Acne and the gut-skin axis Acne is one of the most common dermatological concerns, affecting up to 80% of adolescents and young adults aged 12 to 301 . While often perceived as a cosmetic issue, acne can significantly impact self-esteem, mental wellbeing, and quality of life. For healthcare professionals, understanding the underlying biological drivers of acne, including emerging research on the gut-skin axis, can support informed discussions with patients seeking ways to improve their skin health 2 . The Role of Inflammation in Acne Development Historically, acne was attributed to an overgrowth of Cutibacterium acnes (formerly Propionibacterium acnes), a naturally occurring microorganism on the skin. This led to the widespread use of antibiotics as a first-line treatment3 . However, research now shows that C. acnes is also an important part of the skin’s microbiota, helping maintain a healthy pH and protecting against more harmful pathogens such as Staphylococcus aureus. Problems arise when skin inflammation creates conditions that allow C. acnes to thrive excessively. Inflammation increases oil gland activity, which can block hair follicles and provide an ideal environment for bacterial growth. This triggers an immune response which paradoxically drives further inflammation, resulting in acne lesions such as pimples, papules, and pustules. 4 Factors That Influence Skin Inflammation Several physiological and environmental factors can contribute to increased oil production and skin inflammation, including hormonal changes, insulin resistance, and stress. 5 More recently, research has highlighted a potential connection between gut health and skin health via the gut-skin axis and how gut microbiome imbalances may also contribute to skin inflammation. 4 The Gut-Skin Axis: A Two-Way Connection The health of the gut microbiota is thought to influence skin health and the development of acne through its effects on the immune system and the levels of inflammation it is programmed to create. A healthy gut microbiome can reduce inflammation produced by immune cells in the gut, with these effects transferred to the immune cells in the skin as they travel from site to site via circulation. This can reduce the level of inflammation produced by the skin, and help to reduce processes that support the growth of C. acnes such as excessive oil secretion. The gut microbiome also interacts with the skin microbiota through the production of metabolites which enter circulation and accumulate in the skin, some of which have antibacterial properties that may help keep the levels of key microorganisms under control, including C. acnes. Studies on the gut microbiome of people with acne have also demonstrated low microbial diversity and unfavourable compositions, and these differences may be contributing to inflammatory immune responses in the gut and the skin, worsening the symptoms of acne. 4 Integrating the Science into Clinical Conversations The connection between the gut and skin highlights an exciting area of ongoing research.
