Targeted probiotics for gastrointestinal disorders across the lifespan Functional gastrointestinal disorders are increasingly common and can begin in infancy. Colic, regurgitation, vomiting and constipation often affect infants, while irritable bowel syndrome (IBS) and chronic constipation are leading presentations in adults. Emerging evidence points to the gut microbiome as a central factor in the development and progression of these conditions, and to strain-specific probiotics as a targeted therapeutic option. Infant colic and the gut microbiota Infants with colic show characteristic shifts in gut microbiota composition, including increased gas-producing genera such as Escherichia, Klebsiella and Pseudomonas, alongside reduced bifidobacteria and lactobacilli. In a randomised, placebo-controlled trial, supplementation with Bifidobacterium breve BR03 (DSM 16604) and B. breve B632 (DSM 24706) reduced daily crying time in bottlefed infants by more than 50% versus placebo over three months.
Antibiotic-associated diarrhoea Up to 39% of patients experience antibiotic-associated diarrhoea (AAD) during or after a course of antibiotics. Lacticaseibacillus rhamnosus GG (ATCC 53103) has the strongest evidence base for AAD prevention, with meta-analyses reporting up to a 52% reduction in risk. ESPGHAN strongly recommends LRGG for patients with AAD risk factors, with supplementation commencing alongside the first antibiotic dose.
Constipation Up to 24% of the general population is affected by chronic constipation. In a double-blind, randomised, placebocontrolled trial, a combination of Lactiplantibacillus plantarum LP01 (LMG P-21021) and B. breve BR03 significantly improved bowel movement frequency, stool consistency and ease of evacuation, and reduced bowel-related discomfort and bloating, with benefits sustained 15 days postintervention.
Irritable bowel syndrome IBS is associated with altered gut microbiota composition, intestinal barrier dysfunction and disrupted gut-brain signalling. Lactiplantibacillus plantarum 299v (DSM 9843) has been shown to significantly reduce abdominal pain and bloating, with 78% of patients rating symptom improvement as good or excellent after four weeks. L. rhamnosus GG has also been shown to significantly reduce IBS Symptom Severity Scores after six weeks of supplementation.
