INDEPENDENT FACILITATION

At Easterseals MORC, we respect your right to make the decisions that are best for you. As part of the person-centered planning process, you have the choice of inviting someone you know and trust to help create your plan and to make sure that your wants and needs are fully understood throughout the process.
WHAT IS AN INDEPENDENT FACILITATOR?
An Independent Facilitator is a person who helps you during your person centered-planning process. This person is someone you trust, someone who has been trained in helping with personcentered plans or someone else you choose outside of your IDD Care Coordinator.
WHAT CAN AN INDEPENDENT FACILITATOR DO FOR YOU?
Help you identify your goals and dreams
Help you arrange your person-centered planning meeting
Make sure that your needs and wants are heard and understood during the meeting
Help you incorporate all types of supports, including natural, community and system into your plan to meet your needs
WHAT DOES AN INDEPENDENT FACILITATOR NOT DO?
An Independent Facilitator cannot decide what will be paid for in your plan, authorize your services and supports or benefit from the outcome of your plan.
HOW CAN I FIND AN INDEPENDENT FACILITATOR?
You can ask someone you know and trust or ask your IDD Care Coordinator to provide you with the names and resumes of trained facilitators who have experience coordinating plans like yours.
