COMMUNITY TRANSITION SERVICES At Easterseals MORC, we support those we serve to be as independent as possible for as long as possible. For those currently residing in a nursing home, Easterseals MORC offers a path back to community living through its Nursing Facility Transition Program.
WHO IS ELIGIBLE FOR THE NURSING FACILITY TRANSITION PROGRAM? Services are available to Medicaid beneficiaries residing in a nursing facilities who would like to return to their own home, move in with family members, find an apartment, assisted living or other community-based living option. Services are offered to assist in securing housing and facilitate a successful discharge to the community. Services needed after an individual moves back home may be available to those who meet financial eligibility guidelines.
HOW DOES THE PROGRAM WORK? Referral: A referral for transition services may be made by an individual living in a nursing facility, a family member, friend or social worker. Plan Development: A personal transition plan will be developed by you and your transition navigator, with input from others if desired. This plan may take several months to ensure a successful return to the community. Housing: Help is available if you need to adapt your home or find affordable and accessible.
