s Ministry of Health and defense forces providing needed essential products across the country. “Bahrain is a small country with certain gaps in the market,” explained the Managing Director, Saleh Hamad Alhawas. “Our goal is to find these gaps, form partnerships with suppliers, and fill this need.”

HSH has introduced products to Bahrain from around the world including Saudi Arabia, Turkey, Malaysia, and the United Kingdom. The healthcare division of the company is the sole distributor for numerous companies in Bahrain, stocking both medical and dental equipment and supplies. This proved to be an invaluable resource in February this year, when the Covid-19 pandemic took hold in Bahrain, overwhelming healthcare facili-

There is also the issue of recent social distancing requirements, which mean that patients cannot visit healthcare facilities as freely. However, “remote patient monitoring systems and software can enable options for effectively providing virtual care,” explains Fidanci.

The Philips eICU solutions, for example, reduce risk of infection by upgrading bedside monitoring capabilities and remotely monitoring vital signs of patients in the Intensive Care Unit (ICU).

Bedside staff at the ICU and specialised staff at the eICU center work together to manage patients with COVID-19, providing high-quality medical care from a remote location.

VitalHealth is another Philips solution. This smartphone app allows patients to record their daily status, allowing day-to-day monitoring, and acting as an excellent basis for fully-fledged medical consultation at home to avoid unnecessary visits to the hospital.

Finally, Philips Healthcare Transformation Services (HTS), which is the consulting arm of Philips, offers planning services to new or temporary healthcare sites, as well as the reconfiguration of existing facilities. This expands ICU and diagnostic capacity, including developing external COVID-19 safe triage areas. Of these, Lopinavir-Ritonavir is a product from one of our principals, which we are exclusively authorised to sell all over the Gulf region,” elaborates Khurshid. “We are currently supplying to government hospitals, Ministry of Health, Department of Health Abu Dhabi, Dubai Health Authority, Military hospitals, and private hospitals across the country,” he adds.

Whilst there have been challenges across the supply chain, the company has managed inventories through good connections with suppliers around the world.

“We are committed to helping the government fight this disease. Despite increased global demand for these medicines, we are still able to source them for the UAE. We have the right contacts to get things moving quickly and reliably,” Khurshid concludes.

Equally, over the June 20th weekend, King’s London organised the largest online ante-natal classes in the Middle East.

Post COVID-19 protocols Whilst the hospital is back at pre-pandemic in-patient capacity, there are strict distancing and hygiene protocols in place for patient safety. “We have marked spaces for distancing to ensure that there is no overcrowding anywhere in the hospital and rooms are thoroughly cleaned between appointments,” stresses Schuhmacher.

These precautions extend to antibody testing for travel clearance and treatment diagnostics, through separated, streamlined registration and testing processes. Additionally, King’s Lon-

don has also moved to adaptive schedules for consultations.

“We hold ourselves to higher standards that call upon, and honour, our shared name and heritage with the King’s mothership in London. This aligns clear pathways, systems and techniques that dictate infection-control protocols,” he elaborates.

Shared knowledge Schuhmacher adds that the hospitals and medical centres in the UAE integrate best practices, honed over decades of service in London, so that clients in the Middle East can avail the best healthcare utilizing the shared information collected across the facility’s sister networks in the UK.

As of April 2020, Malaffi comprised 69 million unique clinical records of over 3 million patients, gathered using input from 509 healthcare sites. More than 1350 facilities have signed up to share data with Malaffi in the near future.

It is common for an HIE to begin in one region, then maximise the learnings, investment and benefits by expanding out the successful platform to neighboring regions. For the benefit of patient care in the rest of the UAE, it would be possible to take the Malaffi HIE blueprint and roll it out to the other emirates, saving time and cost in comparison with starting new programmes from scratch. Orion Health’s international customers have pushed for advanced technological interventions that are ripe for emulation in the UAE. In New Zealand, for instance, to support machine learning and AI Orion Health has signed on to deliver a National Algorithm Management Solution, which will facilitate planning mechanisms such as scenario modelling and risk prediction, and thus inform the New Zealand government and healthcare sector’s ongoing COVID-19 plan of action.
